        대학생의 허리통증에 대한 SNAGS(Sustained Natural Apophyseal Glides)기법이 허리통증과 오스웨스트리 장애지수(KODI)에 미치는 영향

        Background: This study investigated the effects of sustained natural apophyseal glides (SNAGS) technique on lower back pain and pain disability among university students with lower back pain. Methods: A total thirty 20~30 year old university students with lower back pain were divided into two groups. A lumbar flexibility exercise was applied to the control group (n=15). The SNAGS technique group (n=15) received L1~L4 segment mobilization using SNAGS technique. Intervention was implemented for 20 minutes, 3 times a week for 3 weeks. The SNAGS were performed using belt with flexion and extension, whereas lumbar flexibility exercise included lumbar flexion and extension. Lower back disability was measured using the Korean Oswestry disability index (KODI), and pain sensitivity was measured by pain pressure threshold (PPT). The groups were assessed for lower back pain disability and pain, before and after the intervention. Results: There were significant improvements in both the KODI and PPT of the SNAGS technique group (p<.05), while the lumbar flexibility exercise group showed no significant change (p>.05). In addition, the SNAGS technique group had a statistically significant difference in KODI and PPT compared to the lumbar flexibility exercise group (p<.05). Conclusion: SNAGS technique is more effective than lumbar spine joint flexibility exercise in the improvement of lower back pain disability and pain among university students with lower back pain.

      • 종합병원 근무 여성의 요통에 대한 요인 분석

        1991년 2월부터 8월까지 7개월간 영남대학교 의과대학 부속병원에 근무하는 여성 471명을 대상으로 요통 발생에 대한 역학적 설문조사를 실시하고 그 자료를 분석한 결과는 다음과 같았다. 1. 배선원과 조리사 및 교환수에서 요통의 발생빈도가 타 직종에 비해 높았다. 2. 요통의 발생 빈도는 정신 신체 증상의 발현 빈도에 주로 관계하였고 부분적으로는 미는 형태의 작업에 관계하였다. 3. 요통에 의한 근무의 지장정도는 드는 형태의 작업 혹은 육체적 노동량의 증가할수록 증가하였고, 정신 신체증상의 발현빈도 및 근무에 대한 불만의 정도와 관계가 있었다. 4. 사소한 요부 손상 혹은 불안정성은 주로 걷는 작업이 긴 형태 혹은 육체적 노동량이 많을수록 증가하였고, 사무적인 작업에서는 감소하였다. 또한 높은 연령과 밀접한 관계가 있었으며 부분적으로는 정신 신체증상의 발현빈도와 관계가 있었다. 이상의 설문지 조사를 통한 역학적 조사를 시행한 결과 요통의 발생 및 이로 인한 장애는 정신 신체적인 요소와 작업의 형태가 관계하여, 이의 예방 및 치료를 위해서는 작업에 대한 인간공학적인 연구 등을 통한 보다 합리적인 접근법이 요구되고 정신적인 인자에 대한 보다 다각적인 접근이 요구되었다. 또한 요부손상은 연령 등의 생리적 요소가 관계하며 이에 대한 보다 합리적인 대처가 요구 되었다. Background : The aim of this study was to confirm the risk factors for low back pain and injury to improve the prevention and treatment of lower back pain. Materials and Methods : An epidemiologic study of low back pain and injury was performed with questionnaires distributed to 471 women working at Yeungnam university hospital. The differences in low back pain and injuries among various hospital departments were analyzed by a one-way analysis of variance (ANOVA), and the relevant factors included in the questionnaire were determined by a factor analysis. Results : The frequency of low back pain in women in the department of diet and in the maintenance department was higher than among the other departments. The frequency of low back pain was mainly related to the frequency of psychosomatic symptoms. In addition, the low back pain was partially related to the frequency of psychosomatic symptoms and partially related to the frequency of pushing during the workday. The degree of disability from low back pain was increased by lifting and hard physical work and was related to the frequency of psychosomatic symptoms and the degree of work dissatisfaction. The frequency of low back injury was increased by increased standing time during work and hard physical work. The frequency of low back injury was related to advancing age and in part to psychosomatic symptoms. Conclusion : Lower back pain and its associated complications are related to psychosomatic factors and type of work. Lower back injury is related to physiological factors such as age. For the prevention and treatment of lower back pain, a multidisciplinary approach is required.

