      • 편마비 환자의 보행훈련 시 기능적 전기자극 병용이 척수신경원의 흥분성 변화에 미치는 효과

        Purpose: The purpose of this study were to analysis the effect on change of spinal neuron excitability during gait training of hemiplegia patients by the functional electrical stimulation. Methods: Thirty six hemiplegia patients participated in this study. Stimulation conditions of FES were pulse rate 35pps, pulse width $250{\mu}s$, and on-time 0.3 second, treatment hour was 30 min. and treatment period was once a day for five days a week through six weeks. For functional evaluations before and after treatment, Modified Ashworth Scale (MAS), active range of motion (AROM), Hmax threshold, H/Mmax ratio were measured and the following conclusions were obtained. Results: Functional evaluation showed significant changes in experimental group as MAS(p<0.01), AROM(p<0.001), compared to control group. In spinal neuron excitability evaluation, change of Hmax threshold was significantly reduced in both non weight bearing (p<0.001) and bearing condition (p<0.05), H/Mmax ratio was significantly reduced in non weight bearing (p<0.05) and bearing condition (p<0.05). Conclusion: In conclusion, application of FES to hemiplegia patients in recovery stage during gait training improved mitigation of muscular spasticity, balance adjustment and moving ability and it was interpreted that it was caused by mitigation of muscular spasticity by the spinal neuron excitability.

        노르딕 워킹이 앞정강근 피로도에 미치는 영향

        Purpose: The purpose of this study was to investigate tibialis anterior muscle fatigue through a Nordic walking when using pole. Methods: This study subject was 38 people who were twenties and randomly assigned to a control group with standard walking and Nordic walking group with Nordic walking. All subjects examined muscle fatigue with surface EMG and visual analogue scale for fatigue. The data were analyzed with SPSS window 18.0 program using ANCOVA. Results: The results were of a significant changes to the both tibialis anterior fatigue(p<0.01). The visual fatigue scales weren't significant difference. Conclusion: It was found that Dool-re-gil (Mt. Ji-Ri) was effective for muscle fatigue during walking, and it appears that it could be used clinically. So it is anticipated that improvement in clinical utilization for the people who's through a Nordic walking when using pole.

        융복합적 수중운동이 유방암 환자의 어깨 기능, 폐기능과 삶의 질에 미치는 영향

        본 연구는 유방암 환자에게 수중 운동과 지상운동을 적용하여 비교 분석하고 암환자의 재활에 수중 운동의 효과를 규명하고자 시행하였다. 연구대상자는 유방암 환자 40명을 모집하여 수중 운동을 적용한 실험군 20명, 지상 운동을 적용한 대조군 20명으로 설정하였고, 수중운동과 지상운동을 각각 주 5회 8주간 중재한 후 어깨기능과 폐기능 삶의 질을 비교 분석하였다. 그 결과 그룹 간 어깨 기능장애는 실험군에서 대조군에 비해 더 큰 변화를 보여 통계적으로 유의한 차이를 보였고(p<.001), 노력성 폐활량과 1초간 노력성 호기량도 실험군에서 대조군에 비해 더 큰 변화를 보여 통계적으로 유의한 차이를 보였다(p<.05)(p<.001). 삶의 질의 변화 비교에서도 실험군이 대조군에 비해 더 큰 변화를 보여 통계적으로 유의한 차이를 보였다(p<.001). 본 연구 결과가 유방암 환자의 신체적 건강과 심리적, 사회적 건강을 회복시키고 관리함에 있어 효율적인 방법을 제시할 것으로 생각되며, 향후 연구를 발전시켜 폐암, 대장암 등의 환자들을 대상으로 한 연구와 다양한 수중운동 방법들의 적용으로 연구를 발전시켜 국내 유방 암 재활 치료의 기초자료를 제시하고자 한다. This study applied aquatic and land-based exercises to breast cancer patients for comparative analysis and elucidation of their effects on the rehabilitation of cancer patients. Participants included 40 breast cancer patients who were divided into two groups: 20 participants in the experimental group using aquatic exercise and 20 participants in the control group using land-based exercise. After the addition of aquatic or land-based exercises five times per week for eight weeks, differences in each group's shoulder and pulmonary functions as well as their quality of life were compared and analyzed. The results showed a statistically significant difference (p<.001) for shoulder dysfunction, with the experimental group showing a greater improvement than the comparison group. Statistically significant differences (p<.05 and p<.001, respectively) were also observed for forced vital capacity (FVC) and forced expiratory volume in one second (FEV1), with the experimental group showing greater improvements. Finally, a statistically significant difference (p<.001) was observed for change in the quality of life, with greater improvements in the experimental group than in the control group. The results of further research in this field are expected to indicate the most efficient method for the recovery and management of physical, psychological, and social health impacts of breast cancer on patients. Furthermore, the research aims to provide fundamental resources for breast cancer rehabilitation in South Korea by conducting studies on patients with lung cancer or colorectal cancer and the application of various aquatic exercise methods.

