        Ten-Day Concomitant, 10-Day Sequential, and 7-Day Triple Therapy as First-Line Treatment for Helicobacter pylori Infection: A Nationwide Randomized Trial in Korea

        Background/Aims: This nationwide, multicenter prospective randomized controlled trial aimed to compare the efficacy and safety of 10-day concomitant therapy (CT) and 10-day sequential therapy (ST) with 7-day clarithromycin-containing triple therapy (TT) as first-line treatment for Helicobacter pylori infection in the Korean population. Methods: Patients with H. pylori infection were assigned randomly to 7d-TT (lansoprazole 30 mg, amoxicillin 1 g, and clarithromycin 500 mg twice daily for 7 days), 10d-ST (lansoprazole 30 mg and amoxicillin 1 g twice daily for the first 5 days, followed by lansoprazole 30 mg, clarithromycin 500 mg, and metronidazole 500 mg twice daily for the remaining 5 days), or 10d-CT (lansoprazole 30 mg, amoxicillin 1 g, clarithromycin 500 mg, and metronidazole 500 mg twice daily for 10 days). The primary endpoint was eradication rate by intention-to-treat (ITT) and per-protocol (PP) analyses. Results: A total of 1,141 patients were included. The 10d-CT protocol achieved a markedly higher eradication rate than the 7d-TT protocol in both the ITT (81.2% vs 63.9%) and PP analyses (90.6% vs 71.4%). The eradication rate of the 10d-ST protocol was superior to that of the 7d-TT protocol (76.3% vs 63.9%, ITT analysis; 85.0% vs 71.4%, PP analysis). No significant differences in adherence or serious side effects were found among the three treatment arms. Conclusions: The 10d-CT and 10d-ST regimens were superior to the 7d-TT regimen as standard first-line treatment in Korea.

        The Korean Version of the University of California San Diego Performance-based Skills Assessment: Reliability and Validity

        Objective: The study’s aim was to develop and standardize a Korean version of the University of California San Diego Performance-based Skills Assessment (K-UPSA), which is used to evaluate the daily living function of patients with schizophrenia. Methods: Study participants were 78 patients with schizophrenia and 27 demographically matched healthy controls. We evaluated the clinical states and cognitive functions to verify K-UPSA’s reliability and validity. For clinical states, the Positive and Negative Syndrome Scale, Clinical Global Impression-Schizophrenia scale, and Social and Occupational Functioning Assessment Scale and Schizophrenia Quality of Life Scale-fourth revision were used. The Schizophrenia Cognition Rating Scale, Short-form of Korean-Wechsler Adult Intelligence Scale, and Wisconsin Card Sorting Test were used to assess cognitive function. Results: The K-UPSA had statistically significant reliability and validity. The K-UPSA has high internal consistency (Cronbach’s alpha, 0.837) and test-retest reliability (intra-class correlation coefficient, 0.381-0.792; p<0.001). The K-UPSA had significant discriminant validity (p<0.001). Significant correlations between the K-UPSA’s scores and most of the scales and tests listed above demonstrated K-UPSA’s concurrent validity (p<0.001). Conclusion: The K-UPSA is useful to evaluate the daily living function in Korean patients with schizophrenia.

      • Grafting of polyaniline onto the surface of 4-aminobenzoyl-functionalized multiwalled carbon nanotube and its electrochemical properties

        <P>Polyaniline (PANi)-grafted multiwalled carbon nanotube (MWNT) composite is prepared by a two-step reaction sequence. MWNT is first functionalized with 4-aminobenzoic acid in polyphosphoric acid/phosphorous pentoxide as a “direct” Friedel-Crafts acylation reaction medium. The resultant 4-aminobenzoyl-functionalized MWNT is then treated with aniline using ammonium persulfate/aqueous hydrochloric acid to promote a chemical oxidative polymerization, leading to PANi-grafted MWNT composite. The resultant composite is characterized by elemental analysis, Fourier-transform infrared spectroscopy, wide-angle X-ray diffraction, scanning electron microscopy, transmission electron microscopy, thermogravimetric analysis, UV–vis absorption spectroscopy, fluorescence spectroscopy, cyclic voltammetry, and electrical conductivity measurement. The thermooxidative stability and electrical conductivity of PANi-grafted MWNT composite are improved compared to those of PANi. Specifically, the electrical conductivity of PANi-grafted MWNT is improved 10–900 times depending upon the level of doping.         FEA based optimization of semi-submersible floater considering buckling and yield strength

