        총누소관폐쇄에서 단순탐침술 후 시행한 실리콘관 삽입술

        Purpose: To investigate the surgical outcome of silicone tube intubation after simple probing performed in obstruction of distal part of canaliculus or common canaliculus. Methods: Silicone tube intubation was done in 24 patients with obstruction of distal part of canaliculus or common canaliculus, whose obstruction was readily opened by simple probing under mild pushing pressure and who had successful lacrimal irrigation after probing. Silicone tube was removed at about 6 months after operation and the epiphora improvement, dye disappearance test, and lacrimal irrigation test were evaluated at 2 month after the intubation and 2 months after the tube removal. Results: The average of probing to perforate the obstructed canaliculus were 2.1. At 1 month after the intubation, epiphora was improved in 19 patients (79.2%), and 18 patients (75.0%) showed a grade 2+ or lower in dye disappearance test, and lacrimal irrigation was successful in 21 patients (87.5%). 2 months after the tube removal, epiphora was improved in 18 patients (75.0%), and 18 patients (75.0%) showed a grade 2+ or lower in dye disappearance test, and lacrimal irrigation was successful in 20 patients (83.3%). Conclusions: When the obstruction of distal part of canaliculus or common canaliculus is thin membranous nature, good surgical outcome can be expected by silicone tube intubation after simple probing using mild pushing pressure without other special procedure. 목적 : 누소관 말단 또는 총누소관폐쇄 환자에서 단순탐침술 후 시행한 실리콘관 삽입술의 결과를 알아보았다. 대상과 방법 : 누소관 말단 또는 총누소관폐쇄로 진단된 환자 중 누소관열상이 생기지 않을 정도의 세지 않은 압력으로 탐침을 지긋이 밀어 보았을 때 폐쇄부위가 뚫리고 누도관류검사에서 관류가 된 환자 24명에게 실리콘관 삽입술을 즉시 시행하였다. 술 후 6개월경 실리콘관을 제거하였으며, 실리콘관 삽입 후 2개월경과 실리콘관 제거 후 2개월경의 유루증상 호전여부와 형광염색약소실검사 그리고 누도관류검사 결과를 술 전과 비교 조사하였다. 결과 : 폐쇄부위를 뚫기 위해 탐침을 평균 2.1회 정도 밀어 보았으며, 실리콘관 삽입 후 2개월경 19명(79.2%)에서 수술 전보다 유루증상이 호전되었다. 형광염색약소실검사에서 18명(75.0%)이 2+이하의 결과를 보였고, 누도관류검사에서 21명(87.5%)이 관류되었다. 실리콘관 제거 후 2개월경에는 18명(75.0%)에서 유루증상이 호전되었으며, 형광염색약소실검사에서 18명(75.0%)이 2+이하의 결과를 보였으며, 누도관류검사에서 20명(83.3%)이 관류되었다. 결론 : 누소관 말단 또는 총누소관의 폐쇄부위가 얇은 막성 양상일 때 특별한 기구를 사용하지 않고 약간의 압력 하에 일반적인 탐침술만으로 폐쇄부위를 뚫은 후 실리콘관 삽입술을 시행하여 비교적 양호한 결과를 얻을 수 있었다.

        주파수 배가 시야검사계와 하이델베르그 시신경 유두 분석기 II의 복합분석을 이용한 녹내장 판별

        Purpose: This study investigated whether the combined use of Frequency Doubling Technology and Heidelberg Retina Tomography II increases the usefulness of glaucoma detection. Methods: The study subjects were, 59 patients with POAG and 34 normal persons were selected. The authors defined a total of six discriminative criteria: F1, in which the conventional analysis of FDT was used; F2, in which the FDT scoring system was used; F3, in which the Moorfield classification of HRT was used; F4, in which the new HRT discrimination function `Z` was used; F5, in which F1 and F3 were combined, and F6, in which F2 and F4 were combined; The usefulness of each criterion was investigated. Results: The sensitivity, specificity, and area under the ROC curve of the six discriminative criteria were: 86.4%, 58.8%, and 0.740 in F1, 4.5%, 80.0% (cutoff: 12.50), and 0.882 in F2, 79.7%, 61.8%, and 0.679 in F3, 77.6%, 74.3% (cutoff: 0), and 0.857 in F4, 77.6%, 71.4% (cutoff: 1.0805), and 0.767 in F5, and 86.2%, 91.4% (cutoff: 0.495) and 0.931 in F6. Conclusions: Combined use of functional (FDT) and morphological (HRT) aspects, which are early parameters of glaucoma damage, gave more useful results than a single analysis of each. Thererfore, the combined use will help the early detection of glaucoma.

