        정상인의 자세와 시각 교란에 따른 정적 균형능력의 변화

        Purpose: This study examined the difference in the static balance ability according to the visual cues and postural tasks in normal subjects. Methods: Thirty participants (12 male, 18 female; mean age $24.63\pm1.43$ years) stood barefoot on a force platform in a one-legged stance, tandem Romberg stance and tandem Romberg with neck extension stance with a visual cue open and closes. The static balance was assessed by the center of pressure (CoP), surface electromyography root mean square (RMS) of the leg muscles according to the stance position. Results: In the CoP tests, the difference in the unit path length and circumference area was affected by the visual cue according to the stance posture (p<0.01). In the RMS tests, the difference in the tibialis anterior and medial gastrocnemius muscle was affected by visual cue in accordance with the stance posture (p<0.01). Conclusion: The visual cue and postural task affect the balance ability in normal subjects. Therefore, this study provides clinical evidence that the balance and postural control can be improved. Therapeutic intervention, such as an obstacle course, and a lower leg muscle performance program with a change in the base of support can affect the balance and postural control.

      • 수온이 뇌성마비 환자의 상지 경련근의 표재 온도와 근 활동전위에 미치는 영향

        The purpose of this study was to describe the alteration of muscle action potential of spastic upper extremity in a patient with cerebral palsy by the water temperature. We used seven patients with cerebral palsy. Participants classified according to each group in $29^{\circ}C,\;35^{\circ}C$ by the water temperature. All subjects participated 2 session, which at least 1 week between session. The test was measured continuously pre test, post-test by thermometer and surface EMG. The following results were obtained; 1. In changes of surface temperature, wrist flexor and extensor were significantly decreased in $29^{\circ}C$ (p<0.001) group but were significantly increased $35^{\circ}C$ group(p<0.01). 2. In changes of surface temperature, wrist flexor and extensor were significantly differenced between $29^{\circ}C$ and $35^{\circ}C$ group(p<0.001). 3. In changes of muscle action potential, wrist extension antagonist were significantly increased in $29^{\circ}C$ group(p<0.05). 4. In changes of muscle action potential, wrist flexion agonist were more significantly increased in $29^{\circ}C$ group(p<0.01). 5. In changes of muscle action potential, wrist extension antagonist were significantly differenced between $29^{\circ}C$ and $35^{\circ}C$ group(p<0.05). These results lead us to the conclusion that changes of muscle action potential of spastic upper extremity in a patient with cerebral palsy were influenced by the water temperature. Therefore, a further direction of this study will be to provide more evidence that a moderate water temperature have an effect on muscle tone in a patient with cerebral palsy.

      • 뇌졸중환자 내측 비복근의 초음파 영상 분석

        Purpose: The purpose of this study were to analyze ultrasonography on both sides of gastrocnemius medialis of stroke patient. Methods: In fourteen subjects with stroke, the gastrocnemius medialis(GM) muscle were scanned with ultrasonography at the muscle belly at rest at ankle angles of 0 degree (neutral position) and it were measured with ultrasonography on both sides of the leg. Muscle architecture (thickness, pennation angle, fascicle length, density, white area index) was examined using real-time B-mode ultrasonography with 7.5MHz linear-array prob. Results: Thickness of non-paratic side was significantly thicker than paratic side. Pennation angle of non-paratic side was significantly larger than paratic side. Fascicle length of non-para tic side was significantly smaller than paratic side. Density of non-paratic side was significantly smaller than paratic side. White area index of non-paranc side was significantly smaller than paratic side. Conclusion: This study showed that stroke have influenced on mechanical properties and quality properties of skeletal muscle architecture.

        The Halliwick concept was developed by James McMillan over 50 years ago, and began as a method for teaching swimming to children with physical disables. After that the Halliwcik concept was developed as a specific strategy(called the Ten-Point Program) for teaching swimming to children with disabilities. Soon after the changes in the physical and emotional behavior were attributed to a unique teaching/learning philosophy and a psycho-sensory-motor learning program that is enhanced by hydrodynamics. Recognizing the therapeutic effects of the Ten-Pont Program adapted the program as a therapeutic intervention called Water Specific Therapy or the Logic Approach to Therapy in Water.

