Chronic osteomyelitis is often considered difficult to treat and may lead to refratory condition in oral and maxillofacial region. Clinical features of chronic osteomyelitis includes pain, swelling, pus discharge, and radiographic change. There are many kinds of treatment of chronic osteomyelitis. One of the most important factors in treating osteomyelitis are removal of infectious foci and administration of massive antibiotics. Recently we reviewed 29 patients with chronic osteomyelitis who visited in our department and treated successfully with our treatment protocols, consisting of surgical intervention and intravenous antibiotics for 2 weeks, and followed by oral antibiotics for 6 weeks. It is concluded that combination of surgical intervention and antibiotic therapy are sufficient to treat the chronic osteomyelitis in oral and maxillofacial region.

We described three cases of immediate reimplantation of a frozen-thawed autogenous mandible composed of a mixture of iliac bone, marrow, and particulate hydroxyapatite in tumors of the mandible. Acceptable outcomes were obtained in three patients who underwent immediate autogenous mandibular graft reconstruction. The conditions leading to successful outcome of the procedure are also discussed. Reimplantation of frozen autogenous lesioned mandible was performed in three patients with mandibular tumors. Two reimplanted grafts survived without complications following surgery. One case had postoperative infection that resolved with appropriate antibiotic treatment. There were no recurrences of the primary lesions. Satisfactory facial contour after surgery was achieved. These results are most promising, and we believe that, with further refinement, this technique will offer a new and acceptable modality for facial reconstruction in patients with cancer.

Obstructive sleep apnea syndrome (OSAS) is characterized repeated collapae of the upper airway, producing hypopnea, apnea, and ultimately, oxygen destaturation of hemoglobin. The major characteristics of OSAS are male predominance, obersity, hypersomnolence, and excessive snoring. Patients suffer deterioration of memory and judgement, irritability, morning headache, sexual dysfunction, and personality changes. Psychologic changes involving the cardipulmanary system occur and may lead to life-threatening events. The presurgical evaluation includes nocturnal polysomnography, clinical examination, radiologic evaluation, and fiberoptic endoscopy. The current surgical procedures used for OSAS are tracheostomy, tonsillectomy, adeniodectomy, nasal surgery (septoplasty, partial turbinectomy), tongue reduction, uvulopalatopharyngoplasty (UPPP), anterior sagittal osteotomy of the mandible with hyoid myotomy and suspension, and maxillary, mandibular and hyoid advancement. Selection of the surgical procedure is base upon the severity of the sleep apnea, presence of a skeletal dificiency, presence of morbid obesity, and anatomic site of the obsructive process.

The development of TMJ arthroscopy has had a major impact on the surgical management of patients with TMJ disorders. As TMJ arthroscopic technique and instrumentation has improved, many surgeons have reduced the number of open joint surgery they perform. Arthroscopy has provided surgeons a window into the TMJ that is technically rewarding and has contributed new information on the pathophysiologic characteristics and treatment of TMJ disorders. We report a patient who treated with TMJ arthroscope in TMJ adhesion.
