남성 만성기 허혈성 뇌졸중환자의 운동강도 및 시간에 대한 Poor Fibrinolytic Response

Purpose: The purpose of this case study was to investigate three poor fibrinolytic responders with chronic ischemic stroke to acute exercise intensity and time. Methods: Three ischemic stroke patients (male) from the stroke center located at Busan metropolitan area in Republic of Korea volunteered at this study. They performed two single session exercises that were a VO2peak test and a single bout treadmill walking (70-75%HRpeak, 30 min, 50min). Fasting blood samples for determination of tissue Plasminogen Activator (tPA) and Plasminogen Activator Inhibitor-1 (PAI-1) were obtained before, immediately after, 30min after acute exercise. SPSS 12.0 was used for analyzing of data and computing mean and standard deviation, and change rate was conducted between times. Results: In fibrinolytic activity according to the intensity and time of acute exercise, tPA change increased steadily during the recovery stage after the VO2peak in the cases, but PAI-1 activity showed different patterns among the cases. In a single bout treadmill walking (70-75%HRpeak, 30 min, 50min), tPA change increased between 30min and 50min. Conclusion: In conclusion, these results suggest that the exercise prescription for poor fibrinolytic responder with three male chronic ischemic stroke patients without motor disability recommend at 70-75%HRpeak, over 30min.

Purpose: The purpose of this study was to examine correlations among UPDRS, respiratory function, and senior fitness and to investigate the effects of restrictive respiratory function on these factors in Parkinson's disease patients. Methods: Subjects (n=25, Hoehn & Yahr (H&Y) stage: 2-3, $69.3{\pm}5.9$ yrs) from D Hospital Parkinson's Disease Center at Busan metropolitan area in the Republic of Korea volunteered for this study. They performed the pulmonary function test, UPDRS, and the senior fitness test. SPSS 18.0 was used for analysis of data, and the collected data were analyzed using Pearson's correlation coefficient (n=25). In addition, Independent t-test was used for determination of differences between two groups (between the normal pulmonary function group (n=10) and the restrictive pulmonary function group (n=10)). Results: Forced vital capacity (FVC (L)) showed significant negative correlation (r=-0.44, p<0.05) with H&Y stage in Parkinson's disease patients, and chair stand showed significant negative correlations (r=0.41, 0.43, 0.42, p<0.05) with FVC (L), FVC (%), and FEV1 (L). FVC (%) showed significant positive correlations (r=0.44, r=0.44, p<0.05) with right and left back scratch. In addition, the restrictive respiratory function group showed significantly lower FVC (%) (p<0.01) and was significantly slower (p<0.05) in the 8-foot up-and-go test than the normal respiratory function group. Conclusion: In conclusion, these results suggest that restrictive respiratory function in PD was related to H&Y stage. In addition, agility of PD patients was lower in the restrictive respiratory function group than in the normal function group.

Intense pulsed light(IPL) is a photo-therapeutic medical device using xenon lamps with optical characteristics of incoherence, defocusing, and polychromatic. IPL has been recognized as a representative medical therapeutic device with growth of domestic skin care market. However, there is no standard regulation manual and guideline to evaluate the safety and performance of IPL. International Electrotechnical Commission(IEC) is being statued "Technical Report IEC TR 60825-16" which is a guidelines for the safe use of intense light source equipment on humans and animals. In this study, a standard Korea Food & Drug Administration (KFDA) regulation for the safety and performance of IPL was suggested by developing minimal evaluation methods for optical parameters of IPL such as output energy density, pulse mode, spot size, wavelength and uniformity of output light. We expect that this study may be a step to statue a standard regulation manual and guideline for IPLs used in domestic.
학령기 정상 아동의 호흡 특성과 신체 조건에 관한 상관분석

