A Study on The Relationship Between Intraoperative Neuromonitoring and Hemoglobin Changes

This study was conducted in order to determine the effect of intraoperative hemoglobin changes on intraoperative neuromonitoring (IONM). This was a retrospective study that included 339 participants who underwent cerebrovascular surgery. We compared anesthetic agents, intraoperative hemoglobin, hematocrit, blood transfusion, and blood loss. We examined motor evoked potential and sensory evoked potential to patients. There were significant differences in hemoglobin changes, bleeding levels, transfusion, anesthesia time, and postoperative mobility disorders. Moreover, compared with patients who received transfusions, those who did not receive transfusion had a lower average hemoglobin level, as well as a higher bleeding amount, and a need of higher anesthesia time and anesthetic dose. Also, we found vasospasm occurred while surgery can bring adverse results after operation. This study showed that an intraoperative decrease in hemoglobin levels affects the function of cerebral perfusion, which could result in abnormal nerve monitoring results. However, as this study could not find a relation of anesthetics to IONM, there is a need for further research regarding the association between anesthetics and hemoglobin changes and IONM.

OBJECTIVES After the third wave of coronavirus disease 2019 (COVID-19), by mid-February 2021, approximately 0.16% of the Korean population was confirmed positive, which appeared to be among the lowest rates worldwide at that time. However, asymptomatic transmission is challenging for COVID-19 surveillance. Therefore, a community-based serosurvey of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection was conducted to understand the effectiveness of Korea’s strong containment strategy. METHODS We collected 5,002 residual sera samples from January 30 to March 3, 2021, from 265 medical facilities in Seoul, 346 in Gyeonggi Province, and 57 in Incheon. Sixty samples from tertiary institutions were excluded. We defined the sub-regions according to the addresses of the medical facilities where the specimens were collected. Elecsys Anti-SARS-CoV-2 was used for screening, and positivity was confirmed using the SARS-CoV-2 sVNT Kit. Prevalence was estimated using sampling weights and the Wilson score interval for a binomial proportion with a 95% confidence interval. RESULTS Among the 4,942 specimens, 32 and 25 tested positive for COVID-19 in the screening and confirmatory tests, respectively. The overall crude prevalence of SARS-CoV-2 antibodies was 0.51%. The population-adjusted overall prevalence was 0.55% in women and 0.38% in men. The region-specific estimation was 0.67% and 0.30% in Gyeonggi Province and Seoul, respectively. No positive cases were detected in Incheon. CONCLUSIONS The proportion of undetected cases in Korea remained low as of early 2021. Therefore, an infection control strategy with exhaustive tracing and widespread pre-emptive testing appears to be effective in containing community spread of COVID-19.

In this paper, we propose an Energy Harvesting aware Entropy-based Routing protocol, called EHER, in mobile ad-hoc wireless sensor networks. The main features and contributions of this paper are as follows. First, we use physical layer information of RF-energy harvesting when routes are selected in network layer. Second, we use energy aware entropy based mesh route selection strategy adaptively to efficiently support route stability. Third, we use a direction-guided line using location information to support energy saving for the establishment of routing routes in the given networks. The performance evaluation using OPNET shows that the proposed routing protocol can efficiently support PDR, average and minimum remaining energy of nodes, and the lifetime of routes and networks, respectively.
Apolipoprotein(a) Kringles for Gene Therapy of Colon Cancer

Lipoprotein(a) is a proatherogenic, prothrombotic lipoprotein that comprises apolipoprotein(a) (apo(a)) tethered to apolipoprotein B-100, which surrounds a lowdensity lipoprotein (LDL) moiety. Despite its molecular similarity to plasminogen, apo(a) plays distinct physiological and pathological roles both in vitro and in vivo. In this study, we examined possible target molecules and the therapeutic potential of gene therapy using a recombinant adeno-associated virus (AAV) carrying the LK8 and LK68 (LKs) genes, which encode the cryptic kringle fragment of apo(a) (rAAV-LK), for the treatment of colon carcinoma. The results showed that AAV-produced LKs inhibited the migration of HUVECs induced by different angiogenic factors in a dose-dependent manner, and the potency was >1,000-fold than that of recombinant LK8 and LK68 proteins. A single intramuscular injection of rAAV-LKs (1 × 109 infectious particles (IP)/mouse) significantly inhibited the growth of subcutaneously transplanted LS174T tumors and reduced the number of tumor nodules in metastasized livers. Immunohistochemical analysis showed that AAVproduced LKs inhibited microvessel formation within the tumor mass, thereby inducing apoptosis in the tumor periphery, leading to the suppression of both tumor growth and metastasis. Taken together, these results suggest that AAV-produced LKs are multi-potent angiogenesis inhibitors that suppress the growth and metastasis of colon carcinoma.

Cerebral vascular surgery can damage patients’ motor and sensory nerves; therefore, neuromonitoring is performed intraoperatively. Patients with diabetes often have peripheral neuropathy and may be prone to nerve damage during surgery. This study aimed to identify factors that should be considered when diabetic patients undergo intraoperative neuromonitoring during brain vascular surgery and to present new criteria. Methods: In patients with and without diabetes who underwent cerebrovascular surgery (n = 30/group), we compared the intraoperative stimulation intensity, postoperative motor power and sensory, glycated hemoglobin (HbA1c) and glucose levels, and imaging findings. Results: Fasting glucose, blood glucose, and HbA1c levels were 10%, 12.1%, and 9.7%, respectively; they were higher in patients with than in patients without diabetes. Two patients with diabetes had weakness, and 10 required increased Somato sensory evoked potential (SSEP) stimulation, while in 16, motor power recovered over time rather than immediately. The non-diabetic group had no weakness after surgery, but 10 patients required more increased SSEP stimulation. The diabetic group showed significantly more abnormal test results than the non-diabetic group. Conclusion: For patients with diabetes undergoing surgery with intraoperative neuromonitoring, whether diabetic peripheral neuropathy is present, their blood glucose level and the anesthetic used should be considered.

South Korea has been divided into quantities and water quality, and due to a revision of the Government Organization law in June 2018, the controversial water management system was integrated into the Ministry of Environment. The total Maximum Daily Loads System has been called the flower of water quality control, and since 2004, all three major river systems which have been introduced into the Han River system, despite its various difficult environments, and subsequently leading to all of the four major rivers undergoing obligatory implementation since 2013. Currently, the target TMDL (Han River Phase 1 and Other Water Systems Phase 3) for the 2020 stage has been implemented. The domestic TMDL established a basic plan for calculating the load which complies with the unit watershed’s target water quality, as well as an implementation plan for annual load management, both which have been institutionalized in order to evaluate load compliance on a repeated annual basis. Local governments ask external organizations to conduct investigations every year in order to assess the transition, which thereby requires tens of millions of won every year. Therefore, an assessment and management model that can be easily operated at the TMDL personnel level is required. In this study, when the Han river Water System TMDL was implemented in earnest, we confirmed the the water quality improvement effect when TMDL was introduced to major inflow tributaries (TancheonA, JungnangA, AnyangA) under the Seoul City’s jurisdiction through the use of the total amount control unit basin evaluation technique. By presenting customized management measures, we propose the guidelines that are necessary for determining more effective water environmental policies.
