        It is important to examine patients’ subjective evaluation as well as objective measures and clinician’s rating to assess voice disorders. This study aimed to evaluate validity and reliability of Korean-version of Voice Handicap Index (KVHI) and Voice-Related Quality of Life (KVQOL) with 113 adults with voice disorders and 111 normal adults. Content validity was verified by three experienced speech-language pathologists. Concurrent validity was revealed by examining the correlation among KVHI, KVQOL, and Voice Rating Scale as well as item discrimination coefficients. Total scores of KVHI and KVQOL of adults with voice disorders were significantly different from those of normal adults. Test-retest reliability and internal consistencies were significantly high in both KVHI and KVQOL. Correlations among scores of each subscale and total score were also significantly high in each tool. The study revealed that KVHI and KVQOL are suitable tools to be used in clinics and research areas in Korea, which can subjectively evaluate the effects of voice disorders on daily life as well as on quality of life.

        한국 언어재활사의 음성피로 및 음성과 관련된 삶의 질

        배경 및 목적: 한국 언어재활사의 직업적 음성사용실태를 파악하고, 음성건강 증진 방안을 모색하기 위해 언어재활사의 음성피로도 검사와 음성과 관련된 삶의 질 검사의 점수를 비교하였다. 방법: 주당 20회기 이상 치료하는 언어재활사 137명(남 20명, 여 117명)에게 온라인이나 오프라인으로 음성과 관련된 설문과 K-VFI, K-VQOL을 실시하고 그 내용을 분석하였다. 연구에 참여한 언어재활사의 연령은 20대가 79명, 30대가 46명, 40대 이상이 12명이었다. 결과: 언어재활사의 근무기관 유형, 주당 치료 회기 수, 경력, 주관적인 음성문제 여부에 따른 음성피로도 검사(K-VFI)와 음성과 관련된 삶의 질 검사(K-VQOL)의 점수를 비교한 결과 주관적인 음성문제가 있는 집단은 주관적인 문제가 없는 집단에 비해 K-VQOL-P (신체)-F (기능), K-VQOL–S (사회)-E (정서), K-VQOL-T (총점), K-VFI-F (피로), KVFI-P (신체), K-VFI-R (휴식), K-VFI-T (총점)의 점수가 유의하게 더 높았다(p≤.001). 또한, 주관적 음성증상, 음성오남용, 음성피로지수 및 음성과 관련된 삶의 질 간의 유의한 상관(p≤.001)이 나타났다. 근무기관 유형에 따른 주당 치료 회기 수는 20-29회기를 치료하는 경우가 43.8%, 30-39회기가 37.2%, 40회기 이상 치료하는 경우가 19%이었으며, 40회기 이상 치료하는 언어재활사 중 가장 높은 비율을 차지한 근무기관은 병원(10.9%)이었다. 논의 및 결론: 직업적 음성사용으로 인한 언어재활사의 음성피로와 음성과 관련된 삶의 질 증진을 위해서는 주관적 음성문제에 영향을 미치는 음성오남용을 줄일 뿐 아니라 음성오남용에서 비롯되는 신체적 문제를 개선시킬 수있는 적극적인 자가 음성관리프로그램의 적용이 필요하다. Objectives: The purpose of this study was to investigate the vocal fatigue, voice-related quality of life, and their related factors in Korean speech-language pathologists (SLPs) for the improvement of voice health. Methods: A total of 137 SLPs (male 20, female 117) responded to an online or offline survey and then the data were analyzed. The respondents worked 20 sessions per week. Seventy-nine of the participants were in their 20s, 46 in their 30s, and 12 in their 40s and above. Results: SLPs who had subjective problems showed significantly higher K-VQOL-physical functional, K-VQOL-social-emotional, K-VQOL total, KVFI-fatigue, K-VFI-physical, K-VFI-rest, and K-VFI-total (p≤ .001). Additionally, K-VQOL and KVFI based on workplace, sessions per week, and career were not significantly different. There was a significant correlation (p ≤ .000) between subjective voice symptoms, voice abuse and misuse, voice fatigue index and voice-related quality of life. The type of workplace where the highest percentage of SLPs worked for over 40 sessions was the hospital (10.9%). Conclusion: To reduce vocal fatigue and to improve voice-related quality of life of SLPs, vocal abuse and misuse that may affect voice problems should be eliminated and also self voice care need to be implemented in their daily life. These findings may help SLPs improve not only voice health but also overall life satisfaction and job satisfaction.

