        Obstetrics & Gynecology Science : Original Articles ; The ON-Q pain management system in elective gynecology oncologic surgery: Management of postoperative surgical site pain compared to intravenous patient-controlled analgesia

        Objective The goal of this study was to compare postoperative surgical site pain in gynecologic cancer patients who underwent elective extended lower midline laparotomy and managed their pain with either the ON-Q pain management system (surgical incision site pain relief system, ON-Q pump) or an intravenous patient-controlled analgesia pump (IV PCA). Methods Twenty gynecologic cancer patients who underwent elective extended lower midline laparotomy were divided into two groups. One group received a 72-hour continuous wound perfusion of the local anesthetic ropivacaine (0.5%, study group) into the supraperitoneal layer of the abdominal incision through the ON-Q pump. The other group received intravenous infusion pump of patient-controlled analgesia (fentanyl citrate 20 mg/mL · kg+ondansetron hydrochloride 16 mg/8 mL+normal saline). Postoperative pain was assessed immediately and at 6, 24, 48, 72, and 96 hours after surgery using the visual analogue scale. Results Postoperative surgical site pain scores at 24, 48, and 72 hours after surgery were lower in the ON-Q group than the IV PCA group. Pain scores at 24 hours and 48 hours after surgery were significantly different between the two groups (P = 0.023, P<0.001). Overall painkiller administration was higher in the ON-Q group but this difference was not statistically significant (5.1 vs. 4.3, P = 0.481). Conclusion This study revealed that the ON-Q pain management system is a more effective approach than IV PCA for acute postoperative surgical site pain relief after extended lower midline laparotomy in gynecologic cancer patients.

      • Overexpression of miR-9, miR-21, miR-155, and miR-944 in formalin-fixed paraffin-embedded cervical cancer tissues

        목적: MicroRNAs (miRNAs) are small noncoding RNAs of about 22 nucleotides and have negative regulatory action of gene expression at post-transcriptional period through mRNA degradation and/or translation repression. These molecules have been studied as potential biomarkers in many important biological processes, including cell growth, apoptosis, viral infection and cancer development. Several oncogenic miRNAs have been reported to be associated with cervical cancer tumorigenesis as well. 방법: Among many oncogenic miRNAs, miR-9, miR-21, miR-155, and miR-944 are known to promote cell growth, migration and invasion in cervical cancer. However, the relationship between miRNAs and HR-HPV infection has not been fully understood, and few studies have been generated using clinical samples. The aim of this study was to investigate the expression levels of miR-9, miR-21, miR-155, and miR-944 in formalin-fixed paraffin- embedded (FFPE) samples and to find out relationship between miRNA expression and HR-HPV infection using reverse transcription-quantitative polymerase chain reaction (RT-qPCR). In this study, HR-HPV infection was determined by RT-qPCR targeting E6/E7, oncogenes of HPV. 결과: In brief, the results from this study showed that the expression levels of miR-9, miR-21, miR-155, and miR-944 in FFPE cervical cancer tissues were higher than in normal tissues. Among four miRNAs, miR-21 expression levels in both HR-HPV E6/E7-positive cancer tissues and -negative cancer tissues were significantly up-regulated. In other words, miR-21 expression level was up-regulated regardless of HR-HPV E6/E7 expression levels. In conclusion, this study seems to suggest that four miRNAs were up-regulated in cervical cancer tissues compared to normal tissues. 결론: Therefore, miR-9, miR-21, miR-155 and miR-944 seem to can be useful biomarkers for the diagnosis of cervical cancer.

      • Film Session Q&A 1 : 01 ; Laparoendoscopic single-site radical trachelectomy using conventional laparoscopic instruments

        목적: Demonstrate a new instrument, technique or procedure. 방법: A 29 year-old unmarried woman who was clinically diagnosed cervical cancer, FIGO stage IB1 was operated under laparoendoscopic single- site approach, using a transumbilical, home-made surgical glove port system. The procedure was performed through a 1.7 cm umbilical incision with a single multichannel port system consisting of a wound retractor, a surgical glove, and two 5-mm and one 12 mm trocars. 결과: The procedure was completed successfully in 371 minutes and estimated blood loss was 320cc. According to the final pathology report, FIGO stage was IB1 and the resection margin was all negative. 29 negative lymph nodes and no parametrial or lymphovascular invasion was reported. No severe intraoperative and postoperative complication occurred, except 6.7 cm sized lymphocele which was found on day 11 and drained. The patient has been followed up for 18 months without any evidence of recurrence. 결론: Laparoendoscopic single-site surgery may be applied to radical trachelectomy in experienced surgeons but further study in large patients numbers is needed to confirm its feasibility.

