        제주도와 수도권 피부관리실의 현황에 관한 연구

          뷰티산업의 발전은 생활환경이 급속히 변화해가고, 생활수준이 향상되면서 더 가속적으로 발전하였다. 이제는 특정계층만이 아닌 남녀를 불문하고, 신체내적인 건강과 더불어 신체 외적인 건강 및 미에 이르기까지 관심을 집중시키게 되었다.<BR>  2004년 4월 프랑스 겔랑에서 세계 최초로 제주도 신라호텔에 1호점 스파를 개설하고, 2005년 서울 신라 호텔에 2호점을 개설 예정을 발표함으로서 우리나라는 세계적 뷰티 산업국가로 주목받고 있다. 이에 발맞추어 정부나 지방자치단체에서는 급속히 변화하는 뷰티 산업 개방화에 적극성을 보이고 있다. 그러나 이런 국제화, 세계화에 합류하기에는 국내 피부관리 실정은 열악한 상황이다.<BR>  이러한 상황에서 본 연구는 현재 제주도 피부관리실을 운영하는 원장, 경영자와 이들을 지탱하는 종사자를 대상으로 제주도의 피부관리실 실정을 파악 분석하고, 수도권과 비교하여 뷰티산업의 변화와 개방화에 앞서 제주도 피부관리가 나아가야할 방향에 대해 알아보고자 하는 것을 목적으로 하였다.<BR>  제주도의 피부관리실은 체계적이고 적극적인 변화를 시도하지 않으면 더 이상 유지할 수 없는 상황 처해 있다. 국가 차원의 활성화 방안이 추진되어야 하고, 프로그램을 다양하게 개발해야 하며, 국가 보조금 지원방안 및 피부 교육기관의 활성화를 통한 교육기회를 확대하고, 우수한 인력을 양성할 수 있는 교육 방안도 마련되어야 한다.<BR>  제주의 경우, 천혜의 자연환경을 바탕으로 한 관광의 이미지도 이미 지나간 옛말이 되었다. 그 대안으로 현재 제주도만이 가진 해양성과 열대성 기후 그리고 아열대성 기후가 만나는 삼박자가 갖추어진 천혜의 자연조건을 바탕으로, 제주만이 할 수 있는 역점 시책의 하나로 뷰티에 한방을 접목한 뷰티한방산업이 그 막을 올렸고, 조용히 그 가능성을 시험하고 있다.<BR>  이를 완성하기 위한 방안으로 제주만의 특유한 상품을 만들고 개발한다면 지금의 뷰티산업이 더 발전하고 활성화 될 수 있을 것이라 생각한다.   The growth of the skin care industry has been developed by the rapid changes in life style and upgrade of income level.<BR>  Now, people are interested in not only physical health caring but also attractive body figure regardless their gender.<BR>  Now Korea is focused by all of the world as an advanced international beauty industry country after April-2004, Guerlain of France established the first spa shop at Shilla hotel in Jeju for the first time in the world and announcing plans to set up the second shop at Shilla hotel in seoul in 2005. Moreover, the central government and local autonomous entity show their interest positively, Actually there are still shortage in domestic skin care sityation to deal with rapid change and globalization.<BR>  Therefore, the purpose of this research is to find a direction of the skin care industry prior to the rapid change and globalization in beauty industry by analyzing of status in Jeju compare with metropolitan area. The way of research is based on surveying the director, manager, and employee who currently work at the skin care shop in Jeju and collected data was dealt by SPSS statistics program.<BR>  According to the above results, actually the situation of Jeju"s skin-care industry will be stuck in the trap unless we try to change our environment positively in system. The central government and local autonomous must support to this industry more actively, develop various programs, give a more chance of education by support cosmetic academies and provide the education plan to train to produce higher competent worker.<BR>  <Now, Jeju is losing it"s sightseeing image based on an outstanding natural beautiful landscape resources. For the purpose of maintain it"s reputation in the sightseeing industry, they must make variety marketing strategies as joint with beauty-business.><BR>  As the alternative plan, I suggest combines the treasure from the oceanic, tropical, and subtropical climate which is only in Jeju, with beauty industry and the oriental medicine. And, we are carefully testing this possibility.<BR>  If this proposal is accepted and completed, we can kill two birds with one stone which current beauty industry will be more growing as higher value added and can maintain the reputation of sightseeing industry.

