        광선추적과 스펙트럼에 대한 교사와 중학생의 개념 유형 분석

        본 연구의 목적은 중학교 교사와 학생들을 대상으로 광선추적과 스펙트럼에 대한 교사와 학생의 개념 유형을 비교하는 것이다. 본 연구에서는 7학년 '빛 단원에서 가장 중요하다고 생각되는 핵심개념은 광선추적과 스펙트럼에 의해 파악할 수 있다고 전제하고, 이에 대한 교사와 학생의 개념 유형을 조사하였다. 연구 대상은 서울ㆍ경기지역 과학교사 10명과 이들에게 배운 학생 328명이다. 모든 검사문항은 광선추적법과 스펙트럼 방법을 적용하는 문항으로 구성하였다. 연구결과 광선추적과 스펙트럼에 대한 교사와 학생들의 개념유형에는 차이가 있었다. 또한 대부분의 교사와 학생들은 상이 생기는 근본원리에 대하여 정확하게 알지 못하였다. 광선추적법을 알지 못하여 반사와 굴절에 의한 상을 찾을 때, 물체에서 나온 두 개 이상의 광선을 그려서 상을 찾는 경우는 거의 없었고, 하나의 광선으로 임의의 위치에서 상을 찾거나 평소 암기하고 있던 상의 위치를 표시하였다. 그리고 색에 대하여는 교사와 학생들은 개념을 이해하지 못하고, 교사들은 학생들에게 원리에 대한 설명 없이 현상을 제시하였다. 즉 교사와 학생들은 색에 대해서 단순 암기하고 있었다. The purpose of this study is to understand the types of teacher and student's concept on ray-tracing and spectrum in middle school. This study suggest key-concepts that is most important to "Light" unit in the seven grades : the ray-tracing and the spectrum. We examined the types of teacher and student's concept. We selected 10 teacher and 328 students who had learned by 10 teachers in Seoul and Gyeonggi. The problems were composed with the question item regarding all ray-tracing or the spectrum. From the analysis, it was found that the types of teacher and student's concept on ray-tracing and spectrum is different. The most of teacher and student didn't understand the basic principle of image formation. In case of context about formation of image by reflection and refraction, because they don't know ray-tracing, they do not try to find the position of the image by drawing two rays. Most of them used one ray comes from the one position of the object and indicated the position of image by memorized position. Also almost there was not a case which uses a ray tracing accurately. In the case of understanding color, they didn't understand the concept of color(or spectrum) and teachers represented to students the color phenomena without explanation of principles. In the result teachers and students would learn color science by rote.

      • Cutaneous Mycobacterium massiliense Infection associated with bee venom acupuncture

        Bee venom acupuncture is a variant of acupuncture technic of folk medicine using bee sting. Despite controversies surrounding its effectiveness, various adverse events including nontuberculous mycobacterium (NTM) skin and soft tissue infection has been reported. Mycobacterium massiliense is a rapid growing nontuberculous mycobacterium (NTM) that is closely related to the M. abscessus/chelonae group. It is usually ubiquitous in soil and water, but increasing reports of its isolation in human infection arouse attention for it. A 37-year-old woman presented with tender erythematous indurated patches on lateral side of right knee, which was developed following a bee venom acupuncture procedure. Biopsy revealed chronic granulomatous inflammation with abscess and subsequent pus culture revealed Mycobacterium massiliense growth. The patient treated with 6 months of oral clarithromycin and 14 days of intramuscular amikacin and did not show recurrence afterward. Herein, we report a case of Mycobacterium massiliense granulomatous infection following a bee venom acupuncture procedure.

