The Patterns of Mandibular Movement in Relation to Maxillofacial Skeletal Structure

앵글씨 분류에 따른 최대개구시 하악의 수직 변위에 대한 차이를 확인하고, 수직 하악운동에 영향을 주는 골격요소를 확인하기 위하여, 측두하악관절 및 저작계 이상에 대한 증상 및 병력이 없으며 발치 및 교정치료의 경험이 없는 광주지역 대학에 재학중인 학생들을 대상으로, 앵글씨 구치부 관계에 근거하여 1급군(남:30명, 여:49명), 2급군(남:18명, 여:24명)과 3급군(남:18명, 여:33명)으로 분류하여 총 172명(연령 범주:20-30세)의 학생을 선택하였다. 전남대학교 병원 구강내과에서 사용하는 계측용자를 이용하여 최대 개구시 상하악 중절치 절단면간의 거리를 측정하였다. 대상자들의 진단모형을 만들어 상하악궁 길이와 폭경를 측정하였다. 대상자들에 대한 두부 규격방사선 사진을 촬영, 작도하고 방사선학적 지표를 계측, 비교분석하였다. 앵글씨 분류 1급군, 2급군 그리고 3급군 절치간 최대개구량은 3급군이 가장 컸으며 모든 군에서 남자가 여자보다 컸다. 구치간 최대개구량은 앵글씨 분류 1급군, 2급군 그리고 3급군에서 각 군간의 유의한 차이는 없었으나, 모든 군에서 남자가 더 컸다. 앵글씨 분류 1급군과 2급군의 하악운동에서 총 하악골 길이, 하악지 길이, 하악 하연부 길이 그리고 상악궁 폭경이 변수로 나타났으며, 상하악궁 길이와는 역상관관계를 나타내었다. 앵글씨 분류 3급군의 하악운동에서 상하악궁 길이와 안면 부길이가 1급군과 2급군과는 다른 중요한 변수로 나타났으며, 상악궁의 폭경과는 역상관관계를 나타내었다. 이상의 결과로 보아 앵글씨 분류 각 군에서 하악개구운동은 안면 골격구조의 영향을 받으며, 각 군간에 영향을 주는 안면 골격 요소들은 차이가 있었다. 따라서 이러한 골격적 요소들은 개구량 개선을 위한 진단과 치료시 고려되어야 할 것으로 사료된다.

Myofascial Pain, and the Spraying and Stretching Technique

근막동통은 발통점과 중추흥분효과가 나타나는 국소적 근육성 동통이다. 발통점은 근육의 일부나 건 부착점에 나타나는 부위로서 단단한 띠로서 느껴지고 촉진시 통증을 유발한다. 통증은 관련된 발통점의 위치에 따라 연관된 부위에 나타난다. 이러한 근막 발통점에 따른 증상은 약한 기능이상부터 일상생활이 어렵게 되는 정도의 심한 통증까지 다양하다. 이의 발생에는 국소적 근육이상, 심부 동통, 심리적 스트레스의 증가, 수면장애와 전신적인 요인들도 관여한다. 일반적으로 치료는 보존적인 처치로써, fluoro-methane 같은 vapocoolant spray를 사용하는 사용하여 분사신장하는 방법이 있으며, 국소적인 주사, 마사지, 초음파, 온열요법등이 있다. 여러 환자에서 근막동통을 가지는 환자에 분사신장요법을 시행한 결과 대부분 환자에서 증상의 개선을 보였다. 근막동통의 치료시 분사신장요법은 매우 보존적인 처치로 임상가의 적용과 접근이 쉬우며, 환자로 하여금 치료에 따른 불안감을 줄이면서 만족을 느낄 수 있는 치료로 사료된다. 그러나 만성 환자의 경우 분사신장요법의 치료결과에 대한 만족도가 높지 않았다. 분사신장 용법은 근막동통을 가진 환자 모두에서 효과적이지는 않으나 치료의 도입으로, 근막동통을 진단시에 일차적으로 사용할 수 있는 안정한 보존적인 용법으로 사료된다.

