Proceedings of international Symposium on East-West Medicine, Seoul. 44-64, 1999.-Cordyceps sinensis is a major parasitic fungus on larva of Lepidoptera. It is one of the well know fungi used in traditional Chinese medicine for treatment asthma, lung and kideney diseases. Many fungi belonging to the genus Cordyceps have been demonstrated to produce natural products with various biological activities. Reported biological activities of genus cordyceps include:(a)inhibition of DNA and RNA synthesis;(b)enhancement of cell differentiation;(c)restruction of cytoskeleton;(d)inhibition of protein kinase activity;(e)antitumor activity on bladder, colon, lung carcinoma as well as fibroblastoma;(f)inhibition of the infection and revers transciptase activity of human immunodeficiency virus type 1; and(g) inhibition of methylation of nucleic acid. Thus, it is suggested that the extracts of Cordyceps sp. Contain antitumor, antiviral, and many other biological activities. In our previous studies, we proved that C. sinensis contains the antitumor and immunomodulatory agents. In the present studies, effects of C. sinensis on bronchoalveolar lavage fluids(BAL)cells were demonstrated. The crude methanolic extracts of C. sinensis were fractionated into 15 fractions by silica gel column chromatography. Effects of various fraction on BAL cells proliferation and lipopolysaccharide(LPS) stimulated interleukin-1β(IL-8),platelet activating factor(PAF), interleukin-6(IL-6), tumor necrosis factor-α(TNF-α),and interleukin-8(IL-8) production on BAL cells were determined. The result indicated that the CS-19-22 fraction suppressed BAL cells proliferation activated by LPS. The median inhibitory concentration(IC50) was 6㎍/ml. The CS-19-22 fraction also decreased IL-1β,PAF,IL-6, TNF-αand IL-8 production.The results of revers transcription-polymerase chain reaction(RT-PCR) demonstrated that the CS-19-22 fraction did not affect IL-1β,PAF,IL-6, TNF-α,and IL-8 mRNAs expression in BAL cells activated by LPS. By contrast, it inhibited the IL-10 mRNA expression but enhanced IFN-γand IL-12 mRNAs expression in activated BAL cells. Moreover, the CS-19-22 fraction blocked PAF-induced platelet aggregation. It is unlikely that cytotoxicity was involved. Because no cell deths were observable. We hypothesize that inhibitory mechanisms of CS-19-22 on BAL cells proliferation may be related to the impairments of gene expression and production of cytokines in BAL cells. The explanation of C. sinensis antiasthma function may involved the following(1)C.sinenesis contained immunomodulatory agents that stimulated IFN-γand IL-12 expression in TH1cells.(2)Enhanced secretion of IFN-γand IL-12 will inhibit the Tγcells immune responses Ex. Decreasing of IL-6 and IL-10 production.(3) The impairments of cytokines production in TH2 cells suppressed th BAL cells proliferation then more cytokines production Ex.IL-1β,PAF,IL-6,TNF-αand IL-8 in activated BAL cells were decreased. (4)The suppression of TH2cells immune responses will inhibit the growth and differentiation of B cells then IgE production will be decreased. (5)Reduced production of IgE will decreased asthma attack occurring in individuals. However, the results of the present study suggested that C. sinensis may also have acted to treat asthma in part by inhibiting BAL cells proliferation and cytokine gene expression and production. Future, experiments with treatment of animals with asthma with C. sinensis will be necessary to defined whether C. sinensis can reduce asthma injury. Plans are underway for the isolation of pure principal growth inhibitors and their mechanisms of action are subjected for further study.

      • A Novel Active Image AuthenticationSchemefor Block Truncation Coding

        In this paper, a simple active image authentication scheme for the compressed images of block truncation coding (BTC) is proposed. In this scheme, the authentication codes of the compressed blocks are generated from the random value induced by the random seed. The authentication code of each compressed block is embedded into the bit map. The bit length of each authentication code can be chosen according to the user’s requirement. The experimental results indicate that the proposed scheme detects the tampered areas clearly and keeps good image qualities of the embedded images. Meanwhile, a low computational cost is required in the proposed scheme.