        외이전기자극이 슬관절질환 노인의 통증과 보행 및 균형에 미치는 영향

        Purpose: This study examined the application of electric stimulation to the auricle acupoint (frequency 2 Hz, stimulation level: noxious) with 30 elderly people over 65 years that received treatment in a rural hospital to determine the influence of pain, gait and balance in the aged with knee joint disease. Methods: The subjects were divided into three groups; the degenerative joint diseases (DJD) group (1 male, 9 females), the total knee replacement (TKR) group (1 male, 9 females) and the control group (1 male, 9 females). Auricualr electrical stimulation (AES) was applied with low frequency, high intensity transcutaneous electrical nerve stimulation for 10 seconds per each point. Results: 1. For the change of pain according to AES, there was interaction in the resting period (p<0.001) and gait (p<0.001) and pain of the DJD group and TKR group was decreased. 2. The range of motion (ROM) of the knee joint showed a significant difference in interaction for each group of elderly people (p<0.001) and the ROM for the DJD group and TKR group of elderly people was increased. 3. In the analysis of gait speed changes, there was a significant difference in interaction for each group of elderly people (p<0.001) and for gait speed in the DJD group and TKR group of elderly people. 4. It was found in the change of static balance that there was a significant difference in interaction for each group of elderly people (p<0.01) and balance capacity in the DJD group and TKR group of elderly people was increased. Conclusion: The aged with knee joint disease have pain, and a decreased function of gait and balance. AES was an excellent treatment for control of pain, and an excellent treatment to enhanced joint functions. AES was useful for improving gait and balance due to decreased pain.

        노인의 균형 및 보행과 족관절 근력과의 상관관계

        Purpose: This study was designed to investigate the correlations between muscle strength of the ankle and balance, walking in the elderly. Methods: Thirty-nine subjects were selected from a population of female volunteers. Measurement of balance ability included evaluation of timed "up and go", functional reach, and a one leg standing test. Measurement of walking analysis included evaluation of cadence, stride length, step length, and walking speed. Maximal voluntary isometric contraction (MVIC) of the ankle muscle strength was measured by use of a dynamometer. Results: For balance, there were significant negative correlations between timed "up and go" and the MVIC of the ankle dorsiflexor. There were significant positive correlations between one leg standing with the eyes closed and the MVIC of the ankle dorsiflexor. For walking, there were significant positive correlations between cadence, walking speed and the MVIC of the ankle dorsiflexor. Conclusion: This study showed that there were close relationships between muscle strength of the ankle dorsiflexor and walking and balance in the elderly.

        탄력밴드저항운동이 노인의 균형 능력 향상에 미치는 영향

        Purpose: This study examines the effects of elastic band of resistance exercise for balance control of the elderly. Methods: Thirty of eighty participants in experiment subjects who demonstrated balance-impairment through the use of primary screening tests including the one leg standing test (OLST), functional reach test (FRT) and timed up and go (TUG) were selected as subjects. Fifteen subjects that underwent muscle-strengthening exercise using an elastic band were selected as the exercise group and fifteen subjects were selected as a control group. Subjects undertook a home-based exercise program three times per week for 9 weeks. Muscle strength, functional assessment and a balance test were quantitatively measured before and after the exercise regimen. Results: After muscle strengthening exercises, changes in maximal voluntary isometric contraction (MVIC) showed a significant increase in all of the lower extremity muscles of the exercise group subjects. There were statistically significant differences between the exercise and control groups for changes in the OLST, FRT and TUG, which are functional assessments of balance ability, and changes of the unit path length and circumference area, measurement items of quantitative analysis. In addition, from examining correlations between MVIC, balance ability, it was found that an increase of muscle strength in the hip joint group of muscles among the lower extremity muscles had greater improvement in correlation with balance ability in this elderly population. Conclusion: Resistance exercise using elastic bands had significant effects on muscle strengthening in elderly subjects, with a resultant increase of lower extremity muscle strength with significant improvement of balance ability.

