Background/Aims: The fatigue, resistance, ambulation, illnesses, and loss of weight (FRAIL) scale is a screening tool for frailty status using a simple 5-item questionnaire. The aim of this study was to evaluate the clinical feasibility and validity of the Korean version of the FRAIL (K-FRAIL) scale. Methods: Questionnaire items were translated and administered to 103 patients aged ≥ 65 years who underwent a comprehensive geriatric assessment at the Seoul National University Bundang Hospital. In this cross-sectional study, the K-FRAIL scale was compared with the domains and the multidimensional frailty index of the comprehensive geriatric assessment. We also assessed the time required to complete the scale. Results: The participants’ mean age was 76.8 years (standard deviation [SD], 6.1), and 55 (53.4%) were males. The mean overall frailty index was 0.19 (SD, 0.17). For K-FRAIL-robust, prefrail, and frail patients, the mean frailty indices were 0.09, 0.18, and 0.34, respectively (p for trend < 0.001). A higher degree of impairment in the K-FRAIL scale was associated with worse nutritional status, poor physical performance, functional dependence, and polypharmacy. The number of items with impairment in the K-FRAIL scale was positively associated with the frailty index (B = 3.73, p < 0.001). The K-FRAIL scale could differentiate vulnerability from robustness with a sensitivity of 0.90 and a specificity of 0.33. Of all patients, 75 (72.8%) completed the K-FRAIL scale within < 3 minutes. Conclusions: The K-FRAIL scale is correlated with the frailty index and is a simple tool to screen for frailty in a clinical setting.

「화학물질관리법」 시행 이후, 2015년부터 2017년까지 제출된 장외영향평가서는 점차적으로 소규모 수준이 증가하고 있는 추세이다. 소규모 사업장에서 제출된 장외영향평가서의 업종을 15개 분류로 구분하여 확인하였다. 그 결과, 표면처리(도금)(166건), 인쇄(그라비아)(36건) 등으로 분석되었다. 이 에 따라, 동일한 공간에 두 대 이상의 취급설비가 위치한 경우에는 R값 적용이 필히 고려되어야 한다. 개별 취급시설은 소량기준 미만을 만족하더라도 R값이 1 이상이면 안전성 확보방안 및 관리적 대책이 추가된 장외영향평가서를 표준 수준으로 제출해야한다. 이에 따라, 본 연구에서는 소규모 수준 장외영향평가서의 다빈도 업종인 인쇄(그라비아)공정과 표면처리(도금)공정 R값을 산정하였 고, 이를 바탕으로 사각지대에 있는 소규모 및 영세 사업장의 화학 사고를 예방하는데 도움이 될 것으로 기대된다. Since the implementation of the Act on Chemical Substance Management, the number of the small-scale Off-site Risk Assessment (ORA) reports submitted has been gradually increasing from 2015 to 2017. Classifying industrial categories of the ORA reports submitted by small business sites into 15 processes, 166 were found to be surface treatment (plating) and 36 were printing (gravia). When more than 2 facilities are located in the same space, the R value must be applied. If the R value is greater than 1 even if the individual facility meets the small-quantity criteria, the level-2 ORA report should be submitted to the government in addition to safety and administrative measures. Thus, this study determined R values for printing (gravia) and surface treatment (plating) processes, which are the industries that mainly submit a small-scale ORA report. The findings from this study are expected to help prevent chemical accidents in small businesses in blind spots.

