        APPLICATIONS OF THE HILBERT-HUANG TRANSFORM ON THE NON-STATIONARY ASTRONOMICAL TIME SERIES

        The development of time-frequency analysis techniques allow astronomers to successfully deal with the non-stationary time series that originate from unstable physical mechanisms. We applied a recently developed time-frequency analysis method, the Hilbert-Huang transform (HHT), to two non-stationary phenomena: the superorbital modulation in the high-mass X-ray binary SMC X-1 and the quasi-periodic oscillation (QPO) of the AGN RE J1034+396. From the analysis of SMC X-1, we obtained a Hilbert spectrum that shows more detailed information in both the time and frequency domains. Then, a phase-resolved analysis of both the spectra and the orbital profiles was presented. From the spectral analysis, we noticed that the iron line production is dominated by different regions of this binary system in different superorbital phases. Furthermore, a pre-eclipse dip lying at orbital phase ~0:6-0:85 was discovered during the superorbital transition state. We further applied the HHT to analyze the QPO of RE J1034+396. From the Hilbert spectrum and the O-C analysis results, we suggest that it is better to divide the evolution of the QPO into three epochs according to their different periodicities. The correlations between the QPO periods and corresponding fluxes were also different in these three epochs. The change in periodicity and the relationships could be interpreted as the change in oscillation mode based on the diskoseismology model.

        Changes in Glucose Metabolism after Adrenalectomy or Treatment with a Mineralocorticoid Receptor Antagonist for Primary Aldosteronism

        Background: Data on the effects of excess aldosterone on glucose metabolism are inconsistent. This study compared the changes in glucose metabolism in patients with primary aldosteronism (PA) after adrenalectomy or treatment with a mineralocorticoid receptor antagonist (MRA). Methods: Overall, 241 patients were enrolled; 153 underwent adrenalectomy and 88 received an MRA. Fasting glucose, homeostatic model assessment of insulin resistance (HOMA-IR), and homeostatic model assessment of β-cell function (HOMA-β) were compared between the treatment groups after 1 year. Plasma aldosterone concentration (PAC) and factors determining HOMA-IR and PAC were evaluated. Results: No baseline differences were observed between the groups. Fasting insulin, HOMA-IR, and HOMA-β increased in both groups and there were no significant differences in fasting glucose following treatment. Multiple regression analysis showed associations between PAC and HOMA-IR (β=0.172, P=0.017) after treatment. Treatment with spironolactone was the only risk factor associated with PAC >30 ng/dL (odds ratio, 5.2; 95% confidence interval [CI], 2.7 to 10; P<0.001) and conferred a 2.48-fold risk of insulin resistance after 1 year compared with surgery (95% CI, 1.3 to 4.8; P=0.007). Conclusion: Spironolactone treatment might increase insulin resistance in patients with PA. This strengthened the current recommendation that adrenalectomy is the preferred strategy for patient with positive lateralization test. Achieving a post-treatment PAC of <30 ng/dL for improved insulin sensitivity may be appropriate.

        Portable Laser Doppler Flowmeter for Microcirculation Detection

        Purpose The microcirculatory system of the human body is closely related to its physiological conditions. The laser Doppler flowmeter can detect symptoms through variations in the microcirculation; however, the present commercial Doppler flowmeter is very expensive, heavy, and inconvenient to carry. Therefore, an inexpensive, small, and portable laser Doppler flowmeter is proposed. Methods In this study, we designed a laser Doppler flowmeter transmitting receiving module based on the fundamental principles of laser Doppler, using a common computer for flow calculation. A self-designed correction system was used to validate the portable laser Doppler flowmeter. Results The coefficient of correlation between the mean frequency offset <ω> of the flowmeter and the actual mimicked blood flow rate reached a high value of R = 0.99, and the variations in microcirculation flow could be accurately measured. Conclusions The results proved that the low-cost portable laser Doppler flowmeter can be used extensively in early symptom detection for disease prevention and control.

        臺灣「後現代」觀察

        After mid-80’s, the comparative concept in Taiwan contemporary cultural arts of “localization” and “internationalization” becomes the era topic. “Localization” as “Taiwan” and “internationalization” as “modernism” seem to be regarded as synonymous. However, it is not easy to clarify the complicated definition by simple dichotomy. Localism is a multiple culturalism instead of a conservative thinking, it should be a flexible concept. “Localization” is not outdated but preserving the main culture and integrating the value of both old and new cultures. On the other side, “internationalization” represents that the Western mighty culture affects the Third World but not equals to modernism, and it is a compromised sense. The manner that how Western “Postmodernism” subverts the contemporary ink-painting is the main reason of the consequence in divergence; nevertheless, it contains positive “freedom” and “possibilities” in stimulating the creativity of ink-painting arts. Currently, Taiwan “postmodernism” combines a great deal of cultural resources and it conserves traditional ideas and creates brand-new opinions simultaneously. Therefore, the delimitation of “localization” and “internationalization” is no more distinct. As my observation, an open-minded cultural awareness makes a new definition in contemporary arts of focusing on subjects themselves and the spirit of era. The development of Taiwan principal artistic thoughts are effected by post-colonialism and postmodernism, as well as domestic trouble and foreign invasion. These lessons become a profound energy of humanities, not less than a core value of “modern” and “postmodern” ink-painting arts. Accordingly, the interpretation of “localization” and “internationalization” could be representative of the primary culture of contemporary Taiwan.

