Twenty-nail dystrophy is an idiopathic nail dystrophy in which all twenty nails are uniformly and simultaneously affected with excess longitudinal ridging and loss of lustre. The pathogenesis is controversial, and the treatment is unsuccessful. It is thought to have a self-limiting and reversible nature when it develops in childhood, but in adults, it is unusual and exists persistently. We report here two cases of adult patients with dystrophy of all twenty nails, whose ages were 58 and 55. The disease had been present for one year and may be associated with alopecia areata in the 58 year old and an idiopathic condition in the 55 year old. Negative results were obtained on mycological studies. Biopsies taken from the nail bed revealed marked hyperkeratosis consistent with nail dystrophy.

Background/Aims: The aim of this study was to evaluate whether the mucosa-tracking technique is effective for improving precutting-related pancreatitis and the sustained failure of bile duct cannulation in precut papillotomy (PP) with the Iso-Tome (MTW Endoskopie). Methods: From September 2004 to June 2006, PP was performed with the Iso-Tome if biliary cannulation failed by conventional methods for approximately 5 minutes. The pink intrapapillary mucosa (PIPM) exposed by PP was tracked and classified into four groups: fully exposed and oriented to the direction of the bile duct (group A) or the pancreatic duct (group B), partially exposed (group C), or unexposed (group D). The success rate of bile duct cannulation (SRBC), the procedure time required for successful bile duct cannulation (PTBC), and the complications in the first session were compared between the mucosa-exposed groups (MEGs; group A, B, and C) and the mucosa-unexposed group (MUEG; group D). Results: A total of 59 patients (25 females, 34 males) with a mean age of 65.2 years were enrolled. The MEGs and MUEG comprised 52 (88.1%) and 7 (11.9%) patients, respectively. SRBC in the first session was 86.4% (51/59) in total and 92.3% (48/52) in the MEGs, compared to only 42.9% (3/7) in the MUEG (p=0.005). The mean PTBC in the MEGs and MUEG was 8.7 minutes and 16.3 minutes, respectively (p=0.23). Complications occurred in 6.8% of the patients (4/59; all pancreatitis); there were no differences between the MEGs (5.8%, 3/52) and MUEG (14.3%, 1/7; p=0.41). All four patients with pancreatitis were managed medically. Conclusions: The mucosa-tracking technique in PP with the Iso-Tome is a feasible and useful method of enhancing SRBC. PIPM is an important endoscopic landmark for successful PP. (Gut Liver 2010;4:76-83)

<P><B>Abstract</B></P><P>To understand the depositional processes and environmental changes during the initial flooding of the North China Platform, this study focuses on the Lower to Middle Cambrian Zhushadong and Mantou formations in Shandong Province, China. The succession in the Jinan and Laiwu areas comprises mixed carbonate and siliciclastic deposits composed of limestone, dolostone, stromatolite, thrombolite, purple and grey mudstone, and sandstone. A detailed sedimentary facies analysis of seven well‐exposed sections suggests that five facies associations are the result of an intercalation of carbonate and siliciclastic depositional environments, including local alluvial fans, shallowing‐upward carbonate–siliciclastic peritidal cycles, oolite dominant shoals, shoreface and lagoonal environments. These facies associations successively show a transition from an initially inundated tide‐dominated carbonate platform to a wave‐dominated shallow marine environment. In particular, the peritidal sediments were deposited during a large number of depositional cycles. These sediments consist of lime mudstone, dolomite, stromatolite and purple and grey mudstones. These shallowing‐upward cycles generally resulted from carbonate production in response to an increase of accommodation during rising sea‐level. The carbonate production was, however, interrupted by frequent siliciclastic input from the adjacent emergent archipelago. The depositional cycles thus formed under the influence of both autogenetic changes, including sediment supply from the archipelago, and allogenic control of relative sea‐level rise in the carbonate factory. A low‐relief archipelago with an active tidal regime allowed the development of tide‐dominated siliciclastic and carbonate environments on the vast platform. Siliciclastic input to these tidal environments terminated when most of the archipelago became submerged due to a rapid rise in sea‐level. This study provides insights on how a vast Cambrian carbonate platform maintained synchronous sedimentation under a tidal regime, forming distinct cycles of mixed carbonates and siliciclastics as the system kept up with rising relative sea‐level during the early stage of basin development in the North China Platform.</P>
Influence of Calcium on the Formation of Aluminosilicate Inorganic Polymer Binder