<P><B>Abstract</B></P> <P> <I>Corynebacterium glutamicum</I> was metabolically engineered for the production of glutaric acid, a C5 dicarboxylic acid that can be used as platform building block chemical for nylons and plasticizers. <I>C. glutamicum gabT</I> and <I>gabD</I> genes and <I>Pseudomonas putida davT</I> and <I>davD</I> genes encoding 5-aminovalerate transaminase and glutarate semialdehyde dehydrogenase, respectively, were examined in <I>C. glutamicum</I> for the construction of a glutaric acid biosynthesis pathway along with <I>P. putida davB</I> and <I>davA</I> genes encoding lysine 2-monooxygenase and delta-aminovaleramidase, respectively. The glutaric acid biosynthesis pathway constructed in recombinant <I>C. glutamicum</I> was engineered by examining strong synthetic promoters P<SUB>H30</SUB> and P<SUB>H36</SUB>, <I>C. glutamicum</I> codon-optimized <I>davTDBA</I> genes, and modification of <I>davB</I> gene with an N-terminal His<SUB>6</SUB>-tag to improve the production of glutaric acid. It was found that use of N-terminal His<SUB>6</SUB>-tagged DavB was most suitable for the production of glutaric acid from glucose. Fed-batch fermentation using the final engineered <I>C. glutamicum</I> H30_GA<SUB>His</SUB> strain, expressing <I>davTDA</I> genes along with <I>davB</I> fused with His<SUB>6</SUB>-tag at N-terminus could produce 24.5 g/L of glutaric acid with low accumulation of <SMALL>L</SMALL>-lysine (1.7 g/L), wherein 5-AVA accumulation was not observed during fermentation.</P> <P><B>Highlights</B></P> <P> <UL> <LI> <I>C. glutamicum</I> strains were metabolically engineered for the production of glutaric acid. </LI> <LI> Optimization of glutaric acid biosynthesis pathway could enhance glutaric acid production. </LI> <LI> Recombinant <I>C. glutamicum</I> strains efficiently produced glutaric acid from glucose. </LI> <LI> Fed-batch culture of recombinant <I>C. glutamicum</I> H30_GA<SUB>His</SUB> supported high-level production of glutaric acid from glucose. </LI> </UL> </P>

Anti-cancer effects of naringenin derivative diethyl (5,4'-dihydroxy flavanone-7-yl) phosphate were evaluated in human lung cancer cells. The effect of diethyl (5,4'-dihydroxy flavanone-7-yl) phosphate (dEdHF-7-p) on A549 cell viability was measured using MTS assay and cell counting. Morphological changes were detected using phase-contrast microscopy. Apoptosis was analyzed using Hoechst staining. The influence of dEdHF-7-p on cell cycle distribution was determined using propidium iodide (PI) staining, and protein expression was determined by Western blot analysis. A newly synthesized naringenin derivative dEdHF-7-p suppressed cell growth of A549 though mechanisms including inhibition of cell cycle and increased apoptosis. Apoptotic and cell cycle modulators were changed by dEdHF-7-p in A549 cells; cyclins, ppRB, and anti-apoptotic factor Bcl-2 were down-regulated, whereas apoptotic factor Bax and cyclin-dependent kinase inhibitors p21 and p53 were enhanced, thereby releasing cytochrome c into the cytosol of dEdHF-7-p-treated-A549 cells. dEdHF-7-p treatment processed caspases-3/-8/-9 and cleavage of poly ADP-ribose polymerase. The dEdHF-7-p treatment enhanced Fas expression and decreased expression of cell survival factors such as PI3K and p-Akt in a dose-dependent manner. Taken together, dEdHF-7-p induces apoptosis by inhibiting the PI3K/Akt survival signaling pathway and modulating mitochondria-emanated intrinsic and Fas extrinsic pathways in A549 cells.

<P>Schisandra chinensis Turcz. fruit is widely used to treat skin diseases. The aim of this study was to determine the anti-inflammatory effects of the methanol extract of S. chinensis (MESC) on 1-fluoro-2,4-dinitrofluorobenzene (DNFB)-induced contact dermatitis (CD) in mice. The effects of MESC on ear thickness and weight, histopathological changes, immune cell filtration and cytokine production were investigated in DNFB-induced CD mice. Topical application of MESC effectively inhibited ear swelling (30 or 300 μg on the left ear, P<0.001; 30 μg on the right ear, P<0.001). MESC also inhibited hyperplasia, spongiosis (100 μg/ear, P<0.05 and 300 μg/ear, P<0.001, respectively) and immune cell infiltration (100 μg/ear, P<0.05; 300 μg/ear, P<0.001) induced by DNFB. In addition, MESC suppressed increases in tumor necrosis factor (TNF)-α levels (100 or 300 μg/ear, P<0.05), interferon (INF)-γ (30 μg/ear, P<0.05; 100 μg/ear, P<0.01; 300 μg/ear, P<0.001), interleukin (IL)-6 (300 μg/ear, P<0.05) and monocyte chemoattractant protein (MCP)-1 (30 μg/ear, P<0.05; 100 μg/ear, P<0.01; 300 μg/ear, P<0.001). These results suggest that the anti-inflammatory effects of MESC are mediated by the reduced production of TNF-α, IFN-γ, IL-6 and MCP-1, and that MESC has potential use for the treatment of inflammatory skin diseases.</P>
소아에서 혈청 25-hydroxyvitamin D와 알레르기비염의 연관성

