        우심실에 생긴 심근내막섬유증에서 시행한 양 방향성 상대정맥-폐동맥 단락 수술

        환자는 44세 남자로 호흡곤란과 동반된 복부팽만을 주소로 입원하였다. 심초음파상 우심방내의 종괴와 삼첨판막 협착증, 우심실내강의 감소를 관찰할 수 있었다. 수술을 시행하여 우심방내의 종괴를 제거하였고 삼첨판막의 개구부를 확장시킴과 동시에 심실내부의 심내막절제술을 시행하였다. 또한 우심방의 용적부하를 낮추기 위해 심방중격결손을 만들어주었다. 술 후 환자는 증세의 호전을 보였고 별다른 문제없이 퇴원하였고 조직검사상 진단은 심근내막섬유증으로 내려졌다. 술 후 18개월 뒤 환자는 진행된 호흡곤란과 청색증으로 인해 다시 입원하였고 심초음파를 시행하여 우심실내강이 이전보다 더 좁아져 있으며 폐동맥의 혈류가 거의 없음을 확인하였다. 기능적 단 심실 상태였기 때문에 양 방향성 상대정맥폐동맥 단락수술을 시행하였고 술 후 호흡곤란과 청색증은 많은 호전을 보였다. 진행된 우심실의 심근내막섬유증에서 양 방향성 상대정맥-폐동맥 단락수술의 시행은 술 후 초기에 효과적인 증상완화를 제공하지만 장기간의 추적관찰이 요구된다.

      • 메니에르병 환자에서 고실내 Gadolinium-diethylen -triamine pentaacetic acid 주입 후 자기공명영상을 통한 내림프공간의 시각화

        Background and Objectives: Endolymphatic hydrops are known as major causes of Meniere’s disease. MRI (Magnetic resonance imaging) with contrast tried recently makes it possible to visualize perilymphatic and endolymphatic space without invasive procedures. There are no tryouts in the interior of our country. We attempted MRI after injection of gadolinium-diethylen –triamine pentaacetic acid (Gd-DTPA) in normal adults and patients with Meniere’s disease to make sure 3D-FLAIR (fluid-attenuated inversion recovery) MRI parameters and to visualize endolymphatic spaces. Materials and Methods: Five normal adults and Five patients with Meniere’s disease were included in this study. Twenty-four hours after Gd-DTPA intratympanic injection, we performed 3D-FLAIR and 3D-IR imaging at 3T. MRI region of interest signal intensity was used to determine the diffusion of Gd-DTPA into the perilymphatic fluid spaces over time. Results: Five of five in normal group, using 3D-IR MRI after Gd injection, had enhanced imagings (perilymphatic spaces) of inner ears. Five of five in patients group, using 3D-IR after Gd injection, had enhanced perilymphatic spaces and non-enhanced endolymphatic hydrops. Conclusions: Delayed contrast imaging of the inner ear with 3D-IR MRI after Gd-DTPA intratympanic injection revealed in vivo visualization of endolymphatic hydrops.         Contrast medium-induced transient neurologic deteriorations following cerebral angiography and coil embolization for unruptured aneurysm -report of two cases-

        Cerebral angiography with contrast medium (CM) is a key method for diagnosis and interventional treatment of intracranial cerebral vascular lesions. Cerebral angiography causes few neurologic complications. There have been rare reports of complications related to disruption of the blood-brain barrier due to the administration of nonionic CM. We observed two patients with transient neurologic complications following cerebral angiography and coil embolization for unruptured aneurysm. (Kor J Cerebrovascular Surgery 10(4):245-9, 2010)

        Expression of α1 Receptor and Nitric Oxide Synthase in Oophorectomized and Estrogen-Supplemented Rat Bladder and Urethra

        Purpose: To investigate the effects of estrogen on the expression of the α1 receptor andnitric oxide synthase (NOS) in rat urethra and bladder after oophorectomy. Materials and Methods: Forty-five mature female Sprague-Dawley rats (aged 10–11weeks, 235–250 g) were randomly assigned to one of three groups: control group, oophorectomygroup (Opx), or oophorectomy and estradiol replacement group (Opx+ Est). The degree of expression of α1 receptor (α1A and D) and NOS (neuronal NOS [nNOS]and endothelial NOS [eNOS]) in bladder and urethral tissues was investigated by usingimmunohistochemical staining and Western blotting. Results: In the bladder, the expression rates of α1 receptor (α1A and α1D) increasedin the Opx group but decreased in the Opx+Est group. These changes were not statisticallysignificant. The α1A and α1D receptor of the urethra decreased in the Opx groupbut increased in the Opx+Est group. These changes were not statistically significant. In the bladder and urethra, the expression rates of nNOS and eNOS significantly increasedin the Opx group but decreased in the Opx+Est group (p<0.05). Conclusions: These data suggest that estrogen depletion increases NOS and α1 receptorexpression in the rat bladder. However, these changes could be restored by estrogenreplacement therapy.

        방사선 치료 후 재발한 결절외변연부세포 림프종

        Extranodal marginal zone B-cell lymphoma of mucosa associated lymphoid tissue (MALT lymphoma) is a low-grade B-cell lymphoma that presents with an indolent clinicopathologic nature. Although this tumor can occur in various sites, including the gastrointestinal, skin, salivary gland and ocular adnexa, radiation therapy shows high local control in that disease. We report a case of recurrent MALT lymphoma presenting a huge cheek mass after radiotherapy on ocular adnexal lymphomas. An 65-year-old man had a history of treatments on MALT lymphomas of ocular adnexa came again with a huge cheek mass, not recurred and not originated from salivary gland, skin, other related mucosa tissues. He got excisional biopsy and was confirmed immunohistochemically as MALT lymphoma. After diagnosis, he was sent urgently to a radiotherapist for further treatment. He responded well to added radiotherapy with 36 Gy dose and has remained well at 6 months after his initial presentation.

        소아 장중첩증 단순복부X선사진의 진단적 신뢰성에 관한 고찰

        1. The plain abdominal films of 246 cases of intussusception preceding hydrostatic barium enema were analysed in order to reassess the diagnostic reliability of the abdominal plain film examination. Then, these were compared with 42 cases in which the diagnosis had been rejected by means of barium enema. 2. It was found that diagnosis of the intussusception was possible in 80% of 246 cases on the basis of plain film findings alone. 3. The most common and reliable findings of intussusception in plain film diagnosis are those of sparse faecoloid bowel content and sparse amount of intestinal gas. 4. The loops of distended small bowel in cecal or ascending colon area and intraabdominal soft tissue mass are considered to be relatively reliable findings. 5. In 42 cases in which the diagnosis had been rejected by means of barium enema, intussusception was excluded in 74% of the cases on the bases of plain film findings alone.
