        의학적 충고에 반한 퇴원의 특성과 퇴원결정 요인에 관한 연구 - 사회사업가의 개입사례와 역할을 중심으로 -

        Objectives : DAMA cases were analyzed to examine what the main casual factors of DAMA were and how to deal with these cases effectively in hospital with the DAMA interdisciplinary team including medical social worker whose role is to perform psycho-social assessment, family counsel, to evaluate family's DAMA need. Patients and Methods : The content analysis of medical record and social work record were reviewed in 37 cases referred by medical doctor to DAMA team. These cases were reported by patients' self discharge request or family's request for discharge from September 1998 to February 2000. The DAMA team consists of Assistant Director of Hospital as team leader, medical staff in-charge, social worker, QI nurse, other staff members who are not involved in direct treatment for patient, and administrative clerk. Results : The results of content analysis are as follows : 1) The most causal factors of DAMA consist of combination of more than 2 factors. 2) The major decision-maker is revealed to be son and daughter of patient. 3) In 59.4% of cases, family was not informed of patients' prognosis, alternatives, the consequence of DAMA at all. 4) In cases of DAMA report, the rapid intervention of social worker is carried out. Conclusion : In this study, we propose the interdisciplinary team approach to make decision legitimately and ethically for DAMA. The suggestions from this study are as follows : 1) To deal with DAMA case properly, the interdisciplinary team approach should be considered. 2) The criteria for DAMA case should be formed carefully. For the explicit selection of DAMA case, preliminary system for high-risk patient screening is recommended. 3) The medical social worker is available for the psycho-social problems of the patient once family members. For the effective family counselling, discharge planning and nursing home placement, the participation of medical social worker should be mandatory.

        노인의 여가활용을 위한 경로당 프로그램의 활성화 방안에 관한 연구

        The purpose of this study was to investigate the actual condition of leisure activity for elderly peoples at $quot;Kyungnodang(the senior citizen center)$quot; located in Suwon. This survey was carried out 45 elderly people whose a president and vice-president, general business. The results of this study are summauzed as follows : First, the leisure activities in $quot;Kyungnodang$quot;were very simple and inactive. They generally played korean cards, pa-duk, or the game of chess and talked to each other meaningless, and they watching television or listening to radio. Second, the activities program in $quot;Kyungnodang$quot; were very diversity. They enjoyed a trip once or twice in a year. And they participated volunteer activities in community and to meet to eating service, medical exam once more in a month. Third, they concerned about that health and meal service. Also, they desired for enhancement program medical exam and meal service. They wished to linking services which hospitals, health center, restaurant and sports center in the community. Such a results, some suggestions are as follows : First, the expanding of leisure program in $quot;Kyungnodang$quot; should be fully financial supports. Second, the way of enhancement leisure program in $quot;Kyungnodang$quot; should be linked to community resources Third, the leisure program in $quot;Kyungnodang$quot; should be developed as providing the elderly people with various leisure programs and services. Fourth, Its required leader education for the aged`s orientation about leisure life should be changed positively.

      • 의료사회복지의 장기적 발전 방향

        본 연구는 의료사회복지의 장기적 발전 방향을 모색하기 위하여, 의료의 1차, 2차, 3차 수준을 포괄하여 고찰하였다. 연구결과, 1차 수준에서는 보건소에 사회복지사를 배치해야 하며, 2차 수준에서는 종합병원과 국공립병원의 사회복지사 채용을 확대해야 하고, 수가체계를 확대인정 받도록 하며 병원표준화 심사과정이나 서비스 평가과정에 사회복지사가 참여해야하며, 전문 프로그램의 활성화를 도모해야 한다. 3차 수준에서는 호스피스 프로그램의 참여 및 요양병원에서 의료사회복지사의 역할이 확대되어야 한다. 이러한 포괄적인 접근이 이루어지기 위해서는 의료사회복지의 명확한 관점 확립과 사회복지사 자격제도의 개선 및 수련제도의 확대 시행, 의료사회복지학과의 신설 등이 필요하다. The purpose of this study is to explore for long term developing direction of social work in health care. Thus, comprehensive explored including primary care, secondary care and tertiary care. Major results are as follows: Firstly, medical social worker employed a public health center within a each community, for linkage of medical service and welfare service. Secondly, medical social worker`s employing expended to general hospitals and a public hospitals, medical social worker`s fee items enlarged to social worker`s all activities, take part in the conduct of evaluation process for certification of hospitals services, and specialized program development for professional of medical social worker. Thirdly, medical social worker`s activities enlarged to hospice program and nursing home facilities. Finally, improvement of social worker`s degree, management of internship program for medical social worker` training, new establishment curriculum for school of medical social work.