Purpose: Vitamin D is known to play an important role in the regulation of the immune system and in the prevention of allergic disease. Herein, we examined the association between vitamin D and allergic rhinitis (AR) in children. Methods: A total of 72 aeroallergen-proven AR and 57 healthy control subjects who visited Severance Children`s Hospital between 2012 and 2014 were evaluated. The mean serum 25-hydroxyvitamin D (25(OH)D) level was measured in all subjects to evaluate their vitamin D status. Results: The mean serum 25(OH)D levels was significantly lower in patients with AR than in controls (19.5±6.4 ng/mL vs. 22.3±5.8 ng/mL, P=0.013). The incidence of AR is higher in the 25(OH)D-insufficient group (15-20 ng/mL) and in the 25(OH)D-deficient group (≤15 ng/mL) compared with the 25(OH)D-sufficient group (>20 ng/mL) (40.3% vs. 84.4% vs. 22.2%, P<0.001). There was no significant difference in the serum 25(OH)D level between subgroups with mild and moderate-to-severe AR symptoms (19.8±6.5 vs. 19.0±6.3, P=0.596). After adjusting for confounding factors, the lower serum vitamin D level groups had increased odds of AR compared with the 25(OH)D-sufficient group (odds ration [95% confidence interval]; 3.67 [1.29-10.38] in the 25(OH)D-insufficient group; P=0.014 and 6.04 [1.49-24.51] in the 25(OH)D-deficient group; P=0.012). Conclusion: The serum vitamin D levels were significantly lower in children with AR compared with healthy controls. Therefore, lower serum level of 25(OH)D may be associated with the development of AR. (Allergy Asthma Respir Dis 2016:4:423-428)

Foreign body aspiration commonly occurs, especially among children younger than 3 years of age. Most endobronchial foreign bodies may be easily removed by bronchoscopy and have a good prognosis. Despite advances in bronchoscopic procedures, difficulties in management of and complications from foreign body aspiration are still noted. We present a foreign body aspiration case with acute respiratory distress syndrome caused aspiration pneumonia, which was successfully treated by bronchoscopy under extracorporeal membrane oxygenation (ECMO). ECMO can be an effective and potentially life-saving method in complicated foreign body aspiration with uncompensated respiratory failure. (Allergy Asthma Respir Dis 2016;4:140-144)
Prediction of Mortality by Modified Multidimensional Frailty Score in Hip Fracture Elderly Patients

Background: Hip fractures are serious injuries among frail older patients that often result in long-term functional impairment, institutionalization, and increased mortality. However, clinical indicators predicting prognosis are limited. Methods: We included a total of 535 low trauma hip fracture patients (age ≥ 65) undergoing surgery from May 2009 to June 2014. The modified-multidimensional frailty score (mMFS) was calculated by comprehensive geriatric assessment. The primary outcome was 6-month all-cause mortality rate. The secondary outcomes were 1-year mortality, post-operative complication (e.g., pneumonia, urinary tract infection, delirium, acute pulmonary thromboembolism, and unplanned intensive care unit admission), length of hospital stay and institutionalization. Results: 39 patients (7.3%) died during 6 months after operation (median [interquartile range], 2.4 [1.3-3.9] months), including 3 in-hospital deaths. The fully-adjusted odds ratios per 1 point increase in mMFS were 1.425 (95% CI; 1.176- 1.725 p <0.001) for 6 month mortality, 1.329 (95% CI; 1.158-1.527 p<0.001) for postoperative complications, 0.974 (95% CI; 0.899-1.054, p=0.510) for institutionalization, and 1.121 (95% CI; 1.017-1.235, p=0.022) for prolonged total hospital stay. High risk patients (mMFS >9) showed higher risk of 6-month mortality (OR: 5.158, 95% CI; 2.027-13.213, p<0.001) and postoperative complication (OR: 2.110, 95% CI; 1.373-3.244, p=0.001) than low risk patients after adjustment. Conclusions: Modified MFS based on preoperative comprehensive geriatric assessment is useful for predicting 6 months mortality, postoperative complications and prolonged hospital stay in elderly hip fracture patients after surgery
