A rare case of primary intraosseous squamous cell carcinoma(PISCC) arising from lining epithelium of a dentigerous cyst is described. The case occurred at the left mandibular 3rd molar region in a 56-year-old Japanese woman. Clinical observation revealed cyst formation with an impacted 3rd molar, a common finding in dentigerous cyst, in the left mandible. Histopathologically, the lining epithelium of the cyst demonstrated transition from epithelial dysplasia to invasive squamous cell carcinoma(SCC). This case was diagnosed as PISCC arising from lining epithelium of a dentigerous cyst.
Dysplastic Odontogenic Cyst with the Manifestation of Inflammatory Radicular Cyst

This report describes a case of odontogenic cyst with keratinization and dysplastic change of lining epithelium, which showed the manifestation of inflammatory radicular cyst, clinically. A 28-year-old man complained of dull pain in the right mandibular molar region. Radiographically a well-defined oval cystic lesion with non-vital teeth, a common finding in radicular cyst, was observed. Microscopically, the lining epithelium of the cyst demonstrated both keratinization and severe epithelial dysplasia. Atypical findings such as hyperchromatic nuclei, increase of N/C ratio and drop shaped rege ridge were observed in the lining epithelium. However,definite invasion into fibrous connective tissue was not found. Immunohistochemically, the dysplastic lining epithelium was highly positive for proliferative marker, Ki-67. Based on the dysplastic changes of lining epithelium, this periapical lesion would be considered to be signs of malignant change. From this case, we conclude that definitive diagnosis by microscopical examination should be made,even if the periapical lesion would be clinically considered as inflammatory radicular cyst.

Carcinoma ex pleomorphic adenoma is a rare malignant salivary gland tumor. The carcinomatous component of the carcinoma ex pleomorphic adenoma is mostly one type such as adenocarcinoma NOS, salivary duct carcinoma and undifferentiated carcinoma. We present a case of carcinoma ex pleomorphic adenoma including two carcinomatous components. The tumor occurred in the palate of a 70-year-old man. Histopathologically, the tumor was composed of both benign pleomorphic adenoma and the carcinoma area that showed adenocarcinoma NOS and squamous cell carcinoma. Finally this case was diagnosed as carcinoma ex pleomorphic adenoma including two carcinomatous components

<P>We investigated the expression of p75<SUP>NGFR</SUP>, a proliferative and basal cell marker, in the mouse buccal mucosa epithelium during wound healing in order to elucidate the role of epithelial stem cells. Epithelial defects were generated in the epithelium of the buccal mucosa of 6-week-old mice using CO<SUB>2</SUB> laser irradiation. BrdU was immediately administered to mice following laser irradiation. They were then sacrificed after 1, 3, 7, and 14 days. Paraffin sections were prepared and the irradiated areas were analyzed using immunohistochemistry with anti-p75<SUP>NGFR</SUP>, BrdU, PCNA, and CK14 antibodies. During re-epithelialization, PCNA (–)/p75<SUP>NGFR</SUP> (+) cells extended to the wound, which then closed, whereas PCNA (+)/p75<SUP>NGFR</SUP> (+) cells were not observed at the edge of the wound. In addition, p75<SUP>NGFR</SUP> (–)/CK14 (+), which reflected the presence of post-mitotic differentiating cells, was observed in the supra-basal layers of the extended epithelium. BrdU (+)/p75<SUP>NGFR</SUP> (+), which reflected the presence of epithelial stem cells, was detected sparsely in buccal basal epithelial cells after healing, and disappeared after 7 days. These results suggest that p75<SUP>NGFR</SUP> (+) keratinocytes are localized in the basal layer, which contains oral epithelial stem cells, and retain the ability to proliferate in order to regenerate the buccal mucosal epithelium.</P>
Re-epithelialization of the Buccal Mucosa after Alkaline Chemical Injury

<P>Alkaline conditions in the oral cavity may be caused by a variety of stimuli, including tobacco products, antacids, alkaline drinking water and bicarbonate toothpaste. However, the effects of an alkaline pH on the oral mucosa had not been elucidated. The purpose of this study was to investigate how basal keratinocytes are actively involved in re-epithelialization after alkaline chemical injury. We generated epithelial defects in the oral mucosa of mice by applying an alkaline chemical, and the localization of cytokeratin 13, cytokeratin 14, PCNA and p63 was investigated during the re-epithelialization process. PCNA- and p63-positive staining was seen in basal cells covering the wound surface at 1 day after the chemical injury. Cytokeratin 14-positive and PCNA-negative basal keratinocytes were localized in a few layers of the wound epithelium during epithelial outgrowth. Cytokeratin 14-positive and PCNA-positive basal keratinocytes, indicating proliferation, were localized over the entire layer of the epithelium at the wound margin. These results imply that basal keratinocytes at the wound margin migrate to the wound surface, provoke differentiation and keratinization during epithelial outgrowth and that epithelial cells are supplied from the wound margin to the epithelial outgrowth after alkaline chemical injury.</P>

Significance of rescue hybrid endoscopic submucosal dissection in difficult colorectal cases

Background/Aims: Hybrid endoscopic submucosal dissection (ESD), in which an incision is made around a lesion and snaring is per-formed after submucosal dissection, has some advantages in colorectal surgery, including shorter procedure time and preventing per-foration. However, its value for rescue resection in difficult colorectal ESD cases remains unclear. This study evaluated the utility of res-cue hybrid ESD (RH-ESD). Methods: We divided 364 colorectal ESD procedures into the conventional ESD group (C-ESD, n=260), scheduled hybrid ESD group(SH-ESD, n=69), and RH-ESD group (n=35) and compared their clinical outcomes. Results: Resection time was significantly shorter in the following order: RH-ESD (149 [90–197] minutes) >C-ESD (90 [60–140] min-utes) >SH-ESD (52 [29–80] minutes). The en bloc resection rate increased significantly in the following order: RH-ESD (48.6%), SH-ESD (78.3%), and C-ESD (97.7%). An analysis of factors related to piecemeal resection of RH-ESD revealed that the submucosal dis-section rate was significantly lower in the piecemeal resection group (25% [20%–30%]) than in the en bloc resection group (40% [20%–60%]). Conclusions: RH-ESD was ineffective in terms of curative resection because of the low en bloc resection rate, but was useful for avoid-ing surgery.
