        An Anthropometric and Three-Dimensional Computed Tomographic Evaluation of Two-Point Fixation of Zygomatic Complex Fractures

        Background Maintaining stability and restoring the aesthetic appearance are the fundamental goals when managing zygomatic fractures. We aimed to evaluate the stability and anthropometric outcomes of zygomatic fracture patients who underwent two-point fixation involving the infraorbital rim and zygomaticomaxillary buttress via the transconjunctival and gingivobuccal approaches without any skin incisions. Methods We examined 15 zygomatic fracture patients who underwent two-point fixation during a 3-year period. Stability was evaluated using three-dimensional facial bone computed tomography. Superoinferior and anteroposterior displacement of the zygoma was quantified. The aesthetic appearance of the periorbital region was evaluated using indirect anthropometry with standardized clinical photographs. The ratios between the eye fissure height and width, and lower iris coverage ratio were used to evaluate aesthetical changes. The bony displacement and aesthetic ratios were analyzed using Wilcoxon or Friedman tests. The correlation between the preoperative zygoma position and anthropometric values was analyzed. Results The positions of the zygoma were similar to those on the contralateral side at the long-term follow-up. The preoperative anthropometric measurements on the fractured side differed from those on the contralateral side, although these values were close to the normal values at the long-term follow-up. Furthermore, we noted that the anteroposterior displacement strongly positively correlated with the lower iris coverage rate (Spearman's coefficient=0.678, P=0.005). Conclusions Two-point fixation of zygomatic fractures achieved stable outcomes on long-term follow-up, and also appeared to be reliable in restoring the aesthetic appearance of the periorbital region.

        Risk Factors of Donor Site Seroma Formation and Laboratory Analysis of Seromas after Breast Reconstruction with a Latissimus Dorsi Flap

        Background Donor site seroma is the most frequent and troublesome complication of latissimus dorsi (LP) flaps. This study aimed to identify the risk factors of seroma formation after an LD flap and to evaluate the biochemical composition of seromas. Methods The medical records of 84 patients who underwent an LD flap from September 2007 to May 2017 were reviewed. Age; body mass index (BMI); the type of breast surgery, reconstruction, and nodal dissection; the usage of fibrin glue; smoking; chemotherapy; and history of diabetes mellitus or hypertension were evaluated. In 11 of the 84 patients, the levels of electrolytes, glucose, proteins, lipids, and inflammatory markers present in seromas were investigated. Results The overall incidence of seroma was 66.7%. Advanced age (≥45 years) and overweight (BMI ≥23 kg/m2) were significant risk factors for seroma. Patients who underwent an extended LD flap had a higher incidence of seroma than those who underwent a standard LD flap, while those who underwent breast-conserving surgery had a lower incidence of seroma than those who underwent other breast procedures. Lactate dehydrogenase (LDH) levels in seromas on postoperative day 2 demonstrated a positive linear correlation with the duration of drainage, but this relationship did not reach statistical significance. Conclusions Advanced age, overweight, wider excision or mastectomy, and use of an extended LD flap were found to be risk factors for seroma formation after breast reconstruction with an LD flap. It may be possible to infer whether a seroma will be long-lasting by measuring LDH levels in the seroma.

        Volumetric changes of the pedicled transverse rectus abdominis musculocutaneous flap and the contralateral native breast during long-term follow-up

        Background Serial volumetric changes of reconstructed breasts have not been studied in detail. In this study, we analyzed serial volumetric changes of reconstructed and contralateral normal breasts during long-term follow-up, with a focus on the effect of various adjuvant therapies. Methods Among all patients who underwent immediate breast reconstruction with a unilateral pedicled transverse rectus abdominis musculocutaneous (p-TRAM) flap, 42 patients with valid data from ≥3 postoperative positron emission tomography-computed tomography (PET-CT) scans were included. The volumes of the reconstructed and normal breasts were measured, and the ratio of flap volume to that of the contralateral breast was calculated. Serial changes in volume and the volume ratio were described, and the effects of chemotherapy, radiation therapy, and hormone therapy on volumetric changes were analyzed. Results The mean interval between the initial reconstruction and each PET-CT scan was 16.5, 30, and 51 months respectively. Thirty-five, 36, and 10 patients received chemotherapy, hormone therapy, and radiation therapy, respectively. The flap volume at each measurement was 531.0, 539.6, and 538.0 cm3, and the contralateral breast volume was 472.8, 486.4, and 500.8 cm3, respectively. The volume ratio decreased from 115.1% to 113.4%, and finally to 109.6% (P=0.02). Adjuvant therapies showed no significant effects. Conclusions We demonstrated that the p-TRAM flap maintained its volume over a long-term follow up, while the volume of the contralateral native breast slowly increased. Moreover, adjuvant breast cancer therapies had no statistically significant effects on the volume of the reconstructed p-TRAM flaps or the contralateral native breasts.