        음성장애 관련 자기보고식 평가도구의 최신 동향

        The patient-oriented evaluation methodology increases the importance of self-reporting evaluation tools measuring the voice-related quality of life for the voice disorder patients. The present study was performed to investigate a current trend of self-reporting evaluation tools related with voice and voice and/or by literature review. By using Pubmed, one of the most frequently used electronic journal database, articles related with self-reporting evaluation tools for the voice disorders patients from 2009 to 2019 were searched. Among them, 113 articles related with existing self-reporting evaluation tools (adapted with specific languages, comparison and/or correlation study among the tools, and statistic analysis) and articles of newly developed tools were selected. 94 among 113 articles were related with existing self-reporting evaluation tools, and 19 articles were related with newly developed tools. 67 among 94 articles related with existing tools reported the adapted version of specific languages; 11 articles, comparison and/or correlation study with other self-reporting tools; 16 articles, the statistic analysis results of the tools (eg., reliability, validity). Articles of newly developed tools includes self-reporting tools for the older adults, ones using the concepts of clinical psychometry, ones related with vocal fatigue, and ones for their own native peoples. The focus of self-reporting evaluation tools have changed into more various ages and more specific domains with newly introduced concepts. With these results, more comprehensive and characterized self-reporting evaluation tools for Korean voice disorder patients should be developed.

        연기 및 연기관련 전공 대학생간 삶의 질 차이 연구 : 한국판 음성장애지수(K-VHI)와 음성관련 삶의 질(K-VRQOL)을 중심으로

        직업적 음성사용자에 대한 음성평가도구는 음성이 삶의 질적인 측면에 미치는 영향을 잘 반영할 수 있어야 한다. 이 연구의 목적은 연기 및 연기 관련 분야를 전공을 하고 있는 대학생을 대상으로 한국판 음성장애지수(K-VHI)와 한국판 음성관련 삶의 질(K-VRQOL)을 이용한 삶의 질적인 차이를 살펴봄과 동시에 삶의 질과 관련된 평가도구인 K-VHI와 K-VRQOL간의 상관관계를 살펴보고자 하였다. 연구 대상자는 연기 및 연기관련 분야를 전공하고 있는 대학교 1-2학년에 재학중인 97명(연기전공 39명, 무대전공 17명, 연출전공 41명)이었다. 97명의 연구 대상자에게 K-VHI 및 K-VRQOL 검사지를 배부하기 전 본 검사의 취지와 주의사항등을 설명하였고, 두 검사지를 배부한 후 각 문항에 대한 설명을 듣게 한 후 질문에 답하도록 요구하였다. 연기 및 연기관련 전공에 따른 K-VHI와 K-VRQOL의 차이를 살펴보기 위해서 일원분산분석(One way ANOVA)을 실시하였고, K-VHI와 K-VRQOL간의 상관관계를 이변량 상관분석을 통해 살펴보았다. 그 결과, K-VHI에서는 기능적 영역에서 세 집단간 차이가 있었으며[F(2,94)=6.307, p<.05], K-VRQOL에서는 세 집단간 유의한 차이가 있었다[F(2,94)=4.181, p<.05]. K-VHI와 K-VRQOL간 Pearson 상관계수가 0.811로 통계적으로 유의하였으며, 강한 양적 선형관계가 있었다. 그럼에도 불구하고 전공에 따라 겪는 주관적 음성의 인식정도가 하위 영역별로 차이가 있으므로, K-VHI와 K-VRQOL의 결과를 포괄적으로 이해하고 활용하는 것이 바람직할 것으로 사료된다. Voice assessment tools for professional voice users should be able to reflect the impact of one’s voice on quality of life. The purpose of this study is to examine the differences in quality of life using the Korean version of the voice handicap index (K-VHI) and the Korean version of voice-related quality of life (K-VRQOL) assessment tool for college students majoring in acting and acting-related fields. We simultaneously examined the correlation between the K-VHI and K-VRQOL. The study had 97 participants (39 acting majors, 17 stage majors, and 41 director majors), who were students in the first and second year of the university majoring in acting and acting-related fields. The purpose of and precautions in this test were explained before distributing the K-VHI and K-VRQOL tests to 97 participants. After receiving the two questionnaires, the participants were provided with an explanation of each question and asked to answer each item. A one-way ANOVA was conducted to examine the differences between the K-VHI and K-VRQOL with regard to the majors in acting and acting-related fields. The correlation between the K-VHI and K-VRQOL was examined through bivariate correlation analysis. There were significant differences among the three groups in the functional domain of the K-VHI [F (2,94) = 6.307, p<.05], and significant differences among the three groups in the K-VRQOL [F (2,94). = 4.181, p<.05]. The Pearson’s correlation coefficient between the K-VHI and K-VRQOL was 0.811, which was statistically significant, and there was a strong quantitative linear relationship. Even so, subjective speech perception differs according to one’s major, and it is desirable to comprehensively understand and utilize the results of the K-VHI and K-VRQOL.