        A semi-submersible structure has been widely used for offshore drilling and production of oil and gas. The small water plane area makes the structure very sensitive to weight increase in terms of payload and stability. Therefore, it is necessary to lighten the substructure from the early design stage. This study aims at an optimization of hull structure based on a sophisticated yield and buckling strength in accordance with classification rules. An in-house strength assessment system is developed to automate the procedure such as a generation of buckling panels, a collection of required panel information, automatic buckling and yield check and so on. The developed system enables an automatic yield and buckling strength check of all panels composing the hull structure at each iteration of the optimization. Design variables are plate thickness and stiffener section profiles. In order to overcome the difficulty of large number of design variables and the computational burden of FE analysis, various methods are proposed. The steepest descent method is selected as the optimization algorithm for an efficient search. For a reduction of the number of design variables and a direct application to practical design, the stiffener section variable is determined by selecting one from a pre-defined standard library. Plate thickness is also discretized at 0.5t interval. The number of FE analysis is reduced by using equations to analytically estimating the stress changes in gradient calculation and line search steps. As an endeavor to robust optimization, the number of design variables to be simultaneously optimized is divided by grouping the scantling variables by the plane. A sequential optimization is performed group by group. As a verification example, a central column of a semi-submersible structure is optimized and compared with a conventional optimization of all design variables at once.

        Investigation of mechanical/dynamic properties of carbon fiber reinforced polymer concrete for low noise railway slab

        <P><B>Abstract</B></P> <P>In this paper, the mechanical/dynamic property of the carbon fiber reinforced polymer concrete (CFRPC) was investigated to reduce the vibration/noise problems on rail slab. The CFRPC was fabricated with the different weight fraction of carbon fiber and different fiber length. The oxygen plasma treatment was performed to enhance the adhesion between the carbon fiber and epoxy resin. The surface of the carbon fiber with the oxygen plasma treatment was characterized using X-ray photoelectron spectroscopy (XPS) analysis and scanning electron microscope (SEM). The flexural strength of the CFRPC was measured using 3-point bending test. Also, the frequency dependent variation of the stiffness and damping of the CFRPC were measured by impact test to characterize their ability to reduce the vibration. In this study, the CFRPC with 12mm of optimal fiber length and 2wt% of the optimal fiber content showed 25.12MPa flexural strength and 0.01954 damping value. Finally, it was found that the noise level of the railway using CFRPC sleeper was decreased by 4dB compared to that using the polymer concrete sleeper, from the impact test of the scaled model of the railway structure.</P>

        Liver X receptors alpha gene ( <i>NR1H3</i> ) promoter polymorphisms are associated with systemic lupus erythematosus in Koreans

        <P><B>Introduction</B></P><P>Liver X receptors are established sensors of lipid and cholesterol homeostasis. Recent studies have reported that these receptors are involved in the regulation of inflammation and immune responses. We attempted to identify single nucleotide polymorphisms (SNPs) of the <I>NR1H3</I> gene associated with the susceptibility to systemic lupus erythematosus (SLE).</P><P><B>Methods</B></P><P>SNPs were genotyped using SNaPSHOT assay in 300 Korean patients with SLE and 217 normal controls (NC), and in replication samples (160 SLE patients and 143 NC). Also, the functional effects of <I>NR1H3</I> gene promoter polymorphisms were analyzed using a luciferase assay, real-time polymerase chain reaction, B cell proliferation assay and an electrophoretic mobility shift assay.</P><P><B>Results</B></P><P>We identified five polymorphisms: -1851 T > C (rs3758673), -1830 T > C (rs3758674), -1003 G > A (new), -840 C > A (rs61896015) and -115 G > A (rs12221497). There was a significant and reproducible difference in the -1830 T > C, -1003 G > A and -115 G > A polymorphisms between the SLE and the NC. Luciferase activity of the structure containing -1830 C was less enhanced compared to the structure containing -1830 T in basal, GW3965 and T0901317 treated Hep3B cells (<I>P</I> = 0.009, <I>P</I> = 0.034 and <I>P</I> <0.001, respectively). Proliferation of the -1830 TC type was increased compared to the -1830 TT type in basal, GW3965 and T0901317 treated B cells from SLE patients (<I>P</I> = 0.011, <I>P</I> = 0.040 and <I>P</I> = 0.017, respectively). Transcription factor GATA-3 preferentially bound the -1830 T allele in the promoter.</P><P><B>Conclusions</B></P><P><I>NR1H3</I> genetic polymorphisms may be associated with disease susceptibility and clinical manifestations of SLE. Specifically, -1830 T > C polymorphism within <I>NR1H3</I> promoter region may be involved in regulation of <I>NR1H3</I> expression.</P>

        Improvement in the Stabilities of White Organic Light Emitting Diodes Using a Partially Doped Emission Layer

