        신경섬유종증과 동반되지 않은 악성 신경집종양

        Malignant peripheral nerve sheath tumor (MPNST) is a rare entity of all soft tissue sarcomas. It has four different types of glandular, melanocytic, rhabdomyoblastic, and epithelioid. Epithelioid MPNST is composed of predominantly epithelioid cells. In contrast, typical MPNST is consisted of spindle cells, which is arranged in a fascicular or whirling pattern. A 33-old man showed two skin-color to reddish brown protruding nodules on the left thigh. Microscopically, one nodule was made up of epithelioid cell, primarily. In addition, the other nodule was composed of epithelioid cells and spindle cells, which also showed Antoni A and B. Immunohistochemical study showed the cells being positive for S-100 protein (weak and focal) and neuron-specific enolase, and negative for HMB-45, smooth muscle actin. Herein, we report a case of two different forms of malignant nerve sheath tumor on the left thigh, which was not associated with neurofibromatosis-1. (Korean J Dermatol 2012;50(4):362∼366)

        Erlotinib HCL (Tarceva(R)) 복용에 의한 방사선 회상 피부염

        Erlotinib (Tarceva(R)) is a new anti-cancer agent that acts by inhibiting epidermal growth factor receptor (EGFR) signal transduction. It is currently used in the treatment of advanced stage non-small cell lung cancer and pancreatic cancer. A patient consulted our clinic and she had erythematous patches and vesicles on the submandibular area. She had taken erlotinib for the treatment of recurred lung cancer for 2 weeks. We report here on a case of radiation recall dermatitis that was induced by erlotinib in a 69-year-old woman who was previously irradiated on her submandibular area.         소아에서 발생한 수장족저 균상 식육종

        Mycosis fungoides palmaris et plantaris (MFPP) is a rare form of mycosis fungoides that is confined to the palms and soles. The clinical manifestation of MFPP is often confused with inflammatory palmoplantar dermatoses. Mycosis fungoides is usually considered as a disease of middle age, but it is rarely developed at any age. A 10-year-old girl was referred to us with a 2-year history of recalcitrant palmoplantar dermatoses. Other clinics had treated her for more than 2 years, but all medical treatments turned out to have had no effect, despite her young age. She had not had any atopic dermatitis or allergic contact dermatitis. Histopathologic findings showed inflammatory cell infiltration and lymphocytic epidermotrophism. Monoclonal TCR-rearrangement was positive, so we diagnosed her as having MFPP. We tried to treat her with topical PUVA therapy and she improved within 3 months. Herein, we report on a case of mycosis fungoides Palmaris et plantaris in a 10-year-old girl that was treated successfully with topical PUVA. (Korean J Dermatol 2011;49(3):260∼264)

