      • [P357] Giant trichoblastoma developed on the calf

        Trichoblastoma is a benign dermal basaloid epithelial and stromal neoplasm that arises from rudimentary hair follicles. The clinical appearance is a skin-colored, well-circumscribed nodule located predominantly in the head and neck and rarely in the trunk, lower extremities. The majority of trichoblastomas are less than 2cm, but giant trichoblastoma can be seen. The recommended treatment is complete surgical excision. We report a case of 49-year-old female patient presenting asymptomatic mass on the right calf. Physical examination revealed a solitary 3x3cm size skin colored dome shaped subcutaneous mass on the right calf. Histopathologic findings revealed a well circumscribed mass of epithelial cell forming various-sized lobular nests. Neoplasm was situated in deeper dermis away from superficial epidermis and there was no artifact cleft. Immunohistochemical studies revealed that tumor stroma was positive for CD10, CD34 and the expression of Bcl-2 was limited to the outermost basaloid cells in tumor nests. CK7, CK20 was negative. Based on these findings, we diagnosed this case as a trichoblastoma. We completely excised the tumor and no evidence of recurrence was observed for 6 months. It is difficult to distinguish trichoblastoma from basal cell carcinoma and trichoepithelioma. Therefore, complete surgical excision of the tumor is recommended. We report a rare clinical variant of trichoblastoma with unusual site, calf, and unusual size.

      • A case of trichoblastoma with plasma cell infiltration

        Trichoblastoma is a rare benign follicular neoplasm, presenting as solitary, dark colored nodule on the head or neck. It can arises within a nevus sebaceous and can also coexist with BCC. Meanwhile, cutaneous plasma cell infiltration can be seen in various disorders, including inflammatory disease and infectious diseases such as syphilis and deep fungal infections. It can also occur with lymph node disease and polyclonal hypergammaglobulinemia. A 53-year-old woman presented with scaly erythematous plaque on left cheek. Skin biopsy specimens were taken by punch biopsy. Smooth-bordered large basaloid proliferation and fibrotic stroma were seen in entire dermis. Interestingly, mixed inflammatory cell infiltrations were seen in upper dermis and they were mainly plasma cells. There was no evidence of other organ involvement and infection in laboratory tests. Preceding unnoticeable infection might independently leave a plasma cell infiltration regardless of trichoblastoma. However, we cannot exclude the possibility of association between trichoblastoma and plasma cell infiltration. She was diagnosed with trichoblastoma with plasma cell infiltration based on histological findings. In our literature review, there was no reported study about the association between plasma cell infiltration and adnexal neoplasm except syringocystadenoma papilliferum. Herein we report a peculiar case of trichoblastoma with plasma cell infiltration.

        In the tumor, several histologic variants have been described including a rippled-pattern variant. The unique finding of rippled-pattern trichoblastoma is a peculiar arrangement of the basaloid cells in linear rows parallel to one another, resembling the palisading of nuclei of Verocay bodies. Only a few case reports of rippled-pattern trichoblastoma have been reported in the English literature, but there has been no report in the Korean literature. Herein we report two cases of rippled-pattern trichoblastoma. (Korean J Dermatol 2007;45(9):934∼937)

        Trichoblastoma is a benign cutaneous neoplasm that consists of follicular germinative cells. There have only been a few case reports describing the malignant counterpart of trichoblastoma. We report trichoblastic neoplasms that show histological features of malignancy. The histological sections revealed locally aggressive neoplasms showing prominent differentiation towards hair germ and follicular sheath with formation of papillary mesenchymal bodies and follicular bulb-like structures. The tumor was widely excised with a clear margin. No sign of recurrence has been noted 1 year later. (Korean J Dermatol 2008;46(1):94∼96)

        Nevus sebaceus is located on the scalp or the face as a single lesion and is already present at birth, various types of appendage tumor may develop secondarily within the lesion of the nevus sebaceus. A syringocystadenoma papilliferum is the most common secondary benign tumor that has been found in 8% to 19% of the lesion of nevus sebaceus. The malignant carcinoma and malignant eccrine poroma. The combination of a variety of tumor was found in the lesion of nevus sebaceus. We report a case of nevus sebaceus in a 66-year-old male associated with trichilemmoma, trichilemmal carcinoma and trichoblastoma. (Korean J Dermatol 2004;42(4):443~446)

