퇴행성 관절염에서 Interleukin-6와 Soluble Interleukin-6 Receptor

Objective: Unlike other soluble receptors, the soluble interleukin-6 receptor (sIL-6R) cooperates with IL-6 to activate gp130 of effector cell. As the IL-6 and sIL-6R are important in the rheumatoid disease, this study was designed to measure concentration of IL-6 and sIL-6R in synovium and synovial fluid of the degenerative arthritis. Methods: The synovium and synovial fluid were obtained during total knee replacement arthroplasty. The synovium was taken from eleven patients, and synovial fluid taken from sixteen patients. Same patients between two groups were seven. Tissue cultures of the synovial tissues were done with 10% FBS for 72 hours. After irrigation, thery were incubated for 48 hours without FBS, and the culture media and the synovial fluid were collected after centrifuged at 2500rpm for 10 minutes. The level of IL-6 and sIL-6R were measured by quantitative sandwich enzyme immunoassay technique. Results: In the synovium, the IL-6 level was 5.1±0.12ng/ml, and the sIL-6R level was 0.41±0.25ng/ml. In the synovial fluid, the IL-6 level was 0.09± 0.15ng/ml, and the sIL-6R level was 10.37±3.28ng/ml. These results show that IL-6 concentration was measured highly in two groups, especially in synovium (sixty times), and the sIL-6R concentration was measured significantly high in synovial fluid (twenty-five times). Conclusion: The IL-6 and sIL-6R were elevated in degenerative arthrits. We confirmed the source of IL-6 was synovium (very high in synovial tissue culture media), but we need further study for the source of sIL-6R as it was remarkably elevated as IL-6 and its level was lower than serum.

인대는 인체의 관절 주위에서 기능학적으로 매우 중요한 역할을 한다 인대의 기계적 성상과 손상 기전에 대한 많은 연구가 지금까지 진행되고 있으며, 많은 치료 방법들이 발달되어 사용되어 지고 있으나 치유의 질적 면이나 치유 촉진 및 완전한 치유에 대한 의문점들이 아직 도 남아있다. 관절의 기능에 중요한 역할을 하는 인대의 치유에 대한 연구와 이해를 하여 저자들은 인대의 조직학적 특성과 기계적 성상, 치유 과정 등에 관하여 여러 문헌들을 고찰하여 이 연구에서 알아보고자 한다 Ligaments are functionally very important structures in the joints of human. Many studies have been performed about injury mechanisms and mechanical properties of ligaments. Although treatment methods of injured ligaments have continually improved, many questions still remain about enhancing the rate, quality and completeness of ligament healing. For studies and understanding of the healing of ligaments that have important functions in the joint, this article reviews histological characteristics, mechanical properties and process of healing.

76 magnetic resonance (MR) studies of the knee were reviewed to evaluate the diagnostic validity of MR imaging compared with the arthroscopic findings and to identify the'eharacteristic findings of acute versus chronic anterior cruciale ligament tears on MR imaging. The sensitivity, specificity, accuracy, negative predictive value and positive predictive value 84.2%, 90.0%, 86.2%, 94.1% and 75% respectively for acute anterior cruciate ligament tears and 86.4%, 100%, 93.6%, 100% and 89.3% respectively for chronic tears. In acute anterior cruciate ligament tears, MR imaging showed discontinuity with or without high signal intersity of the ligament, high signal intersity without discontinuity and high signal intersity at the lateral compartment of the knee on T2 images. In chronic tears, MR imaging showed discontinuity, irregular contour and attenuation of the bgament. High bone signal infersity on the )ateral compartment of the knee and high signal intersity in the anterior cruciate ligament were characteristic fmdings of acute anterior cruciate ligament tears.

Acute patellar dislocation is a painful, terrifying experience that always occurs suddenly. The recurrence is unfortunately common and each experience is just as painful to the patient. With each acute dislocation, additional intraarticular derangement occurs. Therefore the goals of treatments are not only to relieve acute pain by prompt reduction of the dislocation but also to remove any chondral or osteochondral fragments that may be present within the knee joint and to restore normal patellofemoral anatomy, thereby preventing recurrent dislocation and avoiding future patellar subluxation. To present the details of arthroscopic medial retinacular repair in acute patellar dislocation. we report 2 patients with treatment of acute patellar dislocation with osteochondral loose body.

