Objectives: The purpose of this study is to review safety of taking herbal medicines during breastfeeding and Korean medicine approach to problems that occur to mothers during breastfeeding. Methods: Literature search was done using database such as Pubmed, Google scholarship, RISS and Uptodate scholastic. Search words were ‘breastfeeding TCM’, ‘herbal medicine’, ‘Korean medicine’ and ‘safety’. Only English and Korean studies were assessed. Results: For most single herbs used in Korean medicine, there were few studies on the safety during lactation, and the reported studies show conflicting results. In terms of herbal medicine decoctions, there were several studies reporting of no side effects when taken by breastfeeding mothers. Meanwhile, Korean medicine can be used for problems arising from breastfeeding such as insufficient milk supply, laceration of nipples, pain, and mastitis. Conclusions: Although clinical studies reported in Korea are still at a basic level, studies assessing efficacy and safety of Korean medicine for breastfeeding mothers are being conducted, and there are many Korean medicine literatures explaining Korean medicine approaches to breastfeeding mothers. Therefore, it is considered that the possibility of Korean medicine treatment for the problems that mothers face during lactation has great potential.
Advanced Design of Birdcage RF Coil for Various Absorption Regions at 3T MRI System

Purpose: The purpose of this study was to design and build an optimized birdcage resonator configuration with a low pass filter, which would facilitate the acquisition of high-resolution 3D-image of small animals at 3T MRI system. Methods and Materials: The birdcage resonator with 12-element structures was built, in order to ensure B1 homogeneity over the image volume and maximum filling factor, and hence to maximize the signal to noise ratio (SNR) and resolution of the 3-dimensional images. The diameter and length of each element of a birdcage resonator were as follows: (1) diameter 13 cm, length 22 cm, (2) diameter 15 cm, length 22 cm, (3) diameter 17 cm, length 25 cm. Spin echo pulse sequence and fast spin echo pulse sequence were employed in obtaining MR images. The quality of the manufactured birdcage resonators wes evaluated on the basis of the return loss following matching and tuning process. Results: The experimental MR image of phantoms by the various manufactured birdcage resonators were obtained to compare the SNR in accordance with the size of objects. The size of an object to that of coil was identified by parameters that were estimated from the image of a phantom. First, the diameter of the birdcage resonator was 15cm, and the ratio of the tangerine to the birdcage resonator accounted for approximately 27%. The Q factor was 53.2 and the SNR was 150.7. Second, at the same birdcage resonator, the ratio of the orange was approximately 53%. The SNR and the Q parameter was 212.8 and 91.2, respectively. Conclusion: The present study demonstrated that if birdcage resonators have the same forms, SNR could be different depending on the size of an object, especially when the size of an object to that of coil is approximately 40~80%, the former is bigger than the latter. Therefore, when the size of an object to be observed is smaller than that of coil, the coil should be manufactured in accordance with the size of an object in order to obtain much more excellent images.

