Study to the Effect of Involutional Blepharoptosis Surgery Using Objective and Subjective Parameters

Background We investigate the effect of involutional blepharoptosis (IB) surgery based on dry eye symptoms by analysis using objective and subjective measures. Methods We recorded various parameters from patients that underwent levator advancement surgery for IB, totaling 125 eyes (total 65 patients, 5 unilateral, 60 bilateral). Subjective assessment comprised a questionnaire on dry eye-related quality of life score (DEQS), a summary score calculated from DEQS, and six-grade evaluation, the patient’s own measure of eye comfort. Objective assessment comprised marginalreflex distance-1 (MRD-1), measurement of tear film breakup time, and superficial keratopathy (SPK) existence by slit lamp microscope. Results Subjective assessments showed that IB patients had improvement of dry eye symptoms and eye comfort when surgery increased MRD-1. On the other hand, objective assessments showed that the presence of SPK is suspected when the postoperative MRD-1 level is 3mm or higher. Conclusion IB surgery must not only increase MRD-1 value, but also to perform maintenance of the appropriate ocular surface condition. From our parameters, we suggest postoperative MRD-1 value should be maintained at<3mm to safe and effective of IB surgery.

Purpose: The purpose of this study was to evaluate whether neo-adjuvant hormonal therapy (NHT) prior to radical retropubic prostatectomy (RRP) for prostate cancer (PCa) is beneficial in terms of surgical outcomes and for preventing or delaying biochemical recurrence via single-surgeon case series study. Materials and Methods: Fifty-three men underwent RRP by a single surgeon. The patients were divided into two groups according to whether or not NHT was performed prior to RRP. The study was analyzed retrospectively. We evaluated clinical parameters,surgical parameters, and biochemical recurrence rate. Group 1 (n=34) was treated with RRP only, while Group 2 (n=19) underwent RRP along with NHT. Results: There were no significant differences in clinical, operation-related and pathological factors between the two groups (p>0.05). There was also no significant difference in biochemical recurrence rate between the two groups at the last follow-up, although Group 2 tended to have a lower PCa recurrence rate than Group 1 and the initial prostate-specific antigen (PSA) level was significantly higher in Group 2 than Group 1 (p=0.0496). Conclusion: The present single-surgeon case series study revealed a trend toward a lower rate of PCa recurrence in NHT+RRP treated patients compared to those treated with RRP alone, but this did not reach statistical significance, despite the fact that NHT+RRP patients exhibited higher serum PSA levels preoperatively. Prospective studies with a longer duration of observation and a greater number of patients would be helpful in evaluating NHT more definitively.
Preliminary Study of Needle Tracking in a Microsurgical Robotic System for Automated Operations

Surgical needle tracking is an important element of high-level automated operations conducted by surgical robotic systems. However, conventional needle tracking algorithms lack robust performancewith different needle postures and are not applicable to the small needles used during microsurgery. This paper discusses a robust, efficient needle tracking algorithm, which is capable of estimating all of the positions and of the postures of a microsurgical needle. In the preoperative preparation stage, contour models of the microsurgical needle are generated using a 3-D CAD model and saved in a database. During the operation, the system extracts the contours of the microsurgical needle from the microscopic image using the edge and the color information. The system then calculates the likelihood of the contour models in the database bymatching the contours extracted fromthemicroscopic image. The experimental results indicated that our proposed method has high accuracy when tracking a microsurgical needle, and that it performed robustly with different needle postures.
PCB design for 13.56MHz half-bridge class D inverter for wireless power transfer system

At 13.56MHz, the inverter is strongly affected by parasitic elements. The print circuit board (PCB) design is very critical because it control the parasitic elements on the circuit. This paper analyzes the effect of parasitic elements on switching performance of MOSFETs in 13.56MHz class D inverter. This work also proposes an improved PCB design which can provide a 23.4% decrease in parasitic inductance comparing with conventional PCB design.

Background: Considering concerns about conventional toe grip strength, we devised a method to measure toe pressure strength in the standing position, which is close to the actual motion. This study examined the association between toe pressure strength in the standing position and walking speed among older adults. Methods: This cross-sectional study included 150 community-dwelling older adults (81±8 years, 73% female) who participated in the physical fitness test. We analyzed the correlation between the participants’ maximum walking speed and physical function. Furthermore, we performed regression analysis with the maximum walking speed as the dependent variable to examine the association with toe pressure strength in the standing position. We also examined the association between maximum walking speed and toe pressure strength in the standing position by introducing a covariate. Results: Correlation analysis revealed a significant positive correlation between maximum walking speed and toe pressure strength in the standing position, with a moderate effect size (r=0.48, p<0.001). Moreover, multiple regression analysis with covariates showed an association between maximum walking speed and toe pressure strength in the standing position (standardization factor=0.13, p<0.026). Conclusion: Toe pressure strength in the standing position was associated with maximum walking speed. This finding clarifies the significance of assessing toe pressure strength in the standing position and suggests that enhanced toe pressure strength in the standing position may increase maximum walking speed.

Welding technology has been developed to improve welding quality. Especially, plasma arc welding has been used for a wide range of areas. However, welding defects occur because of inappropriate arc conditions and lack of arc control with metal vapor contamination in the arc column and anode root. The metal vapor contamination from the anode is varied by the electrical conductivity of the arc in high-temperature medium. The arc emits intense radiation. In this paper, the temperature distribution of the plasma arc contaminated with iron vapor from the anode under consideration of self-absorption was elucidated to develop the plasma arc welding technology and avoid the welding defect. Electromagnetohydrodynamic (MHD) simulation is used to calculate the temperature. Results show that the temperature distribution was varied under consideration of self-absorption because of the iron vapor distribution, and because ionized argon exists near the cathode.
