        Layer-adapted meshes for solute dispersion in a steady flow through an annulus with wall absorption: Application to a catheterized artery

        This paper describes the longitudinal dispersion of passive tracer molecules injected in a steady, fully developed, viscous, incompressible, laminar flow through an annular pipe with a first order heterogeneous boundary absorption at the outer wall, numerically using layer-adapted meshes. The model is based on steady advection-diffusion equation with Dirichlet and Robin boundary conditions. The solutions are discussed in the form of iso-concentration contours of the tracer molecules in the vertical plane. An artanh transformation is used to convert the infinite domain into a finite one. A combination of central finite difference and 2-point upwind scheme is adopted to solve the governing advection-diffusion equation. It is shown that how the mixing of tracers is affected by the shear flow, aspect ratio and the first-order boundary absorption. When the flow becomes convection dominated, the monotone finite difference on a uniform mesh does not work properly, so a layer-adapted mesh, namely a “Shishkin” mesh, is used to capture the layer phenomena at the different downstream stations. The present results are compared with existing experimental and numerical data and we have earned an excellent agreement with them. It is observed that, due to the use of layer adapted mesh, we have achieved a better agreement with the experimental data than some other previous results available in the literature, especially in the closest downstream location. The results of this study are likely to be of interest to understand the basic mechanism of dispersion process of solute in blood through a catheterized artery with an absorptive arterial wall.

        Prognostic value of lymph node ratio in surgically treated cases of vulvar cancer: a tertiary care centre experience

        Objective This study aimed to study the patterns of recurrence in surgically treated cases of vulvar cancer (VC) and determine the factors associated with recurrence, with a special emphasis on lymph node ratio (LNR). Methods This retrospective study examined VC patients primarily treated with surgery at our institute from January 2005 to December 2015. Demographic data, clinical characteristics, surgicohistopathological data, adjuvant treatment, follow up, and recurrence site and treatment were studied. Results Among the 111 cases treated, a recurrence rate of 18.9% was noted. Recurrence was most commonly local (61.9%). On univariate and multivariable analyses of clinicopathological parameters, an LNR >20 had the highest hazard ratio for recurrence. Conclusion LNR may provide useful prognostic information in VC patients with positive inguinal lymph node status.

      • GEMS OR FAKES? USING LOCATION AWARE TWEETS TO ASSESS ONLINE REVIEW-RELIABILITY

        Online review sites have become both popular and indispensable for many industries that have recognized the importance of word-of-mouth as advertising tools. Hotels and restaurants that are rated highly by travel site “Trip advisor” proudly put a sticker outside their business locations demonstrating their popularity. The review site logos, and the business scores on stickers and badges regularly serve as seals of approval and symbols of reliability. This has given rise to a cottage industry that misuse the trust. While some businesses post flattering reviews as advertising, competitors sometimes falsely slander reputation of competitors. There has been some research which explores the issue of reliability of online reviews, for example, Luca and Zervas, (2015)* identify different restaurant characteristics that cause them to use fake reviews. Ney (2013)* identifies factors consumers use to assess credibility of online reviews. The problem of unreliable reviews creates an interesting set of issues that we attempt to address in this paper. First, if there is a way to confirm whether the reviews are reliable without engaging in primary data collection. Second, what explains the underreporting or over reporting of the quality of a place? To answer the above questions, in this paper the authors extract emotions embedded in location-based tweets emerging from restaurant locations to verify the reliabilities of their online review scores on Yelp. Due to the real-time nature of the feedback, location based tweet content is free of certain survey response biases like social desirability bias. In order to collect location based tweets, we mined data from consumers checking-in via Foursquare (a location based social network application) at restaurants, across six regions in USA. These regions were chosen because of the high volume of check-ins emanating from them on foursquare. Using this data set we were able to extract specifics such as the name of the restaurant, the content of the tweet and related temporal variables impacting the consumer’s experience in a particular business location. Over twenty five thousand tweets were analyzed which were posted by approximately 14000 users. Further, we developed a scale measuring emotions embedded in the tweets with the help of University of Florida’s Affective Norms for English Words (ANEW) scale. Each of the tweets were divided into its constituent’s words and the words were checked against the Anew scale items. When a word was identified, we allotted a numerical pleasure value to that word. At the end of the processing we had an average numerical pleasure score for each tweet. Using the tweet pleasure score and the Yelp score, an index was computed that could reveal whether Yelp overrated or underrated the restaurant. Further analysis led to preliminary findings that demonstrated how underrated or overrated a restaurant was varied with the type of cuisine served in the restaurant. Among all restaurants, over 75% of the restaurants were classified as overvalued. In other words, based on tweet emotion content, most Yelp ratings appear positively biased. Asian restaurants were the most overvalued (100%) followed by Latin restaurants, which were 88% overvalued. One interesting initial finding was that American category restaurants were the most undervalued. 43% of the restaurants were undervalued on yelp as compared to their pleasure ratings.