        10주간의 고교 남자 핸드볼 선수 신체 안정화 프로그램 후 시각 반응 속도, 기능적 움직임, 신체 균형, 폐활량 변화

        무안 Y고등학교 남자 핸드볼 선수 21명을 대상으로 10주간 신체안정화 훈련 후 시각반응속도, 기능적 움직임, 균형, 폐활량의 결과를 비교 분석하여 부상을 감소시킴으로써, 경기력 향상을 시킬 수 있는 추후 관리 및 훈련 자료를 제공할 목적으로 연구하였다. 훈련 결과 핸드볼선수에게 요구되는 순발력, 민첩성이 크게 증가하였고, 반응시간이 유의하게 단축되었다 (p <0.001). 기능적 움직임 평가 총점은 훈련 전 10.43±1.16, 훈련 후 12.05±1.07로 유의하게 증가하였다 (p <0.001). 와이발런스 측정은 훈련 전 89.18±6.99, 훈련 후 91.39±7.33으로 유의하게 증가하였다 (p <0.001). 폐활량은 유의하게 증가하였다(p <0.001). 결론적으로, 10주간 신체안정화 훈련 프로그램을 적용하면 속도, 민첩성, 기능적 움직임, 와이발런스 측정, 폐활량의 향상에 효과적이다. 결론적으로, 부상을 예방하거나 줄일수 있고 핸드볼 경기 중 경기력을 향상시킬 수 있을 것이다. This study aims to see, a 10-weeks program of Body Stability Exercise(BSE) was conducted on 21 male handball players at Muan Y High School and then, the results of changes in Visual Response Speed Test, Functional Movement Screen, Y-Balance Test, and Vital Capacity were observed, compared and analyzed to provide follow-up management and training data that can be used to reduce injuries and improve performance. After training, strength and quickness were increased significantly and reaction time was significantly shortened (p <0.001). The total score of Functional Movement Screen was increased from 10.43±1.16, before training to 12.05±1.07 after training, which was statistically significant(p<0.001). Y-Balance Test was significantly increased from 89.18±6.99 before training to 91.39±7.33 after training(p <0.001). The Vital Capacity increase was also shown to be more than statistically significant(p<0.001). The 10-week BSE program improved speed, agility, Functional Movement Screen, Y-Balance Test, and Vital Capacity. In conclusion, it`s can prevent or reduce injuries and performance could be improved while playing handball.