Background/Aims: Primary biliary cirrhosis (PBC) is a slowly progressing autoimmune disease of the liver that is characterized by portal inflammation and immune-mediated destruction of the intrahepatic bile ducts. Serum total bilirubin is one of the various prognostic factors that have been proposed. A recent study found that PBC with accompanying autoimmune hepatitis (AIH) carries a negative prognosis. This study examined the clinical characteristics of PBC and analyzed the factors that affect its prognosis. Methods: Patients diagnosed with PBC between January 1998 and December 2010 based on clinical and histopathological findings were compiled and analyzed retrospectively. Results: Among 27 patients, 24 (1 male and 23 females, ages 50.0±9.3 years) were followed up. The follow-up period was 8.6±0.9 years. Of the 24 patients, 9 patients progressed to liver cirrhosis (LC). Comparison between patients who did and did not progress to LC revealed statistically significant differences in the patients` serum total bilirubin (2.7±1.8 vs. 0.8±0.4, P=0.012), the Mayo risk score (5.1±0.7 vs. 3.9±0.6, P=0.001), the revised IAHG (International Autoimmune Hepatitis Group) score (9.2±2.3 vs. 5.4±3.0, P=0.004) and frequency of AIH overlap (5/9 [55.6%] vs. 0/15 [0%], P=0.001) at the time of diagnosis. Conclusions: We propose that serum total bilirubin, the Mayo risk score, and the revised IAHG score at the time of diagnosis are helpful for predicting PBC prognosis. In particular, since all of the patients with accompanying AIH progressed to LC, the presence of overlap syndrome at the time of diagnosis is helpful for predicting PBC prognosis and providing an adequate treatment. <P>We have developed two-photon (TP) pH-sensitive probes (BH-2 and BHEt-1) that exhibit absorption and emission maxima at 370 and 466 nm, and TP absorption cross-section values of 51 and 61 GM (1 GM = 10(-50)cm(4)s/photon), respectively, at 750 nm and pH 3.0 in a universal buffer (0.1 M citric acid, 0.1 M KH2PO4, 0.1 M Na2B4O7, 0.1 M Tris, 0.1 M KCl)/1,4-dioxane (7/3) solution. The TPM images of CCD-18co (a normal colon cell line) and HCT116 cells (a colon cancer cell line) labeled with BH-2 were too dim to be distinguished. When the same cells were labeled with BHEt-1, however, the TPM image of the HCT116 cells was Much brighter than that of CCD-18co cells, and the relative proportion of the acidic vesicles (P-acid) of the former was 5-fold larger than that of latter. BHEt-1 could also differentiate HepG2 cells (a human liver cancer cell line) from LX-2 cells (a human hepatic stellate cell line) with a 6-fold larger P-acid value. Human colon cancer tissues labeled with BHEt-1 showed similar results, demonstrating much brighter TPM images and 6-fold larger P-acid values compared to normal tissue. These results suggest the potential utility of BHEt-1 for detecting colon cancer in human tissues using TPM.</P>

Objective: This study aimed to assess the safety and effect on glucose level of Bojungikgi-tang in patients with type 2 diabetes mellitus. Methods: To review patients' clinical characteristics and laboratory tests retrospectively, we investigated 15 hospitalized patients with type 2 diabetes mellitus who took Bojungikgi-tang at Kyung Hee University Korean Medical Hospital for at least one day between January 2012 and December 2022. The blood glucose levels, including fasting blood sugar (FBS), 2-hour postprandial glucose (PP2) levels, and glycated hemoglobin level, were collected to determine the effect of the Bojungikgi-tang on blood sugar changes. Furthermore, to evaluate the safety of Bojungikgi-tang, hepatic function and renal function tests were implemented, including aspartate aminotransferase, alanine aminotransferase, alkaline phosphatase, γ-glutamyltransferase, blood urea nitrogen, creatinine, and Modification of Diet in Renal Disease estimated glomerular filtration rate. Results: When Bojungikgi-tang and a standard treatment for diabetes were administered for patients with type 2 diabetes, it was confirmed that there were no statistically significant changes in FBS and PP2 levels in the analysis of each medication taken. There was no significant difference in the safety profile after taking Bojungikgi-tang. Conclusions: The combined administration of Bojungikgi-tang with standard hypoglycemic medication for patients with type 2 diabetes may not affect blood glucose levels and safety.

Objective: This study was conducted to investigate whether acupuncture and moxibustion treatment could improve proteinuria in immunoglobulin A nephropathy. Methods: A 25-year-old man diagnosed with immunoglobulin A nephropathy had severely increased proteinuria. We conducted acupuncture treatment and moxibustion treatment on the abdominal area between the abdominal aorta and kidney at level L2. Results: After five months of acupuncture and moxibustion treatment, the patient's urine albumin/creatinine ratio and urine protein/creatinine ratio had improved from 0.571 to 0.28 and 0.79 to 0.47, respectively. Conclusions: This case report suggests that acupuncture and moxibustion treatment may ameliorate proteinuria in immunoglobulin A nephropathy.
