Background : 6.1% of red blood cells and whole blood issued to the operating room was not transfused to the patients and discarded in Seoul National University Hospital in 1994. Objectives : We planned to set up an effective management program of blood in the operating room and we investigated whether this program could reduce the disposal rate of blood. Methods : We made a guideline of blood management in the operating room through a workshop. The guideline was revised after a preliminary application. The revised guideline was applied for 5 months from May to September in 1996. The disposal rate was compared before and after the installation of the new program. Results : 5,336 units of blood were issued to the operating room for 5 months. Disposal rate of red blood cells and whole blood was markedly reduced from 6.2% in May to 2.1% in September(p<0.05). The average disposal rate was 3.7% during the five months. Conclusion : We were able to reduce the disposal of unused blood in the operating room through the development and the application of a new blood management program.
좁은 다중 동축 석영관 내부서의 예혼합 화염의 전파 특성에 대한 기초 실험

Flame stabilization characteristics of premixed flames in narrow annular coaxial tubes (NACT) were investigated experimentally. The NACT burner was proposed as a model of a cylindrical refractory burner, and it was made of quartz tubes. Flame stabilization conditions affected by the characteristic length of the burner was investigated with the variation of the equivalence ratio and the flow rates. Flame behaviors in narrow spaces could be directly observed. Conclusively, more wide flame stabilization conditions could be obtained at the case of the decreased channel scale. A flame instability, such as combustion noise was detected concerned with the flame oscillation observed at the surface of multi channel stage. Some flame propagation characteristics had complicated tendencies that may exist in practical porous-media combustors. Therefore, this NACT burner can be a basic configuration for the development of flame stabilization model in porous media combustor, and it will enhance our understanding about the behavior of flames in meso-scale combustion spaces.
환형 5중 동축관 연소기 내부에서의 예혼합 화염의 전파 특성 직접 관찰

Flame stabilization characteristics of premixed flames in an annular coaxial 5-tubes burner (AC5TB) were investigated experimentally. The AC5TB was made of five quartz tubes, and the flame stabilization conditions in that burner were investigated with the variation of equivalence ratio and the flow velocities. Flame behaviors inside of narrow annular tubes could be observed directly. Overall flame stabilization conditions were similar to that of the previous study, while the flame behaviors and structures were different mainly due to the controlled uniform distribution of the velocities in channels. Flame flashback conditions were thought to be governed by the competition between heat release rate, heat loss and heat recirculation in each channel. Stationary flames at a fixed location were compared in its velocity distribution and burned gas temperature across the channel. This AC5TB can be a basic configuration for the development of flame stabilization model of porous media combustors, and it will help understand about the real behavior of flames in meso-scale combustion spaces.

본 논문에서는 콤팩트 메탈헬라이드 램프용 가로등 기구의 반사관에 대해 2차원 설계법을 기반으로 하는 3차원 설계법을 제시하고 포토피아 20의 반사판 설계 툴을 사용하여 150W 콤팩트 메탈헬라이드 램프용 도로 조명 지구의 반사판을 설계하였다.

Koilonychia is dystrophy characterized by eversion of the marginal portion of the nail. As a result, the central portion of the nail is concave. It has been associated with systemic or skin-related conditions. Hereditary koilonychia is inherited as an autosomal dominant trait. Koilonychia can also be caused by exogenous factors such as certain toxic chemicals or repeated mechanical trauma. In this case, koilonychia on the fingernail of the left thumb was caused by repeated trauma resulting from continuous suction since birth. Herein, we report a rare case of koilonychia induced by habitual thumbsucking. (Korean J Dermatol 2011;49(10):915∼917)

Background: Fractional carbon dioxide laser (FCL) has been used to treat keloid and hypertrophic scars as monotherapy or combination therapy, including intralesional triamcinolone acetonide (ITAC). However, whether the combination of FCL and ITAC is really effective compared with other treatments, such as ITAC monotherapy in treating keloid and hypertrophic scars, remains unclear. Objective: To evaluate the effectiveness of FCL plus ITAC compared with ITAC monotherapy in treating keloid and hypertrophic scars. Methods: A computerized search was performed in different databases, including Cochrane, Embase, and PubMed. One randomized controlled trial and two controlled clinical trials were included. Statistical analyses of the extracted outcome data from the studies were then calculated using the Rex Software (version 3.0.1). Results: A total of 203 records were identified by searching databases, including Cochrane, Embase, and PubMed. The meta-analysis results, including three studies, demonstrated that although FCL combined with ITAC showed a slightly pronounced improvement in keloid and hypertrophic scars than ITAC monotherapy, there was no statistical difference (SMD: 0.26, 95% confidence interval [CI]: -0.10∼0.61, p=0.1541). Conclusion: FCL combined with ITAC may not be a cost-effective treatment for the treatment of keloid and hypertrophic scars due to similar effectiveness compared with ITAC monotherapy. However, this study had limitations, including insufficiency of published articles and data. Therefore, further investigations are needed. (Korean J Dermatol 2020;58(7):445∼452)
