증례 : 소화기 ; 메트로놈 항암요법을 통하여 완전관해를 경험한 진행성 간세포암 2예

메트로놈 요법은 독성이 나타나지 않는 낮은 용량으로 항암제를 지속적으로 빈번하게 투여하는 방법으로, 기존의 항암화학요법과 비교해 보았을 때 독성이 적고, 내성이 발생할 가능성이 낮다는 장점이 있다. 동물실험에서 간세포암에 대해 우수한 항신생혈관작용과 항암 효과를 보여 의미있게 생존기간이 연장되었음을 보고되었으나, 임상적으로는 간세포암 치료에서 메트로놈 요법을 적용한 예는 보고된 바 없었다. 본 증례는 각각 폐전이와 주문맥 혈전을 동반한 진행성 간세포암 환자에서 대퇴동맥을 통해 간동맥에 삽관하여 반영구적 포트를 장착한 후 이를 이용한 메트로놈 항암요법을 통하여 완전관해의 뛰어난 치료 효과를 경험한 첫번째 예로, 문헌고찰과 함께 보고하는 바이다. Advanced hepatocellular carcinoma (HCC) has a poor prognosis and few effective therapies. Recently, low-dose antiangiogenic (also called metronomic) chemotherapy has been tested in patients with advanced HCC. Here, we report two patients with advanced HCC who showed a good response after metronomic chemotherapy. The first was a 54-year-old man who was diagnosed with advanced HCC with lung metastasis. After three cycles of metronomic chemotherapy, the size of tumor and pulmonary metastatic lesions had decreased markedly on follow-up computed tomography. The second was a 54-year-old woman who was diagnosed with HCC with portal vein thrombosis. Metronomic therapy was performed. After 9 months, tumor enhancement in the arterial phase had disappeared completely, and the portal vein thrombus was decreased slightly. Metronomic therapy is an interesting treatment option for patients with advanced HCC. More clinical data and studies are needed to confirm this result. (Korean J Med 78:741-746, 2010)
A Turbidimetric Determination of Protein by Trichloroacetic Acid

Based on the turbidimetric response of protein with 50% trichloroacetic acid (TCA), this study aims to introduce an assay method for protein in solution. The standard procedure consists of mixing equal volume of sample solution (standard or unknown) with 50%-TCA solution and measuring the absorbance at 450 nm after 20 min. The absorbances of the solutions were almost stable over 120 min at room temperature. This assy method is simple, reproducible, and tolerant to many interfering substances. It can detect less amount than $10\mu$g/ml of bovin serum albumin. The assay method has low protein-to-protein variability over wide range of molecular weight.
Annealing Effect of Photoluminescence Intensity on Eu-doped Y2O3 Nanocrystals

We synthesized Y2O3 : Eu (15, 20, 25 wt%) nanoparticles by using the liquid-phase reaction method and studied their luminescence characteristic according to the amount of doped europium. The particle size and luminescence intensity of the synthesized nanocrystals annealed at 500 C in air showed europium concentration dependence. We compared thesenanocrystals with Y2O3 : Eu (20 wt%) annealed in oxygen at 500C. The nanocrystal annealed in oxygen showed a better luminescence characteristic and crystallinity. With these annealing conditions, the crystallinity and luminescence characteristic of the Y2O3 : Eu nanocrystals are improved.

In order to investigate the safety of a water extract (ADP) of 1 : 1 mixture of Anemarrhena rhizoma and Phellodendron cortex for alleviating benign prostate hyperplasia, genotoxicity studies in bacterial and mammalian cell assay systems, namely, the Ames bacterial reverse mutation and chromosomal aberration assays were performed. As shown by the results of the Ames bacterial reversion assay, ADP in the range of 625-5000 $\mu\textrm{g}$/plate did not induce mutagenicity in Salmonella typhimurium TA 98, TA 100, TA 1535 and TA 1537 strains in the absence or in the presence of S9 (the microsomal fraction of rat liver homogenate) metabolic activation. The $IC_{50}$ (50% cell growth inhibition concentration) values of ADP for the chromosomal aberration assay were determined; these were 2425 $\mu\textrm{g}$/ml in the absence and 8126 $\mu\textrm{g}$/ml in the presence of S9 metabolic activation in Chinese hamster lung (CHL) fibroblast cell culture. No chromosomal aberration was observed in CHL cells treated with ADP at 2425, 1212.5 and 606.25 $\mu\textrm{g}$/ml in the absence, or at 8126, 4063 and 2031.5 $\mu\textrm{g}$/ml in the presence of S9 metabolic activation. These results show that under the conditions used, ADP does not harmfully affect the bacterial or mammalian cell system at the gene level.

<P><B>Abstract</B></P> <P>Concurrence of both endometrial adenocarcinoma and ovarian adult granulosa cell tumor (aGCT) is believed to be related to high estrogen milieu, but genomic alterations of the concurrent endometrial adenocarcinoma and aGCT are not known. For this, we analyzed an uterine endometrial adenocarcinoma and an ovarian aGCT in a same patient by a targeted next generation sequencing (NGS). We found a germline mutation in <I>STK11</I> (p.L113fs). The endometrial adenocarcinoma harbored <I>FGFR2</I> and <I>TP53</I> mutations and the aGCT harbored a <I>FOXL2</I> (p.C134 W) mutation. These germline and somatic mutations have been reported in non-concurrent tumors. These two tumors harbored 20 CNAs but only one CNA was exactly overlapped in the tumors. Our findings indicate that the concurrent endometrial adenocarcinoma and aGCT in this patient might not be genetically related to each other at germline or somatic level and suggest that such concurrence might be originated from non-genetic backgrounds including stimulated estrogen milieu.</P>
Swelling-activated Cl- Channels in Human Salivary Gland Acinar Cells

The role of Cl channels in regulatory volume decrease (RVD) in human salivary gland acinar cells was examined using a whole-cell patch clamp technique. Human tissues were obtained from healthy volunteers or from patients with oromaxillofacial tumors. During the measurements, K+-free solutions were employed to eliminate contamination of whole-cell conductance by K+ currents. When the cells were exposed to a 70% hypotonic solution, outward-rectifying currents, which were not observed in the resting state, were found to have significantly increased both in human labial and parotid gland acinar cells. The amplitudes of the currents were reduced in a low Cl bath solution. Furthermore, the addition of 100 μM 5-Nitro-2- (3-phenyl propylamino) benzoic acid (NPPB) or 100 μM 4,4'-diisothio cyanatostilbene -2,2'-disulphonic acid (DIDS), known to partially block Cl channels, significantly inhibited these currents. Its outward-rectifying current profile, shift in reversal potential in a low Cl bath solution and pharmacological properties suggest that this is a Ca 2+-independent, volume activated Cl current. We conclude therefore that volume activated Cl channels play a putative role in RVD in human salivary gland acinar cells.

<P> Pure and Nb-doped Bi4Ti3O12 ceramics were prepared by using the conventional solid state reaction method. The dielectric permittivity and electrical modulus in pure and Nb-doped Bi4Ti3O12 ceramics were measured in the temperature range of 30 ∼ 700 °C and the frequency range of 1 Hz∼1 MHz through an impedance spectroscopy method. The obtained results showed well-defined relaxation peaks. The conductivity relaxation times are determined from the peaks of the modulus. The activation energies associated with hopping condition are 0.42 eV and 0.53 eV in pure and Nb-doped Bi4Ti3O12 ceramics, respectively.</P>
