        Non-atypical endometrial hyperplasia: risk factors for occult endometrial atypia and malignancy in patients managed with hysterectomy

        ObjectiveTo determine the risk factors for occult endometrial atypia and malignancy in patients diagnosed with non-atypicalendometrial hyperplasia (NEH) on endometrial biopsy. MethodsAll new cases of NEH diagnosed between April 2015 and March 2016 at KK Women’s and Children’s Hospital, whounderwent hysterectomy as first-line treatment, were included in the study. Patients with a history of endometrialhyperplasia or malignancy were excluded from the study. Patient demographics (e.g., age, parity, body mass index[BMI]), medical history, and clinical presentation were obtained for analysis. ResultsIn total, 262 patients were diagnosed with NEH, of which 18.3% (n=48) underwent hysterectomy as first-linemanagement. The average time to surgery was 77.0±35.7 days. All cases were diagnosed by dilation and curettage,and hysteroscopy. The mean age was 51 years, and the mean BMI was 26.9±5.8 kg/m2. Histology from thehysterectomy specimen showed 9 (18.8%) patients with atypical hyperplasia and 2 (4.2%) with grade 1, stage 1Aendometrioid adenocarcinoma. Patients with higher grade final pathology had significantly lower median parity(1 vs. 2, P=0.039), higher mean BMI (30.1±6.5 vs. 25.9±5.3 kg/m2, P=0.033), and BMI ≥30 kg/m2 (54.5% vs. 13.5%,P=0.008, odds ratio 7.68), compared to patients whose final histology showed NEH or no residual hyperplasia. ConclusionOccult endometrial atypia and malignancy were found in 18.8% and 4.2% of patients with an initial diagnosis of NEH,respectively. High BMI and low parity were identified as significant risk factors for high-grade endometrial lesions inpatients with NEH.

        Different Catabolism Pathways Triggered by Various Methylxanthines in Caffeine-Tolerant Bacterium Pseudomonas putida CT25 Isolated from Tea Garden Soil

        The degradation efficiency and catabolism pathways of the different methylxanthines (MXs) in isolated caffeine-tolerant strain Pseudomonas putida CT25 were comprehensively studied. The results showed that the degradation efficiency of various MXs varied with the number and position of the methyl groups on the molecule (i.e., xanthine > 7-methylxanthine ≈ theobromine > caffeine > theophylline > 1-methylxanthine). Multiple MX catabolism pathways coexisted in strain CT25, and a different pathway would be triggered by various MXs. Demethylation dominated in the degradation of N-7-methylated MXs (such as 7- methylxanthine, theobromine, and caffeine), where C-8 oxidation was the major pathway in the catabolism of 1-methylxanthine, whereas demethylation and C-8 oxidation are likely both involved in the degradation of theophylline. Enzymes responsible for MX degradation were located inside the cell. Both cell culture and cell-free enzyme assays revealed that N-1 demethylation might be a rate-limiting step for the catabolism of the MXs. Surprisingly, accumulation of uric acid was observed in a cell-free reaction system, which might be attributed to the lack of activity of uricase, a cytochrome c-coupled membrane integral enzyme.

        The Prognostic Impact of Heat Shock Proteins Expression in Patients with Esophageal Cancer: A Meta-Analysis

        Purpose: Heat shock proteins (HSPs) are highly conserved molecular chaperones. There are various studies that assess the prognosticvalue of HSPs in patients with esophageal cancer, but the conclusion remains controversial. This is the first meta-analysisstudy aiming to summarize the evidence on the suitability of HSPs to predict patients’ survival. Materials and Methods: Searching PubMed, Web of science and Medline until May 31, 2014, data were compared for overall survivalin patients with down-regulated HSPs level with those with up-regulated level. We conducted a meta-analysis of 9 studies(801 patients) that correlated HSPs levels with overall survival. Data were synthesized with hazard ratios (HRs). Results: The estimated risk of death was 2.93-fold greater in HSP27 negative patients than HSP27 positive patients [95% confidenceinterval (CI), 1.12–7.62]. When limited to esophageal squamous cell carcinoma (ESCC), the risk of death in HSP27 negativepatients seemed more significant (HR, 3.90; 95% CI, 2.35–6.49). Decreased expression of HSP70 was also associated with worsesurvival in esophageal cancer (HR, 2.83; 95% CI, 1.90–4.23) and, when limited to ESCC, HR was 3.21 (95% CI, 1.94–5.30). Data collected,however, were not sufficient to determine the prognostic value of HSP90 in patients with ESCC nor esophageal adenocarcinomas(EADC). Conclusion: In this meta-analysis, reduced HSP27 and HSP70 expressions were associated with poor survival in patients withesophageal cancer, especially esophageal squamous cell carcinoma.

        A Facile Hg2+-related Quenching Photoluminescence Sensor Based on Nitrogen-doped Graphene Quantum Dots

        A Hg2+-related quenching photoluminescence (PL) sensor was fabricated based on nitrogen-doped graphene quantum dots (N-GQDs) as the luminescent agent and glutathione as the masking agent to detect Hg2+ in tap water. The addition of Hg2+ significantly reduced the PL intensity of N-GQDs, which was attributed to coordination reaction inducing the aggregation of N-GQDs. The Hg2+-related quenching PL sensor with glutathione as the masking agent has good selectivity and accuracy. The sensor showed a linear relationship ranging from 0.5 to 110?nM with the detection limit 0.08?nM (S/N = 3). The proposed method was applied to the determination of Hg2+ in tap water, and the results were consistent with atomic absorption spectroscopy (AAS).