        10주간 초등학교 축구선수의 신체안정화 프로그램을 통한 기능적 움직임 평가, 신체균형, 폐활량 효과

        본 연구의 목적은 초등학교 축구 선수들의 신체 안정화 운동 프로그램 후 부상예방과 경기력 향상 여부를 측정하기 위하여 실시하였다. 목포 Y 초등학교 축구 선수 23명을 대상으로 10주 동안 신체 안정화 운동 프로그램을 통해 경기 중 필요한 능력 향상을 위해 시각 반응속도 측정, 기능적 움직임 평가, 신체 균형 능력 평가(와이발런스), 폐활량의 결과를 비교 분석하여 부상예방을 통해, 경기력 향상을 시킬 수 있는 추후 관리 및 운동 자료를 제공하고자 실시하였다. 시각반응속도 평가 결과 힘과 민첩성이 유의하게 증가하였고, 반응시간이 유의하게 단축되었다(p<.001). 기능적 움직임 평가 프로그램 총점은 운동 전 10.83±0.12, 운동 후 12.09±0.90으로 유의하게 증가했다(p<.001). 신체 균형 능력 평가 프로그램은 운동 전 85.87±5.75, 운동 후 87.78±6.03으로 유의하게 증가했다(p<.001). 폐활량 측정 종목 중 노력성 폐활량은 운동 전 1.53±0.23 운동 후 1.76±0.19, 1초량은 운동 전 1.74±0.39 운동 후 1.93±0.24로 운동 후에 유의하게 증가하였다(p<.001). 10주간 신체 안정화 운동 프로그램을 통해 속도, 민첩성, 기능적 움직임, 신체 균형(와이발런스), 폐활량이 향상되었다. 결론적으로, 축구를 수행하면서 부상을 예방하거나 감소시킬 수 있고, 경기력을 향상 시킬 수 있을 것이다. This study aimed to see how the Body Stability Exercise (BSE) program can prevent injuries and improve performance for elementary school soccer players. A 10-week BSE program was conducted on 23 soccer players at Mokpo Y Elementary Soccer School. The results of changes in Visual Response Speed Test (VRST), Functional Movement Screen (FMS), Body Balance Ability Evaluation (Y-Balance Test, YBT), and Vital Capacity (VC) were observed, compared, and analyzed to provide follow-up management and training data that can be used to reduce injuries and improve performance. After VRST training, strength and agility increased significantly and reaction time significantly shortened (p<.001). The total score of FMS evaluation increased from 10.83±0.12 before training to 12.09±0.90 after training, which was statistically significant (p<.001). YBT significantly increased from 85.87±5.75 before training to 87.78±6.03 after training (p<.001). Among the VC tests, FVC significantly increased from 1.53±0.23 before training to 1.76±0.19 after training, and FEV1.0 increased from 1.74±0.39 before training to 1.93±0.24 after training(p<.001). After the 10-week BSE, improved speed, agility, FMS, Y-Balance, and VC were observed. In conclusion, the BSE program can prevent or reduce injuries and improve performance while playing soccer.

        10주간의 신체 안정화 운동프로그램이 초등학교 태권도 선수의 민첩성, 기능적 움직임, 균형, 폐기능에 미치는 효과

        Purpose : The purpose of this study is to evaluate whether physical ability is improved by body stabilization, develop training methods to improve physical fitness factors suitable for body conditions, and provide future management and exercise materials to improve Taekwondo athletes' performance, after conducting the Body stabilization exercise (BSE) program on 22 elementary school Taekwondo athletes for 10 weeks to strengthen core muscles and evaluating Visual response speed test (VRST), Functional movement screen (FMS), Y-Balance test (YBT) and Vital capacity (VC). Methods : In order to examine the impact of the BSE program on Taekwondo athletes’ VRST, FMS, YBT and VC this study conducted individual evaluations on all subjects, including VRST, FMS, YBT and VC before applying the programs, and compared exercise effects before and after the program, before and after exercise by a certain period. Results : After exercising, strength and quickness were improved significantly and reaction time was significantly shortened (p<.001). The total score of FMS was improved from 10.86±1.17 before to 11.68±0.99 after exercising, was statistically significant (p=.014). YBT was significantly improved from 91.11±4.84 before to 95.68±5.15 after exercising (p<.001). VC improved was statistically significant (p<.001). Conclusion : In conclusion, the BSE athletic program is expected to help Taekwondo athletes perform well by preventing and reducing injuries through the improvement of muscular strength, strength, speed, agility, step, kick, balance, flexibility, full body endurance and the combination of technical abilities.

