      • 고유수용성 신경근 촉진법이 편마비 환자의 균형에 미치는 영향 -율동적 안정화와 등장성 수축 결합기법을 중심으로-

        Purpose : The purpose of this study was to examine the effects of Rhythmic Stabilization(RS) and Combination of Isotonic(CI) of Proprioceptive Neuromuscular Facilitation on balance in patients with hemiplegia. Methods : Ten patients with hemiplegia were trained with RS and CI for six weeks. Two Standard Scale(TSS), Functional Reach Test(FRT), Time Up and Go test(TUG) and Berg Balance Scale(BBS) were used to prove their improvement of balance. Results : Body weight bearing decreased on less affected side and increased on more affected side. Distance of forward reaching for FRT increased. Time for TUG decreased. BBS score increased. They all showed significant difference. Conclusion : This study suggests that balance training with RS and CI of Proprioceptive Neuromuscular Facilitation improved balance in patients with hemiplegia.

        굽힘하중에 대한 퇴행성 추간판의 생체역학적 특성 분석

        Aging has been recognized as the primary cause of disc degeneration. A biomechanical characteristics of disc degeneration has been demonstrated that intradiscal pressure is reduced. With the increasing population of elderly people, disc degeneration and associated problems of nerve entrapment are becoming more prevalent. Presently, research on reduced intradiscal pressure associated with degeneration is insufficient. In this study. we used the Finite Element Method (FEM) of computerized simulations to investigate the effects of variation in intradiscal pressure on mechanical behaviours of L4-5 intervertebral disc degeneration. Degeneration was classified using four grades based on initial intradiscal pressure; Normal (135 kPa), mild(107 kPa), moderate (47 kPa) and severe (15 kPa). The predicted results f3r bending loads were as follows; 1 . Range of motion increased progressively with severity of degeneration with flexion and lateral bending moments, but decreased with extension moments. 2. Discal bulging of posterolateral aspect was larger in lateral bending and extension moment. But bulging was increased with severity of degeneration in lateral bending and torsion(same side).3. The rate of increasing intradiscal pressure was decreased in all bending motions with severity of degeneration. In conclusion, lateral bending and extension moment yield greatest bulging in severe degeneration. In torsion, although bending load produces disc bulging, disc bulging was associated more strongly with severity of degeneration than increasing torsional moments. Clinical Implications: Discal bulging may produce nerve root impingement and irritation. The effect of loading and posture on the varying degrees of disc degeneration has important implications especially in the elderly. In the presence of disc degeneration, avoidance of end range postures, especially extension and lateral bending may help reduce discal bulging and in turn, nerve entrapment.

      • 컴퓨터 작업 후 유지-이완운동과 능동신장이 근피로 회복에 미치는 영향

        Purpose : The purpose of this study was to identify that the effect of hold-relax and active stretching on recoveries of upper trapezius and sternocleidomastoid muscle fatigue after computer work. Methods : This study was conducted by 30 volunteer at the age of 23~33(resting group=10, hold-relax group=10, active stretching group=10). The muscle fatigue measured by tracing the median frequency(MDF) decrement from power spectrum analyses of EMG signals and the data were analyzed repeated ANOVA. Result : In case of upper trapezius, there was significant difference in group I,II and group II, III. In case of sternocleidomastoid, there was significant difference in group I,II and group I,III. Conclusion : The active stretching was effective for sternocleidomastoid and the hold-relax was effective for upper trapezius and sternocleidomastoid. The hold-relax was effective better than active stretching on recoveries of muscle fatigue.

      • 네발기기 자세에서 PNF 후방흔듬 운동 시 견갑골 안정근의 근활성도

        Purpose : The purpose of this study was to examine activities of the scapular stabilizers during push-up plus and PNF backward rocking exercises. Methods : 9 subjects performed randomly push-up plus and PNF backward rocking exercises in four point kneeling. The activities of 4 scapular stabilizing muscles were compared between push-up plus and PNF backward rocking exercises. Results : PNF backward rocking exercise showed significantly higher lower trapezius and lower serratus anterior activations compared to push-up plus exercise. Push-up plus exercise showed significantly higher middle serratus anterior activation than PNF backward rocking exercise. Conclusion : PNF backward rocking exercise facilitates the lower trapezius and serratus anterior activations compared to push-up plus exercise.

