        백서 심근경색모델에서 시간경과와 경색의 크기에 따른 심자도의 변화

        N/A Background: Magnetocardiogram (MCG), which records the changes of magnetic fields generated by the heart`s electrical activity, theoritically can provide unique data for clinical application. To date, MCG has been investigated only at a single time point after myocardial infarction (MI) with severe left ventricular dysfunction in rats. The purpose of the present study was to investigate sequential changes of MCG after MI and to evaluate effects of infarct size on MCG. Methods: Acute MI were induced by the permanent ligation of left coronary artery in 22 rats. Magnetic fields were recorded just above a rat with Nb Superconducting Quantum Interference Device (SQUID) gradiometer inside a magnetically shielded room. MCG was measured before and immediately after surgery and it was subsequently recorded at the time points of 1, 4 and 6 hours post operatively. MCG was also measured at 1, 3, 7 and 21 days after surgery. Results: Elevation of ST segment and appearance of pathological Q wave on the MCG were evident immediately after the ligation of coronary artery and persisted to 6 hours after MI. On MCG, ST segment was depressed and T wave was inverted from 1 day after MI. In rats with small-and moderate- sized MI (infarct size〈30%), ST depression returned to near the isoelectric level and Q wave disappeared from 7 days after MI. However, ST depression and Q wave were still present in rats with larger infarct (infarct size≥30%). Conclusion: Evolutional changes of MCG were well-recognized up to 21 days after MI. Furthermore, the infarct size can be expressed by the extent of Q wave and ST segment depression on MCG. Taken together, these data indicate that MCG is a helpful modality for the diagnosis, evaluation of infarct size and follow up after MI.(Korean J Med 62:42-48, 2002)

      • Could cluster of risk factors predict clinical outcome in patients with acute myocardial infarction?

        Purpose: Conventional risk factors are differently contributed to short-term prognosis of acute myocardial infarction (AMI); hypertension and diabetes increase adverse outcome, whereas hyperlipidemia, smoking, and obesity paradoxically decrease adverse outcome of post-MI patients. The aim of this study is to assess whether cluster of conventional risk factors, PARADOCS (Pressure of ARtery Abnormality, Diabetes, Obesity, Cholesterol, Smoking) score, would improve the ability to predict major adverse cardiac events (MACEs) in patients with AMI. Methods: Between November 2005 and July 2011, 24,020 patients (17,232 men; mean age=63.0±12.8) with AMI were analyzed from Korean AMI registry. PARADOCS score was calculated as follows; [number of non-paradoxical risk factors (NRF) -number of paradoxical risk factors (PRF)]+3 in which non-paradoxical risk factors are hypertension and diabetes, and paradoxical risk factors are the rest. The 1-year MACEs were defined as death and non-fatal MI. Results: NRF including hypertension and diabetes were present in 49.6% and 27.0%, whereas PRF including hyperlipidemia, smoking, and obesity were present in 12.3%, 43.0%, and 34.6%, respectively. During the follow-up, 1,409 (5.9%) MACEs occurred. PARADOCS score was significantly higher in patients with 1-year MACEs (3.38±1.03 versus 2.81±1.08, p<0.001). In Cox proportional hazards model, PARADOCS score was an independent predictor of 1-year MACEs (hazards ratio 1.26, 95% confidence interval 1.18-1.34; p<0.001) after adjusting for confounding variables. Patients were categorized into 3 groups according to the PARADOCS score; PARADOCSLOW (0-1, n=2,446), PARADOCSMID (2-3, n=14,594), and PARADOCSHIGH (4-5, n= 6,980). Kaplan-Meier survival curve showed that there were significant differences in the 1-year MACEs among three groups including 3.5% in PARADOCSLOW, 8.5% in PARADOCSMID, and 16.4% in PARADOCSHIGH, respectively (long-rank p<0.001). Conclusions: In post-MI patients, conventional risk factors are differently associated with short-term prognosis, and score of these risk factors, PARADOCS score, can provide useful prognostic information to clinicians.

