This paper proposes a new communication system for e-learning applications to mitigate the negative impacts of COVID-19 where the online massive demands impact the current commu-nications systems infrastructures and capabilities. The proposed system utilizes high-altitude platforms (HAPs) for fast and efficient connectivity provision to bridge the communication in-frastructure gap in the current pandemic. The system model is investigated, and its performance is analyzed using adaptive antenna arrays to achieve high quality and high transmission data rates at the student premises. In addition, the single beam and multibeam HAP radio coverage scenarios are examined using tapered uniform concentric circular arrays to achieve feasible communication link requirements.

Acknowledging the relevance of work environment in influencing job-related choices, this paper examines the relationship between emotional intelligence, role-based job crafting i.e. task, relational and cognitive crafting, job autonomy and internal employability. The role of job autonmy as a moderator and role-based job crafting as mediator between emotional intelligence and internal employability has also been examined. The study was conducted on 296 managers working in Indian public sector enterprises and found interesting findings including a positive and direct effect of emotional intelligence on all three forms of role-based job crafting and internal employability. The results for mediation effect of job crafting and moderation effect of job autonomy varied depending upon the form of role-based job crafting. The paper discusses theoretical implications and practical relevance of these findings and may provide a direction for interlinking job demands-resources and conservation of resource theories.

Backgrounds/Aims: Transglutaminase 2 (TG2) is known to be an important mediator of inflammation induced carcinogenesis pathway. Chronic inflammation is the most important causative factor in Gallbladder cancer (GBC) carcinogenesis. We analyzed the expression of TG2 in GBC and its role as potential prognostic marker, first of its kind study. Methods: We analyzed TG2 expression in 100 cases of GBC and 28 cases of non-cancer gallbladder specimen (calculus cholecystitis). We studied TG2 expression in GBC in comparison to control group and evaluated its role as a potential prognostic marker. Results: TG2 score (1-9) was calculated by multiplying percentage cytoplasmic expression (P) with intensity of expression (I) in tumor cells. Positive TG-2 expression was observed in 62% of GBC patients compared to only 21% (n=6) in control group (p=0.001). In curative resection subgroup (n=54), TG2 positive patients showed shorter disease free survival rate (p=0.04) and higher rate of recurrence (p=0.03) compared to TG2 negative patients. TG2 positive expression was observed in 15/16 of patients with recurrent disease. In palliative treatment subgroup, patients with strong TG2 positive expression had poorer disease specific survival (p=0.01) as compared to weakly positive group. On multivariate analysis, lymph node status (p=0.03) and TG2 expression (p=0.037), were found to be significant predictor of recurrence and eventual survival. Conclusions: Positive TG2 expression was related to higher recurrence rates post curative surgery, shorter disease free and overall survival and ultimately portended poor prognosis. It may be helpful in better prognostication and tailoring therapeutic approach for better management of GBC.
Quality indicators in inflammatory bowel disease

Inflammatory bowel disease (IBD), which includes Crohn’s disease and ulcerative colitis, is a chronic, debilitating, and expensive condition affecting millions of people globally. There is significant variation in the quality of care for patients with IBD across North America, Europe, and Asia; this variation suggests poor quality of care due to overuse, underuse, or misuse of health services and disparity of outcomes. Several initiatives have been developed to reduce variation in care delivery and improve processes of care, patient outcomes, and reduced healthcare costs. These initiatives include the development of qualityindicator sets to standardize care across organizations, and learning health systems to enable data sharing between doctors and patients, and sharing of best practices among providers. These programs have been variably successful in improving patientoutcomes and reducing healthcare utilization. Further studies are needed to demonstrate the long-term impact and applicability of these efforts in different geographic areas around the world, as regional variations in patient populations, societalpreferences, and costs should inform local quality improvement efforts.

The nutritional and medicinal potential (antioxidant and antibacterial activities) of 2 wild edible mushroom species (Lentinus sajor-caju and Lentinus torulosus) of Similipal Biosphere Reserve were determined. The macronutrient profile of these mushrooms in general revealed high source of protein (27.31-28.36 g/100 g), carbohydrate (64.95-68.24 g/100 g), and low amounts of fat (1.36-2.42 g/100 g) and possessed good quantities of micronutrients (vitamins and carotenoids) and minerals (P, K, Mn, Ni, and Fe). The solvent extracts (ethanol, methanol, and water) of the mushrooms exhibited strong antioxidant properties (ABTS, DPPH, H2O2, and metal chelating activities) with scavenging activity upto 70.54% along with phenol,flavonoid, and total antioxidant capacity. Both the mushrooms showed moderate antibacterial activity (11.0-18.33 mm inhibition zones) against Streptococcus aureus and Vibrio cholerae. Being a rich source of nutritional and medicinal potential, these 2 studied mushrooms can be used in human diet as nutraceuticals and functional foods.

Purpose: The DNA methylation mediated by specific DNA methyltransferases (DNMTs), results in the epigenetic silencing of multiple genes which are implicated in human breast cancer. We hypothesized that the natural compounds modulate the expression of DNMTs and their associated proteins in the breast cancer cell lines and affect the methylation mediated gene silencing. Methods: The DNMTs transcript expression was analyzed by reverse transcription-polymerase chain reaction (RT-PCR) in the tumors and the adjacent normal breast tissues of the patients with invasive ductal breast carcinoma. We tested the hypothesis that the natural compounds, viz., epigallocatechin gallate (EGCG), genistein, withaferin A, curcumin, resveratrol, and guggulsterone, have demethylation potential. To investigate this hypothesis, we analyzed the DNMTs expression at the transcript levels, followed by the analysis of DNMT1 and its associated proteins (HDAC1, MeCP2, and MBD2). Results: The increased DNMTs transcripts expression, viz., DNMT1, DNMT3a, and DNMT3b, in the breast cancer tissues suggest involvement of the DNMTs in the breast carcinogenesis. Quantitative RT-PCR analysis revealed that the treatment with natural compounds, viz., EGCG, genistein, withaferin A, curcumin, resveratrol, and guggulsterone, resulted in a significant decrease in the transcript levels of all the DNMTs investigated. Importantly, these natural compounds decreased the protein levels of DNMT1, HDAC1, and MeCP2. Conclusion: Our results demonstrate that the natural compounds, EGCG, genistein, withaferin A, curcumin, resveratrol, and guggulsterone, have the potential to reverse the epigenetic changes. Moreover, their lack of toxicity makes these natural compounds promising candidates for the chemoprevention of the breast cancer. In-depth future mechanistic studies aimed to elucidate how these compounds affect the gene transcription are warranted.
Posterior Retroperitoneoscopic Adrenalectomy for Large Adrenal Pheochromocytoma: A Case Report

Minimally invasive adrenalectomy has now become the preferred technique for adrenalectomies and can be done via transperitoneal laparoscopic and retroperitoneoscopic approaches. The retroperitoneal approach is generally suitable for <5 cm tumors, whereas the transperitoneal approach is preferred for larger tumors. We report an 8 cm right adrenal pheochromocytoma operated with a posterior retroperitoneoscopic approach without any surgical complications. The posterior retroperitoneoscopic approach for large adrenal tumors (>5 cm) considering its additional advantages is a reasonable alternative to the transperitoneal laparoscopic approach, in experienced hands.