        네발기기 자세에서 실시하는 안정화 운동에 따른 체간 근육의 활성도

        Purpose: This study examined the activation of the rectus abdominis (RA), external abdominal oblique (EO), transversus abdominis, internal abdominal oblique (TrA/IO), and Multifidus (MF) muscles while stabilization exercise was performed in a four-point kneeling position. Methods: Experiments were conducted on 21 healthy male adults (mean age=25.29 years) who voluntarily agreed to participate in the study. Each subject was instructed regarding maximal voluntary contractions (MVC) and stabilization exercise in four-point kneeling. While MVC and stabilization exercise of individual muscles were being performed, activation of the muscles was measured using surface electromyography (EMG). Activation of the muscles while performing stabilization exercise in four-point kneeling was normalized to a percentages of the MVC. Results: Left RA, right TrA/IO, right and left MF muscles showed significant differences among the positions. Conclusion: Selecting an optimal position can aid subjects on physical conditions while performing stabilization exercises in the four-point kneeling position.

        뇌졸증으로 인한 편마비 보행의 시간적 공간적 요소 분석: 지팡이가 보행에 미치는 영향

        The purpose of this study was to assess of cane use on the hemiplegic gait of strock patients in temporal and spatial variables. Subjects were thirteen including 8 men and 5 women. They could walk independently without cane, To compare the effect of walking with and without a cane, temporal and spatial variables was measured using GAITRite. Cane walking demonstrated increased stance time on the affected side, and swing time, step length on the sound side were increased, and base of support was decreased. In conclusion, hemiplegic gait pattern of strock patients were improved with cane than those not using a cane.

        근수축 형태(유지-이완과 수축-이완 기법)에 따른 어깨 굽힘근과 폄근의 근활성도 비교

        Purpose: This study aimed to examine the activity of the shoulder flexor and extensor when hold-relax and contraction-relax techniques were applied with shoulder joint flexion. Methods: The subjects of this study were 15 healthy women. With the shoulder joint flexion at $0^{\circ}$ and $90^{\circ}$, hold-relax and contraction-relax techniques were applied for the same submaximal resistance to measure the activities of the deltoid muscle anterior fiber, deltoid muscle posterior fiber, pectoralis major fiber, and latissimus dorsi muscle with surface electromyography. An independent t-test was conducted in order to compare activities of each muscle according to the two techniques. Results: When the hold-relax and contraction-relax techniques were applied with the shoulder joint flexion at $0^{\circ}$, the activities of the shoulder flexor and extensor were not significantly different, but the activity of the flexor was higher when the contraction-relax technique was applied than when the hold-relax technique was applied. When the hold-relax and contraction-relax techniques were applied with the shoulder joint flexed at $90^{\circ}$, the activities of the shoulder flexor and extensor were not significantly different, but the activity of the extensor was relatively higher than when the flexor was at $0^{\circ}$ Conclusion: When the hold-relax and contraction-relax techniques were applied with the shoulder joint flexion at $0^{\circ}$, the activities of the shoulder flexor and extensor were not significantly different, but the activity of the flexor was higher when the contraction-relax technique was applied than when the hold-relax technique was applied. When the hold-relax and contraction-relax techniques were applied with the shoulder joint flexed at $90^{\circ}$, the activities of the shoulder flexor and extensor were not significantly different, but the activity of the extensor was relatively higher than when the flexor was at $0^{\circ}$.

      • 복부 할로잉 운동의 이해와 임상적 적용 : 문헌적 고찰

        Purpose : To provide the understanding of abdominal hollowing exercise, this study reviewed literatures related with TrA and AHE. Methods : We reviewed the prior studies related with TrA and AHE. Results : Crook lying is easier to facilitate isolated contraction of TrA from EO than the others. The contraction of the TrA is shown to be the highest muscle activity in prone lying. Additionally, wall support standing(or standing) is shown a higher contraction of entire abdominal muscle than the others. However, learning and teaching correct AHE have innate difficulties in four positions. Conclusion : We have to consider that Rehabilitative Ultrasonic Imaging(RUSI) can facilitate accurate AHE. In the country, physical therapists will be necessary more training and efforts to use ultrasound because very few use ultrasound in clinical field. It will be necessary to study the effects of RUSI feedback and examine effects of exercises in combination with AHE.
