        전층각막이식후 초음파유화흡인술의 결과

        Purpose: To evaluate the factors that affect visual outcome and transplanted corneal endothelial cell density in eyes that have undergone cataract surgery after penetrating keratoplasty, and to compare surgical outcomes with those of a control group. Methods: We conducted a retrospective review of case records of 20 eyes that had undergone cataract surgery between January 2003 and April 2006 after previous penetrating keratoplasty (group I) and 20 eyes that had undergone cataract surgery alone with low cornea endothelial cell density (group II). Results: For both groups, the grade of nucleosclerosis affected postoperative best corrected visual acuity (BCVA) and the total phaco energy (phaco power×time) did not affect either BCVA or endothelial cell density. In group I, the factor that most affected change in corneal endothelial cell density after cataract surgery was the time interval between keratoplasty and phacoemulsification. Comparison of the surgical results showed an increase in BCVA for both groups, and the difference between the two groups was not statistically significant. However, group I showed a significantly greater decrease in corneal endothelial cell density than did group II. Conclusions: Our results suggest that the corneal endothelial cells of the eye having previously undergone penertrating keratoplasty are vulnerable to surgical insult occurring in procedures such as cataract surgery, and that the time interval between the two procedures can affect the change in endothelial cell density.

        눈안삽입콘택트렌즈 삽입술 후 각막내피세포의 변화

        Purpose: To evaluate the stability of corneal endothelial cells after ICL (implantable contact lens) implantation. Methods: Retrospective review of 50 eyes were subjected to implantation of ICL by one surgeon from December 2003 to April 2008. During the follow-up period, the number of eyes with regular follow-up decreased and remained at 18 eyes at the time of a three-year follow-up. Patients were examined preoperatively and at one, three, and six months and one, two, and three years postoperatively. The main outcome measures were the change in corneal endothelial cell density (CD), coefficient of variation (CV), and hexagonality (HA). This analysis was done in all eyes and especially in the 18 eyes with regular follow-up for three years and 11 eyes with changes in CV and HA corresponding to the normal endothelial wound healing process for three years. Results: Corneal endothelial CD, CV, and HA were not significantly changed compared with their preoperative values throughout the three-year follow-up period for all eyes. In the early postoperative period, there was a slight decrease in CD, but it increased after the second year of follow-up (range: -3.12%~+2.84%); there was a slight increase in CV, but it decreased after the six-month follow-up (-6.63%~+5.56%); and there was a slight decrease in HA, but it increased after the three-month follow-up (range: -0.88%~10.64%). Similarly, there were no significant changes in corneal endothelial cell density in the 18 eyes with regular follow-up or the 11 eyes with changes in CV and HA corresponding to normal endothelial wound healing process for three years. Conclusions: The results show stability of corneal endothelial cells after ICL implantation.

        단안의 시력감소과 비전형적인 시야장애를 보인 뇌하수체 종양 1예

        목적: 뇌하수체 종양의 임상양상이 단안의 시력과 시야장애를 보이는 경우는 드물며 본 증례와 같은 비전형적인 시야장애를 보이는 경우는 국내에 보고된 바가 없어서 저자들은 1예를 보고하고자 한다. 대상과 방법: 65세 남자가 약 2개월 전부터 진행되는 우안의 시력장애를 주소로 내원하였다. 시력은 우안의 교정시력이 0.32였고 좌안의 교정시력이 0.6이었다. 양안의 경도의 백내장이 관찰되었으나 안저검사 및 시신경에서는 특이한 이상이 없었고, 좌안은 전혀 시력이나 시야장애를 호소하지 않았다. 반복된 시력검사와 시야검사, 시유발전위검사에서 점점 악화되는 양상을 보여 우측 구후 시신경염이나 시신경의 종양을 의심하고 안와 전산화 단층촬영을 실시하였다. 우측 안구 및 시신경에는 이상소견이 없었으나 몇 개의 단층에서 뇌하수체의 비정상적인 음영을 관찰하여 뇌자기공명영상을 다시 하여 뇌하수체 종양으로 확진되었다. 환자는 신경외과의 절제 수술 후 경과관찰 중이다. 결과: 뇌하수체 종양은 다양한 임상양상과 시야변화를 보일 수 있으나 단안에 국한된 주증상을 보이는 경우는 매우 드물며 본 증례와 같은 시야결손을 보이는 경우는 안과 의사의 진단이 매우 중요할 것으로 사료된다. Purpose: We report a rare case of pituitary tumor showing decreased visual acuity and atypical visual field defect in one eye. Methods: A 65-year-old patient presented with decreasing visual acuity in right eye. There were mild cataratous change of lens in both eyes and no abnormal findings of retina and optic nerve in fundus examination except mild epiretinal membrane in left eye. And there showed an atypical constricted visual field and abnormal wave in visual evoked potential in right eye. Orbit CT was done to observe optic nerve lesion but there was no abnormality in optic nerve. But we found an abnormal enhancement in pituitary gland. Pituitary tumor was confirmed in brain MRI and the patient was undergone neurosurgical excision of tumor. Results: Pituitary tumor shows variable clinical signs and visual field defect. But signs confined to unilateral eye is rare and visual field defect such as our case is atypical. So ophthalmologists need more attention in diagnosis of patients with atypical visual field defect.