        Lichen amyloidosis is a type of primary localized cutaneous amyloidosis characterized by multiple pruritic discrete hyperkeratotic papules commonly seen on extensor surfaces of legs. Various therapeutic modalities include topical and intralesional steroid, Oral retinoic acid, topical DMSO, UVB have been employed. Although beneficial effects of oral retinoic acid on lichen amyloidosis were reported, topical retinoic acid has not been reported to be effective in the treatment of lichen amyloidosis. We report a 50-year-old woman with lichen amyloidosis; severely itchy brownish papuloplaques of 2-3 months duration on both shins. She was suffering from liver cirrhosis due to hepatitis B infection, and was being treated with tenofovir. The cutaneous lesions improved after four times of intralesional injections of triamcinolone, but there were residual lesions and pruritus. Due to her underlying liver cirrhosis, treatment with oral retinoic acid was not a valid option. We therefore tried retinoic acid topically on the lesions focusing on the anti-inflammatory and improvement effect on hyperpigmentation of topical retinoic acid. After treatment with topical retinoic acid, the patient showed improvement of residual lesions and pruritus was almost disappeared. We hereby report a case successfully treated with topical retinoic acid in a patient with lichen amyloidosis

      • A case of accessory tragus on buccal area

        Accessory tragus is a congenital anomaly of the external ear arised from the abnormal development of the auricular tubercles or from the tissues surrounding the second, third and fourth branchial clefts. It presents in males and females equally with a prevalence of approximately 2-6 per 1000 live births. Clinically, this anomaly is usually presented as a small skin-colored tag or nodule near the tragus, but rarely, along an imaginary line drawn back from the tragus to the angle of the mouth, or along the anterior edge of the sternocleidomastoid muscle and the sternoclavicular region. The diagnosis is made by the age of onset, site of localization, and affirmative histopathology. We report a 12-month-old male who congenitally presented with a solitary flesh colored pedunculated nodule on the left buccal area. Shave biopsy was performed on this lesion. Histologically there are numerous follicles, prominent connective tissue framework in the sub cutaneous fat and central core of cartilage. It diagnosed as accessory tragus and the patient is under observation without complications. Herein, we report a case of accessory tragus arising in buccal area as an interesting example.

      • Adult-onset lichen striatus versus adult blaschkitis from clinicopathological review of adult-onset blaschkolinear dermatosis

        Background: Since the first description of a case of adult blaschkitis by Grosshands in 1990, the existence of this entity has been a matter of great debate. Furthermore, although there were many arguments to distinguish adult blaschkitis from lichen striatus, clear distinction between the two diseases lacks evidence. Objectives: We aimed to conduct a clinicopathological review of all adult-onset blaschkolinear dermatosis in our institution and to provide readers a more updated understanding of these conditions especially focusing on comparing lichen striatus and adult blashkitis. Methods: We retrospectively reviewed the clinicopathological features of all patients who clinically showed linear dermatosis along Blaschko’s lines. Results: After clinicopathological review, 27 cases of lichen striatus, three of adult blaschkitis, eight of linear lichen planus(LPP), and two of linear psoriasis were identified. The differences between lichen striatus and adult blashkitis were insignificant except for age at onset and multiple site involvement. The mean age at onset was younger in lichen striatus and multiple site involvement was more frequent in adult blaschkitis. Conclusion: Considering their overlapping features, the distinction between lichen striatus and adult blaschkitis is not necessary. Therefore, we recommend that clinicians only use the term lichen striatus when describing acquired inflammatory linear dermatosis along Blaschko’s lines after careful exclusion of LLP and linear psoriasis.

        Tumor Necrosis Factor-Alpha Inhibitor-Associated Psoriatic Alopecia in a Patient with Ulcerative Colitis: A Case Report and Review of the Literature

        Paradoxical reactions in patients treated with tumor necrosis factor-alpha inhibitors (TNFis) have an estimated prevalence of 1.5% to 5%. Such reactions usually present as psoriasiform eruptions on the trunk and extremities along with palmar and flexural involvement. When affecting the scalp, new-onset psoriasis induced by TNFi can result in non-scarring or scarring alopecia. Although the paradoxical reaction was first reported in 2003, this TNFi-associated psoriatic alopecia (TiAPA) has been recently reported with increasing frequency. This condition is characteristically reversible and requires clinical and histopathological identification from other diseases for proper treatment. The cessation of TNFi therapy may not be mandatory, and decision to continue TNFi therapy depends on the severity of TiAPA and the riskbenefit ratio of treatment modification on the underlying disease. Herein, we report a case of TiAPA in a patient with inflammatory bowel disease whose alopecia improved following suspension of TNFi.       • Granular cell tumor of lip, a rare tumor in children

