        뇌졸중 환자의 엉덩관절 폄,굽힘근에 대한 등속성 운동이 등속성 근력과 보행속도 및 강직에 미치는 영향

        PURPOSE: The purpose of this study was to evaluate isokinetic muscle strength, spasticity, and gait speed in adults with stroke following 6 weeks isokinetic muscle strength exercises. METHODS: 26 adults stroke patients were recruited, and divided 2 groups. The exercise training performed 6 weeks general exercise therapy with isokinetic exercise (n=13), and control group (n=13) a general exercise therapy only. The rate of change in isokinetic strength, gait speeds between groups, and spasticity between groups using Modified Ashworth Scale (MAS) were compared before and after training. Paired and independent t-tests were used for statistic analysis (p<.05). RESULTS: The results were as follows: First, the hip flexor and extensor of the exercise group were significantly (p<.05) higher than control group at each velocity. However, the hip flexor of the control group was only significant at 30 °/sec and 120 °/sec, and the hip extensor of the control group was significant at 30 °/sec (p<.05). As a result of comparing the rate of change in isokinetic muscle strength at each velocity before and after treatment, the hip extensor was seen to best statistically significant at each velocity but the hip flexor was significant at 30 °/sec and 120 °/sec (p<.05). Secondly, gait speeds increased after 6 weeks training in both groups; but, there was no significant improvement (p>.05). The rate of changes in gait speeds was significant (p<.05). Thirdly, two adults with stroke at level 1+ in the Modified Ashward Scale of the control group decreased to 1 and there was no difference in the exercise group. However, one of nine adults with stroke in the control group and one of ten adults with stroke in the exercise group increased level from 1 to 1+ and three of four adults with stroke in control group and one of three adults with stroke decreased level from 1+ to 1. There was no significant difference in spasticity between groups (p>.05). CONCLUSIONS: Stroke with mild spastic hemiplegia patient``s isokinetic exercise for hip extensor and flexor is approximately six weeks in strength, therefore it helps to strengthen and improve walking speed, spasticity its impact on the big it can not.

        뇌졸중 환자에 있어서 Mini-Mental State Examination과 Motor Assessment Scale을 통한 인지기능과 기능적 회복의 상관관계 연구

        The objective of this study was to identify the effects of the cognitive performance of stroke patients on their motor function recovery after comprehensive rehabilitation management. The subjects of this study were 41 stroke in-patients of the Rehabilitation Hospital, College of Medicine, Yonsei University, hospitalized during the period from September 1, 1997 to May 5, 1998. The cognitive performance was measured using a Mini-Mental State Examination(MMSE) and the motor function recovery using Motor Assessment Scale(MAS). The data were analyzed by the paired t-test, independent t-test, a one way ANOVA, and Pearson's correlation coefficiency. The findings were as follows: 1. There was a significant difference found in the motor function recovery level after the comprehensive rehabilitation management. 2. There was no significant difference found in relation to sex, age, cause of stroke, laterality of paralysis and the level of spasticity. However, there was a big difference between pre- and post-treatment regarding the treatment period. 3. In line with the cognitive performance level, there was a significant difference found in the motor function recovery level after the comprehensive rehabilitation management. 4. The correlation between the elements of the cognitive performance and the motor recovery was found to be high in orientation, attention, calculation, and language. Those elements were expected to give larger effects on motor recovery after the comprehensive rehabilitation management. Based on this study, the cognitive performance level was found to play an important role in bringing effects on motor recovery after the comprehensive rehabilitation management of stroke patients. And the evaluation on the motor recovery based on quality would be also expected to be examined, as well as the cognitive performance level test accompanied by Intelligence Quality(IQ) test.

