Background/Aims: This study elucidated the prognostic factors for neurocardiogenic syncope in males in their late teens and early twenties. Methods: Tilt-table testing (TTT) was performed on 665 males (age range, 17 to 27 years) following the Italian protocol. The subjects were tilted head-up at a 70° angle on a table for 30 minutes during the passive phase. If the passive phase was negative, the subjects were given sublingual nitroglycerin and tilted to the same angle for 20 minutes during the drugprovocation phase. The subjects with positive results were followed without medication. We analyzed factors related to the recurrence rate of syncope. Results: Of 305 subjects (45.8%) with positive results, 223 (age range, 18 to 26 years) were followed for 12 months. The frequency of previous syncopal episodes ≥ 4 (p = 0.001) and a positive result during the passive phase (p = 0.022) were significantly related to a high recurrence rate. A positive result during the early passive phase (≤ 12 minutes) was significantly related to a higher recurrence rate than was that during the late passive phase (> 12 minutes; p = 0.011). Conclusions: A positive result during the early passive phase of TTT and frequent previous syncopal episodes were prognostic factors for neurocardiogenic syncope in men in their late teens and early twenties.

본 연구는 남한강과 북한강 유역 내 경작지 각 3개소(site A, B, C)를 대상으로 Web GIS-based VFSMOD System의 HUFF & SCS UH-based VFS Design Module을 적용하여 강우 시 경작지에서 유출되는 고형물질 유출저감을 모의하였다. 북한강 유역 내에 위치한 site A의 노지 밭의 토지경사도 5%, 강우 106.2mm 조건에서 고추 및 옥수수를 재배할 경우, 70% 이상의 고형물질 유출저감 효율을 얻기 위해서는 초생대 폭이 최소 0.5m 이상, 토지경사도 10%인 경우 70% 이상의 고형물질 유출저감 효율을 얻기 위해서는 초생대 폭이 최소 1.0∼1.5m 이상 유지해야 한다. 남한강 유역 내에 site B의 노지 밭의 토지경사도 5%, 강우량 95.1mm 조건에서 토마토 및 땅콩을 재배할 경우, 70% 이상의 고형물질 유출저감을 위해서는 초생대의 폭이 최소 0.5m, 토지경사도 10%인 경우 50% 이상의 고형물질 유출저감을 위해서는 최소 1.0m 이상의 폭을 갖는 초생대를 설치 해야 하는 것으로 나타났다. 남한강 유역 내에 site C의 노지 밭의 토지경사도 5%, 강우량 102.6mm 조건에서 고구마 및 콩을 재배할 경우, 64.0% 이상의 고형물질 유출저감을 위해서는 초생대의 폭이 최소 1.0m, 초생대를 설치해야 하는 것을 분석 되었다. 또한, 동일한 조건에서 강우지속시간 2시간의 강우량을 151.2mm로 증가시킬 경우, 62.0% 이상의 고형물질 유출저감을 위한 초생대의 최소 폭은 1.0m 이상 설치해야 하는 것으로 분석되었다. 초생대 시설은 경작지와 같은 토지이용 시 친환경적인 고형물질 유출저감 방안으로 검토될 수 있으며, Web GIS-based VFSMOD System을 이용하여 다양한 토지특성과 강우 사상을 반영하여 적정한 규모의 초생대 설계가 가능할 것으로 분석되었다. The study was performed to simulate the reduction efficiency of suspended solids (SS) for cultivated land located at riverine area at the Namhan River and the Bukhan River watershed sites (site A, B, C) under the rainfall conditions using HUFF & SCS UH-based VFS Design module of Web GIS-based VFSMOD System. The study indicates that the field 5% sloped, located at Bukhan River watershed (site A), requires at least 0.5m width of Vegetative Filter Strip (VFS) to reduce 70% of SS while the field 10% sloped requires the at least 1.0∼1.5m width of VFS to reduce 70% SS, under the condition 106.2mm of rainfall event and bell pepper or corn of crops. Against the conditions 95.1mm of rainfall event and sweet potato or soy bean of crops, the field 5% sloped, located at Namhan River watershed (site B) requires at least 0.5m width of VFS to reduce 70% of SS while the field 10% sloped requires at least 1.0m width of VFS to reduce 50% SS. The crops sweet potato and soy bean are cultivated in the site C, located at Namhan River watershed, 1m of VFS is capable of 64.0% and 62.0% of SS reduction against 102.6mm and 151.2mm rainfall conditions respectively, for the 5% sloped field. The result supports that VFS is one of most potential methods to reduce SS from cultivated area, which is environment-friendly hydrologic structure. The VFS design, however, needs to be simulated before its installation in the field, the simulation needs to consider not only various characteristics of the field but also different conditions affecting the VFS, using a model capable to consider a lot of factors.
석회 기반 신소재(RePhos)를 활용한 all-in-one 정석인회수공정의 반응속도 및 제거효율 개선

본 논문에서는 부영영화의 주된 원인인 인을 제거하기 위해 하수처리수 방류수 고도처리의 대안으로 새로운 정석인회수공법을 제안하고, 정석반응의 반응속도와 제거효율을 개선하고자 하였다. 정석재로 석회 기반 인회수 신소재 RePhos를 사용하였고, 시험수는 하수처리장 방류수 수준의 원수를 제조하여 실험을 진행하였다. RePhos 투입 후 반응시간에 따른 수질을 모니터링한 결과, 수온이 높을수록 처리효율이 개선됨을 확인하였고, 교반 및 폭기 등의 물리적 처리에 의해 반응속도와 처리효율이 제고됨을 확인하였다.