        이압요법이 여대생의 월경시 요통과 하복부 통증에 미치는 효과

        Purpose: The purpose of this study was to examine the effects of auricular acupressure therapy on back pain and lower abdominal pain in female undergraduate students during menstruation. Methods: The study used a quasi-experimental design. Data was collected from March 15th to June 31st, 2011. The participants were 20 female university students for the experimental group, and 22 female university students for the control group. visual analogue scale (VAS) with the opening records was used to assess back pain and lower abdominal pain. Data was analyzed using the χ²-test, Fisher’s exact test, Mann-Whitney U test, and Wilcoxon signed rank test using the SPSS/WIN 11.0 program. Results: There were statistically significant differences in back pain and lower abdominal pain between the two groups. Auricular acupressure therapy decreased back pain and lower abdominal pain in the experimental group compared to the control group. Conclusion: The study results show that auricular acupressure therapy is effective in improving back pain and lower abdominal pain in female university students during menstruation. Therefore, auricular acupressure therapy can be considered a useful nursing intervention to promote improvements in back pain and lower abdominal pain related to menstruation.

        만성 허리통증에 대한 Kinesio taping 적용이 허리압통과 균형능력에 미치는 영향

        Background: This study investigated the effects of kinesio taping application on lower back pressure pain and balance ability among university students with chronic lower back pain. Methods: A total of thirty university students between 20 to 30 years of age with chronic lower back pain were divided randomly into two groups, the control and the experimental group. In the control group (n=15), placebo taping was applied to the lumbar region. In the experimental group (n=15), kinesio taping was applied to the erector spine muscles of the lower back. The groups were assessed for lower back pressure pain and balance ability, before and after the taping application. Pain was measured by the pain pressure threshold (PPT), and balance was measured using the good balance system (GBS). Results: There were significant improvements in both the PPT and GBS of the kinesio taping group compared to pre-treatment values (p<.05), while the placebo taping group showed no significant change (p>.05). In addition, the kinesio taping group had a statistically significant difference in PPT and GBS compared to the placebo taping group (p<.05). Conclusion: The kinesio taping application is more effective than the placebo taping application in the improvement of lower back pressure pain and balance ability among university students with chronic lower back pain.

        Effect of Kinesio Taping with Magnets on the Pain Level of Patients with Chronic Lower Back Pain

        본 연구의 목적은 자석을 동반한 키네시오 테이핑 적용에 따른 만성 요통환자의 통증 조절 효과를 살펴보는 것이 다. 이를 위하여, 6개월 이상 만성 요통을 호소하는 40∼50대 여성 29명을 대상으로 하였으며, 통제군 14명과 자석을 동반한 키네시오 테이핑을 적용하는 실험군 15명으로 나누었다. 실험군의 자석을 동반한 키네시오 테이핑 처치는 8주간 이루어졌다. 이후, 통증정도를 평가하는 14가지 항목의 VAS(Visual Analogue Scale)를 통해 실험처치 적용 전, 후의 평가가 이루어졌다. 연구결과에서 만성 요통환자에게 자석을 동반한 키네시오테이핑 적용 8주 후에 실험군은 통증정도를 평가하는 총 14가지 전체 항목에서 유의한 감소를 나타냈다(p<.05). 하지만, 통제군에서는 통증정도의 전체 항목에서 유의한 변화가 나타나지 않았다. 본 연구에서 만성 요통환자에게 자석요법을 동반한 키네시오 테이핑 적용에 따른 통증 조절 효과를 확인할 수 있었으며, 만성요통환자의 통증 감소를 위한 새로운 보완요법으로 임상에서 유용하게 쓰이게 되는 계기가 될 것이다. The general morbidity of chronic lower back pain cannot be completely removed via medical treatment. If the purpose of treatment for patients with chronic lower back pain is to consider symptom relief and functional recovery to return to one’s daily and socio-economic routine, moderate sport medicine complementary therapy in real life has become more important. The purpose of study is to investigate the effect of kinesio taping with magnets on the pain level of patients with chronic lower back pain. Twenty nine women in their 40s and 50s who had chronic lower back pain for at least six months were recruited as randomized control group pre test-post test design. Fourteen subjects were allocated to the control group while 15 were assigned to the experimental group, both of which were administered kinesio taping with magnets. The experimental group was treated for 8 weeks. This investigation used Visual Analogue Scale (VAS) containing 14 items to assess pain level before and after treatment. Eight weeks later, pain level assessed based on 14 items decreased significantly in the experimental group treated with kinesio taping with magnets, indicating pain relief. In the control group, there was no significant change. This study demonstrated that kinesio taping with magnetotherapy relieved pain among patients with chronic lower back pain, making it a clinically useful complementary therapy in sport medicine for pain relief among patients with chronic lower back pain