근육세포 분화에 대한 TGF-β1과 OP-1의 억제 효과

In order to investigate the effect of Transforming growth factor ${\beta}1$(below TGF-${\beta}1$) and osteogenic protein-1(below Op-1) onto the myogenic differentiation, C2C12 satellite myoblastic cell line was cultured and treated with both growth factors. At first morphological changes with microscopical examination were examined, and isolated total RNA to analyse mRNA expression of bone marker proteins, muscle regulatory proteins, TGF-${\beta}$ receptor and their ligands by Northern blot analysis. And cellular proliferative inducibility of both growth factors was also tested to C2C12 cells. Incubating the cell with $5ng/m{\ell}$ of TGF-${\beta}1$ until 4 days almost inhibited multinucleated myotube formation expressing muscular regulatory proteins, and induced decreasing Id proteins. However, no osteoblastic phenotypes was induced by TGF-${\beta}1$ in C2C12 cells. The mRNA expression of TGF-${\beta}$ receptors with TGF-${\beta}1$ was conversed after 48 hours cultured. Type I TGF-${\beta}$ receptor was seemed to play a role in negative signalling for inhibition of myogenic differentiation. OP-1 dose dependently induced ALP activity, osteopontine production and bone sialoprotein production at concentrations above $100ng/m{\ell}$ and osteocalcin production at concentrations above $300ng/m{\ell}$. The concentration of OP-1 required to induce these osteoblastic phenotypes was the same as that required to almost completely inhibit myotube formation. Incubation with above $100ng/m{\ell}$ OP-1 suppressed the expression of mRNA for muscular egulatory proteins from 2 days after incubation. Expression of Id-1, 2, 3 mRNA were stimulated by OP-1 at concentration above $300ng/m{\ell}$. When C2C12 cells were treated with both growth factors, TGF-${\beta}1$ potentiated the inhibitory effect of OP-1 on myotube formation and expression of mRNA for myogenin at 12 days. And TGF-${\beta}1$ reduced osteocalcin and bone sialoprotein production induced by OP-1 at 12 days in C2C12 cells. Both growth factor had no mitogenic effect. These results indicate that OP-1 converts the differentiation pathway of C2C12 myoblasts into that of osteoblastic lineage cells and it's not heritable, but TGF-${\beta}1$ does not and has reversible inhibitory activity on the myogenic differentiation. TGF-${\beta}1$ and OP-1 play a role in myogenic differentiation via different mechanism between them.

이 논문은 기독교를 일본에 토착화시킨 우치무라 간조의 무교회주의와 복음주의 신학에 관한 연구이다. 우치무라 간조의 무교회주의는 외국 선교회의 지배와 감독을 벗어나는 한편 일본 국내의 황교주의자, 즉 충효도덕을 내세워 기독교를 반국가적ㆍ비윤리적 종교로 몰아세우려 하는 세력과 맞서 기독교의 일본화ㆍ토착화시키고자 했던 일본종교사에서 또 하나의 복합주의적 절충주의의 표본이었다. 그것은 일본기독교인들의 공통적인 목적의식의 실현이자 무사도와 같은 일본의 전통 사상과 서양 기독교문화와의 습합으로 이루어낸 일본문화사에서 시도가 대개 천황제 절대주의와의 야합 혹은 국가에의 종속으로 끝난 데 반해 우치무라의 무교회주의는 표면적으로 예수와 일본이라는 “두 개의 J”에의 사랑을 표시하면서도 내면적으로는 오직 그리스도 이외의 어떠한 외부적인 세력에도 기대지 않는 독립적인 복음주의 입장을 고수했다는 점에서 차별적 독자성을 가지고 있었다고 말할 수 있다. 그럼에도 불구하고 그의 무교회주의는 일본문화의 본질적 속성인 잡종성(hybridism)에서 벗어날 수 없는 한계성을 드러내고 있음도 부인할 수 없다. 그는 성서를 주재료로 하고 청교도정신과 무사도를 부재료로 삼아 칵케일함으로써 오자와 사부로(小澤三郞)가 메이지문화의 특징을 가리켜 ‘보수와 진보의 각테일’이었다고 하는 평가에서도 예외일 수 없었다. The paper examined Uchimura Ganjo's non-church movement and evangelism that implanted the Christian religion in Japan. Uchimura Ganjo's non-church movement was free from control and supervision of foreign evangelists to rely upon the Emperor of Japan, in other words, the thought of loyalty and filial piety, and to fight against the power that blamed the Christian religion as anti-national and ethical impropriety religion, and to implant the Christian religion in Japan, so that it was an example of composite eclecticism in the Japanese religion history. The evangelism realized common purpose consciousness of the Japanese Christians to integrate the Japanese traditional thought with the Christian culture of the Western countries and produce an integrated outcome in the Japanese culture history. In particular, common Christian religion made efforts to implant it in Japan by making coalition with the system of the Emperor of Japan or subordinating it to the Japanese government : Uchimura Ganjo's non-church movement superficially expressed love "Two of J", in other words, Jesus Christ and Japan, to keep independent evangelism not relying upon external power and to have a differentiated originality. Nevertheless, the non-church movement had its limitation that could not be free from hybridism being inherent nature of the Japanese culture. Uchimura Ganjo adopted the Bible as main material and Puritanism as well as chivalry as sub material so that Ojawa Saburo's Meiji Culture was said to have characteristics of 'a mixture of conservatism and progressivism'.