      • GO-06 : The Evaluation of Conization Margin Positivity Corresponding to 2011 International Federation of Cervical Pathology and Colposcopy (IFCPC) Transformation Zone-type Classification

        A new colposcopic terminology was released by the Nomenclature Committee of the International Federation of Cervical Pathology and Colposcopy (IFCPC) in 2011. The document added three types of transformation zone (T-zone) and corresponded excisional types. This study aimed to evaluate the margin positivity of conization according to 2011 IFCPC transformation zone-type classification. A total of 293 patients who underwent conization by large loop excision of the transformation zone (LLETZ) between January and October 2011 at Severance Hospital, Yonsei University Health System were retrospectively reviewed. Colposcopic images of each patient were reviewed independently by 2 experienced colposcopists through Picture Archiving Communication System (PACS) and classified by 3 transformation zone-types and compared resection margins after following conization. Among 247 eligible cases, type 2 transformation zone was the most common type (type 2 T-zone: 98 cases, 39.7%) and CIN 3 was the most frequent final pathologic diagnosis of conization (CIN 3: 85 cases, 34.4%). However, the margin positivity was dominantly shown in type 3 transformation zone and statistically significant (30 cases, 48.4%; p=.017). Colposcopy using the new 2011 IFCPC transformation zone-type classification is a potentially effective screening method to determine the excisional type of conization. And it might be important to describe localization of cervical lesion in relation to the each type of transformation zone for the margin-free procedure.

      • GG-34 : Internal intestinal herniation of small bowel through a broad ligament without intestinal obstruction: A experience with distinguishing feature of transvaginal ultrasonography

        Symptomatic internal herniation through a broad ligament defect with no intestinal obstruction is a very rare condition. A 32-year old gravida 4, para 3, married woman presented at emergency room with left-side dominant, lower abdominal colicky pain accompanied by several times of vomiting. The patient had a history of one normal pregnancy and 2 times of cesarean section and no other medico-surgical history. According to the physical examination, she had moderate direct tenderness in the bilateral lower abdominal quadrants with mild guarding in the left lower quadrant, and moderate left adnexal tenderness without cervical moving tenderness. Initial laboratory investigations were all normal except mild leukocytosis change in 2 hours later follow up. Transvaginal ultrasonography was requested under impression of ovarian cyst torsion or rupture and revealed no pathologic gynecology finding but invisible left side ovary maybe hiding behind 4.2x3.6 cm sized hyperechoic oval shape cross section view of double-lumen of partial small bowel loop. Computed tomography scan showed no pathologic finding, neither. Although supportive therapy was commenced with prophylactic antibiotics and analgesics, there was nothing to improve patient clinical condition. Emergency diagnostic laparoscopy disclosed that small bowel was internally herniated through a defect in the left broad ligament. The herniation was reduced, and the broad ligament defect was laparoscopically sutured. The patient was fully recovered without any event and discharged on postoperative day four.

      • The Validation of MicroRNA Profiling in Fromalin-Fixed, Paraffin-Embedded Ovarian Cancer Sample

        MicroRNAs are known as non-coding RNA molecules working for post-transcriptional regulation with critical biologic functions including differentiation, proliferation, apoptosis and stress resistance of cell. Owing to the unique riboregulatory function of gene expression, global microRNA profiling of human malignancies, especially extracted from formalin-fixed, paraffin-embedded (FFPE) samples, are increasingly carrying out. To assess the validation of FFPE ovarian cancer tissues for microRNA profiling, we compared the expressions of several microRNAs of FFPE samples of ovarian cancer to that of matched fresh-frozen samples. Three patients operated for benign uterine disease (control group) and five patients diagnosed with serous ovarian carcinoma between June 2009 and October 2011 were enrolled. Profiling of microRNAs of each patient`s FFPE samples and matched fresh-frozen samples were performed using DNA microarray. Then the expression of eight over-regulated microRNAs and six down-regulated microRNAs were quantitatively analyzed by real-time PCR. Among eight over-regulated microRNAs, six microRNAs were demonstrated more than control group as a same pattern in both FFPE and matched fresh-frozen sample. All the six down-regulated microRNAs were less demonstrated as a same pattern in both samples. The profiling of microRNA in fromalin-fixed, paraffin-embedded ovarian cancer sample is valid as fresh-frozen sample.

        어린이집 원장이 지각한 조직커뮤니케이션과 인구사회학적 변인이 어린이집 조직효과성에 미치는 영향

        본 연구는 어린이집의 조직커뮤니케이션과 원장의 인구사회학적 변인이 조직효과성에 미치는 영향을 살펴보고자 함을 목적으로 하였다. 대상은 제주를 제외한 전국의 어린이집을 운영하는 원장 178명이었다. 연구를 위하여 조직커뮤니케이션 질문지와 조직효과성 질문지가 척도로 사용되었다. 자료 분석은 빈도, 백분율, 신뢰도, 회귀분석을 실시하였다. 연구결과, 첫째, 원장이 기혼일 때 조직효과성 전체에 유의한 영향을 주었다. 하위변인 중 조직헌신의 경우 기혼이며 연봉이 높을수록, 직무성과의 경우 기혼이며 학력이 높을수록 정적인 영향을 주었다. 둘째, 커뮤니케이션 만족과 능력이 높을수록 조직효과성 전체와 직무만족이 높아졌고, 커뮤니케이션 만족이 높을수록 조직헌신이 높아지고, 커뮤니케이션 만족과 능력이 높을수록 그리고 불안이 적을수록 직무성과가 높아졌다. This study is about how the organizational communication in child-care center and the sociology population of teachers influence on organizational effectiveness. The 178 number of teachers, expecially beginning level, working in child-care centers in whole nation except Jeju Island were the investigated subject. The questionnaire listed about the organizational communication and the organizational effectiveness were used in this study. For the analysis of questionnaire, frequency, percentage, reliability, regression were used as statistical tool. In the result, the directer’s high educational, age, marrage, pay background seem to give a positive influence on the organizational effectiveness. The communication satisfaction, apprehension, competence on child-care center give a positive influence on organizational effectiveness.