        불교의식무용과 영산재 - 경전에 보이는 작법무와 영산재 작법무 분류 중심으로 -

        기원전 6세기 중 인도에서 발생한 불교는 여러 나라에 전파되어 각 민족 고유의 역사와 사상, 문화에 알맞은 새로운 믿음과 불교문화를 형성하였다. 불교무용은 석가모니 가르침을 바탕으로 한 모든 무용을 불교무용이라 한다. 불교무용은 범패승 사이에 작법무, 법무, 승무와 같은 명칭으로 불리며, 한국 사찰에서 전승되어지는 불교무용은 바라무, 나비무, 법고무, 타주무의 4종으로 구분하고 있다. 경전에서 나타나는 작법과 관련한 악·가·무에 대한 기술은 『경율리상』·『장아함경』·『화엄경』·『열반경』·『누탄경』·『대지론』 등에서 찾아볼 수 있다. 또한, 한국불교의식에 관련된 불교무용이 나타나있는 여러 자료들 중 16세기말 조선시대에 불교회화인 감로탱화에서 의식을 치르는 승려들의 舞의 형태와 복식, 범패의 형태로 보아 작법무의 역사를 짐작해 볼 수 있다. 16세기 이후 현재까지 보존되고 있는 감로탱화는 부처님의 감로의 설법을 형상화 한 것으로 불교무용과 의식승의 모습이 세밀하게 그려져 있어 그 연원을 짐작해볼 수 있다. 이러한 불교무용인 작법무는 전통형식으로 스님들에 의해 전승되거나, 영산재보존회 부설교육기관 등에서 교육과정을 이수할 수 있고, 유네스코 세계문화유산으로 지정되어 현재 중요무형문화재 제50호 영산재를 통해 보존하고 있으며 시대의 맞춰 대중예술의 문화컨텐츠로 발전하고 있다. Buddhism, which took place in India during the 6th century BC, has have profound impact not only on India but also on Korea and other Asian countries. In many countries where Buddhism has spread, a, new faith and culture appropriate to the history, thought and culture of each nation have been formed based on Buddhism. Buddhist dance is all Buddhist dance based on Buddhist teachings. Beompae monks say that Buddhist dance is classified into Jakbeopmu, Beopmu, Seungmu. There are four types of Buddhist dance, Bara dance, Nabi dance, Beopgo dance, Taju dance that are handed down into Korean Buddhist temples. Nectar of Immortality passed down and preserved since the 16th century is a symbol of the Buddhist teachings and Buddhist dance and rituals are depicted in detail in these paintings. Instruments·songs·dance in the scriptures can be seen in 『Gyeongyulrisang』, 『Dirghagama-sutra』·『Avatamska sutra』·『Nirvana sutra』·Loka- Upapatti sutra』·『Platform theory』and many other scriptures, and the origin of Jakbeop can be found in them. It can be seen that Jakbeopmu which is Buddhist ritual dance is actually recorded with the appearance of the age in the scriptures and Nectar of Immortality of Buddhist paintings. It can be found that Jakbeopmu, Buddhist ritual dance has been traditionally handed down by monks in Yeongsanjae, UNESCO World Heritage Site Important Intangible Cultural Property No.50 and Nectar of Immortality, this traditional ritual dance is developing into performing arts as one cultural content.

        영아의 피부질환에 대한 임상적 분석

        Background: The incidence of infantile dermatoses shows a tendency to increase, despite medical development and environmental improvement. Infantile dermatoses must be regarded discretely, due to special characteristics of the neonatal and infantile skin. Objective: The purpose of this study is to evaluate the incidence of dermatologic diseases in the infant patients. Methods: One hundred fifty nine out-patients, younger than 1 year who had visited the department of dermatology from January 2001 to December 2011, were analyzed, retrospectively. Results: The 0∼1 year age group accounted for 0.58% of all new patients (261/44,718). The ratio of male to female was 1 to 0.69. Patients visited the hospital most frequently in the winter (34.5%). The most common disease was atopic dermatitis (30.3%), followed by hemangioma (9.2%), and irritant contact dermatitis (7.7%). The most common disease group was eczema (44.8%), followed by an infectious skin disease (19.2%), and dermal and subcutaneous tumors (10.7%). Conclusion: This study provides useful data on the incidence and characteristics of dermatologic diseases in infant patients.

        Neurofollicular hamartoma is an unusual, previously described neoplasm characterized by a stroma composed of proliferation of spindle cells and hyperplastic pilosebaceous units. The stroma has light microscopic and immunohistochemical features consistent with neuroid differentiation showing positive S-100 antigen. Lesions are almost invariably located on the face with strong predilection for the nose or nasolabial fold. A 35-year-old man presented with a solitary, skin-colored, dome-shaped papule that had been present on his right temple area for a few years. Clinical differential diagnosis included basal cell carcinoma, angiofibroma and intradermal nevus. Clinically, the lesion presents as a small, skin-colored, dome-shaped firm papule, 3 to 7 mm in diameter. Although clinical aspects of the lesion are not diagnostic, the histological features are very typical. Herein, we report a rare case of neurofollicular hamartoma on the right temple area.         조선시대 감로탱화 중단에 나타난 작법무의 특징연구