        Structural analysis of a ligatured rat sciatic nerve in the ex vivo state using synchrotron small-angle X-ray scattering (SAXS)

        Background: To understand the fundamentals of neural tissue injury, experiments on the nano-structured nerve system of animals are essential. This study was designed to reveal the nanostructure changes of an isolated ligatured rat sciatic nerve using the synchrotron small-angle X-ray scattering (SAXS) technique. Methods: Male Sprague-Dawley rats (weighing approximately 250 grams) were used in this study. The SAXS patterns of 1 week after ligatured nerves (N = 5) and the normal sciatic nerves (N = 5) for the control were acquired after extracted approximately 15 mm before the experiment. Experiments were conducted at the 4C1 beam line at the Pohang Accelerator Laboratory in Korea. The exposure time was 60 sec, and 8 to 12 images per sample were acquired in 0.5 mm intervals, including the regions above, around and below the ligatured position. Results: The periodic peaks of the myelin sheath and the interfibrillar space of collagen completely disappeared at the ligatured position. Farther from the ligatured point, weak and quite different SAXS patterns were observed for the myelin sheath and interfibrillar space. However, the collagen fiber peaks appeared at all positions, although they were weaker near the ligatured position. Conclusions: The ligature treatment totally destroyed the myelin sheath and interfibrillar space of collagen. In addition, retrograde degeneration developed 2 mm above the ligatured site. The myelin sheath and interfibrillar space of collagen were damaged 6 mm below the ligatured site. However, the collagen fiber structure was not significantly affected by the ligature, indicating a much different structural organization.

        황반부 장액성망막박리가 동반된 시신경유두소와의 수술적 치료 1예

        Purpose: To report the clinical results of a planned vitrectomy, laser photocoagulation and gas tamponade for the treatment of serous macular detachment associated with optic disc pit. However, laser photocoagulation was performed on a postoperative outpatient basis for subretinal fluid. Case summary: A 13-year-old women presented with central visual disturbance in the left eye. Her best-corrected visual acuity (BCVA) was 0.16. On ophthalmic examination, the patient was diagnosed with optic disc pit combined with serous macular detachment. After 3 months, the serous macular detachment increased and visual acuity decreased to 0.1. Pars plana vitrectomy combined with complete posterior vitreous detachment was performed. After intravitreal injection of perfluorodecalin, internal drainage of the subretinal fluid using the back flush needle around the optic disc pit was performed, but the fluid was not drained. The planned intraoperative photocoagulation was not performed due to a large serous macular detachment. Gas tamponade was performed and a facedown position was maintained for 1 week followed by laser photocoagulation. The subretinal fluid was completely absorbed and visual acuity was improved.         베체트병에서 백내장수술 후 발생한 황반부종 및 혈관염을 덱사메타손삽입술로 치료한 1예

