Pituitary tumors occur in 15-50% of patients with multiple endocrine neoplasia of type 1 (MEN1). To the best of our knowledge, no MEN1 case in which the initial lesion was an invasive giant prolactinoma has been reported from Korea. We describe a patient in whom a skull-base tumor involved the sellar and parasellar spaces. A 49 year-old female presented with headache and diplopia. The tumor was ultimately identified as a giant prolactinoma; the serum prolactin concentration increased from 155.6 ng/mL to 3,234.3 ng/mL after cranial irradiation. She was evaluated in terms of incidental hypercalcemia and was found to have parathyroid hyperplasia. Genetic analysis revealed a missense mutation in the MEN1 gene (c.643G>A, p.Val215Met). Two years of treatment with a dopamine agonist reduced, but did not normalize, the serum prolactin concentration. We highlight the aggressive behavior of the giant skull-base tumor, and the diagnostic delay caused by a high-dose hook effect of the MEN1-related prolactinoma. (Korean J Med 2016;91:300-305)
다구찌 방법을 이용한 Inconel 718 소재의 선삭가공에서 표면거칠기 최적화

Inconel 718 alloy, widely used as material of aircraft engine, has a good mechanical property in high temperature, strong anti-oxidation characteristics in oxidated current over 900℃, and also is not easily digested in the air containing sulfur, therefore, its usage as mechanical component is expanding rapidly. Even though Inconel alloy 718 is difficult to machine, it requires highly precise processing/machining to sustain its component quality of high accuracy. In this paper, general turning operation conditions are tested to select the best cutting process condition by measuring surface roughness through implementing experiments with orthogonal array of cutting speed, feeding speed and cutting depth as processing parameters based on the Taguchi method. Optimal turning operation conditions are extracted from the proposed experimental models.

Recently the technology of capsule endoscopy has been developed dramatically and many researchers are making efforts to combine surgical function into the capsule endoscope. In this paper, the micro biopsy device which is a part of the capsule endoscope is proposed. The proposed device consists of a trigger with paraffin block, rotational tissue-cutting razor with a torsion spring, and controller. They are designed to operate sequentially so that the tissue sampling, sealing, and fixing are achieved in one operation. It becomes possible for the capsule endoscope to get sample tissue inside the small intestine which can not be reached by a conventional biopsy device. Experiments were performed with the small intestine of a cow and rabbit. In conclusion, the tissue samples of 1.0㎜ long were extracted successfully by the proposed biopsy device of the capsule endoscope.
