A 46 year-old woman who had worked repetitive manual job for 3 years visited our hospital because of aggravated right hand sensory symptom and newly developed limitation of 3, 4, 5th finger extension. In physical examination, moderate swelling of volar aspect of wrist and limitation of finger extension was noticed as well as positive tinnel sign. We confirmed carpal tunnel syndrome after electrodiagnostic studies and associated flexor tenosynovitis of 4th flexor tendon in the carpal tunnel through MRI (magnetic resonance imaging) study. She was successfully treated with conservative program including wrist splint and steroid injection. We also tracked the result of treatment using MRI and electrodiagnositic studies. Analyzing her work procedure, we concluded that repetitive and forceful exertional nature of job impose a high burden on hand and wrist region and it caused her carpal tunnel syndrome.
척수손상 장애인 사격선수들의 운동 관련 특성 및 기능적 의무등급 분류

Objective: To categorize the spinal cord injury level and American Spinal Injury Association (ASIA) impairment scale into functional classification of shooting and to give information about shooting sport for the disabled to medical staffs, and to remind importance of sports activities for rehabilitation. Method: Seventy nine spinal cord injured players were classified into each functional classes by two international classifier. During the classification process, demographic data such as age, sex, duration, injury type and questions including attitude toward disabled shooting or benefits or difficulties from shooting were collected by interview. Results: Functional classes of players were closely related to injury level and ASIA impairment scale. Average dura-tion of shooting experience of men (8.01 years) was significantly different from that of women (5.29 years)(p<0.05). Self-motivated (39.3%) and disabled friends (21.5%) were leading motive providers followed by medical staff (20.2%), other shooting player or coach (12.7%), family (6.3%). Promoting self confidence (32.5%) was number one benefit from shooting and followed by making friends (22.1%), increasing independence (18.3%), and increasing in social participation (12.7%). Conclusion: We report the functional classification result of spinal cord inured player and we found that sports activities promote social activities and help to develop self confidence.

노인 연령층에서 비교적 흔히 발생하는 골절로는 척추압박골절과 고관절 및 손목의 골절이다. 65세이상 여성의 33~50%에서 1곳 이상의 척추압박골절이 발생하고 80세 이상 노인의 약 17%에서 고관절 골절이 발생한다. 특히 여성 노인들은 골절 발생 후 3개월 이내 20%에서 사망하고 생존자의 50%는 영구히 보호시설에서 지내게 된다는 점에서 큰 의미를 지니고 있다고 하겠다.

장애인들도 이성에 대한 감정을 표현하고, 사랑을 하고 성취할 권리가 있다. 이는 인간으로서 추구할 당연한 권리이다. 그러나 결혼과 자녀의 출산 문제에 있어서는 여전히 커다란 장벽에 부닥치게 된다. 가족과 사회에서 체계적인 도움을 줄 수 있다면 그다지 어려운 문제가 아닐 수 있다.

화학적 변형 방식에 의한 트로포마이오신과 액틴의 상호작용을 규명하기 위하여 액틴결합에 중요한 역할을 하는 C-말단부위의 아미노산 잔기 276 또는 277을 단일 시스테인 잔기로 치환한 돌연변이 트로포마이오신을 제조하여 대장균에서 대량 발현시킨 후 액틴 결합력을 측정하였다. 잔기277을 시스테인 잔기로 치환시킨 TM24(QC) 및 TM29(HC)는 액틴 결합 성질을 잃어버렸을 뿐만 아니라 트로포닌 존재 하에서도 액틴결합력이 증가하지 않았다. 이 결과는 잔기 277이 트로포마이신 기능에 중요한 역할을 한다는 것을 제시한다. 반면 잔기 276을 시스테인 잔기로 치환한 TM22(CT) 및 TM23(CA)는 액틴과 비교적 잘 결합하였을 뿐만 아니라 트로포닌 존재 하에서 액틴결합력이 증가하였다. 따라서 TM23(CA)는 시스테인 잔기를 도입하여도 트로포마이오신의 기능을 유지하였으며 향후 화학적 변형 연구를 위한 도구로 중요하게 사용될 수 있을 것이다. It has been previously reported that the carboxyl terminal residues 276 and 277 of α-tropomyosin are important for actin affinity. In order to investigate actin affinities of these two residues of skeletal (HA) and smooth (QT) muscle α-tropomyosins, a series of mutant tropomyosins were constructed in which residues at either 276 or 277 were individually replaced with a cysteine residue for chemical modification. These mutants were overexpressed in E. coli as unacetylated and Ala-Ser (AS) dipeptide fusion forms. While actin affinities of unacetylated tropomyosins were considerably low, those of AS/TMs were remarkably higher than those of corresponding unacetylated tropomyosins. However, actin affinities of AS/TM24 (QC) and AS/TM29 (HC) were dramatically lower than those of other AS/TMs and were close to those of unacetylated tropomyosins. In addition, actin affinities of unacetylated TM24 (QC) and TM29 (HC) failed to be restored in the presence of troponin, unlike unacetylated TM10 (HA) and TM23 (CA). These results indicated that the presence of a cysteine residue at 277 caused a drastic decrease in actin affinity, and also that the residue 277 is important for actin affinity of α-tropomyosin. Since TM23 (CA) showed high actin affinity, it may serve as a valuable tool for chemical modification studies for investigating the interaction of the carboxyl terminal residues of α-tropomyosin with actin and/or troponin.
