확장광배근피판을 이용한 화상 후 반흔 유방의 재건 2례

Purpose: Authors present the case of 2 patients who underwent extended Latissimus dorsi myocutaneous flap to reconstruct postburn breast deformity. Methods: A 39-year-old woman and 18-year-old woman with postburn breast deformity visited for reconstruction. The nipples were preserved but normal breast development did not occur due to scar contracture. Burn scar contracture was released by excision of the restricting burn scar and breast mound was reconstructed with extended Latissimus dorsi myocutaneous flap. Additional contracture release with multiple z-plasty was performed at the axillae and medial portion of breast. Results: Postburn breast reconstruction using Latissimus dorsi myocutaneous flap showed natural shaped breast mound and inframammary fold. There was no significant complication in both cases. Conclusion: Latissimus dorsi myocutaneous flap provide sufficient skin and soft tissue and it could be an effective method for reconstruction of postburn breast deformity.
한선 및 피부절제술을 이용한 액취증 수술 후 발생한 Axillary Web Syndrome의 치험례

Purpose: The axillary web syndrome (AWS) is a selflimiting cause of morbidity in the early postoperative period after axillary surgery. This article presents a rare complication developed after surgical treatment for axillary osmidrosis. Methods: A 55-year-old male patient underwent surgical excision of skin and glandular tissue for axillary osmidrosis. Three weeks after the surgery, he visited our department due to a visible web of left axillary skin overlying palpable cord extends into the medial ipsilateral arm. There was a taut and tender cord of tissue under the skin and shoulder abduction was limited to less than 90 degrees. He was diagnosed with AWS and treated with conservative management. Results: Four months later, AWS resolved completely without any treatment and patient was free of pain or motion restriction. Conculsion: Axillary web syndrome has been described as frequent complication after axillary lymph node dissection or sentinel node biopsy, but not after surgical treatment of axillary osmidrosis. Surgeons must be aware of the risk of axillary web syndrome after treatment of axillary osmidrosis.
노선버스 영향을 고려한 트램사업 투자평가방법론 개선 연구

트램은 타 경전철과는 달리 도로에서 자동차와 혼용운행이 가능하다는 큰 특징을 가지고 있으며, 베를린, 뮌헨, 파리, 리스본 등 국외에서는 노선버스와 일부 노선 및 정거장을 공유하는 사례도 존재한다. 현재 투자평가제도 상에서는 교통수요예측 및 편익산정 시 트램을 경전철 중 하나의 수단으로 구분하고, 도시철도의 분석방법론을 준용하고 있으나, 노선버스 특성과 유사한 트램사업 중 비수도권 사업의 경우 KTDB 배포자료에 노선버스의 O/D 및 Transit Line을 제공하지 않아 사업노선 주변 노선버스에서의 수단전환 등의 통행행태를 반영할 수 없는 구조적인 문제점이 존재한다. 본 연구는 현재 비수도권 사업에서 고려되지 않고 있는 노선버스가 트램사업에 미치는 영향을 검토하기 위하여, 현황 검토 및 문제점을 도출한 후, 노선버스 반영 여부에 따른 트램사업의 효과를 분석하여, 수요예측 및 편익산정 개선방안을 제시하였다. 본 연구를 통해 향후 투자평가체계 개선을 통한 트램사업 도입여건에 변화를 줄 수 있을 것으로 기대한다. In contrast to standard train tracks, tramlines are often set along public roads, with trams running among pedestrians and other vehicles. In some cities and towns, trams and buses share the same routes and stations. Under the current investment appraisal system, trams are classified into light rail when predicting traffic demand and calculating benefits, but in the case of non-capital areas, it is notable that the origin-destination and transit lines of buses are not provided in the Korea Transport Database distribution data. Due to this problem, it is difficult to reflect proper mode changing behaviors between route buses and trams. This study examines the impact on tramlines of bus routes that are not currently considered in non-capital areas. Following an analysis of the effect of tram projects according to whether bus routes are considered or not, an improvement in methodology is proposed. Through this study, it is expected that the investment appraisal system for the planning of new tramlines will be improved in the future.