        Professionals’ experiences and attitudes toward use of Traditional Chinese Medicine in hospice palliative inpatient care units: A multicenter survey in Taiwan

        Background: Medical staff may have difficulties in using conventional medicine to manage symptoms among terminally ill patients, including adverse effects of the treatment. Traditional Chinese medicine (TCM) is regarded as a complementary or alternative medicine, and has been increasingly used in the field of palliative medicine in recent years. This study aimed to investigate the experiences of and attitudes toward using TCM among palliative care professionals, and to provide preliminary information about its use in palliative care. Methods: This was a cross-sectional survey study conducted in eight inpatient hospice wards in Taiwan between December 2014 and February 2016. The questionnaire was self-administered, and was analyzed with descriptive statistics including Pearson’s Chi-square test and Fisher’s exact test. Results: A total of 251 palliative care professionals responded to the questionnaire, of whom 89.7% and 88.9% believed that the use of TCM could improve the physical symptoms and quality of life in terminally ill patients, respectively. Overall, 59.8%, of respondents suggested that TCM had rare side effects, and 58.2% were worried that TCM could affect the liver and kidney function of patients. In total, 89.7% and 88.0% of professionals agreed there were no suitable clinical practice guidelines and educational programs, respectively, for TCM use in palliative care. Conclusions: Most of the respondents agreed there was insufficient knowledge, skills-training, and continuing education on the use of TCM in terminally ill patients in Taiwan. These results show that to address patient safety considerations, guidelines about use of TCM in palliative care should be established.

        Catheter Ablation of Ventricular Tachycardia in Arrhythmogenic Right Ventricular Dysplasia/Cardiomyopathy

        Arrhythmogenic right ventricular dysplasia/cardiomyopathy (ARVD/C) is predominantly an inherited cardiomyopathy with typical histopathological characteristics of fibro-fatty infiltration mainly involving the right ventricular (RV) inflow tract, RV outflow tract, and RV apex in the majority of patients. The above pathologic evolution frequently brings patients with ARVD/C to medical attention owing to the manifestation of syncope, sudden cardiac death (SCD), ventricular arrhythmogenesis, or heart failure. To prevent future or recurrent SCD, an implantable cardiac defibrillator (ICD) is highly desirable in patients with ARVD/C who had experienced unexplained syncope, hemodynamically intolerable ventricular tachycardia (VT), ventricular fibrillation, and/or aborted SCD. Notably, the management of frequent ventricular tachyarrhythmias in ARVD/C is challenging, and the use of antiarrhythmic drugs could be unsatisfactory or limited by the unfavorable side effects. Therefore, radiofrequency catheter ablation (RFCA) has been implemented to treat the drug-refractory VT in ARVD/C for decades. However, the initial understanding of the link between fibro-fatty pathogenesis and ventricular arrhythmogenesis in ARVD/C is scarce, the efficacy and prognosis of endocardial RFCA alone were limited and disappointing. The electrophysiologists had broken through this frontier after better illustration of epicardial substrates and broadly application of epicardial approaches in ARVD/C. In recent works of literature, the application of epicardial ablation also successfully results in higher procedural success and decreases VT recurrences in patients with ARVD/C who are refractory to the endocardial approach during long-term follow-up. In this article, we review the important evolution on the delineation of arrhythmogenic substrates, ablation strategies, and ablation outcome of VT in patients with ARVD/C.

        THE UPDATED ORBITAL PERIOD OF LOW MASS X-ray BINARY 4U 1323-62

        4U 1323-62, a low mass X-ray binary with an orbital period of 2.94 hr, exhibits periodic X-ray dips, which are due to absorption by the bulge of the outer accretion disk. The purpose of this study is to search for orbital period changes using archived X-ray data over a time span of 20 years. We present our preliminary results from analyzing light curves observed by RXTE, BeppoSAX, XMM-Newton and Suzaku. We used the method proposed by Hu et al. (2008) to estimate dip center time and adopted the Observed - Calculated method to measure changes in period. We obtained an orbital period of 2.941917(36) hr and a period derivative of $\dot{P}_{orb}/P_{orb}=(-9.9{\pm}3.5){\times}10^{-7}yr^{-1}$. The F-test result shows that the quadratic ephemeris is describes the evolution of the dip phases better than the linear ephemeris at a greater than 95% confidence level. More X-ray data collected from the early 80s will be included to further refine the orbital ephemeris.