        수중 운동 후 유방암 환자의 허파 기능 변화

        본 연구는 유방암 환자를 대상으로 수술치료와 보존치료 후 환자의 허파 기능을 알아보기 위해 실시했다. 45세 이상 유방암 환자를 대상으로 유방암 1~2기 진단 받은 후 수술치료와 보존치료 중이거나 종료된 자로 재활치료를 위해 입원한 유방암 환자 59명을 대상으로 하였다. 연구를 위해 수중 운동을 적용한 29명(실험군)과 지상 운동을 적용한 30명(대조군)으로 구분했다. 허파 기능 평가를 위해 폐활량계를 이용했다. 본 연구는 8주간 수중 운동과 지상 운동을 동일 조건하에 실시하여 유방암 치료 후 수중 운동의 허파 기능 향상 여부를 비교 조사했다. 집단 간 운동 중재 기간이 증가함에 따라 노력성폐활량은 수중 운동군(p=.030)이 지상 운동군(p<.001)보다 증가를 보였다. 효과 검증 결과 중재 기간(p<.001), 기간 · 집단(p<.001), 개체 간(p=.019) 유의한 차이를 보였다. 또한 수중운동군이 1초간 노력성 날숨량이 증가했지만, 효과 검증은 유의한 차이가 없었다. 수중 운동 맞춤 프로그램을 시행 한다면, 호흡근 강화에 의해 허파 기능이 향상되고, 삶의 질 향상에 기여할 수 있다. 따라서, 수중운동 프로그램은 유방암 환자의 신체활동을 향상시키기 위한 재활 프로그램으로 활용할 수 있을 것이다. This study was conducted on breast cancer (BC) patients to assess their pulmonary function (PF) of patients after surgical and conservative treatment. We recruited 59 female patients aged over 45 who had been diagnosed with stage one or two BC, had been under or had completed conservative treatment after surgery, and had been hospitalized for rehabilitation. This study was divided into 29 patients in the Aquatic Exercise Group (AEG) and 30 patients in the Land-Based Exercise Group (LBEG). PF was assessed with a spirometer. Aquatic and land-based exercises were conducted for the respective groups for 8 weeks under the same conditions to compare and investigate whether PF improved in the AEG after BC treatment. There was a significant increase in the forced vital capacity (FVC) in the AEG (p=.030) compared to the LBEG (p<.001). Effectiveness verification showed significant differences in the intervention period (p<.001), time?group (p<.001), individuals (p=.019). The forced expiratory volume (FEV1.0) also increased significantly with the duration of the intervention. But, effectiveness verification showed no significant difference. Customized AE programs may improve the PF by strengthening respiratory muscles, which will, in turn, contribute to an improvement in quality of life. Therefore, AE programs can be used as rehabilitation programs to improve the physical activity of BC patients.

