        Sofosbuvir/velpatasvir plus ribavirin for Child-Pugh B and Child-Pugh C hepatitis C virus-related cirrhosis

        Background/Aims: Real-world studies assessing the effectiveness and safety of sofosbuvir/velpatasvir (SOF/VEL) plus ribavirin (RBV) for Child-Pugh B/C hepatitis C virus (HCV)-related cirrhosis are limited. Methods: We included 107 patients with Child-Pugh B/C HCV-related cirrhosis receiving SOF/VEL plus RBV for 12 weeks in Taiwan. The sustained virologic response rates at off-treatment week 12 (SVR<sub>12</sub>) for the evaluable population (EP), modified EP, and per-protocol population (PP) were assessed. The safety profiles were reported. Results: The SVR<sub>12</sub> rates in the EP, modified EP and PP were 89.7% (95% confidence interval [CI], 82.5-94.2%), 94.1% (95% CI, 87.8-97.3%), and 100% (95% CI, 96.2-100%). Number of patients who failed to achieve SVR<sub>12</sub> were attributed to virologic failures. The SVR<sub>12</sub> rates were comparable regardless of patient characteristics. One patient discontinued treatment because of adverse events (AEs). Twenty-four patients had serious AEs and six died, but none were related to SOF/VEL or RBV. Among the 96 patients achieving SVR<sub>12</sub>, 84.4% and 64.6% had improved Child-Pugh and model for endstage liver disease (MELD) scores. Multivariate analysis revealed that a baseline MELD score ≥15 was associated with an improved MELD score of ≥3 (odds ratio, 4.13; 95% CI, 1.16-14.71; P=0.02). Patients with chronic kidney disease (CKD) stage 1 had more significant estimated glomerular filtration rate declines than patients with CKD stage 2 (-0.42 mL/min/1.73 ㎡/month; P=0.01) or stage 3 (-0.56 mL/min/1.73 ㎡/month; P<0.001). Conclusions: SOF/VEL plus RBV for 12 weeks is efficacious and well-tolerated for Child-Pugh B/C HCV-related cirrhosis. (Clin Mol Hepatol 2021;27:575-588)

      • Inhibition of Tumor Growth in Vitro by a Combination of Extracts from Rosa Roxburghii Tratt and Fagopyrum Cymosum

        Objective: Traditional Chinese herbal medicines have a very long history. Rosa roxburghii Tratt and Fagopyrum cymosum are two examples of plants which are reputed to have benefits in improving immune responses, enhancing digestive ability and demonstrating anti-aging effects. Some evidence indicates that herbal medicine soups containing extracts from the two in combination have efficacy in treating malignant tumors. However, the underlying mechanisms are far from well understood. The present study was therefore undertaken to evaluate anticancer effects and explore molecular mechanisms in vitro. Methods: Proliferation and apoptosis were assessed with three carcinoma cell lines (human esophageal squamous carcinoma CaEs-17, human gastric carcinoma SGC-7901 and pulmonary carcinoma A549) by MTT assay and flow cytometry, respectively, after exposure to extract from Rosa roxburghii Tratt (CL) and extract from Fagopyrum cymosum (FR). $IC_{30}$ of CL and FR were obtained by MTT assay. Tumor cells were divided into four groups : control with no exposure to CL or FR; CL with $IC_{30}$ CL; FR with $IC_{30}$ FR; CL+FR group with 1/2 ($IC_{30}$ CL + $IC_{30}$ FR). RT-PCR and Western blot analysis were used to detect the expression of Ki-67, Bax and Bcl-2 at mRNA and protein levels. Results: Compared with the CL or FR groups, the combination of CL+FR showed significant inhibition of cell growth and increase in apoptosis; the mRNA and protein expression levels of Ki-67 and Bcl-2 in CL+FR group were all greatly decreased, while the expression of Bax was markedly increased. Conclusions: These results indicate that the synergistic antitumor effects of combination of CL and FR are related to inhibition of proliferation and induction of apoptosis.

        Single-Phase Current-Source Bidirectional Converter for V2G Applications

        In this paper, a single-phase current-source bidirectional converter topology for V2G applications is proposed. The proposed converter consists of a single-phase current-source rectifier (SCSR) and an auxiliary switching network (ASN). It offers bidirectional power flow between the battery and the grid in the buck or boost mode and expands the output voltage range, so that it can be compatible with different voltage levels. The topology structure and operating principles of the proposed converter are analyzed in detail. An indirect control algorithm is used to realize the charging and discharging of the battery. Finally, the semiconductor losses and system efficiency are analyzed. Simulation and experimental results demonstrate the validity and effectiveness of the proposed topology.