        어린이집 원장이 지각한 조직커뮤니케이션이 조직갈등에 미치는 영향

        본 연구는 어린이집 원장이 바라본 조직커뮤니케이션이 어린이집 조직갈등에 미치는 영향을 규명하기위하여 실시되었다. 전국의 어린이집 원장 196명을 대상으로 하여 조직커뮤니케이션질문지와 조직갈등질문지를 척도로 사용하였다. 결과처리는 빈도분석, 회귀분석을 실시하였다. 결과는 어린이집의 실제적인 운영자인 원장의 시각에서도 원장의 결혼과 연령이 어린이집의 갈등과 조직커뮤니케이션에 영향을 주며, 어린이집의 조직갈등에 조직커뮤니케이션만족은 긍정적 영향을, 조직커뮤니케이션불안은 부정적 영향을 주는 것으로 나타났다. This study is about how the Organizational communication in child-care center and the sociology population of Directors influence on Organizational Conflict. The 196 number of Directors, expecially beginning level, working in child-care centers in whole nation except Jeju Island were the investigated subject. The questionnaire listed about the organizational communication and the Organizational Conflict were used in this study. For the analysis of questionnaire, frequency, percentage, reliability, regression were used as statistical tool. In the result, the more number of Directors in child-care center give the more influence on the Organizational communication. The Director`s age and marrage background seems to give a negative influence on the Organizational communication. The Communication communication satisfaction on child-care center give a positive influence on Communication Conflict. The Communication communication apprehension on child-care center give a negative influence on Communication Conflict.

        초임 보육교사의 특성과 보육시설 조직커뮤니케이션이 조직효과성에 미치는 영향

        본 연구는 보육시설의 조직커뮤니케이션과 초임보육교사의 인구사회학적 변인이 조직효과성에 미치는 영향을 살펴보고자 함을 목적으로 하였다. 대상은 제주를 제외한 전국의 보육시설에 근무하는 초임교사 108명이었다. 연구를 위하여 조직커뮤니케이션 질문지와 조직효과성 질문지가 척도로 사용되었다. 자료 분석은 빈도, 백분율, 신뢰도, 상관관계, 회귀분석을 실시하였다. 연구결과, 초임보육교사의 학력이 높을수록 직무만족도에 부정적으로 유의한 영향을 주었고, 보육시설당 교사수가 많을수록 커뮤니케이션 불안에 정적으로 유의한 영향을 주었고, 커뮤니케이션 만족이 높을수록 조직효과성에 정적으로 유의한 영향을 주었다. 또한 커뮤니케이션 불안이 높을수록 조직효과성의 하위변인인 직무성과에 부정적으로 유의한 영향이 있었다. The communication satisfaction on child-care center give a positive influence on organizational effectiveness.

        경력교사가 지각한 보육시설 조직커뮤니케이션이 교사의 직무성과에 미치는 영향

        본 연구는 보육시설의 조직커뮤니케이션과 보육교사의 인구사회학적 변인이 직무성과에 미치는 영향을 살펴보고자 함을 목적으로 하였다. 대상은 수도권과 강원, 충청지역 보육시설에 근무하는 교사 271명이었다. 연구를 위하여 조직커뮤니케이션 질문지와 조직성과 질문지가 척도로 사용되었다. 자료분석은 빈도, 백분율, 신뢰도, 상관관계, 회귀분석을 실시하였다. 연구결과, 보육교사의 인구사회학적 변인 중 이직유무 변인이 직무성과에 유의한 결과를 나타냈으며, 현 기관 재직기간 변인은 조직커뮤니케이션 만족에 유의한 영향을 주었다. 또한, 보육교사의 조직커뮤니케이션 만족과 능력 변인은 직무성과에 정적으로 유의한 영향을 주었으며, 조직커뮤니케이션 불안 변인은 부적으로 유의한 영향을 주었다. The purpose of this study is to understand how these three variables, organizational communication satisfaction, organizational communication apprehensiveness, and organizational competence, can influence on-the-job performance of teachers in child-care centers. The subjects in this study were selected according to a questionnaire given to 271 teachers in child-care centers. Analysis of the questionnaires was conducted by using statistical tools, such as frequency, percentage, reliability, and regression. Organizational communication satisfaction and organizational competence positively influence job performance, however, organizational communication apprehensiveness negatively affect the job performance of teachers in child-care centers.
