Posterior reversible encephalopathy syndrome (PRES) is a clinical radiologic syndrome which is characterized by reversible neurological and radiological findings. It is associated with an abrupt and severe increase in blood pressure, eclampsia, chronic kidney disease with hypertension and using various immunosupressive drugs. Patient with chronic kidney disease could be especially vulnerable to this syndrome because they are frequently exposed to several possible causes as uremia, hypertension, or electrolyte imbalance. We report a case of posterior reversible encephalopathy syndrome in a patient having continuous ambulatory peritoneal dialysis.
복막 투석 환자에서 Brevundimonas vesicularis에 의한 복막염 1예

A 53-year-old male was hospitalized with abdominal pain and turbid peritoneal fluid. He was diagnosed with hypertension, diabetic nephropathy and started continuous ambulatory peritoneal dialysis (CAPD) 4 years ago. Initial peritoneal fluid analysis demonstrated CAPD peritonitis. As initial antibiotic therapy, ceftazidime/vancomycin were injected intraperitoneally. But drug sensitivity test revealed these regimens were ineffective. On sixth hospital day, Brevundimonas vesicularis (B. vesicularis) was cultured from peritoneal fluid, this strain was susceptible to imipenem, piperacillin and resistant to ceftazidime. Accordingly we changed the antibiotics to imipenem, which was administered for 14 days, but analysis of peritoneal fluid was seldom improved. Finally, CAPD catheter was removed, and hemodialysis was started. After CAPD catheter removal, peritonitis improved rapidly. B. vesicularis is a rare opportunistic organism in CAPD peritonitis. Because this peritonitis may not improve in spite of medical treatment with susceptible antibiotics, CAPD catheter must be removed finally.
Primary Empty Sella Syndrome 에 동반된 뇌하수체 기능저하중 및 중추성 요붕증 1 예

The empty sella syndrome results from and extension of the subarachnoid space into an intrasellar portion with expansion of sella and flattening of the pituitary gland. The empty sella syndrome is distinguisted between $quot;primary$quot; (without prior surgical or radiotherapeutic procedurea) and $quot;secondary$quot; (following such procedure) cases. In this syndrome, the endocrine function is usually normal and this syndrome which is accompanied with endocrinologic abnormalities and diabetes insipidus is rarely reported. We discribed a patients of central type of diabetes insipidus, hypoptuitarism including growth retardation and sexual infantilism in the primary empty sella syndrome. So we present this case with a review of the literature.

'h A 50-year old female patient admitted with a 1-week-istory of pain and bulging in the left lower abdominal area following a bout of coughing. The pain was exacervated when a Valsalva's manuver was performed and was somewhat relieved by lying supine. The patient reported a 10-day history of severe coughing, but no fever. She had been taking antitussive for 10 days, but no aspirin and anticoagulant. She had been suffered from bronchial asthma for 8 years. The ultrasound, CT & needle aspiration confirmed the diagnosis of a hematoma between the internal oblique & external oblique muscle of abdominal wall. She was treated conservatively with a heating pad, analgesics, elastic bandages, and instructed to rest. Her bronchial asthma was treated with antitussives, aminophilline, B2 agonist inhalator, and prednisolon. This case report describes the association between relative minor strain and the formation of a hematoma of anterior abdominal wall. A review of the literature was undertaken to discuss the cause, diagnosis, and treatment of a anterior abdominal wall hematoma.