        Remarkable impact of amino acids on ginsenoside transformation from fresh ginseng to red ginseng

        Background: Amino acids are one of the major constituents in Panax ginseng, including neutral amino acid, acidic amino acid, and basic amino acid. However, whether these amino acids play a role in ginsenoside conversion during the steaming process has not yet been elucidated. Methods: In the present study, to elucidate the role of amino acids in ginsenoside transformation from fresh ginseng to red ginseng, an amino acids impregnation pretreatment was applied during the steaming process at 120℃. Acidic glutamic acid and basic arginine were used for the acid impregnation treatment during the root steaming. The ginsenosides contents, pH, browning intensity, and free amino acids contents in untreated and amino acid-treated P. ginseng samples were determined. Results: After 2 h of steaming, the concentration of less polar ginsenosides in glutamic acid-treated P. ginseng was significantly higher than that in untreated P. ginseng during the steaming process. However, the less polar ginsenosides in arginine-treated P. ginseng increased slightly. Meanwhile, free amino acids contents in fresh P. ginseng, glutamic acid-treated P. ginseng, and arginine-treated P. ginseng significantly decreased during steaming from 0 to 2h. The pH also decreased in P. ginseng samples at high temperatures. The pH decrease in red ginseng was closely related to the decrease in basic amino acids levels during the steaming process. Conclusion: Amino acids can remarkably affect the acidity of P. ginseng sample by altering the pH value. They were the main influential factors for the ginsenoside transformation. These results are useful in elucidating why and how steaming induces the structural change of ginsenoside inP. ginseng and also provides an effective and green approach to regulate the ginsenoside conversion using amino acids during the steaming process.

        Intracellular Polysaccharide and its Antioxidant Activity by Pleurotus citrinopileatus SM-01

        The extraction parameters of intracellular polysaccharide (IPS) from Pleurotus citrinopileatus SM-01mycelia were optimized, and the in vitro and in vivo antioxidant activities of IPS were investigated. The optimum conditions of IPS extraction were predicted to be an ultrasonic treatment time of 664.09 s, precipitation time of 23.03h and pH 7.36, and IPS yield was estimated at 16.13%. The in vitro inhibition effects of IPS at a dosage of 5 g/L on the superoxide anion, 1,1-diphenyl-2-picrylhydrazyl (DPPH) and hydroxyl radicals were 73.96±4.62%, 69.2±4.37%,and 50.75±4.39%, respectively, which were 72.56±5.08%, 22.83±1.94%, and 43.93±3.26% higher than that of butylated hydroxytoluene (BHT), respectively. The reducing power of IPS was 0.9±0.07, 69.81±5.24% higher than that of BHT. The activities of glutathione peroxidase (GSH-Px), superoxide dismutase (SOD), catalase (CAT) and alanine aminotransferase (ALT) in mice blood were 241.38±23.19, 454.95±42.39, 60.32±5.16, and 32.39±2.54 U/mL,respectively, and the malonaldehyde (MDA) level was 9.54±0.72 nmol/mL. The results provided a reference for the large-scale extraction of IPS by P. citrinopileatus SM-01 in industrial fermentation, suggesting that the IPS can be used as a potential antioxidant, which enhances adaptive immune responses.

        An Improved Steganography Method Based on Least-Significant-Bit Substitution and Pixel-Value Differencing

        This research was based on the study conducted by Khodaei et al. (2012), namely, the least-significant-bit (LSB) substitution combined with the pixel-value differencing (PVD) steganography, and presented an improved irreversible image steganography method. Such a method was developed through integrating the improved LSB substitution with the modulus function-based PVD steganography to increase steganographic capacity of the original technique while maintaining the quality of images. It partitions the cover image into non-overlapped blocks, each of which consists of 3 consecutive pixels. The 2nd pixel represents the base, in which secret data are embedded by using the 3-bit LSB substitution. Each of the other 2 pixels is paired with the base respectively for embedding secret data by using an improved modulus PVD method. The experiment results showed that the method can greatly increase steganographic capacity in comparison with other PVD-based techniques (by a maximum amount of 135%), on the premise that the quality of images is maintained. Last but not least, 2 security analyses, the pixel difference histogram (PDH) and the content-selective residual (CSR) steganalysis were performed. The results indicated that the method is capable of preventing the detection of the 2 common techniques.