        White organic light emitting devices were fabricated to improve the stability through a structural change using the two peak emission method. The fabricated devices were composed of indium tin oxide (100 nm)/ $\alpha$-NPD (30 nm)/4,40-bis(2,20-diphenylvinyl)-1,10-biphenyl (DPVBi, d: variable)/DPVBi: Rubrene (40 nm)/2,9-dimethyl-4,7-diphenyl-1,10-phenanthroline(5 nm)/ $Alq_3$(5 nm)/ Al (100 nm). A DPVBi for blue emissions was used as the host material in the emitters. The doping concentration of the Rubrene was fixed at 2.0% (by weight). The white emission with Commission Internationale De L'Eclairage coordinates of (0.3342, 0.3439) occurred at 14 V with a thickness d of 1 nm. It was insensitive to the drive voltage, and the devices had a maximum luminance of $211\;cd/cm^2$. At 19 V, the current density and maximum external quantum efficiency were $173\;mAcm^2$ and 0.478%, respectively.

      • Metalloid tellurium-doped graphene nanoplatelets as ultimately stable electrocatalysts for cobalt reduction reaction in dye-sensitized solar cells

        <P><B>Abstract</B></P> <P>One feasible alternative to Pt catalyst in dye-sensitized solar cells (DSSCs) is metalloid tellurium (Te)-doped graphene nanoplatelets (TeGnPs). These were prepared by ball-milling graphite in the presence of Te crystals. Introduction of Te at the edges of TeGnPs was confirmed with various analytical techniques including time of flight secondary ion mass spectrometry (TOF-SIMS). The resultant TeGnPs are herein evaluated as counter electrode (CE) materials for the cobalt reduction reaction (CRR) in DSSCs. TeGnP-CE exhibits much lower charge transfer resistance (<I>R</I> <SUB>ct</SUB>=0.15Ωcm<SUP>2</SUP>) than that (<I>R</I> <SUB>ct</SUB>=1.77Ωcm<SUP>2</SUP>) of Pt-CE. More importantly, TeGnP-CE displays an extreme electrochemical stability for the Co(bpy)<SUB>3</SUB> <SUP>2+/3+</SUP> (bpy=2,2'-bipyridine) redox couple even after 1000 potential cycles. The SM315-based DSSC fabricated with TeGnP-CE shows a better power conversion efficiency (PCE=11.58%) than that with Pt-CE (11.03%), suggesting that TeGnP-CE could be one of the best alternatives to Pt-CE in DSSCs.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Tellurium (Te)-doped graphene nanoplatelets (TeGnPs) was, for the first time, prepared by mechanochemical reaction. </LI> <LI> TeGnPs were tested as a material for counter electrode (CE) in dye-sensitized solar cell (DSSC). </LI> <LI> TeGnPs showed much low charge transfer resistance, extreme electrochemical stability and high power conversion efficiency compared with conventional Pt-CE. </LI> <LI> The results suggested that TeGnPs can be one of the best alternatives to expensive Pt-CE. </LI> </UL> </P> <P><B>Graphical abstract</B></P> <P>Tellurium-doped graphene nanoplatelets (TeGnPs) electrocatalysts for cobalt reduction reaction (CRR) in dye-sensitized solar cells (DSSCs) displayed much higher electrocatalytic activities with almost ‘zero-loss’ electrochemical stability than did Pt.</P> <P>[DISPLAY OMISSION]</P>

        Expression and immunohistochemical localization of TMEM16A/anoctamin 1, a calcium-activated chloride channel in the mouse cochlea.

        <P>Sound transduction in the cochlea depends on the unique high concentrations of K(+) in the endolymph. The production and maintenance of high K(+) concentrations are accompanied by Cl(-) cycling. In this study, we report on an investigation of the expression and localization of TMEM16A/anoctamin 1 (ANO1), a recently cloned Ca(2+)-activated Cl(-) channel, in the mouse cochlea by Western blot and immunhistochemistry. The ANO1 protein was identified in the cochlea by Western blotting. The immunoreactivity was found in stria vascularis as a line and in the organ of Corti as three plaques. The cellular localization of ANO1 was examined by means of double-labeling experiments with anti-claudin 11, a marker for basal cells of the stria vascularis. The results demonstrated that ANO1 colocalized with claudin 11, indicating its expression in basal cells. We also examined ANO1 localization in the organ of Corti by double- and triple-labeling techniques with anti-myosin VI, a marker for hair cells, and anti-synaptophysin, a marker for olivocochlear efferent nerve endings under hair cells. The results clearly showed that ANO1 is colocalized with synaptophysin, but not with myosin VI, indicating that ANO1 is localized at medial olivocochlear efferent nerve endings under outer hair cells. These results suggest that ANO1 may be specifically involved in synaptic transmission from medial olivocochlear efferent nerve endings to outer hair cells in the organ of Corti, as well as Cl(-) cycling in basal cells of the stria vascularis.</P>