        사회복지사의 직무특성이 직무만족에 미치는 영향에 관한 연구

        이 연구는 사회복지사의 직무특성이 직무만족에 미치는 영향을 고찰함으로써 사회복지사의 직무 설계 시 고려해야 할 요소를 밝혀내기 위해 시행되었다. 설문은 사회복지관의 사회복지사와 의료사회복지사 240명이 응답한 결과를 토대로 분석하였다.<br/> 연구결과 사회복지기관 사회복지사의 직무만족에는 자율성, 피드백, 정체성, 전문성, 필요성의 직무특성과 자기효능감이 영향을 미쳤으며, 의료사회복지사의 직무만족에는 필요성과 자기효능감, 성별, 직위만이 영향을 미쳐, 직무특성요인이 부분적으로 결과변수에 영향을 미치는 것으로 나타났다. 특히 핵크맨과 올드햄(1975)의 핵심직무특성보다는 정체성, 전문성, 필요성의 직무특성 영향력이 보다 강한 것으로 나타나, 사회복지사의 지식과 기술, 기법 외에 관점이나 정체성, 필요성 등을 강조하는 접근이 필요한 것으로 나타났다. 또한 자기효능감이 직무만족에 미치는 영향력이 다른 변수보다 강하였고, 직무특성이 직무만족에 영향을 미침에 있어 부분적인 매개역할을 하고 있는 것으로 나타나, 사회복지사의 자기효능감을 증진시키기 위한 교육과 지지적인 환경, 수퍼비전 등이 필요한 것으로 나타났다.<br/> 이 연구는 핵크맨과 올드햄(1975)의 핵심직무특성 모형을 사회복지조직에 적용한 연구로, 직무설계 시 사회복지실천현장의 특성별로 달리 나타나는 직무특성 요인을 감안한 직무의 설계와 재설계를 통하여 사회복지조직의 효율성을 도모해야 할 필요성을 제기하고 있다. The objective of this study examined the effect of job characteristics on the job satisfaction of social worker employed by 1st social agency, hospitals. Data were collected by 240 social workers. Based on Hackman and Oldham's(1975) theory of core job characteristics, this study was to defined other element of job characteristics in social worker's duties, focused on moderating effect of self-confidence for job satisfaction, and compared to social worker's job characteristics between 1st social agency and hospitals.<br/> The results of this study were summarized as follows; First, job autonomy, feedback among Hackman and Oldham's 5 core job characteristics showed positive influence on job satisfaction. The other job characteristics found out significantly effected social worker's job satisfaction than Hackman and Oldham's 5 core job characteristics. Second, identities of social worker's job, cognition of professional level, necessity, and self-confidence variables were found to have a statistically significant strong relationship with job satisfaction of 1st setting social worker. The other side, necessity, self-confidence, sex, social worker's academic career variables positive influenced medical social worker's job satisfaction. Third, the moderating variables which had significant difference among social worker's working conditions. 1st setting social worker's self-confidence partial moderated the effect of job characteristics on social worker's job satisfaction. But, medical social worker's self-confidence not moderated the effect of job characteristics on job satisfaction.<br/> As a result, Social worker's job satisfaction depended on job characteristics and self-confidence. Therefore, to assure the job satisfaction, redesign for social worker's job characteristics, reinforced their self-confidence. Staff-development education program, supportive environment, supervision program is strongly recommended.

