Herpes simplex virus (HSV) is one of Herpesviridae family viruses which belong to DNA viruses. HSV-associated diseases are among the most widespread infections, affecting nearly 60% to 95% of human adults. Labial herpes typically results from infection with HSV type 1 (HSV-1), whereas most genital herpes is caused by HSV type 2 (HSV-2). They are incurable and persist during the lifetime of the host, often in latent form. Antiviral agents do not cure HSV infections, but rather modify the clinical course of the disease. Topical, oral, or intravenous antiviral agents may be used in the management of HSV infections. Acyclovir, valacyclovir hydrochloride, and famciclovir are the 3 antiviral drugs commonly used to treat symptomatic HSV infections. However, it is very difficult to choose an appropriate drug and dosing regimen.

Protothecosis is unusual phycological disorder caused by algae, named Prototheca. Cutaneous protothecosis has been reported worldwide including Korea. The patient was eighty-eight year old female who complained of skin lesions developed 15 days prior to the first visit. She has been living near the seashore and enjoyed bathing in the sea. Her skin lesions were erythematous, purulent plaque and patch, ulceration on right forearm. Skin biopsy revealed the possibility of cutaneous protothecosis, and this finding was confirmed by histological, mycological and electron microscopical studies. The causative organism was identified as Prototheca wickerhamii. Surgical debridement with KMnO4 wound irrigation combined with intravenous fluconazole 400 mg/day for 3 weeks resulted in remarkable improvement of skin lesions. We report a case of cutaneous protothecosis which showed severe clinical feature and was successfully treated with combination of systemic antifungal agent and surgical debridement.