        발목관절 가동술과 결합한 전신진동운동이 뇌졸중 환자의 보행 기능과 균형 능력에 미치는 영향: 무작위 대조 예비연구

        PURPOSE: This study was performed to investigate the effects of the whole-body vibration exercise combined with ankle joint mobilization on the gait and balancing ability in patients with hemiplegic stroke. METHODS: A total of 19 patients at a rehabilitation hospital who had suffered a hemiplegic stroke were randomly assigned to the experimental group (whole-body vibration exercise combined with ankle joint mobilization, n=10) or control group (whole-body vibration exercise, n=9). All participants underwent 30 min of comprehensive rehabilitation therapy (5 × /week for 6 weeks). Additionally, the experimental group performed the whole body vibration exercise and ankle joint mobilization (15 minutes each, 30 minutes total, 3 × /week for 6 weeks). In the control group, only the whole- body vibration exercise was performed in the same manner and not the ankle joint mobilization. The gait and balancing abilities were measured before and after the 6-week training. RESULTS: Significant improvements were observed in the 10-m walk test, timed up-and-go (TUG) test, center of pressure (COP) path length, and COP path velocity in the experimental group (p < .05). The experimental group showed a larger decrease in the COP path length and velocity than the control group (COP path length, -10.27 mm vs. -3.67 mm, p < .05; COP path velocity, -.33 cm/sec vs. -.13 cm/sec, p < .05, respectively). CONCLUSION: The whole-body vibration exercise combined with ankle joint mobilization could be effective in improving the gait and balancing ability of stroke patients and could also be more effective for improving the static balance ability than the general whole-body vibration exercise alone.

        등뼈가동운동과 결합한 트레드밀 보행 훈련 프로그램이 뇌졸중 환자의 보행 기능 및 균형 능력에 미치는 효과: 무작위 대조 예비연구

        PURPOSE: This study sought to investigate the effects of treadmill gait training combined with a thoracic mobility exercise on gait and balance in patients with stroke. METHODS: In this single-blinded, randomized, controlled, comparative study, a total of 20 patients at a rehabilitation hospital who had suffered a hemiplegic stroke were randomly assigned to the experimental group (treadmill gait training combined with a thoracic mobility exercise, n = 11) or control group (treadmill gait training without the thoracic mobility exercise, n = 9). All the participants underwent comprehensive rehabilitation therapy (5 × /week for 4 weeks). Additionally, the experimental group underwent 20 min of treadmill gait training combined with 10 min of a thoracic mobility exercise (3 × / week for 4 weeks) and the control group underwent the former but not the latter. Gait and balance were measured before and after the 4-week training. RESULTS: Significant improvements were observed in the 10-m walking test (10 MWT), timed up-and-go (TUG) test, center of pressure (COP) velocity, and COP length in the experimental group (p < .05). This group also showed a larger decrease in the 10 MWT and COP velocity than the control group (10 MWT, -3.02 sec vs. -1.68 sec, p < .05; COP velocity, -.07 mm/sec vs. .08 mm/sec, p < .05). CONCLUSION: Treadmill gait training, combined with the thoracic mobility exercise, could be effective in improving the gait and balance of stroke patients. It could also be more effective in improving walking speed and static balance than the treadmill gait training alone.

        남성 심부전 환자의 넙다리 네갈래근 근력이 최대 유산소 능력과 재입원에 미치는 영향

        Purpose: The cardiopulmonary exercise test (CPET) is widely used to evaluate the clinical prognosis in patients with heart failure (HF). However, some patients with HF cannot perform CPET at maximal exercise intensity because of unstable medical condition or musculoskeletal problems. Thus, the study aimed to investigate the effects of quadriceps maximal voluntary isometric contraction (Q-MVIC) strength on aerobic capacity and re-hospitalization (RH). Methods: Patients with HF (n = 60, 59.03 ± 13.4 years, male 100%) were recruited in this study. We evaluated the Q-MVIC strength and aerobic capacity after discharge from hospitalization and followed up the RH rate during home-based self-care periods. Results: The RH (n = 15, 64.00 ± 14.69) group showed significantly lower Q-MVIC strength (p = .028) than the no RH(NRH, n = 45, 57.78 ± 13.15) group. In addition, the RH group showed significantly lower peak VO2 (p < .001) than the NRH group. Moreover, the Q-MVIC strength and peak VO2 demonstrated significant correlation (r = .414, p < .001). Conclusion: Our study showed that the knee extensor strength has strong predictive values on aerobic capacity and RH rate in patients with HF.