        Systemic and Local Phenotypes of Barium Chloride Induced Skeletal Muscle Injury in Mice

        Skeletal muscle regeneration in mice has traditionally been studied using local freeze burn or snake venom injection models. More recently, a barium chloride (BaCl2)-induced muscle injury model has been established and is gaining popularity due to the relatively simple procedure and accessibility to required reagents. Here we sought to character-ize the local and systemic effects of BaCl2-induced muscle injury. For this study, a 1.2% BaCl2 solution was locally administered to the tibialis anterior (TA) muscle and local and systemic phenotypes were analyzed at different timepoints. When 50 μL of the solution was injected unilaterally in the TA muscle, no mortality was observed. However, when 100 μL of the solution was injected, 50% of the mice died within 24 h. Serum analysis of the mice injected with 50 μL of BaCl2 solution at days 1 and 7 revealed changes resembling rhabdomyolysis. At day 1 post-injection of 50 μL of the BaCl2 solution, acute suppurative inflammation was observed in gross examination of the TA muscle, while extensive hemor-rhagic necrosis was revealed on histological examination. At day 7, regenerated myofibers with centralized nuclei appeared with the resolution of acute inflammatory infiltration and the muscle tissue displayed molecular signatures consistent with myofiber differentiation. The overall muscle injury and regeneration phenotypes in the BaCl2-induced muscle injury model were similar to those of the well-established freeze burn or snake venom injection models. Taken together, the BaCl2-induced muscle injury model is comparable to conven-tional muscle injury and regeneration models, with considerations for possible systemic effects.

        As an electrochemical water electrolysis for green hydrogen production, both polymer electrolyte membrane (PEM) and alkaline electrolyte are being developed extensively in various countries. The PEM electrolyzer with high current density (above 2 A/cm<sup>2</sup>) has the advantage of being able to design a simple structure. Also, it is known that it has high response to electrical output fluctuations. However, the cost problem of major components is the most important issue that a PEM electrolyzer must overcome. Instantly, there are platinum group metal (PGM)-based electrocatalysts, fluorine-based polyfluoro sulfuric acid (PFSA) membrane, Ti felt (porous transport layer, PTL) and so on. Another challenging issue is productivity. A securing outstanding productivity brings price benefits of the electrolytic cells. From this point of view, we conducted basic studies on manufacturing electrode and membrane electrode assembly (MEA) for PEM electrolyzer production.

        Operational Characteristics of High-Performance kW class Alkaline Electrolyzer Stack for Green Hydrogen Production

        Polymer electrolyte membrane (PEM) electrolyzer or alkaline electrolyzer is required to produce green hydrogen using renewable energy such as wind and/or solar power. PEM and alkaline electrolyzer differ in many ways, instantly basic materials, system configuration, and operation characteristics are different. Building an optimal water hydrolysis system by closely grasping the characteristics of each type of electrolyzer is of great help in building a safe hydrogen ecosystem as well as the efficiency of green hydrogen production. In this study, the basic operation characteristics of a kW class alkaline water electrolyzer we developed, and water electrolysis efficiency are described. Finally, a brief overview of the characteristics of PEM and alkaline electrolyzer for large-capacity green hydrogen production system will be outlined.