        배추의 형질전환용 선발항생제로서 Paromomycin의 이용

        정상' 배추와 '서울' 배추의 배축절편을 선발마커로서 hygromycin 저항성유전자를 갖고 있는 pCAMBIA1301와 paromomycin저항성유전자를 갖고 있는 pPTN290으로 각각 형질 전환된 LBA4404 또는 EHA101균주와 공동배양한 후 선발배지에서 배양하면서 형질전환체를 선발하였다. 형질전환빈도는 사용된 항생제와 품종에 따라서 현저하게 차이가 있었으며, 특히 paromomycin은 hygromycin보다 효과적이었고 정상 배추는 서울배추보다 양호하였다. 가장 높은 형질전환빈도는 (0.70%) 100mg/L paromomycin이 첨가된 선발배지에서 정상배추의배축을 배양할 경우 얻어졌다. GUS양성반응으로 확인 한 결과 정상배추에서 9개체와 서울배추에서 3개체를 각각 얻었으며, 온실에서 생장한 후 $T_1$종자를 수확하였다. $T_1$ 종자를 다시 발아시켜 유식물체를 얻은 후 GUS양성반응을 확인함으로서 외래유전자가 안정적으로 발현하고 있음을 확인하였다. Hypocotyl explants of Chinese cabbage (us. 'Jeong Sang' and 'Seoul') produced adventitious shoots on Murashige and Skoog (MS) basal medium supplemented with 4mg/L $AgNO_3$, 5 mg/L acetosyringone, 4 mg/L 6-benzyladenine and 3mg/L alpha-naphthaleneacetic acid (SI) after cocoultivation with strains of Agrobacterium tumefaciens (LBA4404) harboring the pCAMBIA1301 and the $_PPTN290$ containing hygromycin-resistance gene and paromomycin-resistance gene as a selectable marker genes, respectively. There was a significant difference in the frequency of transgenic plants depending on antibiotics and cultivars used. Paromomycin was better than hygromycin, and cultivar 'Jeong-sang' was higher than 'c.v. Seoul' in the frequency of transgenic plants. In particular, the highest frequency (0.70%) of transgenic plants was obtained from selection medium (SI) containing 100mg/L paromomycin in c.v., 'Jeong-sang' GUS positive response were obtained 9 plants and 3 plants from the cultivars, 'Jeong-sang' and 'Seoul', respectively. They were grown to maturity in a greenhouse and normally produced $T_1$ seeds. GUS histochemical assay for progeny $(T_1)$ revealed that the transgenes were expressed in the plant genome.

        배추의 배축절편으로부터 캘러스와 뿌리 발생을 통한 안정적 형질전환

        '정상' 배추의 배축절편을 선발마커로서 paromomycin 저항 성유전자를 갖고 있는 pPTN290으로 각각 형질전환된 EHA101, LBA4404, GV3101균주와 공동배양한 후 갤러스유도배지에서 형질전환캘러스를 얻은 후, 뿌리유도배지에서 부정근을 그리고 신초유도배지에서 신초를 각각 순차적으로 유도하였다. 형질전환캘러스를 얻은 후, 뿌리유도배지에서 부정근을 그리고 신초유도배지에서 신초를 가각 순차적으로 유도하였다. 형질전환캘러스 형성은 Agrobacterium균주에 따라 차이가 있었으며, 특히 EHA101균주에 공동배양된 배축절편으로부터 최대 6.1%까지 얻어졌다. 또한 각각의 형질전환캘러스 클론으로부터 형질전환 부정근과 신초 발생은 EHA101균주에서 60.7%와 38.2%, LBA4404에서 8.3%와 0%, GV3101에서 20.5%와 85.7%까지 각각 얻을 수 있었다. 형질전환식물체는 특별한 형태적 이상 없이 온실에서 정상적으로 자라 $T_{2}$종자를 얻을 수 있었다. GUS방법으로 7개의 후대 유식물체를 분석한 결과 gus유전자가 안정적으로 발현하고 있음을 확인하였고, 배추 genome에 single 또는 multiple copy로 전달되고 있음을 추측할 수 있었다. Hypocotyl explants of Chinese cabbage (cvs. "Jeong Sang") produced transgenic calli on callus induction medium (MS salt, B5 vitamin, 5 mg/L acetosyringone, 1 mg/L 2,4-D, 3% sucrose, 400 mg/L cefotaxime, 100 mg/L paromomycin, pH 5.8) after cocultivation with strains of Agrobacterium tumefaciens (EHA101, LBA4404, GV3101) harboring the pPTN290 containing paromomycin-resistance gene as a selectable marker, and then they transferred to root induction medium (1/2MS salt, MS vitamins, 2% sucrose, 100 mg/L paromomycin, 100 mg/L cefotaxime, pH 5.8) and shoot induction medium (MS salt, B5 vitamin, 4 mg/L $AgNO_3$, 4 mg/L 6-benzyladenine, 3 mg/L alpha-naphthaleneacetic acid, 100 mg/L paromomycin, 100 mg/L cefotaxime, 3% sucrose, pH 5.8) in order. There was a significant difference in the frequency of transgenic calli depending on Agrobacterium strains. In particular, the highest frequency (6.1%) of transgenic calli was obtained from the hypocotyls cocultivated with EHA101 strains. Also, the frequency (%) of transgenic root and plants from each transgenic callus clone were obtained with 60.7% and 38.2% in EHA101, with 8.3% and 0% in LBA4404, with 20.5% and 85.7% in GV3101 strains, respectively. They were grown to maturity in a greenhouse and normally produced $T_2$ seeds. GUS histochemical assay for progeny ($T_2$) revealed that the transgenes was expressed in the plant genome, and progeny analysis from 7 independent transgenic events demonstrated that the transformants transmitted the transgene as a single or multiple functional locus.