Aluminosilicate inorganic polymer binder has been studied as an alternative to ordinary Portland cement due to its higher physical properties, chemical resistance and thermal resistance. This study has been carried out in an attempt to understand the hardening characteristics of aluminosilicate binder by varying the content of calcium. Samples with four different ratios of Al, Si, and Ca were synthesized in this study with the Al:Si:Ca mol ratio being 1.00:1.85~1.98:0.29~2.12. Furthermore, an alkali silicate solution was prepared with the sodium hydroxide (NaOH) and sodium silicate (NaSi). The hardening characteristics of the specimens were analyzed using XRD, SEM, and TG/DTA. In addition, compressive strength and sintering time of specimens were measured as a function of calcium content. The results showed that the specimen containing 2.12 mol% calcium offered the highest compressive strength. However, the compressive strength of the specimen containing 0.26 mol% calcium was lower relative to the other specimens. The results displayed a distinct tendency that as more calcium was added to the inorganic polymer, setting time became shorter. When calcium was added to the inorganic polymer structure, a second phase was not formed, indicating that the addition of calcium does not affect the crystalline structure.

Objective : The treatment of complex intracranial aneurysms remains challenging. One approach is the application of surgical flow alteration to treat aneurysms that are neither clippable, trappable, or coilable. The efficacy and limitations of surgical flow alteration have not yet been established. Methods : Cases of complex aneurysms treated with surgical flow alteration (proximal occlusion with or without bypass, distal occlusion with or without bypass and bypass only) were included in this retrospective study. Results : Among a total of 16 cases, there were 7 giant aneurysms (${\geq}25mm$ diameter) and 9 large aneurysms (>10 mm diameter); 15 of 16 aneurysms were unruptured. There were 8 aneurysms located in the anterior circulation, while the other 8 were in the posterior circulation. Aneurysms were treated with proximal occlusion in 10 cases and distal occlusion in 5 cases; in 1 case, the aneurysm occluded spontaneously after bypass without parent artery occlusion. All but 2 cases underwent prior or concurrent bypass surgery. Complete obliteration of the aneurysm at the latest imaging follow-up was shown in 12 of 16 cases (75.0%). Bypass patency was confirmed in 13 of 15 cases (86.7%). Surgery-related morbidity developed in 3 cases (18.8%, Glasgow outcome scale of 4) and all were perforator infarctions. There were no mortalities. Conclusion : Surgical flow alteration resulted in a high rate of aneurysmal obliteration with acceptable morbidity. Although several limitations remained, it could represent an alternative method for treating complex aneurysms.

In this paper, we propose a new architecture of Fuzzy Polynomial Neural Networks(FPNN) by means of genetically optimized Fuzzy Polynomial Neuron(FPN) and discuss its comprehensive design methodology involving mechanisms of genetic optimization, especially Genetic Algorithms(GAs). The conventional FPNN developed so far are based on mechanisms of self-organization and evolutionary optimization. The design of the network exploits the extended Group Method of Data Handling(GMDH) with some essential parameters of the network being provided by the designer and kept fixed throughout the overall development process. This restriction may hamper a possibility of producing an optimal architecture of the model. The proposed FPNN gives rise to a structurally optimized network and comes with a substantial level of flexibility in comparison to the one we encounter in conventional FPNNs. It is shown that the proposed advanced genetic algorithms based Fuzzy Polynomial Neural Networks is more useful and effective than the existing models for nonlinear process. We experimented with Medical Imaging System(MIS) dataset to evaluate the performance of the proposed model.