        코어운동장비 기반의 요부강화 운동프로그램이 만성요통 노인의 요부근력에 미치는 효과

        This study was conducted to investigate the effect of back strengthening exercise program using core exercise equipment on the back muscles strength in elderly patients with chronic lower back pain. The subjects of this study were 40 elderly people with chronic back pain who are 70 years or older using Seniors Welfare Centers located in Seoul city and suffer from back and lower limb pains that interrupt daily activities. The change in peak torque of adduction and abduction of back extensor, hip and pelvic was measured after a 12-weeks back strengthening exercise program using core exercise equipment. The back strength showed statistically significant difference by measurement point (105°, 120°), between groups (105°, 120°, 135°, 150°, 165°)and by interaction effect (105°, 120°) at each angle (p<.05). The muscle strength of hip also showed significant difference based on interaction effect (p<.05) and the change of strength during abduction and adduction showed significant interaction effect between the measurement points and groups (p<.05). This study applied the exercise program to the range of motion of pelvic and hip, which is greater than that of those in the back. It extended back exercise treatment to the pelvic and hip muscles, which showed positive effect on chronic lower back pain. As such, the effect of back strengthening exercise program using core exercise equipment would be used to wider area of rehabilitation program of chronic lower back pain with expected positive outcome.

        만성요통 치료를 위한 중재적 요법

        Chronic low back pain has become a major public health problem in Korea. The lifetime prevalence of low back pain has been reported to be as high as 90%. The prevalences of discogenic pain, facet joint pain, and sacroiliac joint pain are 42%, 30%, and 18%, respectively. Increasing age is associated with a decreasing probability of discogenic pain but increasing probabilities of facet joint pain and sacroiliac joint pain as the source of low back pain. Physical examination of low back pain is limited and imaging may lack objective clues. Diagnostic interventions allow us to determine the etiology of back pain. Techniques include lumbar disc stimulation,facet joint block, medial branch block, and sacroilliac joint block. Initial therapy includes modification of activity, pharmacological analgesic therapy, and education of patients. In patients with chronic low back pain, a multimodal and interdisciplinary treatment approach is necessary. Use of interventional treatments for chronic low back pain are increasing but their utilization remains uncertain or controversial. Therefore, management of patients with chronic and disabling low back pain remains a clinical challenge.

        고유수용성신경근촉진법 운동이 만성허리통증환자의 통증과 기능장애지수에 미치는 영향

        Purpose: This study attempts to identify the effects of stretching and core exercise using proprioceptive neuromuscular facilitation (PNF) on the pain and functional disability index of patients with chronic lower back pain. Methods: A total of 20 patients with chronic lower back pain were randomly divided into either the experimental group (n=10), who received PNF stretching and core exercise, or the control group (n=10), who received conventional physiotherapy. Both interventions were applied three times a week for six weeks. The visible analogue scale (VAS) was measured in order to determine the level of pain, while the Oswestry Disability Index (ODI) was used to measure the change in the functional disability index. We conducted a paired t-test to compare the within-group change before and after the intervention. To compare the between-group difference, we used an independent t-test. The statistical significance level was set at ${\alpha}=0.05$ for all the variables. Results: The experimental group showed a significant within-group change in both the VAS and the ODI (p<0.01). The control group also showed a significant change (p<0.01). A significant difference was observed between the experimental group and the control group with regard to the change in both the VAS and the ODI after the interventions (p<0.01). Conclusion: In this study, the application of stretching and core exercise using PNF for subjects who complain of chronic lower back pain proved effective in reducing both pain and functional disability. We therefore expect that this intervention can be applied in the future as a useful program for patients with chronic lower back pain.

