Development of a prediction model for bothersome stress urinary incontinence after prolapse surgery

Objective: To develop a prediction model for bothersome stress urinary incontinence (SUI) after prolapse surgery. Methods: This retrospective cohort study included 708 women who underwent prolapse surgery for pelvic organ prolapse quantification stage 2-4 prolapse and had SUI data available 1 year after surgery. Bothersome SUI was defined as bothersome symptoms (“moderately” or “quite a bit” response to the question regarding SUI on the Urogenital Distress Inventory-6) and/or subsequent anti-incontinence surgery 1 year after index surgery. To construct a prediction model, we performed stepwise variable selection using logistic regression model and measured the area under the curve (AUC) using R software. To avoid overfitting and apply internal validation, ten-fold cross-validation was applied. Results: Postoperative bothersome SUI occurred in 113 women (16%). A model containing five predictors (vaginal parity ≥4, diabetes mellitus, subjective urinary incontinence, apical prolapse surgery, and mid-urethral slings) showed excellent performance for predicting bothersome SUI (AUC 0.79, 95% confidence intervals 0.75-0.82) and significantly more accurate than prolapse-reduction stress test-based midurethral sling model and a previously developed prediction model (p<0.01). Conclusion: We successfully developed a prediction model for bothersome SUI after prolapse surgery. External validation will be required to verify its utility in clinical practice.
Flavonoids from <i>Machilus japonica</i> Stems and Their Inhibitory Effects on LDL Oxidation

<P>Stems of <I>Machilus japonica</I> were extracted with 80% aqueous methanol (MeOH) and the concentrated extract was successively extracted with ethyl acetate (EtOAc), normal butanol (<I>n</I>-BuOH), and water. Six flavonoids were isolated from the EtOAc fraction: (+)-taxifolin, afzelin, (−)-epicatechin, 5,3'-di-<I>O</I>-methyl-(−)-epicatechin, 5,7,3'-tri-<I>O</I>-methyl-(−)-epicatechin, and 5,7-di-<I>O</I>-methyl-3',4'-methylenedioxyflavan-3-ol. The chemical structures were identified using spectroscopic data including NMR, mass spectrometry and infrared spectroscopy. This is the first report of isolation of these six compounds from <I>M. japonica</I>. The compounds were evaluated for their diphenyl picryl hydrazinyl scavenging activity and inhibitory effects on low-density lipoprotein oxidation. Compounds <B>1</B> and <B>3</B>–<B>6</B> exhibited DPPH antioxidant activity equivalent with that of ascorbic acid, with half maximal inhibitory concentration (IC<SUB>50</SUB>) values of 0.16, 0.21, 0.17, 0.15 and 0.07 mM, respectively. The activity of compound <B>1</B> was similar to the positive control butylated hydroxytoluene, which had an IC<SUB>50</SUB> value of 1.9 µM, while compounds <B>3</B> and <B>5</B> showed little activity. Compounds <B>1</B>, <B>3</B>, and <B>5</B> exhibited LDL antioxidant activity with IC<SUB>50</SUB> values of 2.8, 7.1, and 4.6 µM, respectively.</P>

The purpose of this study is to investigate the effect of balancing side occlusal contacts on temporomandibular joints (TMJ) through sonography. In this study, 100 adults with no symptoms of temporomandibular disorder (TMD) such as pain or opening disorder but with the possibility of clicking sounds were investigated. Subjects were checked for occlusal interferences with articulating paper, and the presence of clicking sounds with TMJ palpation and stethoscopy. Subsequently, they were classified into four groups according to their status of occlusal interferences or clicking sounds for each category. The joint vibration of the subjects was recorded using joint vibration analysis (JVA). JVA indexes (total integral, integral 300 Hz, peak amplitude, peak frequency, >300/< 300 ratio, med. frequency, max. opening) were measured and compared. All JVA indexes showed no statistically significant difference between the groups. Only JVA index of > 300/< 300 ratio, med. frequency showed a statistically significant difference in the group with both interference and clicking sound. If there are balancing side interferences and clicking sounds at the same time, a larger >300/<300 ratio, med. frequency showed that the interferences and the TMJ clicking sounds were chronic and affected the TMJ vibration.