        Purpose: To report a case of macular edema and retinal vasculitis treated with intravitreal dexamethasone implantation in a Behcet’s disease patient after cataract surgery. Case summary: A 26-year-old male with Behcet’s disease, without recurrence of uveitis for 3 months, underwent phacoemulsification and posterior chamber intraocular lens implantation for a cataract in the left eye. The preoperative best corrected visual acuity (BCVA) of the left eye was 0.15. At day 1 after surgery, his BCVA was 0.4, and slit lamp examination of his left eye revealed a mild anterior chamber reaction. At 1 week after surgery, he presented with left vision impairment, his BCVA was 0.04, and mild inflammation was observed in both the anterior chamber and the vitreous in his left eye. Unilateral macular edema was confirmed with optical coherence tomography, and fluorescein angiography revealed retinal vasculitis on the fovea and peripheral retina. Intravitreal dexamethasone implantation was performed in his left eye, 1 month after treatment, the macular edema and retinal vasculitis had improved, and the BCVA of his left eye was 0.8. Conclusions: In patients with Behcet’s disease, intravitreal dexamethasone implantation can be an effective treatment option for macular edema and retinal vasculitis after cataract surgery. 목적: 베체트병 환자에서 백내장수술 후 발생한 황반부종 및 망막혈관염을 유리체강내 덱사메타손삽입술로 치료한 1예를 보고하고자 하였다. 증례요약: 베체트병으로 진단받고 양안 포도막염 재발 없이 3개월간 유지되던 26세 남자가 좌안의 백내장으로 수정체유화술 및 인공수정체후방삽입술을 시행받았다. 술 전 좌안 교정시력이 0.15였으며, 술 후 1일째 경도의 전방염증, 교정시력이 0.4로 확인되었다. 술 후 1주일 때 좌안 시력저하로 내원하였으며, 교정시력 0.04, 경도의 전방염증과 유리체염 소견이 보였다. 빛간섭단층촬영에서 황반부종이 관찰되었으며, 형광안저혈관조영술에서 황반부 및 주변부에 혈관염 소견이 관찰되었다. 유리체강내 덱사메타손주입술을 시행하고 1개월째 황반부종과 망막혈관염이 호전되었으며 교정시력이 0.8로 회복되었다. 결론: 베체트병 환자에서 백내장수술 후 발생한 망막혈관염 및 황반부종을 유리체강내 덱사메타손삽입술로 효과적으로 치료할 수있었다.

        범망막광응고술 전 유리체내 베바시주맙 주입술의 황반부종 감소의 단기 효과

        Purpose: To investigate the effect of intravitreal bevacizumab injection in preventing panretinal photocoagulation (PRP)-induced macular edema and visual dysfunction in proliferative diabetic retinopathy. Methods: We conducted a prospective study of 40 consecutive eyes (20 patients) with proliferative diabetic retinopathy whose visual acuity (logMAR) was 0.2 or less, foveal thickness (μm) was 280 or less, and retinopathy was bilaterally symmetrical. In Group 1 (20 eyes), PRP was performed with intravitreal bevacizumab injection 1 week before the initiation of PRP, but in Group 2 (20 eyes) PRP alone was performed. In all eyes, PRP was completed using the same technique. Best corrected visual acuity (BCVA) and foveal and parafoveal thicknesses were measured before treatment and 1 and 3 months after PRP. Results: Before treatment, the mean BCVA and foveal and parafoveal thicknesses were not statistically significantly different between the two groups (p >0.05). At 1 and 3 months after PRP, the mean BCVA improved and the foveal and parafoveal thicknesses were significantly smaller in Group 1 than in Group 2 (p <0.05). Conclusions: Intravitreal bevacizumab injection before PRP could help prevent PRP-induced macular edema and visual dysfunction in patients with proliferative diabetic retinopathy and good vision. 목적: 증식성 당뇨망막병증에서 범망막광응고술 후 발생할 수 있는 시력저하와 황반부종에 대해 술 전 유리체내 베바시주맙 주입술의 예방효과를 알아보고자 하였다. 대상과 방법: 전향적으로 양안에 비슷한 증식성 당뇨망막병증 소견이 보이고, 최대교정시력(logMAR)이 0.2이하, 중심와 두께(μm)가 280 이하인 2형 당뇨환자 20명 40안을 대상으로 하였다. 1군 20안에는 1주 전 유리체내 베바시주맙을 주입하고 범망막광응고술을 시행하였고, 2군 20안에는 범망막광응고술만 시행하였다. 범망막광응고술은 동일한 방법으로 시행하였으며, 시술 전, 시술 후 1개월, 3개월에 최대교정시력과 중심와, 중심와부근 두께를 측정하였다. 결과: 두 군에서 시술 전 최대교정시력과 중심와, 중심와부근 두께는 차이가 없었다(p>0.05). 광응고술 후 1개월, 3개월에 2군에 비해 1군에서 최대교정시력이 유의하게 좋았으며, 중심와와 중심와부근 두께 역시 유의하게 적게 측정되었다(p<0.05). 결론: 범망막광응고술 전 유리체내 베바시주맙 주입술은 술 후 유발될 수 있는 시력저하 및 황반부종에 대해 단기 예방효과가 있으며, 특히 시력이 양호한 환자에서 시행 시 유용할 수 있다.