        스마트기기를 활용한 과학 교사의 교수 실행과 과학교육에서 스마트교육 적용 방안에 대한 의견 조사

        이 연구에서는 스마트기기를 활용한 과학 수업의 사례를 분석하고, 스마트교육의 실행에서 과학 교사들이 겪는 어려움과 교육 요구, 과학 교육에서 스마트교육의 적용 방안에 대한 교사들의 인식을 조사하였다. 연구 참여자는 서울 지역의 과학 교사 모임에서 스마트교육 관련 소모임에 참여하고 있는 교사 7명이었다. 교사들이 수업에 사용한 교수학습 자료를 수집한 후 스마트기기를 사용한 실제 수업에 대해 1차면담을 실시하였다. 그리고 이 자료들을 종합적으로 분석하여 과학교육에서 스마트교육의 요소별 정의를 정교화 하였다. 이를 바탕으로 2차면담에서 스마트기기를 활용한 과학 수업에 대한 교사들의 응답 내용을 귀납적으로 범주화하여 분석하였다. 연구 결과, 모든 교사들이 필요할 때 즉시적 상호작용 요소를 활용하였으나, 질의응답이 일어나는 정도의 단순한 수준이었다. SNS, 위키 기반의 서비스를 이용한 협력적 상호작용 요소를 활용하는 경우는 적어서 스마트기기가 상호 작용 지원을 위해 효과적으로 활용되고 있지 않았다. 또한, 대부분의 교사들이 학습자 수준에 따른 개별화 측면에서 학생들의 학습 결과를 수집하여 피드백을 하였으나, 학습자의 맥락, 상황 및 위치 인식 요소에 대해서는 인식하지 못하였다. 모든 과학 교사들은 학습 기회의 확대 요소를 수업에서 주로 활용하였으나, 자기주도 학습 지원 요소에 대해서는 인식하지 못하였다. 과학 교사들은 현재 학생들에게 다양한 자료를 제공하거나 학습자의 수준을 판단하기 위해 스마트기기를 활용하고 있으나, 향후 스마트교육이 학생들의 협력적 상호작용과 자기주도 학습을 돕는 방향으로 발전할 필요가 있다는 의견을 많이 제시하였다. 그리고 이를 위한 교사 연수와 환경적인 지원에 대한 다양한 의견을 제시하였다. 이러한 연구 결과를 바탕으로 과학교육에서 스마트교육을 정착시키고 활성화하기 위한 방안에 대해 논의하였다. In this study, we analyzed the teaching practices of science teachers using smart technologies and investigated their difficulties in implementing smart education and their educational needs. We also studied their opinions on the application of smart technologies in science education. The participants were seven science teachers who participated in the smart education study group of a science teacher association in Seoul. We elaborated on the characteristics of smart education in science education during comprehensive analyses of instructional materials used in science classes and the initial interviews. We then analyzed the second interviews by categorizing their responses inductively. All the science teachers used the ‘instant access as needed’, but their interactions, simply answering students’ questions, were found to be at a low level. They did not effectively use the ‘collaborative interaction with SNS or wiki-based service’ for the support for interactive learning. While most collected learning results of their students and provided feedback in the aspect of ‘individualization according to leaner level’, they were not aware of ‘context, situation, and location of learners’ in smart education. While all the teachers extended learning opportunities by using learning resources widely in smart education, most were not aware of ‘support for self-directed learning’. Most teachers believed that smart education should be developed to help students learn interactively and in a self-directed manner. They also provided many opinions on teacher training programs and environmental support for smart education. Based on the results, some considerations for implementing smart education in science instructions effectively are discussed.

        Reconstruction of a pathologic fracture following osteomyelitis of the mandible using a fibula osteocutaneous flap

        The use of a fibula osteocutaneous flap is currently the mainstay of segmental mandibular reconstruction. This type of flap is used to treat tumors, trauma, or osteoradionecrosis of the mandible. However, a fibula osteocutaneous flap may also be a good option for reconstructing the mandible to preserve oropharyngeal function and facial appearance in cases of pathological fracture requiring extensive segmental bone resection. Chronic osteomyelitis is one of the various causes of subsequent pathologic mandibular fractures; however, it is rare, and there have been few reports using free flaps in osteomyelitis of the mandible. We share our experience with a 76-year-old patient who presented with a pathologic fracture following osteomyelitis of the mandible that was reconstructed using a fibula osteocutaneous flap after wide segmental resection.
