Intertrochanteric fracture of the femur, of which reported mortality ranges from l5% to 20%, usually occurs in the elderly and is usually comminuted and unstable. Many operative techniques have been developed for early exercise and walking ambulation to reduce mortality and mod$gt;idity, but they still have disadvantages. Few papers has been reported on hemiarthroplasty for intertrochanteric fracture in elderly patient, but they didn't preserve the calcar femorale which is very impartant anatomic portion for weight transmission after hemiarthroplasty. Therefore, the purpose of this paper is to analyze clinical outcome of hemiarthroplasty with preservation of calcar femorale and wiring of greater trochanter for intertrochanteric fracture. From March 1993 through March l995, 28 elderly patients had undergone hemiarthroplasty with preservation of calcar femorale and wiring of greater trochanter for the treatment of severely comminuted and osteoporotic intertrochanteric fractures. Mean follow-up period was 17.5 months ranging from l2 months to 28 months. The results obtained were as follows: 1. The most common type by Boyd-Griffin classification was II (73%). 2. Twenty five cases (83%) had osteoporosis, which was evaluated by Singh index, 3. The average operating time was 42.8 minutes and average hospitalization period was 20.6 days. 4. Excellent functional results according to hip rating scale of Merie D'Aubigne were obtained in 24 cases (80%). 5. Greater trochanter fixed with wire was united in average 4.4 months postoperatively. 6. One case (3%) developed loosening of implant at postoperative 13 months.
해부학적 지지 금속판 고정술의 이용한 경골과 골절의 치료

Purpose: We analyzed the anatomical results of operative treatment in acetabular fractures according to fracture pattern and surgeon's experience. Material and methods: From December, 1993 to December, 1999, 100 cases of acetabular fractures operated by single surgeon were analyzed. Fracture configuration was classified by Letournel's method. The anatomical results were classified by Matt s criteria and Claude Martimeau score. Results: There were 36 elementary fractures(anterior column type: 4 cases, posterior wall type: 29 cases, transverse type: 3 cases) and 64 complex fractures(posterior wall and posterior column type: 4 cases, anterior and posterior hemitransverse type: 8 cases, transverse and posterior wall type: 16 cases, T type: 6 cases, both column type: 30 cases). According to Matta's criteria, all elementary fractures were seen anatomic or satisfactory results however, eleven cases of combined fracture were seen unsatisfactory results. Mean Claude Martimbeau's score was 6.1 point. Elementary fracture was 7.5 point and combined fracture was 5.4 point. Both column fracture was 4.4 point and it was the lowest point among the fractures. In both column fracture, the improvement of surgeon's skill made more good anatomical results. There were four cases of infection and 2 cases of nerve injury. Conclusion: In operative treatment of acetabular fractures, complexity of fracture demands more appropriate surgical approach and skillful surgeon,