        Targeted, Stimuli-Responsive, and Theranostic <sup>19</sup>F Magnetic Resonance Imaging Probes

        <P>Unlike conventional <SUP>1</SUP>H magnetic resonance imaging (MRI), <SUP>19</SUP>F MRI features unambiguous detection of fluorine spins due to negligible background signals. Therefore, it is considered a promising noninvasive and selective imaging method for the diagnosis of cancers and other diseases. For <SUP>19</SUP>F MRI, fluorine-rich molecules such as perfluorocarbons (PFC) have been formulated into nanoemulsions and used as its tracer agent. Along with advancements in other types of nanoparticles as targeted theranostics and stimuli-triggered probes and combined with the advantages of <SUP>19</SUP>F MRI, PFC nanoemulsions are being empowered with these additional functionalities and becoming a promising theranostic platform. In this Review, we provide an overview of fluorine-based materials for sensitive <SUP>19</SUP>F MRI of biological and pathological conditions. In particular, we describe designs and applications of recently reported stimuli-responsive and theranostic <SUP>19</SUP>F MRI probes. Finally, challenges and future perspectives regarding the further development of <SUP>19</SUP>F MRI probes for their clinical applications are described.</P> [FIG OMISSION]</BR>

        The immunotherapeutic effects of recombinant Bacillus Calmette-Guérin resistant to antimicrobial peptides on bladder cancer cells

        <P><B>Abstract</B></P> <P><B>Purpose</B></P> <P>Although <I>Mycobacterium bovis</I> Bacillus Calmette-Guérin (BCG) is the most widely used bladder cancer immunotherapy, innate immune responses involving antimicrobial peptides (AMPs) cause BCG failure and unwanted side effects. Here, we generated genetically modified BCG strains with improved immunotherapeutic effects by adding genes that confer evasion of AMPs.</P> <P><B>Materials and methods</B></P> <P>We constructed recombinant BCG (rBCG) strains expressing Streptococcal inhibitor of complement (<I>Sic</I>), which confers resistance to human α-defensin-1 and cathelicidin, and d-alanyl carrier protein ligase (<I>dltA</I>), which confers resistance to cationic AMPs. <I>Sic</I> and <I>dltA</I> were separately cloned into the pMV306 plasmid and introduced into BCG via electroporation. Then, the efficacy of the rBCGs was tested in a growth inhibition assay using two bladder cancer cell lines (5637, T24).</P> <P><B>Results</B></P> <P>We confirmed the presence of cDNA segments corresponding to the <I>Sic</I> and <I>dltA</I> genes in total mRNA of the rBCG strains containing <I>Sic</I> (rBCG-Sic) and <I>dltA</I> (rBCG-dltA), and these rBCGs showed higher survival against AMPs. The growth inhibitory effects of rBCGs on bladder cancer cells were significantly enhanced compared to those of the parent BCG, and THP-1 migration also increased. After 8 h of infection, the levels of internalization were higher in rBCG-infected bladder cancer cells than in BCG-infected cells, and cells infected with rBCGs showed increased release of antitumor cytokines, such as IL-6/12, TNF-α, and INF-γ, resulting in inhibition of bacterial killing and immune modulation via antimicrobial peptides.</P> <P>Conclusions: rBCG-Sic and rBCG-dltA can effectively evade BCG-stimulated AMPs, and may be significantly improved immunotherapeutic tools to treat bladder cancer.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Our rBCG strains evade the action of AMPs and show enhanced internalization in bladder cancer cells. </LI> <LI> The rBCG strains increase the secretion of anticancer cytokines. </LI> <LI> The rBCG strain can improve the efficacy of BCG as a treatment for bladder cancer. </LI> </UL> </P>

