We investigated the therapeutic potential of bone marrow-derived mesenchymal stem cell-conditioned medium (BMSC-CM) on immortalized renal proximal tubule epithelial cells (RPTEC/ TERT1) in a fibrotic environment. To replicate the increased stiffness characteristic of kidneys in chronic kidney disease, we utilized polyacrylamide gel platforms. A stiff matrix was shown to increase α-smooth muscle actin (α-SMA) levels, indicating fibrogenic activation in RPTEC/TERT1 cells. Interestingly, treatment with BMSC-CM resulted in significant reductions in the levels of fibrotic markers (α-SMA and vimentin) and increases in the levels of the epithelial marker E-cadherin and aquaporin 7, particularly under stiff conditions. Furthermore, BMSC-CM modified microRNA (miRNA) expression and reduced oxidative stress levels in these cells. Our findings suggest that BMSC-CM can modulate cellular morphology, miRNA expression, and oxidative stress in RPTEC/TERT1 cells, highlighting its therapeutic potential in fibrotic kidney disease.
SIR 모형과 LSTM 기반의 COVID-19 확진자 예측 비교 및 분석

2019년 12일 중국 후베이성 우한 시에서 시작된 코로나바이러스감염증-19(이하 COVID-19)은 2020년 1월부터 전 세계로 퍼져, 일부 국가 및 지역을 따라 제외한 대부분의 나라와 모든 대륙으로 확산되었다. 이에 따라 WHO는 2020년 1월 30일 국제적 공중보건 비상사태(PHEIC)를 선포한 후, 2020년 3월 11일 범 유행 전염병(Pandemic)을 선언하였다. COVID-19의 영향은 생명 손실에 국한되지 않고, 사회경제적 영역으로 불안 등을 감소시키기 위해 전염병 확산에 대한 예측은 필수 불가결 하다. 본 논문은 이처럼 최근 유행하고 있는 전염병인 COVID-19에 대해 전염병 모형과 순환 신경망(Recurrent Neural Network) 두 기법을 통해 확산을 예측하고 결과를 비교한다. 전 세계의 국가별 확진자와 사망자 통계를 활용하여 예측 결과를 도출하였다. 전염병 모형을 통한 예측에는 SIR(Susceptible-Infected-Recovered) 모형을 사용하며, 기계 학습의 최적화 알고리즘 중 L-BFGS를 사용하여 SIR 모형의 곡선 적합을 진행하였다. RNN 기반 예측 모델로는 LSTM 모델을 사용하였다. 예측을 실시하여 각 기법의 결론을 도출하고 실제 확진자 통계와의 정확도를 비교하였다. 본 논문은 특정한 나라들에 대하여 전염병 초기의 SIR 모형을 사용한 예측과, 현재(2020.5.29)까지 진행된 상태에서의 예측을 비교한다. 두 예측의 오차를 통하여 방역 수준에 대한 추론을 제시한다. 추가 확진자와 누적 확진자의 관계 그래프를 제공, 기울기와 두 예측 그래프 사이의 오차와 관계성을 확인하고 대략적 방역 수준을 정의한다.

<P>Following advances in robotic rehabilitation, there have been many efforts to investigate the recovery process and effectiveness of robotic rehabilitation procedures through monitoring the activation status of the brain. This work presents the development of a two degree-of-freedom (DoF) magnetic resonance (MR)-compatible hand device that can perform robotic rehabilitation procedures inside an fMRI scanner. The device is capable of providing real-time monitoring of the joint angle, angular velocity, and joint force produced by the metacarpophalangeal (MCP) and proximal interphalangeal (PIP) joints of four fingers. For force measurement, a custom reflective optical force sensor was developed and characterized in terms of accuracy error, hysteresis, and repeatability in the MR environment. The proposed device consists of two non-magnetic ultrasonic motors to provide assistive and resistive forces to the MCP and PIP joints. With actuation and sensing capabilities, both non-voluntary-passive movements and active-voluntary movements can be implemented. The MR compatibility of the device was verified via the analysis of the signal-to-noise ratio (SNR) of MR images of phantoms. SNR drops of 0.25, 2.94, and 11.82% were observed when the device was present but not activated, when only the custom force sensor was activated, and when both the custom force sensor and actuators were activated, respectively.</P>
Splenosis Mimicking Recurrence of Renal Cell Carcinoma after Radical Nephrectomy

Splenosis refers to the heterotropic autotransplantation of splenic tissue. Sometimes splenosis after surgical resection is difficult to differentiate from recurrence or metastasis of cancer. A 49-year-old male patient was diagnosed with clear cell renal cell carcinoma of left kidney. As there was no evidence of metastasis, he underwent radical nephrectomy with splenectomy. On surveillance computed tomography, masses at nephrectomy site and pleura were found and both were initially considered to be recurrence. After several cycle of pazopanib administration, pleural mass decreased in size while mass at nephrectomy site did not respond at all. Spleen scan showed increased uptake of the mass and therefore the mass was revealed to be splenosis. To avoid unnecessary treatment and planning optimal treatment, considering the possibility of splenosis is important and spleen scan can be helpful.

Objective : This study is a retrospective cost-benefit analysis of cervical anterior interbody fusion and cervical artificial disc replacement, which are the main surgical methods to treat degenerative cervical disc disease. Methods : We analyzed 156 patients who underwent anterior cervical disc fusion and cervical artificial disc replacement from January 1, 2008 to December 31, 2009, diagnosed with degenerative cervical disc disorder. In this study, the costs and benefits were analyzed by using quality adjusted life year (QALY) as the outcome index for patients undergoing surgery, and a Markov model was used for the analysis. Only direct medical costs were included in the analysis; indirect medical costs were excluded. Data were analyzed with TreeAge Pro $2015^{TM}$ (TreeAge Software, Inc, Williamstown, MA, USA). Results : Patients who underwent cervical anterior fusion had a total cost of KRW 2501807/USD 2357 over 5 years and obtained a utility of 3.72 QALY. Patients who underwent cervical artificial disc replacement received 4.18 QALY for a total of KRW 3685949/USD 3473 over 5 years. The cumulative cost-effectiveness ratio of cervical spine replacement surgery was KRW 2549511/QALY (USD 2402/QALY), which was lower than the general Korean payment standard. Conclusion : Both cervical anterior fusion and cervical artificial disc replacement are cost-effective treatments for patients with degenerative cervical disc disease. Cervical artificial disc replacement may be an effective alternative to obtain more benefits.
