ABSTRACT: Numerous studies have demonstrated that cardiac biomarkers are significant predictors of cardiovascular (CV) and all-cause mortality in ESRD patients, but most of the studies were retrospective or included small numbers of patients, only prevalent dialysis patients, or measured 1 or 2 biomarkers. This study was to analyze the association between 3 cardiac biomarkers and mortality in incident HD patients. A prospective cohort of 864 incident HD patients was followed for 30 months. Based on the median values of baseline NT-proBNP, cTnT, and hsCRP, the patients were divided into “high” and “low” groups, and CV and all-cause mortality were compared between each group. Additionally, time-dependent ROC curves were constructed, and the NRI and IDI of the models with various biomarkers were calculated. The CV survival rates were significantly lower in the “high” NT-proBNP and cTnT groups compared to the corresponding “low” groups, while there was no significant difference in CV survival rate between the 2 hsCRP groups. However, all-cause mortality rates were significantly higher in all 3 “high” groups compared to each lower group. In multivariate analyses, only Ln NT-proBNP was found to be an independent predictor of mortality. Moreover, NT-proBNP was a more prognostic marker for mortality compared to cTnT. In conclusion, NT-proBNP is the biomarker that results in the most added prognostic value on top of traditional risk factors for CV and all-cause mortality in incident HD patients.

The efficacy of the radioprotective effect of Callophyllis japonica ethyl acetate (CJEA) extract was studied by comparing it to that of amifostine, a well-known radioprotective agent, and by evaluating the dose reduction factor, an indicator of radioprotective efficacy. Pretreatment with CJEA extract (100 mg/kg body weight) prior to receiving 12 Gy irradiation significantly improved the survival of jejunal crypts at 3.5 day post-irradiation, but attenuated the level of malondialdehyde compared to vehicle alone (P < 0.01). A similar gastroprotective effect was also obtained in the amifostine-treated irradiated group (P < 0.01). The efficacy of the radioprotective effect was further confirmed by the dose reduction factor, 1.41. Collectively, these results suggest that CJEA extract is a useful radioprotectant whose efficacy is similar to that of amifostine and whose radioprotective mechanism is in part the reduction of lipid peroxidation caused by gamma irradiation.

<P>Neurofibromatosis type 2 (NF2) syndrome is a very rare human genetic disease, and there has been no proper treatment for it until now. In our recent study, it has been reported that the loss of NF2 activates MAPK signaling through reduction of RKIP in a mesothelioma model. Here, we show that loss of NF2 induces reduction of the TGFβ receptor 2 (TβR2) expression, and an overwhelming expression of TGFβ receptor 1 (TβR1) is activated by physical stimuli such as pressure or heavy materials. Activated TβR1 induces the phosphorylation and degradation of RKIP. RKIP reduction consequently results in MAPK activation as well as Snail-mediated p53 suppression and occurrence of EMT in NF2-deficient cells by physical stimuli. Thus, TβR1 kinase inhibitors restore cell differentiation and induce growth suppression in NF2-deficient Schwannoma cell line and MEF. Moreover, TEW7197, a specific TβR1 kinase inhibitor, reduces tumor formation in the NF2-model mouse (Postn-Cre;NF2<SUP>f/f</SUP>). Gene expression profiling reveals that TEW7197 treatment induces the expression of lipid metabolism–related gene set, such as NF2-restored cells in HEI-193 (NF2-deficient Schwannoma). Our results indicate that reduction or deletion of TβR2 or NF2 induces the TβR1-mediated oncogenic pathway, and therefore inhibition of the unbalanced TGFβ signaling is a putative strategy for NF2-related cancers (NF2 syndrome and mesothelioma) and TβR2-mutated advanced cancers. <I>Mol Cancer Ther; 17(11); 2271–84. ©2018 AACR</I>.</P>
위암종 및 위암종의 전구병변에서 p27Kip1 의 발현