      • Production of genetically modified pigs expressing human insulin and C-peptide as a source of islets for xenotransplantation

        <P> Islet xenotransplantation is a promising treatment for type I diabetes. Numerous studies of islet xenotransplantation have used pig-to-nonhuman primate transplantation models. Some studies reported long-term survival and successful function of porcine islets in diabetic monkeys. Genetic engineering techniques may improve the survival and function of porcine islets. A recent study reported the generation of transgenic pigs expressing human insulin rather than porcine insulin by changing one amino acid at the end of the β-chain in insulin. However, C-peptide from pigs still existed. In this study, we generated transgenic pigs expressing human proinsulin to express human insulin and C-peptide using fibroblasts from proinsulin knockout pigs as donor cells for somatic cell nuclear transfer. Eleven live piglets were delivered from three surrogates and characterized to confirm the genotype and phenotype of the generated piglets. Genotype analysis of the generated piglets showed that five of the eleven piglets contained the human proinsulin gene. Insulin expression was confirmed in the serum and pancreas in two of the five piglets. C-peptide derived from human proinsulin was also confirmed by liquid chromatography tandem mass spectrometry. Non-fasting blood glucose level was measured to verify the function of the insulin derived from the human proinsulin. Two piglets expressing insulin showed normal glucose levels similar to that in the wild-type control. In conclusion, human insulin- and C-peptide-expressing pigs without porcine insulin and C-peptide were successfully established. These pigs can be used as a source of islets for islet xenotransplantation. </P>

        Comparison of Postextubation Outcomes Associated with High-Flow Nasal Cannula vs. Conventional Oxygen Therapy in Patients at High Risk of Reintubation: a Randomized Clinical Trial

        Background: Liberation and extubation are important for patients supported by mechanical ventilation. Extubation success is related to the duration of an intensive care unit (ICU) stay and mortality rate. High-flow nasal cannula (HFNC) oxygen therapy has physiological and clinical benefits in respiratory care. The present study compared clinical outcomes associated with HFNC and conventional oxygen therapy (COT) among patients at high risk for reintubation. Methods: A single-center randomized clinical trial was conducted between March 2018 and June 2019. Sixty adults admitted to the ICU and who were at high-risk of reintubation and met the inclusion criteria were enrolled in this study. “High risk” for reintubation was defined as having at least one of the following risk factors: age > 65 years, Acute Physiology and Chronic Health Evaluation II score > 12 points on extubation day, obesity, poor expectoration, airway patency problems, difficult or prolonged weaning, and more than one comorbidity. The primary outcome of interest was reintubation within 72 hours. Secondary outcomes included duration of ICU and hospital stay, mortality rate, and time to reintubation. Results: Of 60 patients, 31 received HFNC and 29 received COT (mean age, 78 ± 7.8 vs. 76 ± 6.5 years, respectively). Reintubation rate within 72 hours did not differ between the groups (3 patients [9.7%] vs. 1 patient [3.4%], respectively). Reintubation time was shorter among patients who received COT than among patients who received HFNC (0.5 hour vs. 25 hours), but this difference was not statistically significant. Duration of ICU did not differ between the groups (14.7 ± 9.6 days vs. 13.8 ± 15.7 days, for HFNC and COT, respectively). Conclusion: Among patients at high risk for reintubation, compared with COT, HFNC did not reduce the risk of reintubation within 72 hours.
