Improvement of Dysphagia in Traumatic Neurovascular Compression: A Case Report and Collective Review

Dysphagia is a relatively rare symptom of neurovascular compression of the lower cranial nerve (CN). Many casereviews of neurovascular compression required surgical treatment to improve the neurological symptoms. This reportpresents a 75-year-old female patient who complained of dysphagia due to neurovascular compression of CNsIX-XI at the left side after the onset period. The symptom was improved by treating with balloon swallowing rehabilitation(BSR). In a video fluoroscopic swallowing study (VFSS), no significant manifestations were observed inthe oral phase. On the other hand, profuse post-swallow residue decreased pharyngeal propulsion were observedwithout aspiration or penetration during the semisolid portion of the swallowing test. To reduce the pharyngealremnant and improve pharyngeal propulsion, the BSR protocol was performed and the patient recovered quickly. Follow-up VFSS showed improvement in the post-swallow residue from 42.6 to 7.3%. After discharge, the patientcould resume a normal diet without complications. Based on this observed result, it is advantageous to consider theoption of a prescribed rehabilitation program over surgical treatment.

Trajectory modeling is foundational for 4D-Route modeling, conflict detection and air traffic flow management. This paper proposes a novel algorithm based Vincenty`s fomulas for trajectory calculation, combined with the Dijkstra algorithm and Vincenty`s formulas. Using flight plan simulations our experimental results show that our method of En-route trajectory calculation exhibits much improved performance in accuracy.

Acquired renal tubular disorder can be observed in various disease processes, especially autoimmune diseases. Gitelman syndrome is an autosomal recessive disease characterized by hypokalemic metabolic alkalosis, hypomagnesemia, and hypocalciuria. This disorder is caused by mutation in the SLC12A3 gene, which encodes the thiazide-sensitive NaCl cotransporter (NCCT). Acquired Gitelman syndrome has been reported and the majority has been associated with Sjogren`s syndrome. The presence of circulating auto-antibodies to NCCT was suggested as a mechanism of acquired Gitelman syndrome. Treatment of acquired Gitelman syndrome was done with supplements of potassium and magnesium and prednisone was effective in some cases. Acquired Gitelman syndrome should be included in the differential diagnosis of renal involvement in patients with autoimmune diseases, especially Sjogren`s syndrome.
Applications of kidney organoids derived from human pluripotent stem cells

The establishment of protocols to differentiate kidney organoids from human pluripotent stem cells provides potential applications of kidney organoids in regenerative medicine. Modeling of renal diseases, drug screening, nephrotoxicity testing of compounds, and regenerative therapy are attractive applications. Although much progress still remains to be made in the development of kidney organoids, recent advances in clustered regularly interspaced short palindromic repeat (CRISPR)-CRISPR-associated system 9 (Cas<sub>9</sub>) genome editing and three-dimensional bioprinting technologies have contributed to the application of kidney organoids in clinical fields. In this section, we review recent advances in the applications of kidney organoids to kidney disease modelling, drug screening, nephrotoxicity testing, and regenerative therapy.

A growing number of governments around the world at both national and subnational levels have shored up their efforts at attracting foreign direct investment (FDI) by setting up an investment promotion agency (IPA). A newly emerging literature has sought to ascertain if and how such agencies have fulfilled their missions. In this article, we seek to move it forward by making two contributions. First, taking advantage of the Foreign-Business Friendliness Index (FBFI) data that cover all district-level local governments in South Korea, we are able to conduct a comprehensive study of local IPAs within a single country. Second, the detailed information in the data also enables us to examine precisely which organizational structures and activities of the IPAs are most responsible for the enhanced FDI performance. Key findings suggest that the way IPA s internal organizational structure is set up matters. Specifically, we show that IPAs that have an IP committee in conjunction with a designated IP department significantly outperform those that do not have both at the same time.