        Granular cell tumor(GCT) is a rare soft tissue neoplasms that derived from Schwann cells. Although it mostly benign, it may be malignant in approximately 2% of the cases. GCT often manifests as a single, painless nodule less than 3 cm in size that shows a slow enlargement in the cutaneous, subcutaneous or submucosal tissues. GCT can occurs anywhere in the body, but has predilection for the head and neck area, most commonly on the tongue. It mostly affects adults between ages 30 and 60 years, and is very rare in children. A 12-year-old female presented with a solitary, non-tender, firm, 2x2 cm sized palpable mass on the lower lip. About several years ago, the mass developed and has gradually grown. Histopathological examination revealed an ill-defined dermal nodule composed of large cells with abundant eosinophilic granular cytoplasm. Immunohistochemical study was positive for S-100 protein and CD68. Based on these findings, we diagnosed this neoplasm as GCT on the lip in a child.

      • A case of Intradermal nevus clinically mimicking a tick engorged with blood

        Intradermal nevus is a common benign skin tumor presents as a flesh-colored soft papule usually because nevus cells located in the dermis often lose their capacity to produce melanin. However, intradermal nevus may show diverse clinical manifestations to such an extent as to confuse it clinically from other diseases when the nevus presents in a non-typical area with an unusual shape. Ticks are small arthropod living by sucking mammalian blood. Ticks occasionally stick to human during outdoor activities and become engorged after sucking blood. Axilla or inner thigh area is frequently involved site of tick bite. When fully engorged, the tick looks like a pedunculated pigmented nodule, which can be mistaken as strangulated lesion of pyogenic granuloma, soft fibroma, or intradermal nevus with stalk. Herein, we report an interesting case of intradermal nevus mimicking shape and color of an engorged tick. A 55-year-old man presented with a dusky dark brown to black colored smooth surfaced, pedunculated nodule on perianal area. Clinically, it was not typical for intradermal nevus and resembled a tick which is fully engorged after sucking blood. Biopsy revealed a well differentiated intradermal nevus with pigmented nevus cell in upper dermis.

        Extraocular Sebaceous Carcinoma in situ Concurrent with Actinic Keratosis: A Case Report and Review of the Literature

        Sebaceous carcinoma is a malignant neoplasm frequently classified as periocular or extraocular. Extraocular sebaceous carcinoma is a relatively rare tumor commonly appearing in the head and neck region. Furthermore, extraocular sebaceous carcinoma limited to the epithelium, which is called ‘extraocular sebaceous carcinoma in situ’ is extremely rare with less than 10 cases reported in the literature. Herein, we present the case of a 93-year-old female who diagnosed as extraocular sebaceous carcinoma in situ occurring concurrently with actinic keratosis. In the present case, extraocular sebaceous carcinoma in situ as identified without any connection to an adnexal component but concurrent with actinic keratosis. This case is evidence that extraocular sebaceous carcinoma can arise from the epidermis and the tumor cells of intraepidermal squamous neoplasia can differentiate toward malignant sebocytes.       • Cutaneous plasmacytosis: generalized form versus localized form

        Cutaneous Plasmacytosis(CP) is a rare skin disorder, only seven cases has been reported in Korea. It is characterized by an infiltration of mature plasma cells in skin and other various organ systems like lymph nodes, bone marrow. Therefore, some authors even named this disease entity as ‘Cutaneous and systemic plasmacytosis’. Laboratory tests may show polyclonal hypergammaglobulinemia. The cutaneous manifestation is characterized by round or oval, red-brown to dark brown poorly circumscribed macules, patches, nodules mainly on the face, nape, trunk, and axillae in adults. Histologically, there is dense, superficial and deep perivascular and periadnexal infiltrate of mature plasma cells without atypia. Etiology and pathogenesis are not known, and given the rarity of CP, most experience with treatment response is derived from case reports without consensus on treatment methods. Most CPs feature multiple skin lesions(generalized form), but sometimes they show localized distribution(localized form), so we would like to report by comparing two cases of generalized form CP and other two cases of localized form CP, based on clinical and laboratory differences.