Purpose: Respiratory is an essential vital component for conservation of life in human, which is controlled by respiratory muscles and its related neuromuscular regulation. The purpose of this study is to assess lung capacity and respiratory pressure in healthy children, and to investigate relationship and predictability between respiratory pressure and other related respiratory functions. Methods: A total of 31 healthy children were recruited for this study. Demographic information and respiratory related factors were assessed in terms of body surface area (BSA), chest mobility, lung capacity, and respiratory pressure. Correlation between respiratory pressure and the rested variables was analyzed, and multiple regression using the stepwise method was performed for prediction of respiratory muscle strength, in terms of respiratory pressure as the dependent variable, and demographic and other respiratory variables as the independent variable. Results: According to the results of correlation analysis, respiratory pressure showed significant correlation with age (r=0.62, p<0.01), BSA (r=0.80, p<0.01), FVC (r=0.80, p<0.01), and FEV1 (r=0.70, p<0.01). In results of multiple regression analysis using the backward elimination method, BSA and FVC were included as significant factors of the predictable statistical model. The statistical model showed a significant explanation power of 71.8%. Conclusion: These findings suggest that respiratory pressure could be a valuable measurement tool for evaluation of respiratory function, because of significant relationship with physical characteristics and lung capacity, and that BSA and FVC could be possible predictable factors to explain the degree of respiratory pressure. These findings will provide useful information for clinical assessment and treatment in healthy children as well as those with pulmonary disease.
온습포와 연부조직 가동술이 뇌성마비의 슬괵근 유연성 증가에 미치는 영향

Purpose:The hamstring muscle has an important role in standing, walking and other functions related to daily living. Most people with Cerebral Palsy (CP) have muscle tightness, especially in the hamstring, which may cause abnormal alignment in the lower extremities. To investigate an effective treatment for hamstring muscle tightness by observing the effects of heat pack or Soft tissue mobilization (STM) on hamstring muscles. Methods:Thirteen-two volunteers with CP aged between 9 and 13 years(M±SD: 11±1.46), were recruited and divided into four groups of four: (1) the STM for people with Spastic CP group, (2) the heat pack(HP) for people with Spastic CP group, (3) the STM for people with Mixd CP group, and (4) the heat pack for people with Mixed CP group. All received treatment three times per week for six weeks. A inclinometer was used to measure for hip flexion & knee extension joint range motion (ROM). Results:The ROM of hip flexion & knee extension were improved at the STM and HP for people with Spastic CP groups. The ROM and hip flexion were improved at the STM and HP for people with Mixed CP groups. The amount of increase ROM are differenced by the types of CP. Conculusion:STM and heat pack, both are effective for people with CP especially for Spastic CP.
척추 안정화 운동과 등뼈가동술이 만성 허리통증환자의 척추 유연성에 미치는 영향

PURPOSE: This study aimed to investigate the benefits of thoracic manipulation and the trunk stability exercises on spine trunk flexibility and the pain index of chronic low back pain patients. METHODS: The study included 44 patients suffering from chronic low back pain. The participants were divided into two groups: the first group was assigned for trunk stability exercises (TSE), and the second group was randomly assigned TSE and thoracic manipulation at the same time (TSE+TM). Both groups carried out each assigned treatment thrice a week for 8 weeks. The study outcome was based on assessment of spine trunk flexibility and the pain index. Spine trunk flexibility was measured by spine flexion and extension through a range of motion and thoracic cage circumference. The pain index was measured using a visual analog scale (VAS). RESULTS: Spine flexion and extension range of motion showed a significant difference within each group and between the groups before and after the treatment. The measurement of the thoracic cage circumference also showed a notable difference within each group and between the groups before and after the treatment. There was no change in the pain index. CONCLUSION: These results indicate that thoracic manipulation with the trunk stability exercises is an efficient treatment for improving the spine trunk flexibility and soothing pain for chronic low back pain patients.

In this study, we propose a single multi-task deep learning model for classifying digital pathology images from multiple organs based on the degree of cancer differentiation. For multi-organ cancer classification, there has been two major approaches in digital pathology. One is to develop a separate model per organ. Second is to employ an ensemble model to combine multiple models that were trained on different organs. Both approaches are time- and resource-inefficient. Herein, we propose a single multi-task model that simultaneously utilizes pathology images from multiple organs. Three digital pathology datasets, including colon, prostate, and gastric tissue images, are employed in this study. The experimental results demonstrate that the proposed approach is able to improve the overall cancer classification performance, which outperforms single organ models and ensemble models.
