체강 삼출액의 진단에 있어서 $PLC-{\gamma}1$ 면역 염색의 유용성

Cytologic diagnosis of reactive or malignant effusion is sometimes difficult. Especially, differentiation of benign reactive mesothelial cells from malignant cells in body effusion is more difficult. Recently, immunohistochemistry has been used to diagnose difficult cases. Phospholipase $C(PLC)-{\gamma}1$ is one of the isoenzyme of the PLC which plays central role in signal transduction involving cellular growth, differentiation and transformation by phosphorylating many protein component. Increased expression of $PLC-{\gamma}1$ in human breast carcinoma, colorectal carcinoma and stomach cancers are reported. To evaluate the efficacy of positive $PLC-{\gamma}1$ immunostaining in the diagnosis of malignancy in effusions, paraffin-embedded cell blocks of pleural fluid and ascites from 10 patients(5 metastatic adenocarcinomas, and 5 reactive mesothelial cells) were immunostained with a monoclonal antibody to $PLC-{\gamma}1$. $PLC-{\gamma}1$ immuostained all the adenocarcinomas in cell block(5/5) with intense membrane pattern, however, none of the reactive mesothelial proliferations stained with the diagnostic membrane pattern. Thus, our study strongly supports the conclusion that $PLC-{\gamma}1$ immunopositivity is likely to become a useful adjunct for the diagnosis of malignancy in effusions.

Papillary renal cell carcinoma (RCC) is an uncommon subtype of RCC that has distinctive gross, histologic, and cytogenetic features. The cytologic features of FNA are abundant papillary clusters and relatively few single cells. The cells are usually small and contain uniform nuclei; numerous macrophages with foamy cytoplasm are often found in the background. We describe a case of papillary renal cell carcinoma evaluated by fine needle aspiration cytology(FNAC) in a 42 year-old man. The smear showed a few papillary clusters and numerous macrophages with foamy cytoplasm in the background. With adequate cellularity, papillary RCC can be distinguished reliably from non-papillary RCC by FNAC.

The histological and immunohistoehernical features of hepatocellular differentiation have been described in scene adenocarcinornas of the stomach. The term hepatoid adenoearcinoma has been proposed fot this rare variety of tumors. A 69-year-old female patient was admitted to the hospital with melena, dizmess and general wealcoess, Gastmfibroscopy revealed gastric tumor oc eupying the lesser curvature of the body and fundus. Serum AFP level was markdly increased. But, there was no evidence of liver metastasis on the abdominal CT and liver scan. Near total gastrect omy was done and gross examinatian of the gastrectomy specirnen revealed an ulcerated tumor, 5.0cm in maximum diameter, located in the lesser curvature of the body and fundus. 알코올 섭취가 사염화탄소 간 중독시 나타나는 IL-6 발현에 미치는 영향

The pathogenesis of liver damage in alcoholic liver disease is not well understood, although there is evidence implicating immune mechanism. Among many aspects of the immune mechanism, the involvement such as interleukin-6 was suggest in alcoholic liver disease. The authors studied in order to investigate whether IL-6 play a role in the pathogenesis of alcoholic liver disease. Rabbits were fed with ethanol mixed diet with subcutaneous injection of 1mg/kg of CCl4 (ethanol CCl4 group) or with same injection of normal saline (ethanol group) during the last 6 weeks of the whole 12 weeks periods of ethanol diet. The other groups were given ethanol free control diet with subcutaneous injection of CCl4 (CCl4 only group) or normal saline (control group) by the same method as the former two group. The result were as follows: 1) The serum level of AST and ALT, ethanol only group an CCl4 only group tended to be elevated than control group. Especially those of ethanol- CCl4 group was significantly higher than any other three groups. 2) The serum level of IL-6 activity in ethanol only group and CCl4 only group tended to be elevated than control group. Especially that of ethanol-CCl4 group was significantly higher than any other three groups. 3) Commbining all four groups, there was significant correlation between the serum IL-6 and AST or ALT levels. It can be concluded that IL-6 may reflect hepatic injury and play a role in liver disease, especially by CCl4.