        고유수용성신경근촉진법 복부강화훈련이 만성허리통증환자의 균형능력과 통증에 미치는 영향

        Purpose: This study attempts to identify the effects of abdominal strength training using proprioceptive neuromuscular facilitation (PNF) on the balance ability and pain level of patients with chronic lower back pain. Methods: A total of 30 patients with chronic lower back pain were randomly divided into either experimental group I (n=10), who received abdominal strength training using PNF, experimental group II (n=10), who received abdominal strengthening training, or the control group (n=10), who received conventional physiotherapy. The interventions were applied three times a week for six weeks. We measured Berg's balance scale (BBS) in order to investigate the change in balance ability, while we measured the visible analogue scale (VAS) to determine the severity of pain. We conducted a paired t-test to compare the within-group change before and after the intervention. For the comparison of the between-group difference, we used a one-way ANOVA test. Tukey's test was used as a post hoc test. The statistical significance level was set at ${\alpha}=0.05$ for all the variables. Results: Experimental group I and experimental group II showed a significant within-group change in BBS (p<0.01). A statistically significant between-group difference was observed in terms of the change in BBS (p<0.01). According to the results of the post hoc Tukey test, experimental group I and experimental group II showed a more effective change in BBS than the control group. Experimental group I, experimental group II, and the control group all showed a significant within-group change in the VAS (p<0.01). There was a statistically significant between-group difference in terms of the change in the VAS (p<0.01). According to the results of the post hoc Tukey test, the change in the VAS was more effective in experimental group I than in experimental group II and the control group. Conclusion: In this study, the application of abdominal strength training using PNF for subjects who complained of chronic lower back pain proved effective in improving their balance ability and reducing pain. We therefore believe that this training method could be applied as a useful program for patients with chronic lower back pain.

        요통 치료의 빈용 경혈과 자가 운동 요법에 대한 체계적 문헌 고찰

        ObjectivesThe aim of this review is to figure out which acupoints are frequently used to treat low back pain and which exercises are effective to low back pain.MethodsWe searched the three electronic database (RISS, KISS, KTKP) and manually checked related Korean journals and reference lists up to June 2013. We investigated the frequency of acupoints for using treatment of low back pain. We Introduce exercise thera-pies we can do easily at home without professional skills for treatment low back pain.ResultsWe included 72 articles in this study. The most frequently adopted acupoints were BL25, BL23, BL24, GB30, BL40, BL26, BL60, GB34, BL52, BL57, GB39. The most frequently adopted meridian pathways were BL, GB, GV, ST. The most frequently adopt-ed disease causing low back pain were HIVD (Herniated intervertebral disc), Lumbar sprain, Spinal stenosis, Compression fracture, Cauda equina syndrome, Spondylolisthe-sis, Ankylosing spondylitis. There are many exercise therapies (Lumbar stabilizing ex-ercise, Resistance exercise, Bareunmom physical exercise, Mckenzie exercise, Williams exercise, Emblass exercise, Swiss ball exercise, Thera band exercise, Yoga) for treatment low back pain.ConclusionsThese results suggest that most frequently adopted acupoints were foot taeyang urinary bladder channel acupoints. Beacuse foot taeyang urinary bladder channel is passes through the waist, it will be effective for treatment of low back pain. Also exercise therapy is effective for enhance Muscular strength. In order to treatment of low back pain, Acupuncture treatment is also important, as well as exercise therapy. (J Korean Med Rehab 2013;23(4):59-71)