목적 : 위암종은 한국인 남자에서 가장 흔한 악성 종양으로 알려져 있으며 위암종 환자의 예후를 결정하는 요인에 관한 많은 연구가 있어왔다. 또한 사람의 여러 종양에서 CDKI 중 하나인 p27Kip1의 소실이 암의 성장과 연관이 있다고 알려져 있다. 이에 위암종, 위선종 그리고 위암종의 전구병변에서 p27Kip1에 대한 면역조직화학적 염색을 시행하여 p27Kip1의 발현정도를 조사하고 지금까지 알려져 있는 요인들과의 상관관계를 살펴보아 예후인자로서 유용하게 쓰일 수 있을 지에 대해 알아보고자 하였다. 방법 : 위암종으로 진단 받고 위절제술을 시행 받은 진행성 위암종 43예, 조기 위암종 19예, 위선종 22예, 위암인접 비종양조직 110개 및 조직학적으로 염증이 없거나 거의 미미한 정상군 10예를 대상으로 하였으며 위암인접 비종양조직은 다시 만성위염 32개, 장형화생 29개 그리고 위암종에서 1 cm 이내의 범위에 있는 이행상피 49개로 나누어 면역조직화학 염색을 이용하여 p27Kip1의 발현정도를 확인하였다. 결과 : 위암종에서 위암인접 비종양 조직과 위선종에 비해 p27Kip1의 발현이 유의하게 감소하였고 (p<0.05), 위암인접 비종양 조직은 정상군보다 p27Kip1의 발현이 감소하였으며 위암인접 비종양조직중 이행상피군이 만성위염과 장형화생보다 p27Kip1 발현이 감소되었다 (p<0.05). 위암종에서 진행성 위암종이 조기 위암종보다 p27Kip1 발현이 감소하였으며 (p<0.05), 미만형 위암종이 장형 위암종보다 p27Kip1 발현이 감소하였다 (p<0.05). p27Kip1 발현의 감소는 림프절 전이와 관련이 있었지만 (p<0.05), 원격전이와는 관련이 없었다 (p>0.05). 결론 : p27Kip1 발현이 위암종의 다단계 발생에서 초기에 감소됨을 알 수 있었고 위암종의 진행 및 분화와 연관성이 있었지만 예후인자로서의 가치는 더 많은 연구가 필요할 것으로 사료되었다. Background : The cyclin-dependent kinase inhibitor p27Kip1 is a negative regulator of cell cycle progression at G1/S transition. Recently, the expression level of p27Kip1 was decreased in many cancers such as breast, pituitary gland, colon and stomach. We studied the expression of p27Kip1 in gastric cancers, precancerous lesions and normal gastric tissues and analysed its correlation to clinicopathologic data including tumor differentiation, tumor depth, nodal and distant metastasis in gastric cancers. Methods : p27Kip1 were immunohistochemically stained in the tissue specimens of 62 resected cancers, 110 corresponding adjacent non-neoplastic tissues, 22 gastric adenomas and 10 normal gastric tissues. Adjacent non-neoplastic tissues consisted of 32 chronic gastritis, 29 intestinal metaplasia and 49 transitional mucosa. Results : Gastric cancers showed significantly decreased expression level of p27Kip1 when compared with non-neoplastic lesions and adenomas. Labeling index of p27Kip1 were more decreased in chronic gastritis, intestinal metaplasia and transitional mucosa than in normal mucosa. Early gastric cancers showed significantly decreased expression level of p27Kip1 when compared with advanced gastric cancers. In gastric cancers, p27Kip1 labeling index was significantly decreased in diffuse type and presence of nodal metastasis however did not show relationship with distant metastasis and tumor depth of advanced gastric cancers. Conclusion : We suggest that p27Kip1 may be decreased in the early stage of gastric carcinogenesis and play an important role in the progression and differentiation of gastric cancers. More further studies are thought to be necessary in order to evaluate its prognostic factor in gastric cancers.(Korean J Med 62:396-404, 2002)
A nationwide study of the incidence rate of herb-induced liver injury in Korea

<P>Discrepant incidence has been reported regarding the incidence of herb-induced liver injury (HILI). To address the growing worldwide concern of HILI, we evaluated the risk of HILI in a nationwide prospective study. Between April 2013 and January 2016, 1001 inpatients (360 males and 641 females) from 10 tertiary hospitals throughout South Korea were treated with herbal drugs and had their liver enzymes periodically measured. A total of six patients met the criteria for HILI with RUCAM scores ranging from 4 to 7. All these participants were women and developed the hepatocellular type of HILI. One HILI participant met the criteria for <I>Hy</I>’s law; however, none of six cases presented clinical symptoms related to liver injury. This is the first nationwide prospective study that estimated the extent of the incidence of HILI [total: 0.60%, 95% confidence interval (CI) 0.12–1.08; women: 0.95%, 95% CI 0.19–1.68] and described its features in hospitalized participants.</P><P><B>Electronic supplementary material</B></P><P>The online version of this article (doi:10.1007/s00204-017-2007-9) contains supplementary material, which is available to authorized users.</P>