        만성폐쇄성폐질환에 폐렴과 급성악화로 입원한 환자의 임상적 특성 및 예후 비교

        Background: Data comparing the clinical characteristics and outcomes in chronic obstructive pulmonary disease (COPD) patients hospitalized with community-acquired pneumonia (CAP-COPD) and acute exacerbation (AE-COPD) are very limited. Methods: Eighty episodes of hospitalization in 65 CAP-COPD patients, and 111 episodes of hospitalization in 82 AE-COPD patients were included in this study. The baseline characteristics, clinical presentations, potential bacterial pathogens and clinical outcomes in these patients were retrospectively reviewed and compared. Results: No significant differences were found between the two groups in parameters related to COPD and co-morbidities, except a higher rate of male among CAP-COPD patients. Clinical presentations by symptoms and laboratory findings on admission were significantly more severe in CAP-COPD patients, who showed higher rates of fever and crepitation, but less wheezing than AE-COPD patients. S. pneumoniae and P. aeruginosae were the most common bacterial pathogens in both groups. With no difference in the overall hospital mortality between both groups, the mean length of hospital stay was significantly longer in the CAP-COPD patients than in AE-COPD patients (15.3 vs. 9.8 days, respectively, p<0.01). Additional analysis on CAP-COPD patients showed that systemic steroid use did not influence the length of hospital stay. Conclusion: Although there was no significant difference in bacterial pathogens and overall hospital mortality between the two groups, CAP-COPD patients had more severe clinical symptoms and laboratory findings at presentation, and longer hospital stay than AE-COPD patients.

        Background/Aims: Healthcare-associated pneumonia (HCAP) occurs outside hospitals, but its characteristics are similar to those of hospital-acquired pneumonia (HAP). We evaluated the clinical and microbial characteristics of HCAP in Korea. Methods: Of 130 subjects with suspected pneumonia, 49 were classified as HCAP and 81 as HAP. We retrospectively examined the clinical presentations, outcomes, pathogens, and drug resistance rates of Pseudomonas aeruginosae in both groups. Results: The clinical presentations, including the symptoms and laboratory findings, at the time of hospitalization were comparable in both groups. The hospital mortalities of HCAP (28.6%) and HAP (34.6%) did not differ significantly; the length of the hospital stay was similar for all of the survivors (14 vs. 17 days, respectively). Of the identified pathogens, methicillin-resistant Staphylococcus aureus was significantly less common in HCAP than in HAP (two vs. 18 cases, respectively, p<0.01), whereas Klebsiella pneumoniae was more common in HCAP (five vs. zero cases, respectively, p<0.01). The frequency of other Gram-negative rods was similar in both groups. The rate of resistance to antibiotics in P. aeruginosae in both groups was substantial, with the highest resistant rate to ciprofloxacin (50% and 61.5% in HCAP and HAP, respectively). Conclusions: Although the clinical features and outcomes of HCAP were comparable to those of HAP in the study population, the frequency of methicillin-resistant S. aureus was significantly lower in HCAP compared to HAP.         활동성 결핵의 중증도 및 병변 부위에 따른 전혈 인터페론 감마 분비능 측정의 민감도

        Background: The clinical manifestation of M. tuberculosis infection ranges from asymptomatic latent infection, to focal forms with minimal symptoms and low bacterial burdens, and finally to advanced tuberculosis (TB) with severe symptoms and high bacillary loads. We investigated the diagnostic sensitivity of the whole-blood interferon-y release assay according to the wide spectrum of clinical phenotypes. Methods: In patients diagnosed with active TB that underwent QuantiFERON(R) (QFT) testing, the QFT results were compared with patients known to be infected with pulmonary tuberculosis (P-TB) and extra-pulmonary TB (EP-TB). In addition, the results of the QFT test were further analyzed according to the radiographic extent of disease in patients with P-TB and the location of disease in patients with EP-TB. Results: There were no statistical differences in the overall distribution of QFT results between 177 patients with P-TB and 84 patients with EP-TB; the positive results of QFT test in patients with P-TB and EP-TB were 70.1% and 64.3%, respectively. Among patients with P-TB, patients with mild extents of disease showed higher frequency of positive results of QFT test than that of patients with severe form (75.2% vs. 57.1%, respectively; p=0.043) mainly due to an increase of indeterminate results in severe P-TB. Patients with TB pleurisy showed lower sensitivity by the QFT test than those with tuberculous lymphadenitis (48.8% vs. 78.8%, respectively; p=0.019). Conclusion: Although QFT test showed similar results between overall patients with P-TB and EP-TB, individual sensitivity was different according to the radiographic extent of disease in P-TB and the location of disease in EP-TB.