<P><B>Background:</B> Our objective was to verify the efficacy and safety of ‘docetaxel + 5-fluorouracil + cisplatin’ 3-day combination chemotherapy as a first-line treatment in patients with unresectable gastric cancer.</P><P><B>Methods:</B> Between January and November 2002, we enrolled 43 patients [males 31; median age 55 years (range 24–74)] with inoperable gastric cancer who had not been seen previously in Seoul National University Hospital. The regimen used was docetaxel 70 mg/m<SUP>2</SUP> on day 1, cisplatin 40 mg/m<SUP>2</SUP> on days 2 and 3, and 5-fluorouracil 1200 mg/m<SUP>2</SUP> over 10 h on days 1–3, every 3 weeks.</P><P><B>Results:</B> A total of 168 cycles were administered. Mean cycle number per patient was 3.9. The administered dose intensity of docetaxel was 21.23 mg/m<SUP>2</SUP>/week, 5-FU 1092.14 mg/m<SUP>2</SUP>/week and cisplatin 23.82 mg/m<SUP>2</SUP>/week, which corresponded to 91.1, 91.0 and 89.5% of planned doses. Of the 43 patients, response evaluation was possible in 40 and, of these patients, 17 (42.5%) achieved a partial response, 13 (32.5%) stable disease, and 10 patients (25%) showed progressive disease. The median time to progression was 5.6 months [95% confidence interval (CI) 4.6–6.6 months]. Median overall survival was 9.0 months. (95% CI 4.8–13.2 months). Leukopenia occurred during 21.4% of cycles (36 of 168 cycles); 14.3% grade 1, 5.3% grade 2 and 1.8% grade 3. Anemia occurred in 16.7% (28 of 168 cycles); 11.3% grade 1, 4.8% grade 2 and 0.6% grade 3. Thrombocytopenia was not observed. Diarrhea, stomatitis and hypersensitivity occurred in 4.7% (two out of 43 patients), respectively. Neutropenic fever occurred in two patients (4.7%) and myalgia in three (7.0%).</P><P><B>Conclusion:</B> ‘Docetaxel + 5-fluorouracil + cisplatin’ 3-day combination chemotherapy is an active and tolerable regimen as a first-line treatment in patients with unresectable gastric cancer.</P>
경골 분절 골절의 골수강내 교합정 고정술 후 발생한 불유합에 대한 분석

Tibial segmetnal fracture, usually caused by strong external force has recently been increased in frequency because of increased traffic accident. This type of fracture has been well known to be very difficult to manage on reduction and maintenance of reduction, and also that it has high rate of complications such as delayed union, nonunion, infection and etc., because of high incidence of open fracture and association with other injury. For the management of this type fracture there are various methods from conservative care to operative treatment such as internal or external fixation. We analyzed nonunion of 8 cases who underwent interlocking intramedullary nailing for tibia segmental fracture. The results were as follows; 1. Interlocking intramedullary nailing for the treatment of tibia segmental fractures were done in 29 cases(76.31%) out of total 38 tibia segmental fractures, 2. Nonunion occurred in 8 out of 29 cases(27.6%). 3. Nonunion occurred at the distal fracture site in 7 cases and both proximal and distal fracture site in 1 case. 4. Nonunion occurred in 4 cases of open fractures, 3 cases of closed fractures with compartment syndrome. 5. Nonunion occurred in 5 cases of Melis type IV, fracture, 2 cases of type I and 1 case of type g fracture. Although the interlocking intramedullary nailing is thought to be good method for the tibia segmental fracture, our study showed relatively high rate of nonunion especially, at the distal fracture site, in case of open fracture, associated with compartment syndrome and Melis Type IV fracture. On the basis of this study we recommend that closer attention should be paid to the method of treatment, fracture type, severity of comminution, open or closed fracture, degree of soft tissue injury, and fracture site.

본 연구에서는 부영양화와 조류성장의 대표적 제한영양소인 인을 제거하기 위해 폐석회석을 활용한 수질정화소재를 개발하고자 하였다. 제조된 폐석회석 소재(YW)의 성능을 알아보기 위해 경기도 D하천의 하천수를 대상으로 체류시간 및 YW 투입량에 따른 수질변화를 분석하였다. YW 투입 후 체류시간에 따른 수질(pH, COD, T-N, T-P, PO₄-P)의 변화를 모니터링한 결과, 조건에 따라 T-P 또는 PO₄-P를 95% 이상 제거할 수 있는 것으로 확인되었다. 또한, YW 투입량을 저감한 조건에서는 체류시간이 길어질수록 인의 제거효율이 제고됨을 확인하였다.