<P>Identification of the cellular mechanisms that mediate cancer cell chemosensitivity is important for developing new cancer treatment strategies. Several chemotherapeutic drugs increase levels of the posttranslational modifier ISG15, which suggests that ISGylation could suppress oncogenesis. However, how ISGylation of specific target proteins controls tumorigenesis is unknown. Here, we identified proteins that are ISGylated in response to chemotherapy. Treatment of a human mammary epithelial cell line with doxorubicin resulted in ISGylation of the p53 family protein p63. An alternative splice variant of p63, δNp63α, suppressed the transactivity of other p53 family members, and its expression was abnormally elevated in various human epithelial tumors, suggestive of an oncogenic role for this variant. We showed that ISGylation played an essential role in the downregulation of δNp63α. Anticancer drugs, including doxorubicin, induced δNp63α ISGylation and caspase-2 activation, leading to cleavage of ISGylated δNp63α in the nucleus and subsequent release of its inhibitory domain to the cytoplasm. ISGylation ablated the ability of δNp63α to promote anchorage-independent cell growth and tumor formation in vivo as well to suppress the transactivities of proapoptotic p53 family members. These findings establish ISG15 as a tumor suppressor via its conjugation to δNp63α and provide a molecular rationale for therapeutic use of doxorubicin against δNp63α-mediated cancers.</P>
Immunostimulatory Activity of Hibiscus manihot Flower in Mouse Macrophages, RAW264.7 Cells

In this study, we investigated whether Hibiscus manihot flowers (HMF) exhibits immunostimulatory activity in RAW264.7 cells. HMF increased the production of immunostimulatory factors such as NO, iNOS, IL-1β and TNF-α in RAW264.7 cells. TLR2 and TLR4 blocked HMF-mediated production of immunostimulatory factors in RAW264.7 cells. In addition, the inhibition of MAPK signaling pathway reduced HMF-mediated production of immunostimulatory factors. From these results, HMF is thought to promote the production of immunostimulatory factors through activating TLR2/4/MAPK signaling in macrophages. It is believed that HMF can be developed as an agent related to immune enhancement in the future.

<P>Methylenetetrahydrofolate reductase (<I>MTHFR</I>), a critical enzyme in folate metabolism, plays an important role in DNA methylation. It has been suggested that abnormal DNA methylation contributes to the pathogenesis of schizophrenia and congenital anomalies. The previous findings regarding the genetic relationship between <I>MTHFR</I> and schizophrenia are controversial. This study investigated the association of the two functional polymorphisms of <I>MTHFR</I>, C677T and A1298C, with the risk for schizophrenia. Furthermore, we conducted an updated meta-analysis on the two polymorphisms. In addition, we investigated the relationship between the polymorphisms and minor physical anomaly (MPA), which may represent neurodevelopmental aberrations in 201 schizophrenia patients and 350 normal control subjects. There was no significant association between either of the two polymorphisms and the risk of schizophrenia (chi-square = 0.001, df = 1, <I>P</I> = 0.971 for C677T; chi-square = 1.319, df = 1, <I>P</I> = 0.251 for A1298C). However, in meta-analysis, the C677T polymorphism showed a significant association in the combined and Asian populations (OR = 1.13, <I>P</I> = 0.005; OR = 1.21, <I>P</I> = 0.011, respectively) but not in the Korean and Caucasian populations alone. Neither polymorphism was associated with MPAs measured by the Waldrop scale (chi-square = 2.513, df = 2, <I>P</I> = 0.285). In conclusion, the present findings suggest that in the Korean population, the <I>MTHFR</I> polymorphisms are unlikely to be associated with the risk for schizophrenia and neurodevelopmental abnormalities related to schizophrenia.</P>

Domestic companies supplying electricity must increase obligatory duty to use renewable energy annually. If not met with obligatory allotment, the electricity-supply companies must pay RPS(Renewable Portfolio Standards) penalty. Although the power plants using a pulverizing coal firing boiler could co-fire up to around 3 per cent with wood pellets mixed in with coal feedstock without any major equipment revamps, they recorded only about 60 per cent fulfillment of RPS. Consequently, USD 46 million of RPS penalty was imposed on the six power supplying subsidiaries of GENCOs in 2014. One of the solutions to reduce the RPS penalty is that the power supply companies adopt the co-firing of torrefied lignocellulosic biomass in coal plants, which may contribute to the use of over 30 per cent of torrefied biomass mixed with bituminous coals. Extra binder was required to form pellets using torrefied biomass such as wood chips, PKS (Palm Kernel Shell) and EFB (Empty Fruit Bunch). Instead of corn starch, 30, 50 and 70 per cent of Larix saw dusts were respectively added to the torrefied feedstocks such as Pinus densiflora chips, PKS and EFB. The addition of saw dusts led to the decrease of the calorific values of the pellets but the forming ability of the pelletizer was exceedingly improved. Another advantage from the addition of saw dusts stemmed from the reduction of ash contents of the pellets. Finally, it was confirmed that torrefied oil palm biomass such as PKS and EFB could be valuable feedstocks in making pellets through improved binding ability.
