소세포폐암에서 CAV / PE 교대복합화학요법의 치료효과

N/A Objectives: The aim of this study was to evaluate the efficacy and toxicity of the 3 weekly alternating chemotherapy in previously untreated small cell lung carcinoma. Methods: Thirty-nine patients with SCLC treated with 6 alternating cycles of cyclophophamide/adriamycin/vincristinre(CAV) and cisplatin/ etoposide(PE). Thoracic irradiation and prophylactic cranial irradiation was administered 3 weeks after completion of chemotherapy to limited stage patients who attained a complete response(CR). Results: Thirty-one patients were evaluable. Overall response rate was 77,4% with 25.8% of CR. The response rate was 88.2%(29.4% CR) in patients with limited stage and 64.3%(21,4F% CR) in patients with extensive stage. and 82.4%(35.3% CR) in younger than 60-years and 71.4%(14,3% CR) in over 60-years. There were no significant difference in neither age or stage. The median survival duration was 18.5 months and the 1-year 3-year 5-year 7-year survival rate were 63.3% 14.1%, 10.6%, 10.6%, respectively. And the 3-year survival rate were significantly different according to response(57.1% in CR; 0% in PR, p<0.025), age(27.3% in age<60; O% in age>60, p<0.005), and stage (19.2% in LS; 7.7% in ES, p<0.005). Conclusion: This results suggest CAV/PE alternating chemotherapy is useful as a front-line treatment of small cell lung carcinoma, but is not superior to conventional treatment.

A case of primary non-Hodgkin's lymphoma of scalp is presented. The patient is 16-year-old male who has develped 10×15cm huse scalp tumor on left frontal head with both cervical lymph node enlagement. The CT presentation of slightly isodense soft tissue mass lesion without rim enhancement on left scalp was found. Magnetic resonance imaging of head showed that tumors invaded outer table of the skull focally. There were no other extra cutaneous manifestations. Biopsy of tumor revealed diffuse poorly differentiated lymphocytic type of malignant lymphoma. The patient was treated with six cycles of COPBLAM-V chemotherapy that resulted in complete remission. Four months after the completion of chemotherapy, the left preauricular lymph node was enlarged. Biopsy revealed same pathology with that of scalp. Again he obtained a complete response with combination chemotherapy.

저자들은 식도내에 편평상피암과 선암이 동시에 존재하는 원발성 중복암을 경험하고, 항암화학요법을 실시하여 부분적 관해를 얻은 매우 드문 예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. A 65 year old man with simultaneously occurring squamous cell carcinoma of mid-esophagus and adenocarcinoma of distal esophagus is reported. There was no continuity between the two masses. Bone scan revealed hot spots in right 3rd, 4th, 5th, and 6th ribs. Both esophageal lesions were responded to treatment with 5-fluorouracil and cisplatin. Fourteen months after the start of treatment, spinal metastasis was developed, but histologically not confirmed. Follow-up esophagoscopic findings at that time revealed 2㎝ sized ulcerative lesion in the distal esophagus only. In this report, we discussed the pathogenesis and treatment of double primary cancers.

두경부림프종에서 복합화학요법 및 국소방사선요법의 병용치료효과

Between 1983 and 1992, 18 patients with stage Ⅰ,Ⅱ intermediate grade head neck non-Hodgkin's lymphoma were treated with a combination chemotherapy and involved field radiotherapy. Mean age was 50.3 years and 7 patients were 60 years old. 12(66.7%) achieved a complete response(CR) and 5 (27.8%) had a partial response after chemotherapy, and additional 5 patients achieved a CR after radiotherapy. The 8-year total survival rate was 82.4% and 8-year relapse-free survival of CRs was 87.5% with a median follow-up of 48.9 months. The age, sex, stage, histology and extranodal involvement did not influence the outcome. Also total survival time was not related to age, performance score and various laboratory parameters.

Fluconazole 투여로 양호한 경과를 보인 경부 및 종격동림프절 효모균증 1예

저자들은 선행질환이 없는 건강하던 환자에서 경부 및 종격동림프절종대의 소견을 보여 악성림프종으로 의심했던 환자에서 조직검사를 통해 효모균증을 증명하고 triazole 계 항진균제인 fluconazole을 사용하여 양호한 경과를 보인 1예를 경험하였기에 문헌고찰과 함께 보고 하는 바이다. Cryptococcosis is a subacute or chronic mycotic infection caused by Cryptococcus neoformans. Authors experienced a case of cryptococceosis, resembling malignant lymphoma that restricted in cervical and mediastinal lymph node. A 44-year-old female patient was admitted with complaints of weight loss, fever, chill and palpable multiple neck mass. Chest PA and CT scan showed multiple discrete nodular masses in both cervical, hilar, supraclavicular and mediastinum. In section of cervical and mediastinal lymph node biopsy, Fontana-Masson and mucicarmine positive-cryptcoccus was found. And there was no evidence of malignant lymphoma. The patient was treated with oral fluconazole 400mg/day for 3 months and showed good response.

We report a patient with pernicious anemia who was diagnosed by positive anti-parietal cell antibody test and anti-intrinsic factor antibody test. 2 months after treatment with parenteral vitamin ??, hematologic findings were normalized, and neurologic symptoms were disappeared after 8 months of treatment.

A rare case of ileocolic intussusception in an adult male patient with multiple lymphomatous polyposis of the gastrointestinal tract is presented. The patient, a 57-year-old man, complained of a right upper abdominal pain, diarrhea and abdominal mass. Right hemicolectomy revealed ileocolic intussusception with multiple polypoid masses confined to the ileum. This indolent case of multiple lymphomatous polyposis contradict those of other reports which have shown a rapidly fatal prognosis.