        The accuracy and clinical applicability of a sensor based electromagnetic nonfluoroscopic catheter tracking system

        Background and Objectives: The differences between electromagnetic-based mapping (EM) and impedance-based mapping (IM) in 3D anatomical reconstruction have not been fully clarified. We aimed to investigate the anatomical accuracy between EM (MediGuide™) and IM (EnSite Velocity™) systems. Methods: We investigated 15 consecutive patients (10 males, mean age 58±9 years) who underwent pulmonary veins (PVs) isolation for paroxysmal atrial fibrillation (PAF). Contrast-enhanced computed tomography (CT) image of the left atrium (LA) was acquired before ablation and the 3D geometry of the LA was constructed using EM during ablation procedure. We measured the 4 PV angles between the main trunk of each PV and the posterior LA after field scaling. Additionally, the posterior LA surface area was measured. The variables were compared to those of CT-based geometry. A control group of 40 patients who underwent conventional PVs isolation using IM were also evaluated. Results: The actual and relative changes of EM and CT-based geometry in all PV angles and posterior LA were significantly smaller compared to those of IM and CT-based geometry. Intraclass correlation coefficient (ICC) between EM and CT-based geometry were 0.871 (right superior pulmonary vein [RSPV]), 0.887 (right inferior pulmonary vein [RIPV]), 0.853 (left superior pulmonary vein [LSPV]), 0.911 (left inferior pulmonary vein [LIPV]), and 0.833 (posterior LA). On the other hand, ICC between IM and CT-based geometry were 0.548 (RSPV), 0.639 (RIPV), 0.691 (LSPV), 0.706 (LIPV), and 0.568 (posterior LA). Conclusions: Image integration with EM enables high accurate visualization of cardiac anatomy compared to IM in PAF ablation.

        DISCOVERY OF AN X-RAY-EMITTING CONTACT BINARY SYSTEM 2MASS J11201034−2201340

        <P>We report the detection of orbital modulation, a model solution, and the X-ray properties of a newly discovered contact binary, Two Micron All Sky Survey (2MASS) J11201034-2201340. We serendipitously found this X-ray point source outside the error ellipse when searching for possible X-ray counterparts of 7-ray millisecond pulsars among the unidentified objects detected by the Fermi Gamma-ray Space Telescope. The optical counterpart of the X-ray source (unrelated to the 7-ray source) was then identified using archival databases. The long-term Catalina Real-Time Transient Survey detected a precise signal with a period of P = 0.28876208 (56) days. A follow-up observation made by the Super Light Telescope of Lulin Observatory revealed the binary nature of the object. Utilizing archived photometric data of multi-band surveys, we construct the spectral energy distribution (SED), which is well fit by a K2V spectral template. The fitting result of the orbital profile using the Wilson Devinney code suggests that 2MASS J11201034-2201340 is a short-period A-type contact binary and the more massive component has a cool spot. The X-ray emission was first noted in observations made by Swift, and then further confirmed and characterized by an XMM-Newton observation. The X-ray spectrum can be described by a power law or thermal Bremsstrahlung. Unfortunately, we could not observe significant X-ray orbital modulation. Finally, according to the SED, this system is estimated to be 690 pc from Earth with a calculated X-ray intensity of (0.7 - 1.5) x 10(30) erg s(-1), which is in the expected range of an X-ray emitting contact binary.</P>

        ON THE COMPLEX VARIABILITY OF THE SUPERORBITAL MODULATION PERIOD OF LMC X-4

        LMC X-4 is an eclipsing high-mass X-ray binary exhibiting a superorbital modulation with a period of ~ 30:5 days. We present a detailed study of the variations of the superorbital modulation period with a time baseline of ~ 18 years. The period determined in the light curve collected by the Monitor of All-sky X-ray Image (MAXI) significantly deviates from that observed by the All Sky Monitor (ASM) onboard the Rossi X-ray Timing Explorer (RXTE). Using the data collected by RXTE/ASM, MAXI, and the Burst Alert Telescope (BAT) onboard Swift, we found a significant period derivative, $\dot{P}=(2.08{\pm}0.12){\times}10^{-5}$. Furthermore, the O{C residual shows complex short-term variations indicating that the superorbital modulation of LMC X-4 exhibits complicated unstable behaviors. In addition, we used archive data collected by the Proportional Counter Array (PCA) on RXTE to estimate the orbital and spin parameters. The detected pulse frequencies obtained in small time segments were fitted with a circular orbital Doppler shift model. In addition to orbital parameters and spin frequency for each observation, we found a spin frequency derivative of $\dot{v}=(6.482{\pm}0.011){\times}10^{-13}Hz{\cdot}s^{-1}$. More precise orbital and spin parameters will be evaluated by the pulse arrival time delay technique in the future.