Aim:?It is still unclear which layer (intima or media) is mainly involved in increased carotid intima-media thickness (CIMT) by aging and also unclear regarding CIMT value suggesting high cardiovascular risk, although 75th percentile value of CIMT is known as a high risk in asymptomatic adults. We sought to find the changes of carotid intima thickness (CIT) and carotid media thickness (CMT) by aging and the 75th percentile value of CIMT in asymptomatic Korean adults.?Method:?This is an observational cohort study. Carotid ultrasound findings (n=2,204 from 12 hospital) were prospectively collected. The carotid images were sent to Korea Research Institute of Standards and Science for analysis using specialized software which can measure intima and media wall also.?Results:?Mean age was 58.1±13.5 years old (52% of men). Pearson correlation coefficient between age and right CIMT (r=0.489, p<0.001) and right CMT (r=0.482, p<0.001) were higher than those between age and right CIT (r=0.284, p<0.001). Mean right CIMT in male and female was 0.696±0.163 mm and 0.686±0.167 mm (p=0.180) and the 75 percentile value was 0.778mm and 0.771 mm, respectively. Mean right CIT was 0.311±0.069 mm and 0.303±0.064mm (p=0.009) and mean right CMT was 0.391±0.124 mm and 0.388±0.131 mm (p=0.694) in male and female, respectively. Left carotid ultrasound findings showed similar to the right one.?Conclusions:?An increased CIMT by aging was mainly due to increased CMT rather than CIT in asymptomatic adults. The 75th percentile values of right CIMT were 0.778 mm and 0.771 mm in asymptomatic Korean male and female adults, respectively.?
Highly Regioselective Preparation and Characterization of New 6-O-Substituted Dieckol Derivatives

Dieckol is a brown algae-derived polyphenol that has broad bioactivity and low toxicity. Therefore, it is apromising lead compound for the development of therapeutic agents to treat various pathologicalconditions, including viral infections, allergies, diabetes, skin ageing, cancers, and neurodegenerativeconditions. To exploit the pharmacological potential of dieckol, its stability, solubility, pharmacokinetics,and drug delivery must be improved. This can be achieved by the controlled modification of the hydroxylgroups, but the presence of eleven nearly equivalent hydroxyl groups makes the task highly challenging. In this study, the regioselectivities infive substitution reactions of the hydroxyl groups of dieckol undervarious SN2 reaction conditions were investigated. After reaction optimization,five substituents (methyl,benzyl, methoxymethyl, 3-hydroxypropyl, and 3-(ethoxycarbonyl)propyl) could be introduced at the 6-Oposition of dieckol with surprisingly high regioselectivity, as confirmed by 2D-NMR spectroscopicanalyses. The prepared dieckol derivatives showed antioxidant and anticancer activities analogous tothose of unmodified dieckol, indicating that the mono-O-substitutions did not affect the biochemical andbiological characteristics of dieckol. Therefore, the proposed methodology for the mono-O-substitutionof a specific oxygen of dieckol is a powerful tool to add various pharmaceutical attributes to dieckol, thuscontributing to the development of various dieckol-based drug candidates.
S-193 The impact of glucose control on carotid arterial wall thickness in Diabetes mellitus patients

Objective:?We evaluated the impact of diabetes mellitus (DM) and DM control status on each layers of carotid artery in asymptomatic population.?Methods:?This is an observational cohort study consisted of 1,479 patients. The carotid images were sent to the Korea Research Institute of Standards and Science for core laboratory analysis using specialized software which can measure intima and media thickness respectively.?Results:?DM patients (n=634, 42.9%) were likely to be older, higher prevalence of male, hypertension and dyslipidemia, and higher creatinine level than non-DM patients (n=845, 57.1%). There was no significant demographic difference according to DM control status in DM patients. DM patients showed higher carotid intima-media thickness (CIMT, 0.70±0.15mm vs. 0.66±0.16mm, p<0.001) and media thickness (CMT, 0.41±0.12 mm vs. 0.36±0.12mm, p<0.001) than non-DM patients, whereas intima thickness (CIT) showed no significant difference (0.29±0.07 mm vs. 0.30±0.06 mm, p=0.067) between 2 groups. Well controlled DM patients (HbA1C<7.0%, n=232, 47.4%) showed higher CIT (0.30±0.08mm vs. 0.27±0.06 mm, p=0.003) than poorly controlled DM (HbA1C ≥ 7.0%, n=257, 52.6%). Old age and LDL-cholesterol were the independent factors for CIMT, CIT and CMT in total asymptomatic adults as well as DM patients. DM control status was not significant independent factor for CIMT.?Conclusions:?The increased CIMT in DM patients was mainly due to the increased CMT. DM control status did not impact on carotid arterial wall thickness in this study. Lipid control rather than glucose control may be the most important factor to decrease atherosclerosis progression in subclinical adults.
Ground Speed Calculation Using Wind Component Information for Trajectory Prediction

Ground speed calculation is basic work for trajectory prediction, conflict detection and air traffic flow management. This paper proposes a novel algorithm based on Vincenty’s formulas for ground speed calculation. Our experiments used simulations with wind components and our experimental results show that our ground speed calculation exhibits much better accuracy performance.
