The round-the-clock technique for correction of gynecomastia

Background Gynecomastia is a common condition that can cause severe emotional and physical distress in both young and older men. Patients in whom symptomatic recalcitrant gynecomastia persists for a long time are potential candidates for surgery. Methods From January 2014 to January 2016, 15 patients underwent correction of gynecomastia through a single 3-mm incision at our institution. Only patients with true gynecomastia underwent surgery with this new technique. Through the small incision, sharp dissection was performed in a clockwise and counterclockwise direction describing two half-circles. Health-related quality of life and aesthetic outcomes were evaluated using a modified version of the Breast Evaluation Questionnaire (BEQ). Results The patients' average age was 23.5 years (range, 18-28 years), and their average body mass index was $23.2kg/m^2$ (range, $19.2-25.3kg/m^2$). One case was unilateral and 14 cases were bilateral. The weight of glandular tissue resected from each breast ranged from 80 to 170 g. No excess skin was excised. Bleeding was minimal. The mean operating time was 25 minutes (range, 21-40 minutes). No complications were recorded. All lesions were histologically benign. The patients' average score was 3.5 (on a 5-point Likert scale) in all domains of the BEQ for themselves and their partners. Conclusions In this study, we demonstrated the safety and reliability of a new technique that allows mastectomy through an imperceptible 3-mm incision. We obtained high patient satisfaction scores using our surgical technique, and patients reported considerable improvement in their social, physical, and psychological well-being after surgery.

In this report, we discuss the postoperative protocol for patients undergoing lymphaticovenous anastomosis (LVA) in our unit. Immediately after LVA, the incision site is closed over a small Penrose drain and a simple gauze dressing is applied without compression. In the first 5 days, ambulation is allowed, but limb elevation is actively encouraged to promote lymphatic flow across the newly formed anastomosis. Prophylactic antibiotics are routinely given to prevent infection because this patient group is susceptible to infections, which could trigger thrombosis in the anastomosis.

Several studies have been carried out on biodiesel production in microreactors. Usually, in conventionalscale processes, researchers and industry professionals use modeling and simulation to develop andoptimize equipment and processes. However, modeling and simulation in microfluidics are still newwhen compared to conventional scale, being the literature even scarcer for biodiesel synthesis. In thiscontext, the biodiesel synthesis is discussed in this paper from the viewpoint of modeling andmicroreactors’ simulations. A state-of-art review emphasizing the design of microdevices is presented,followed by a discussion about modeling and simulation. Finally, the conclusions are exposed.
Effect of Dupilumab on Sleep Apnea Severity in Patients With Chronic Rhinosinusitis

Patients with chronic rhinosinusitis (CRS) report improved sleep quality after dupilumab, an anti IL4/13 therapy. Concurrent CRS and obstructive sleep apnea (OSA) cases are not rare, and CRS seemingly raises nasal resistance. Thus, we hypothesized that improved sleep quality by dupilumab therapy in CRS patients might be due to lowered nasal resistance and subsequent improvement of unrecognized comorbid OSA. Patients with concurrent CRS and OSA were recruited. Nasal resistance was measured invasively with transnasal pressure and flow data collected during normal respiration in the supine position. Results from the first five participants did not support our hypothesis. Subjective and objective measures for CRS and nasal resistance values were improved with dupilumab therapy in CRS patients with nasal polyps. However, apnea severity and sleep-related subjective parameters did not change. In the patients with CRS without nasal polyps, no significant changes in either CRS or OSA-related measures were observed.
Droplet microfluidics for double lipase immobilisation using TiO2 and alginate microbeads

A double immobilisation technique was developed for C. antarctica lipase (CALB) that improved its enzymaticactivity and solved difficulties frequently observed in reactions catalysed by lipases. The firstimmobilisation consisted of CALB adsorption onto a TiO2 nanoparticle surface (CALB TiO2). The adsorptionwas carried out by an oriented monolayer formed by CALB’s hydrophilic amino acids and the TiO2 surface,leaving the CALB’s active site accessible for reaction. As a result, an increase in enzymatic activity wasachieved. The Relative Enzymatic Activity ((REA) obtained was 289%. The second immobilisation consistedof CALBTiO2 entrapment into calcium alginate microbeads [(CALBTiO2)EDTA-Ca], obtained by an internalcrosslinking ion-exchange mechanism and using microfluidic droplet technique. The microbiocatalystobtained, (CALBTiO2)EDTA-Ca, retained a high enzymatic activity (REAt=0 = 232%) and stability(REAt=30days = 263%) at a size (diameter, Ø=8.9104 nm) that enabled easier recovery than CALBTiO2(Ø = 2.3102 nm) or CALBFree (Ø=5.0 nm), and showed a favourable porosity for diffusion without releasingCALBTiO2. Although the microbeads showed CALBFree leaching, as demonstrated by the loss of REA after amechanical resistant test, (CALBTiO2)EDTA-Ca maintained an almost constant REA.

Chitin synthase (CHS) is a glucosyltransferase that converts UDP-N-acetylglucosamine into chitin, one of the main components of fungal cell wall. Class III chitin synthases act directly in the formation of the cell wall. They catalyze the conversion of the immediate precursor of chitin and are responsible for the majority of chitin synthesis in fungi. As such, they are highly specific molecular targets for drugs that can inhibit the growth and development of fungal pathogens. In this work, we have identified and characterized a chitin synthase gene of Moniliophthora perniciosa (Mopchs) by primer walking. The complete gene sequence is 3,443 bp, interrupted by 13 small introns, and comprises a cDNA with an ORF with 2,739 bp, whose terminal region was experimentally determined, encoding a protein with 913 aa that harbors all the motifs and domains typically found in class III chitin synthases. This is the first report on the characterization of a chitin synthase gene, its mature transcription product, and its putative protein in basidioma and secondary mycelium stages of M. perniciosa, a basidiomycotan fungus that causes witches’ broom disease of cacao.