        Prediction of Equivalent Chloride Ion Diffusion Coefficient in Cracked Concrete of the in-Service RC Element

        Considering the influence of the visible load-induced transverse cracks on chloride ion diffusion in cracked concrete of the in-service reinforced concrete (RC) elements under marine environment, a typical concrete volume with one transverse crack was taken to establish the governing equation. Assuming the crack widths at the outer lateral and bottom surfaces were equal, a model to predict the equivalent chloride diffusion coefficient in cracked concrete was put forward to consider the influence of the average transverse crack width, crack spacing and crack extending lengths in beam height and width directions on chloride diffusion. Results show that the proposed model can better reflect the variation trend of the equivalent chloride diffusion coefficient with different crack widths in in-service RC elements under marine environment.

      • Depression and the Risk of Breast Cancer: A Meta-Analysis of Cohort Studies

        Background: Whether depression causes increased risk of the development of breast cancer has long been debated. We conducted an updated meta-analysis of cohort studies to assess the association between depression and risk of breast cancer. Materials and Methods: Relevant literature was searched from Medline, Embase, Web of Science (up to April 2014) as well as manual searches of reference lists of selected publications. Cohort studies on the association between depression and breast cancer were included. Data abstraction and quality assessment were conducted independently by two authors. Random-effect model was used to compute the pooled risk estimate. Visual inspection of a funnel plot, Begg rank correlation test and Egger linear regression test were used to evaluate the publication bias. Results: We identified eleven cohort studies (182,241 participants, 2,353 cases) with a follow-up duration ranging from 5 to 38 years. The pooled adjusted RR was 1.13(95% CI: 0.94 to 1.36; $I^2=67.2%$, p=0.001). The association between the risk of breast cancer and depression was consistent across subgroups. Visual inspection of funnel plot and Begg's and Egger's tests indicated no evidence of publication bias. Regarding limitations, a one-time assessment of depression with no measure of duration weakens the test of hypothesis. In addition, 8 different scales were used for the measurement of depression, potentially adding to the multiple conceptual problems concerned with the definition of depression. Conclusions: Available epidemiological evidence is insufficient to support a positive association between depression and breast cancer.

      • Selective Inhibition of Bicyclic Tetrapeptide Histone Deacetylase Inhibitor on HDAC4 and K562 Leukemia Cell

        Histone deacetylase (HDAC) inhibitors of cyclic peptide have been proved to be the most complex but the most stable and relative efficient inhibitors because of their large cap region. In this paper, a series of studies were carried out to evaluate the efficacy of synthetic bicyclic tetrapeptide inhibitors 1-5 containing hydroxamic acid referring molecular docking, anti-proliferation, morphology and apoptosis. Docking analysis, together with enzyme inhibitory results, verified the selective capability of inhibitor 4 to HDAC4, which might closely related to haematological tumorigenesis, with Phe227, Asp115, Pro32, His198 and Ser114 participating into hydrophobic interactions and Van der Waals force which was familiar with former study. Moreover, inhibitor 4 inhibited K562 cell line at the $IC_{50}$ value of 1.22 ${\mu}M$ which was 51-67 times more efficient than that for U937 and HL60 cell lines. Inhibitor 4 exhibited the cell cycle-arrested capability to leukemia at S phase or G2/M phase as well as apoptosis-induced ability in different degrees. Finally, we considered that bicyclic tetrapeptide inhibitors were promising inhibitors used in cancer treatment and inhibitor 4 could prevent K562 cell line well from proliferation, arrest cell cycle and induce K562 towards apoptosis to achieve the goals of reversing cancer cells which could become a potential leukemia therapeutic agent in the future.

        Copper as an Antimicrobial Agent against Opportunistic Pathogenic and Multidrug Resistant Enterobacter Bacteria

        Infections by Enterobacter species are common and are multidrug resistant. The use of bactericidal surface materials such as copper has lately gained attention as an effective antimicrobial agent due to its deadly effects on bacteria,yeast, and viruses. The aim of the current study was to assess the antibacterial activity of copper surfaces against Enterobacter species. The antibacterial activity of copper surfaces was tested by overlying 5×106 CFU/ml suspensions of representative Enterobacter strains and comparing bacterial survival counts on copper surfaces at room temperature. Iron, stainless steel, and polyvinylchloride (PVC) were used as controls. The mechanisms responsible for bacterial killing on copper surfaces were investigated by a mutagenicity assay of the D-cycloserin (cyclA gene), single cell gel electrophoresis,a staining technique, and inductively coupled plasma mass spectroscopy. Copper yielded a significant decrease in the viable bacterial counts at 2 h exposure and a highly significant decrease at 4 h. Loss of cell integrity and a significantly higher influx of copper into bacterial cells exposed to copper surfaces, as compared to those exposed to the controls,were documented. There was no increase in mutation rate and DNA damage indicating that copper contributes to bacterial killing by adversely affecting cellular structure without directly targeting the genomic DNA. These findings suggest that copper’s antibacterial activity against Enterobacter species could be utilized in health care facilities and in food processing plants to reduce the bioburden, which would increase protection for susceptible members of the community.