        Type 2 Diabetes Mellitus Increases Peritonsillar Abscess Susceptibility: Real-World Evidence

        Objectives. Type 2 diabetes mellitus (T2DM) is a risk factor for deep neck infection (DNI) and leads to complications and poor outcomes. Our study aimed to investigate the risk, prognosis, and complications of peritonsillar abscess (PTA) in patients with T2DM. Methods. We extracted data of patients newly diagnosed as having T2DM between January 2000 and December 2011 from Taiwan’s National Health Insurance Research Database. These patients were matched with patients without T2DM, and PTA incidence was compared between both cohorts. Results. In total, 67,852 patients with and 135,704 patients without T2DM were enrolled. PTA incidence was significantly higher in patients with T2DM (incidence rate ratio, 1.91; P<0.001); moreover, PTA incidence was higher at 1 to 5 years after T2DM diagnosis than at <1 and >5 years after T2DM diagnosis. Cox regression analysis showed that patients with T2DM had an approximately 2-fold higher PTA risk (adjusted hazard ratio [aHR]: 1.89, P<0.001). Patients with a higher adapted Diabetes Complications Severity Index (aDCSI) had higher PTA risk than those with a lower aDCSI (aHRs: 2.17 for aDCSI ≥1, P=0.006 and 1.81 for aDCSI=0, P=0.002). T2DM patients with a high aDCSI (≥1) had a nonsignificantly longer hospitalization duration and a higher rate of DNI complications than did those with a low aDCSI (=0). Conclusion. In patients with T2DM, PTA incidence was relatively high, and it increased with T2DM severity. Moreover, T2DM patients should be particularly careful about PTA within 1 to 5 years after the diagnosis, and physicians should keep in mind that the prognosis of PTA was correlated with T2DM severity.

      • Mobilizing Voluntary Organizations in Taiwanese Emergency Response: Citizen Engagement and Local Fire Branch Heads

        This article assesses factors that affect the ability of local fire branch heads in Taiwan to mobilize volunteer organizations in local emergency responses. Data from a survey of local fire branch heads in Taiwan is analyzed by using an OLS model to test three hypotheses regarding the relationship between the dependent variable, perceived ability to mobilize volunteer organizations in emergency response, and three explanatory variables: organizational capacity, quality of communication, and the quality of citizen engagement ex-ante to emergency response. The model indicates a positive relationship between the ability to mobilize volunteer organizations in emergency response, the quality of communications, and the quality of citizen engagement in preparedness. The research suggests that local fire branch heads and volunteer organizations should begin the process of emergency response mobilization in the preparedness stage. The quality of the citizen engagement in preparedness stages should increase the ability of local fire branch managers to mobilize external resources in emergency response.

        Using Chlorophyll Fluorescence and Vegetation Indices to Predict the Timing of Nitrogen Demand in Pentas lanceolata

        The objective of this study was to predict the timing of nitrogen (N) demand through analyzing chlorophyll fluorescence (ChlF), soil-plant analysis development (SPAD), and normalized difference vegetation index (NDVI), which are positively correlated with foliar N concentration in star cluster (Pentas lanceolata). The plants were grown in potting soil under optimal conditions for 30 d, followed by weekly irrigation with five concentrations (0, 4, 8, 16, and 24 mM) of N for an additional 30 d. These five N application levels corresponded to leaf N concentrations of 2.62, 3.48, 4.00, 4.23, and 4.69%, respectively. We measured 13 morphological and physiological parameters, as well as the responses of these parameters to various N-fertilizer treatments. The general increases in Dickson's quality index (DQI), above-ground dry weight (DW), total DW, flowering rate, ${\Delta}F/Fm$', and qP in response to treatment with 0 to 8 mM N were similar to those of SPAD, NDVI, and Fv/Fm. Consistent and strong correlations ($R^2$= 0.60 to 0.85) were observed between leaf N concentration (%) and SPAD, NDVI, ${\Delta}F/Fm$', and above-ground DW. Validation of leaf S PAD, NDVI, and ${\Delta}F/Fm$' revealed that these vegetation indices are accurate predictors of leaf N concentration that can be used for non-destructive estimation of the proper timing for N-solution irrigation of P. lanceolata. Moreover, irrigation with 8 mM N-fertilizer i s recommended w hen leaf N concentration, SPAD, NVDI, and ${\Delta}F/Fm$' ratios are reduced from their saturation values of 4.00, 50.68, 0.64, and 0.137%, respectively.