        심한 기계성 위눈꺼풀 처짐을 유발한 눈물샘 눈꺼풀엽의 거대 다형성 샘종 1예

        Purpose: To report a rare case of giant pleomorphic adenoma of the lacrimal gland involving the palpebral lobe causing severe mechanical ptosis and visual field defect, which was removed successfully. Case Summary: An 82-year-old man visited our clinic with a large protruding mass on his left upper lid that had grown for approximately 10 years causing severe ptosis. Preoperative orbital CT showed a 2.3 × 2.0 cm sized large mass which was suspicious of hemangioma. The mass was completely excised through a lid crease incision. The histopathologic examination showed typical findings of pleomorphic adenoma. After the surgery, preoperative mechanical ptosis was completely resolved and during the 6 months of follow-up, there was no abnormality of tear secretion and evidence of tumor recurrence. Conclusions: The authors experienced a rare case of giant pleomorphic adenoma of the lacrimal gland involving the palpebral lobe and resolved mechanical ptosis by complete removal of the tumor through a lid crease incision.         안와와 전신에 동시 다발성으로 발생한 재발성 포상연부육종 1예

        Purpose: To report a rare case of recurrent alveolar soft-part sarcoma (ASPS) with concurrent involvement of the orbit and multiple sites of the body that was removed successfully by surgery. Case summary: A 37-year-old woman presented with diplopia at the left lateral gaze and proptosis of the left eye. Two years earlier, the patient had a mass excision of the right gluteus maximus and the left orbit, and ASPS was histopathologically diagnosed at that time. In addition, the patient had been treated with chemotherapy and radiation therapy. On radiologic examination, recurrent tumor of the left orbit was found and surgically removed. The histopathologic examination showed that tumor cells were arranged in an alveolar pattern divided by fibrous septa and contained abundant granules in the cytoplasm, typically consistent with ASPS. Postoperatively, the symptoms of diplopia and proptosis improved. Conclusions: ASPS can occur and recur in the orbit as well as systemically. In addition, at the time of surgical removal of ASPS in the orbit, the surgeon should be particularly cautious of massive bleeding. J Korean Ophthalmol Soc 2010;51(10):1403-1408

        백내장 수술 전 마이봄선 질환이 수술 후의 건성안에 미치는 영향

        목적: 마이봄선 질환과 같은 술 전 요인이 백내장 수술 후 건조증에 영향을 주는지 알아보고자 한다. 대상과 방법: 백내장 수술을 받은 85명, 100안이 본 연구의 대상이었다. 술 전 마이봄선 질환(Meibomian gland disease, MGD)의 정도(MGD I, II, III군)에 따라 군을 나누었고, 술 전, 술 후 1, 2, 5, 9주에 눈물막 파괴시간, 쉬르머 검사, 각막 염색을 측정하고 안구표면 질환 지수를 통해 건성안 증상을 평가하였다. 각 MGD군에 따라 술 전과 후의 건성안 지표에 차이가 있는지 관찰하였다. 결과: 술 전 건성안 지표를 세 군에서 비교했을 때, 마이봄선 질환이 심할수록 안구표면질환 지수가 컸고 눈물막 파괴시간이 짧았다(p <0.05). 술 후 건성안 지표를 술 전과 비교했을 때 MGD II, III군에서는 술 전에 비해 술 후 모든 시점에서 안구표면질환 지수가 유의하게 증가하고 눈물막 파괴시간이 짧아졌다. 또한 각 군 간 비교에서, MGD III군이 MGD I, II군보다 모든 시점에서 안구표면질환 지수가 커졌고, 술 전, 술 후 1주와 술 후 9주에 MGD III군이 MGD I, II군보다 눈물막 파괴시간이 유의하게 짧아져 있었다(p<0.05). 결론: 술 전 마이봄선 질환이 심할수록 백내장 수술 후 건성안 악화 가능성이 크므로 저자들은 술 전 마이봄선 질환의 정도에 상응하는 건성안 예방 및 치료가 필요할 수 있음을 제시하는 바이다. Purpose: The aim of this study is to analyze the influence of preoperative meibomian gland disease (MGD) on the postoperative dry eye disease after cataract surgery. Methods: 100 eyes of 85 patients who had undergone cataract surgery were enrolled. Patients were stratified into three groups by the severity of meibomian gland disease (MGD Grade I, MGD Grade II and MGD Grade III). In each group, we recorded the indexes of dry eye such as Ocular surface disease index, tear breakup time, Schirmer’s test and Corneal staining at preoperatively and postoperative week 1, 2, 5, 9. We compared the indexes, preoperatively and postoperatively between three groups. Results: In comparison of the preoperative indexes between groups, the more severe meibomian gland disease the patients have, the higher Ocular surface disease index and the shorter tear breakup time were observed (p < 0.05). In comparison the preoperative with the postoperative indexes, Ocular surface disease index was higher and tear breakup time was shorter at every postoperative moment in MGD Grade (Gr) II and Gr III. In comparison between MGD groups, MGD Gr III showed higher ocular surface disease index than MGD Gr I and Gr II at every follow up point, and shorter tear breakup time than MGD Gr I and Gr II at preoperative and postoperative week 1 and 9 (p < 0.05). Conclusions : The severity of meibomian gland disease which exist preoperatively can affect the aggravation of dry eye disease after cataract surgery. Therefore, the treatment of meibomian gland disease before cataract surgery can be a therapeutic option to prevent aggravation of dry eye after cataract surgery.