        여성 노인 합창단원의 합창단 유형에 따른 청지각적 음성평가(GRBAS) 및 음성관련 삶의 질(K-VRQOL) 비교

        본 연구의 목적은 음성의 청지각적 평가도구(GRBAS)와 음성관련 삶의 질(K-VRQOL) 척도를 통해 합창활동에 참여하는 여성 노인의 음성 특성과 음성관련 삶의 질을 비교하는 것이다. 연구 대상은 서울 및 부산 소재의 합창단에서 활동 중인 만 60세 이상의 여성 노인으로 총 77명이었다. 합창단은 참여 유형에 따라 합창단(Regular choir)과 찬양단(Church choir) 두 개의 집단으로 분류하였다. 청지각적 음성평가는 /a/ 모음을 발성하는 음성을 듣고 전문가가 청지각적 평가(GRBAS) 척도를 사용하여 평정하였다. 연구 결과, 합창활동 참여 유형에 따라 집단 간 차이를 비교했을 때 찬양단에서 활동하는 여성 노인에 비해 합창단에서 활동하는 여성 노인의 경우 주관적 음성 인식 수준에서 대화 시 음성 사용 만족도가 높은 것으로 나타났다. 또한, 음성관련 삶의 질(K-VRQOL) 척도의 신체 기능 영역에 해당하는 문항에서 만족도가 높은 것으로 분석되었다. 본 연구는 합창활동이 노년기 음성기능의 개선뿐 아니라 음성사용의 주관적 인식 수준을 향상시키는데 긍정적인 결과를 기대할 수 있을 것이라는 점을 확인하였으며, 노인 음성개선을 위한 체계적인 음악 중재 프로그램의 필요성을 시사하고 있다. The purpose of this study is to compare voice characteristics and voice-related quality of life (K-VRQOL) of the elderly female choir members using perceptual-auditory voice quality evaluation (GRBAS) and K-VRQOL scales. The participants were 77 women over 60 years old who were actively engaged in the choir in either Seoul or Busan. There are two kinds of choirs that indicate different engagement levels: regular choir and church choir. The perceptual-auditory vocal quality evaluation was listened to by / a / vowels and were graded by experts using the GRBAS scale. As a result, when comparing the differences between groups, the elderly female participants of the regular choir showed higher satisfaction in speech using the subjective speech recognition level than the elderly female members who performed in the church choir. In addition, the analysis showed that the satisfaction level was high in the physical function area of the K-VRQOL scale. This study confirmed that choral activities could yield positive results not only in terms of improving voice function in old age, but also to improve the subjective perception level of voice use, thus suggesting the necessity of systematic music programs to improve voices that are aging.

        음성장애인의 자가음성평가와 정상음성인의 청자음성평가 특성 비교

        This paper will discuss the difference between self assessment of voice disorders and the hearer voice assessment of a comparative group of normal subjects. The study was conducted on 25 voice disorder subjects and 32 hearers of a comparative group of normal subjects. The results are as follows. Firstly, in K-VHI and VHI-H, the hearers of the comparative group of normal subjects perceived more serious voice disorders than the voice disorder group in all sub-domains. Likewise, in K-VQOL and VRQOL-H, the hearers of the comparative group of normal subjects perceived more serious voice disorders than the voice disorder group in all sub-domains. Secondly, the hearer voice assessment of the comparative group of normal subjects showed no difference in gender regarding the perception of the severity of voice disorder issues. Thirdly, the hearer voice assessment of the comparative group of normal subjects states that in the emotional aspects of VHI-H, professional voice users perceive more serious voice disorders than others. Accordingly, in VRQOL-H, there was no difference in use of the voice between professionals and others.

        음성장애가 있는 직업적 음성사용자와 비직업적 음성사용자의 음성장애 중증도와 유형에 따른 자기보고식 음성평가 차이

        The purpose of this study was to compare professional (Pro) and non-professional (Non-pro) voice users with voice disorders in self-reporting voice evaluation using Korean-Voice Handicap Index (K-VHI) and Korean-Voice Related Quality of Life (K-VRQOL). In addition, those were compared by voice quality and voice disorder type. 94 Pro and 106 Non-pro were asked to fill out the K-VHI and K-VRQOL, perceptually evaluated on GRBAS scales, and divided into three types of voice disorders (functional, organic and neurologic) by an experienced speech-language pathologist and an otolaryngologist. The results showed that the functional (F) and physical (P) scores of K-VHI in Pro group were significantly higher than those in Non-pro group. As the voice quality evaluated by G scale got worse, the scores of all aspects except emotional (E) of K-VHI and social-emotional (SE) of K-VRQOL were higher. All scores of K-VHI and K-VRQOL in neurologic voice disorders were significantly higher than those in functional and organic voice disorders. In conclusion, professional voice users are more sensitive to their functional and physical handicap resulted by their voice problems and that goes double for the patients with severe and neurologic voice disorders.