      • 종합사회복지관의 학습조직이 조직유효성에 미치는 영향에 관한 연구 -학습수준의 매개효과를 중심으로-

        본 연구는 종합사회복지관의 학습조직이 조직유효성에 미치는 영향을 고찰함으로써 사회복지조직에서의 학습조직 적용 가능성을 탐색하고자 실시하였으며, 종합사회복지관의 사회복지사 162명이 응답한 내용에 기초하고 있다. 연구결과, 첫째, 학습지원체계는 개인학습, 팀조직학습에 영향을 미쳤으며, 이러한 학습수준이 조직유효성에 영향을 미쳤다. 또한 학습지원체계는 직접적으로 조직몰입에도 영향을 미쳤으며, 학습지원체계는 학습수준을 매개로 조직유효성에 영향을 미치는 것으로 나타나, 사회복지조직의 유효성 제고를 위해서는 학습지원체계의 구축과 학습수준의 활성화가 필요한 것으로 나타났다. 둘째, 개인학습은 팀조직학습을 부분매개로 직무만족에 영향을 미쳐 팀조직학습이 활성화 되어야 할 것으로 나타났다. 이러한 연구결과에 기초하여 사회복지조직의 학습조직 활성화 방안으로 학습지원체계강화, 다양한 학습활동의 촉진, 학습조직 관련요인의 평가지표 반영을 제안하였다. The purpose of this study of improve the organizational efficiency management`s kills through the effects and paths of learning organization on organizational effectiveness of social welfare centers. Data were collected by 162 social workers including social welfare centers. The results of this study were summarized as follows; First, the learning organization have a positive effect on the organizational effectiveness. Learning construction`s levels was particularly shown to have mediative variables between supporting system and organizational effectiveness. Second, individual level of learning construction partial mediate by team and organizational level of learning construction for job satisfaction. Job satisfaction has a positive effect on the organizational commitment, job satisfaction was particularly shown to have mediative variables with organizational commitment. Therefore, to assure the proper organizational effectiveness, strengthening of learning supported system, facilitating to learning construction level(individual, team, organizational level) and learning organizational components included to evaluative`s indicator of social welfare center.

      • 의료사회복지사의 소진이 이직의도에 미치는 영향에 관한 연구 -매개효과를 중심으로

        이 연구는 의료사회복지사의 소진과 이직의도와의 관계에서 임파원먼트가 미치는 영향을 규명하기 위하여 시행되었으며, 116명의 의료사회복지사가 응답한 결과에 기초하고 있다. 연구결과, 소진과 이직의도는 정적인 상관관계를 보였으며, 임파워먼트는 소진과 이직의도와 부적인 상관관계를 보였고, 임파워먼트는 소진과 이직의도를 부분적으로 매개하는 역할을 하는 것으로 나타났다. 이러한 결과로 볼 때, 의료사회복지의 임파워먼트는 소진상태에 있는 사회복지사의 이직의도를 감소시킬 수 있는 하나의 방안으로, 조직 관리의 효율성을 증진시킬 수 있는 차원에서 임파워먼트를 통한 인력관리와 조직관리의 필요성을 제기하였다. This study was performed to analyze the effects of medical social worker`s turn-over by their burn-out with focusing on the mediating effect of social worker`s empowerment. Data were collected by 116 social workers, were identified through the registry of Korean Association of Medical Social Workers. The results were follows: First, empowerment to medical social workers had reduced on their burn-out and turn-over. Second, the correlation of medical social worker`s burn-out and turn-over was showed very positively relationship. Third, medical social worker`s empowerment partial mediating the effect of burn-out on social worker`s turn-over. As a result, medical social worker`s burn-out and turn-over depended on empowerment. Therefore, to reduce the burn-out and turn-over, reinforced their empowerment. Empowerment was recognized as one means by which organizations can effectively management.