        8주간의 수중운동이 유방암 환자의 어깨관절가동범위, 통증역치 및 어깨 기능 장애에 미치는 영향

        본 연구는 유방암 환자의 수중운동 후 어깨관절가동범위와 통증 역치, 어깨기능장애에 미치는 영향을 조사하여 유방암 환자에게 효과적인 운동 방법을 제시하고자 한다. 전남 소재 암재활 병원에서 2019년 3월부터 2020년 3월까지 3개월간 59명의 유방암 환자를 대상으로 8주간 수중운동을 적용하여 어깨관절가동범위와 통증 역치, 어깨기능을 비교 분석했다. 그 결과 수중운동 프로그램 적용 후 견관절의 굴곡, 신전, 외전의 가동범위가 유의하게 증가하였고(p<.001), 통증 정도가 감소하였으며(p<.001), 어깨 관절 기능이 유의하게 향상되었다(p<.001). 따라서 이 연구의 결론은 유방암 환자의 치료에 효과적인 운동 방법에 대한 증거를 제공하는 것으로 생각된다. 향후 연구에서는 유방암 환자의 주요 증상과 신체적 후유증을 치료하기 위한 다양한 물리치료 중재의 효과를 계속 연구해야 할 필요가 있을 것이다. This study investigated the Effect of 8 weeks of aquatic exercise on range of motion of shoulder joint, pain threshold and shoulder dysfunction in breast cancer patients after surgical treatment and adjuvant therapy. To this end, the researcher recruited 59 female patients aged over 45 who had been diagnosed with stage one or two breast cancer, had been under or had completed chemotherapy after surgical treatment, and had been hospitalized for rehabilitation. The research subjects were divided into two groups: an aquatic exercise group(29) and a land-based exercise group(30). For shoulder function, a goniometer was used to measure the range of motion of shoulder joint, and Pain View was used to measure pain. For shoulder function disability, Shoulder Pain And Disability Index (SPADI) was used to assess the level of shoulder disability. And the differences in range of motion of shoulder joint, pain threshold and shoulder dysfunction were analyzed between the two groups. The results showed that the range of motion for shoulder joint flexion, extension, and abduction increased significantly(p<.001). Further more, the level of pain after aquatic exercise decreased(p<.001) and shoulder joint function was enhanced significantly(p<.001). The findings in this study are thus thought to provide evidence for an efficient method of exercise in the treatment of breast cancer patients. Future studies should continue to investigate the effects of various physical therapy interventions for treating the main symptoms of breast cancer patients, as well as the physical aftereffects. By having breast cancer patients do aquatic exercise for eight weeks, this study found that it is effective for improving the range of motion of shoulder joint, pain threshold and shoulder dysfunction in breast cancer patients. This study contributed to enhancing quality of life in breast cancer patients by restoring not only physical health but also psychological and social health and is expected to provide basic data for the improvement of physical function and activity.

        신체안정화운동 후 고등학교 핸드볼, 축구, 배구 구기종목 운동선수들의 유연성과 평형성 향상 비교

        본 연구는 종목별 고등학교 구기종목 운동선수들의 신체안정화운동 후 와이밸런스 실시 후 평형성과 유연성의 향상 정도의 차이점에 대하여 연구하였다. 연구대상은 무안군 M 고등학교 남자 핸드볼 선수 21명, 목포시 M 공업고등학교 남자 축구선수 24명, 목포시 M 여상고등학교 여자 배구선수 19명 대상으로 진행하였다. 2021년 3월 1일부터 8월 31일까지 10주간 신체안정화운동 프로그램 수행 전·후 와이밸런스 검사를 통해 유연성과 평형성의 차이를 SPSS 21.0을 사용하여 t-test, ANOVA, ANCOVA로 통계 분석한 결과 모든 부위에서 유연성 및 평형성이 유의하게 향상되었다. 종목 간 유연성 및 평형성의 차이를 사후검증을 통해 분석한 결과 양쪽 뒤가쪽, 복합점수, 오른쪽 뒤안쪽 부위에서 배구운동선수들이 핸드볼, 축구 운동선수들보다 향상되었다. 결론적으로 핸드볼, 축구, 배구 운동선수들 모두 신체안정화운동 후 유연성 및 평형성이 향상되었으며, 배구 운동선수들이 다른 종목에 비해 유연성과 평형성이 많이 향상됨을 추측할 수 있었다. This study examined the differences in the degree of improvement in balance and flexibility through the Y balance test after the Body Stabilization Exercise (BSE) program for high school players between sports. The study subjects were 21 male handball players from M High School in Muan, 24 male soccer players from M Technical High School in Mokpo, and 19 female volleyball players from M Women"s Commercial High School in Mokpo, Korea. From March 1 to August 31, 2021, the difference in flexibility and equilibrium was statistically analyzed with t-test, analysis of variance (ANOVA), and analysis of covariance (ANCOVA) using SPSS 21.0 through Y-Balance tests before and after the 10-weeks BSE program. The result of analyzing the difference in flexibility and balance between sports through post-hoc test showed that volleyball players improved over handball and soccer players on both sides of posterolateral parts, the composite score, and the right posteromedial part. In conclusion, handball, soccer, and volleyball players all improved their flexibility and balance after the BSE, and volleyball players improved their flexibility and balance more than other sport players.