      • 편마비 환자의 보행훈련 시 기능적 전기자극 병용이 척수신경원의 흥분성 변화에 미치는 효과

        Purpose: The purpose of this study were to analysis the effect on change of spinal neuron excitability during gait training of hemiplegia patients by the functional electrical stimulation. Methods: Thirty six hemiplegia patients participated in this study. Stimulation conditions of FES were pulse rate 35pps, pulse width $250{\mu}s$, and on-time 0.3 second, treatment hour was 30 min. and treatment period was once a day for five days a week through six weeks. For functional evaluations before and after treatment, Modified Ashworth Scale (MAS), active range of motion (AROM), Hmax threshold, H/Mmax ratio were measured and the following conclusions were obtained. Results: Functional evaluation showed significant changes in experimental group as MAS(p<0.01), AROM(p<0.001), compared to control group. In spinal neuron excitability evaluation, change of Hmax threshold was significantly reduced in both non weight bearing (p<0.001) and bearing condition (p<0.05), H/Mmax ratio was significantly reduced in non weight bearing (p<0.05) and bearing condition (p<0.05). Conclusion: In conclusion, application of FES to hemiplegia patients in recovery stage during gait training improved mitigation of muscular spasticity, balance adjustment and moving ability and it was interpreted that it was caused by mitigation of muscular spasticity by the spinal neuron excitability.

      • 염증유발 백서에서 전침자극과 미세전류자극의 항-통각과민 효과

        Purpose: The purpose of this study were to examine the anti-hyperalgesic effects of combination of electroacupuncture and microcurrent on anti-hyperalgesia in local hyperalgesia zone. Methods: It used 24 rats for experiment, divided them into control group, electroacupuncture group (EA group), microcurrent group (MC group), combination of electroacupuncture with microcurrent (EA+MC group), caused hyperalgesia by injecting ${\lambda}-carrageenan$ into hindpaw. Thickness of hindpaw, mechanical pain threshold (MPT), thermal pain threshold (TPT), noxious flexion withdrawal reflex (NFR) and somatosensory evoked potential (SEP) were measured immediately after induction, at 24 hours, 48 hours and 72 hours after induction. The electrical stimulation was given once a day for three days, 20min per session. Results: Change of thickness, MPT, and TPT showed significant difference in all groups compared to control group. In particular, there were remarkable difference in EA+ME group. In particular there were remarkable differences in EA group and EA+MC group. Change of NFR(% threshold, % reaction time, % RMS) and SEP showed mainly significant differences in EA group and EA+ME group compared to control group. In particular, there were remarkable difference in EA+ME group. Conclusion: The above results suggest that appropriate combination of microcurrent with electroacupuncture for pain control will be very desirable.

      • Correlation between MVIC and Muscle Architecture in the Extensor Carpi Radialis Longus Muscle during Maximum Voluntary Isometric Contraction

        Purpose: This study examined the correlation between the maximum voluntary isometric contraction (MVIC) and the muscle architecture in the extensor carpi radialis longus muscle during MVIC. Methods: The muscle area, volume were measured using a ultrasound imaging system to obtain the muscle architecture during the MVIC. For the mechanical muscle strength measurements, the MVIC was obtained using a dynamometer. Results: There was a significant correlation between the MVIC and the muscle area (r=0.498, p<0.01) and muscle volume (r=0.602, p<0.001). There was a significant correlation between the MVIC and density (r=-0.429, p<0.05). The area showed significant correlations with the muscle volume (r=0.699, p<0.001) and density (r=-0.429, p<0.05). In addition, there was a correlation between the volume and muscle density (r=0.555, p<0.01). Conclusion: There is close relationship between the MVIC and the muscle architecture in the extensor carpi radialis longus muscle during the MVIC.