      • Impact of Peri-Operative Anemia and Blood Transfusions in Patients with Gastric Cancer Receiving Gastrectomy

        Background: Potential disadvantages of blood transfusion during curative gastrectomy for gastric cancer have been reported, and the role of peri-operative transfusions remains to be ascertained. Thus, the aim of our study was to survey its impact in patients with gastric cancer undergoinging gastrectomy. Materials and Methods: Clinical data of patients receiving curative gastrectomy at Far Eastern Memorial Hospital were obtained. Findings for pre-operative anemia states, pre-, peri- and post-operative transfusion of red blood cell (RBC) products as well as post-operative complication events were collected for univariate analysis. Results: A total of 116 patients with gastric cancer received gastrectomy at Far Eastern Memorial Hospital from 2011 to 2014. Both pre-operative and intra- and post-operative transfusion of RBC products were markedly associated with post-operative infectious events (OR: 3.70, 95% CI: 1.43-9.58, P=0.002; OR: 8.20, 95% CI: 3.11-22.62, P<0.001, respectively). In addition, peri- and post-operative RBC transfusion was significantly associated with prolonged hospital stay from admission to discharge (OR: 8.66, 95% CI: 1.73-83.00, P=0.002) and post-operative acute renal failure (OR: 19.69, 95% CI: 2.66-854.56, P<0.001). Also, the overall survival was seemingly decreased by peri-operative RBC transfusion in our gastric cancer cases (P=0.078). Conclusions: Our survey indicated that peri-operative RBC transfusion could increase the risk of infectious events and acute renal failure post curative gastrectomy as well as worsen the overall survival in gastric cancer cases. Hence, unnecessary blood transfusion before, during and after curative gastrectomy should be avoided in patients with gastric cancer.

      • Effects of Acupuncture at Pai-Hui on the Deficit of Memory

        Acupuncture has been introduced as one of the complementary therapies in traditional Chinese medicine. In addition to the analgesic effect, acupuncture also has anti-inflammatory, anti-allergic and immunomodulatory effects. Activation of the Bai-Hui by acupuncture was further shown to correct loss of consciousness, as well as to relieve mental abstraction, sluggish speech and hysteria. In this study, we investigated the attenuating effect of Pal-Hui acupuncture on scopolamine (SCOP)-induced or cycloheximide (CXM)-induced impairment of the passive avoidance response in rats. Our preliminary results indicated that acupuncture at Pal-Hui before or immediately after the training trial could significantly attenuate SCOP-induced impairment in rats. The effects were better than those treated before the right-room retention trial. Interestingly, the identical procedure could also significantly attenuate the CXM-induced impairment, but not those treated before the retention trial. The correction effect of acupuncture on CXM-induced impairment was equivalent to that of piracetam treatment. Acupunctures at Pal-Hui 15,30 or 60 min before the training trial could he all effective to attenuate CXM-induced impairment, and its efficacy paralleled acupuncture duration. Furthermore, we demonstrated the mechanism of the attenuating effects of acupuncture at Pal-Hui on CXM-induced memory consolidation deficits by combined with SCOP or p-chloroamphetamine(PCA). The attenuating effects of acupuncture at Pal-Hui was significantly antagonized by PCA but not SCOP. These results suggest acupuncture at Pal-Hui might mainly affect the memory storage process and had preventive and immediate therapeutic effects on the CXM-induced impairment and its efficacy paralleled acupuncture duration. The preventive effect of acupuncture at Pal-Hui on CXM-induced impairment is significantly reduced by serotonergic 5-HT releaser. However, the detailed mechanism of acupuncture on memory function and the differential mechanism of acupuncture on memory function an analgesia must be further investigated.