        Rheological and mechanical properties of PMMA/organoclay nanocomposites prepared via ultrasound-assisted in-situ emulsion polymerization

        This study investigated the rheological and mechanical properties of PMMA/organically modified Cloisite 30B nanocomposites (clay loading 1-5 wt%) synthesized via ultrasound-assisted in-situ emulsion polymerization. XRD patterns and TEM micrographs of the nanocomposites confirm complete exfoliation and uniform distribution of nanoclay platelets in host poly(methyl methacrylate) matrix. As a consequence, the nanocomposites synthesized with sonication were found to have superior properties as compared to the nanocomposites synthesized with mechanical stirring. Measurement of rheological properties using melt rheology revealed that magnitudes of storage modulus, loss modulus and complex viscosity increased with incorporation of clay platelets in polymer matrix due to the flow restriction of PMMA chains induced by nanofiller. Analysis of mechanical properties of nanocomposites showed maximum Young’s modulus of 1.8GPa with weight average molecular weight (Mw) of 581,130 g mol1 for 2wt% clay loading. The enhancements in rheological and mechanical properties of nanocomposites are attributed to the physical and chemical effect of ultrasound and cavitation, i.e. generation of intense microturbulence and free radicals in the emulsion polymerization reaction system.

        Atlanto-occipital assimilation: embryological basis and its clinical significance

        Atlanto-occipital assimilation is an osseous embryological anomaly of the craniovertebral junction in which the atlas (C1) is fused to the occiput of skull. Embryologically, this assimilation may happen due to failure of the segmentation and separation of the caudal occipital and the cranial cervical sclerotome. The segmentation clock is maintained by NOTCH and WNT signalling pathways along with Hox genes and retinoic acid. This condition is likely to be a consequence of mutation in above mentioned genes. The knowledge of this assimilation may be crucial for the clinicians as it may lead to various neurovascular symptoms. The present case report involves the analysis of atlanto-occipital assimilation with its clinical significance and embryological basis.

        Autoimmune Sclerosing Cholangitis in Children: A Prospective Case-Control Study

        Purpose: In children overlap of autoimmune hepatitis (AIH) and primary sclerosing cholangitis is labelled as autoimmune sclerosing cholangitis (ASC). The only prospective pediatric study showed a high prevalence of ASC by using endoscopic retrograde cholangiopancreatography. Aims of our study were to find the prevalence of ASC by using magnetic resonance cholangiography (MRC) in AIH and in non-AIH cirrhosis and to compare clinical presentation and outcome of AIH and ASC. Methods: Prospectively we did MRC in 38 children with AIH (cases) and 19 disease controls (Wilson disease). Multiple biliary strictures with proximal dilatation on MRC were taken as definitive changes of ASC. Detail clinical, laboratory parameters, liver histopathology and treatment outcome were recorded. Results: The median age of cases was 11.5 (3-18) years, 22 (57.9%) were girls and 28 (73.7%) were diagnosed as type 1 AIH. MRC was done in 11 children (28.9%) at the time of diagnosis and in 27 (71.1%) after a median follow-up of 2.5 (0.3-10) years. Abnormal MRC changes were seen in 14/38 (36.8%) of AIH and 8/19 (42.1%) of controls. However, definite changes of ASC were present in four (10.5%) children in AIH and none in controls. None of the clinical, laboratory, histological parameters and treatment response were significantly different between ASC and AIH groups. Conclusion: The prevalence of ASC in children with AIH was just 10.5%. We suggest MRC in select group with cholestatic features, inflammatory bowel disease and in those who showed poor response to immunosuppression instead of all children with AIH.