        Case Report : A Case of Trichogerminoma

        Trichogerminoma is a rare neoplasm which was first described in 1992 and there is still controversy over its inclusion into the spectrum of trichoblastoma. A 79-year-old woman presented with a 5-year history of an asymptomatic nodule on the left posterior neck. Histologically, the lesion revealed a well-demarcated deep dermal nodule surrounded by a pseudocapsule. The tumor was composed of lobules with basophilic cells and some of the lobules displayed a distinctive pattern of densely packed `cell balls` with peripheral condensation. Immunohistochemically, the tumor cells showed zonal CK5/6 immunoactivity in contrast with the negatively stained `cell balls`. These characteristics were compatible with the diagnosis of trichogerminoma. We report here on a rare case of a hair germ tumor called trichogerminoma. (Ann Dermatol 22(4) 431~434, 2010)

        Nevus sebaceous occurs almost on the scalp and face and shows characteristically well demarcated yellowish patch or plaque. With time, benign or malignant skin tumors secondarily may be developed within nevus sebaceous lesion. We herein report a case of nevus sebaceous superimposed with 4 rare types of tumors; sebaceous epithelioma, syringocystadenoma papillifeum, tubular apocrine adenoma, and trichoblastoma.       • Trichoblastoma accompanied with basaloid follicular hamartoma of the axilla

        Trichoblastoma and basaloid follicular hamartoma (BFH) are uncommon benign neoplasms of follicular germinative cells manifested by combinations of mesenchymal and epithelial cells with various proportions. Clinically, trichoblastoma presents as a single, well-defined, skin colored to brown or blue-black papule or plaque or nodule. It is usually about 0.5-1cm in size and located in the deep dermis or subcutaneous tissue. BFH, on the other hand, show diverse clinical expressions; solitary, localized, linear nevoid, and generalized forms. Predilecting areas of trichoblastoma and BFH are the head or neck, especially the scalp and face in adults. As far as we know, there is no case of trichoblastoma with BFH on the axilla in previous reports. A 64-year old man presented with a painful brownish 1.5 x 1.5 cm sized ulcerative plaque on the right axilla, which occurred 2 years prior. Histopathologic examination revealed lesions at the upper and lower dermis. The lobular aggregation was composed of nests and cords of basaloid cells emanating from the infundibular portion of the hair follicle with peripheral palisading pattern surrounded by fibrous stroma. Focal connection to the overlying epidermis was also observed. Based on clinical and histological findings, we diagnosed trichoblastoma with BFH. The tumor was surgically removed and no evidence of recurrence for 6 months. We report a rare case of the trichoblastoma with BFH on the axilla.

      • A case of epidermal cyst coexistent with trichoblastoma

        A 57-years old female showed an erythematous elevated mass with central opening on her right occipital area. She underwent surgical excision under clinical impression of epidermal cyst. In the middle of operation, a different form of mass was detected at deep dermis and on excised specimen, well-demarcated yellowish mass with overlying cyst close to the epidermis was observed. Histopathologic examination showed a cyst lined by stratified squamous epithelium containing keratin material. Below the cyst, basaloid tumor was observed, which had peripheral palisading. No connection between cyst and tumor was observed. The patient was diagnosed with epidermal cyst coexistent with trichoblastoma and there was no recurrence for 2 years. Hair follicle is divided into 4 parts, and each part can develop into different types of tumor. The cells of each part can be mixed together and form a hybrid tumor. Epidermal cyst and trichoblastoma is a benign neoplasm derived from infundibulum and follicular germ cells respectively. Hybrid tumor which consists of epidermal cyst and trichoblastoma emanating from the cyst wall has been reported. Unlike previously reported tumors, both of epidermal cyst and trichoblastoma were found in upper and deep dermis separately without any connection in our case. Here, we experience a case of epidermal cyst coexistent with trichoblastoma and report with literature review.

      • Trichoblastoma and compound nevus arising from nevus sebaceus

        Nevus sebaceus is a common congenital lesion occurring mainly on the face and scalp. It is well known that secondary neoplasms can be easily observed in the primary lesion. The most frequently reported benign neoplasm associated with nevus sebaceus is trichoblastoma and syringocystadenoma papilliferum. However, the case about trichoblastoma and compound nevus concurrently arising from nevus sebaceus has not been reported yet. A 26-year-old woman presented with a well demarcated skin-colored plaque on her scalp from the birth. On the plaque, blackish elevated round mass and brownish nodule with irregular margin were found a week previously without subjective symptoms. Histopathological examination revealed verrucous epidermal hyperplasia and hyperplasia of the sebaceous glands and eccrine glands. The hair follicles remained small and primordial. Section of the blackish mass showed palisading basaloid cells surrounded by abundant fibrous stroma. Nests of nevus cells were found at the dermoepidermal junction and dermis in the brownish nodule. From these findings, the blackish mass and brownish nodule were diagnosed of trichoblastoma and compound nevus, respectively. Herein we report an interesting case of trichoblastoma and compound nevus concurrently arising from nevus sebaceus, which is the first report in dermatologic literatures to our knowledge.