      • Understanding Breast Cancer Screening Practices in Taiwan: a Country with Universal Health Care

        While the incidence of breast cancer (BC) has been relatively low in Asian countries, it has been rising rapidly in Taiwan. Within the last decade, it has replaced cervical cancer as the most diagnosed cancer site for women. Nevertheless, there is a paucity of studies reporting the attitudes and practices of breast cancer screening among Chinese women. The aim of this study is to assess Taiwanese women's knowledge of and attitudes toward BC screening and to identify potential factors that may influence screening behavior. The study population consisted of a sample of 434 Taiwanese women aged 40 and older. Despite access to universal health care for Taiwanese women and the fact that a majority of the women had heard of the breast cancer screening (mammogram, clinical breast exams, etc.), the actual utilization of these screening modalities was relatively low. In the current study, the majority of women had never had mammograms or ultrasound in the past 5 years. The number one most reported barriers were "no time," "forgetfulness," "too cumbersome," and "laziness," followed by the perception of no need to get screened. In addition, the results revealed several areas of misconceptions or incorrect information perceived by study participants. Based on the results from the regression analysis, significant predictors of obtaining repeated screening modalities included age, coverage for screening, barriers, self-efficacy, intention, family/friends diagnosed with breast cancer. The findings from the current study provide the potential to build evidence-based programs to effectively plan and implement policies in order to raise awareness in breast cancer and promote BC screening in order to optimize health outcomes for women affected by this disease.

        Surgical Outcomes and Predictive Factors in Patients With Detrusor Underactivity Undergoing Bladder Outlet Obstruction Surgery

        Purpose: This study was conducted to evaluate the efficacy of bladder outlet surgery in patients with detrusor underactivity (DU) and to identify factors associated with successful outcomes.Methods: We conducted a retrospective review of men diagnosed with DU in urodynamic studies who underwent bladder outlet surgery for lower urinary tract symptoms between May 2018 and April 2023. The International Prostate Symptom Score (IPSS) questionnaire, uroflowmetry (UFM), and multichannel urodynamic studies were administered. Successful treatment outcomes were defined as either an IPSS improvement of at least 50% or the regaining of spontaneous voiding in patients urethral catheterization prior to surgery.Results: The study included 93 male patients. Men diagnosed with significant or equivocal bladder outlet obstruction (BOO) experienced significant postoperative improvements in IPSS (from 20.6 to 6.0 and from 17.4 to 6.5, respectively), maximum urine flow rate (from 5.0 mL/sec to 14.4 mL/sec and from 8.8 mL/sec to 12.2 mL/sec, respectively) and voiding efficiency (from 48.8% to 86.0% and from 61.2% to 85.1%, respectively). However, in the group without obstruction, the improvements in IPSS and UFM results were not significant. The presence of detrusor overactivity (odds ratio [OR], 3.152; P=0.025) and preoperative urinary catheterization (OR, 2.756; P=0.040) were associated with favorable treatment outcomes. Conversely, an unobstructed bladder outlet was identified as a negative prognostic factor.Conclusions: In men with DU accompanied by equivocal or significant BOO, surgical intervention to alleviate the obstruction may enhance the IPSS, quality of life, and UFM results. However, those with DU and an unobstructed bladder outlet face a comparatively high risk of treatment failure. Preoperative detrusor overactivity and urinary catheterization are associated with more favorable surgical outcomes. Consequently, active deobstructive surgery should be considered for patients with DU who are experiencing urinary retention.