We reviewed the post-reconstructive results and complications of arthroscopic anterior cruciate ligament reconsbuction using the patellar bone-tendon-bone autograft in 91 patients who underwent the index surgery at least one year ago. The changes of Lysholm knee score, limitation of motion, instability of the reconstructed ACL, musc)e strength, presence of anterior knee pain and other complications were evaluated at final fol- low-up(mean, 2.6 year). The results were as follows. 1. The mean Lysholm knee score was 65.5+-15.5 preoperatively and improved to 88.5+-7.4 postoperatively(p<0.05), 2. Of all, nine(9.9%) complained anterior knee pain. 3. Three patients(3.3%) showed limited range of motion. 4. Five(3.5%) showed instability of reconstructed ACL during the followup. Two were caused by trauma. Ln other three, no definite causes were found. 5. In 23 patients, cybex isokinetic thigh muscle strength test was perfornied preoperatively and at post-up. One year. The strength of quadriceps of the operated side at the angular velocity of 60 degrees per second showed mean 41.7% deficit of the normal side at postoperative one year. 6. Other complications were one case of chronic click sound in motion, and one case of the fracture which occurred at the autograft donor site of the proximal tibia.

The purpose of this study is to compare the effects of intraoperative fixation and postoperative rehabili-tation on postoperative results in arthroscopic PCL reconstruction. We analyzed the posterior stability of the knee on 47 patients(48 knees) with posterior cruciate liga-ment injury whose posterior cruciate ligament had been recostructed arthroscopically and followed for minimum 1 year period at Asan Medical Center from March 1993 to May 1998. The patients were divided into the two groups according to intraoperative fixation and postoperative rehabilitation. In A group, one interference screw or staple fixed in distal femur and proximal tibia irre-spective to bone quality and fixation strength during screw insertion, and rehabilitation was started as early as anterior cruciate ligament reconstruction. In B group, additional screw or staple was fixed if fixa-tion strength was weak during screw insertion, and delayed rehabilitation program was performed as schedule. Lysohm knee score was 65.3 in group A and 75.8 in group B preoperatively but 86.0 in group A and 86.5 in group B at last follow up postoperatively. posterior stability was determined by difference in pos-terior tibial translation between the injured and the opposite knee with Telos device. Differences in poste-rior tibial translation on average were 6.9 and 3.0 ㎜in group A and B, respectively. Conclusively, arthroscopic posterior cruciate reconstruction with firm fixation strength and delayed rehabilitation program is effective to restore more reliable posterior stability.

Purpose: The purpose of this study is to prevent the graft-tunnel mismatching by N+7 method and to present the experience of the authors in anterior cruciate ligament reconstruction, using the patellar ten- don by N+7 method. Materials and Methods: Authors analyzed 34 cases who had taken the anterior cruciate ligament reconstruction from April 1998 to July 1999, at Asan Medical Center, by N+7 method. By measuring the patellar tendon length(N), tibial guide was set an angle of N+7 degree. We analyzed the results by dividing the cases into 3 groups by the degree of extrusion of the bone plug from the tibial tunnel. Results: Clinical results were acceptable in 79.4%(27 cases), protrusion in 8.8%(3 cases) and recession in 11.8%(4 cases). When the angle of tibial tunnel was less than 50 degrees, there wasn’t a protruded case in 19 cases. Comparing with 15 cases of the angle which was larger than 50 degrees, there were three protruded cases. Conclusion: Clinical results of anterior cruciate ligament reconstruction using the patellar tendon, with N+7 method is superior to empirical methods which was performed by authors, previously at the point of positioning of ideal tibial tunnel and conveniency of the technique. Especially, if the graft tendon length is shorter than 43mm, N+7 method is preferable.