        본 연구는 조선시대 감로탱화 중단에 나타난 불교의례시 작법의 형태와 구성, 동작, 반주악기를 비교, 분석하여 나타난 특징들을 연구하고자 하였다. 감로탱화 중단에서 보여 지는 배경과 공간구성, 승려들의 작법형태로 각 사찰의 보존된 조선시대 감로탱화들 중에서 의례와 작법의 형태가 사실적으로 표현되고, 각 시대별 흐름이 잘 나타나있는 감로탱화 5점을 중심으로 조사하였으며, 감로탱화와 불교의식 중 작법, 범패와 관련된 논문과 학회지, 관련서적 등 문헌자료를 중심으로 연구하였다. 따라서 감로탱화 중단에 나타나는 승려들의 작법형태, 동작 등을 연구하였고, 작법시 보여 지는 공간의 구성, 내용, 의상을 파악하고, 당시 시대의 흐름에 나타나는 불교사상, 대중들의 신앙을 살펴볼 수 있었다. 본 연구의 결과를 통해 다음과 같은 결론을 내릴 수 있었다. 첫째, 18C이후 감로탱화 도상에서 보이는 의례시 공간의 확대와 무대형식으로 당시의 불교사상이 억불정책에서 변화됨을 느끼고 대중들의 필요에 의해 불교의례가 발전됨을 알 수 있었으며 둘째, 감로탱화 도상에서 보여 지는 영가를 위한 재의식에서 악기연주와 범패, 작법무의 형태 등 전체적인 의례의 구성이 현재의 영가천도재, 영산재에서 보여 지는 것과 상당한 유사성을 찾아볼 수 있었고, 또한 작법무 동작의 경우 범패와는 달리 그 시작을 알기가 역사적으로는 어려우나 불교의례시 작법의 완전한 형태가 도상에 나타나거나 사실적인 동작의 형태로 현재 작법과 범패의 형태와 구성에 대한 정통성을 알 수 있었다. 상기한 결과에 따라 감로탱화는 조선시대의 불교사상과 의례의 형태를 사실적으로 보여주는 그림 자료로서 귀중한 연구 자료가 될 것이라 사료된다. 이러한 연구를 통하여 불교의례 역사자료의 학문자료로 도움이 되기를 바라며, 특히 작법무, 범패와 함께 불교의례시 행하는 의식들을 통해 우리나라 불교예술의 역사적인 정통성과 사상을 연구하는데 그 사료적 가치가 매우 크므로 지속적인 관심과 적극적인 연구가 필요할 것이다. This research is to find out the differences and the comparative features of Buddhist ceremony`s composition, constitution, motion and the instrumental accompaniment from discontinuance of Gamrotangwha during chosun Dynasty. To see the back ground, the method of drawing and the composition as a results of the discontinuance of Gamrotangwha, we have done a research base on the five of Chosun Dynasty`s Gamrotangwhas which well reveal the stream of each era and describe the ceremonies and compositions. However, the appearance of monks` composition, the motion from the discontinuance of Gamrotangwha have been studied, we could understand the composition of the place, subject and the clothes as well as the aspect of Buddhism and public`s religious belief. From this research, we can conclude two things. First, the improvement of Buddhist ceremony driven by people who felt the change of the policy appeared for suppressing Buddhism. The improvement is that the expansion of ceremony area and the form of the ceremony shown in Gamrotangwha. Secondly, the general consitituions of Gamrotangwha`s instrumental performance, Pompae and Jakbeop dance have the similarity with modern day`s Yeongsanjae. Although, Jakbeop dance`s historical origin is hard to figure out, the orthodoxy of the modern day`s Pompae and Jakbeop dance appears while Buddhist Ceremony held. As a result, Gamrotanwha would become an important historical picture material which shows Buddhism and features of ceremony very realistic. Through this research, hopefully Gamrotangwha could be the centric material of studying historical material about Buddhist Ceremony. On top of everything else, studying historical orthodoxy and that idea of Buddhist artistic value, Gamrotangwha deserves to be carefully researched and positively studied, and also constant interest is need.
